1.Effects of lipopolysaccharide on the tryptophan-kynurenine metabolic pathway

Objective To investigate the effects of lipopolysaccharide(LPS) on the tryptophan-kynurenine(TRP) metabolic pathway in rat brains and provide new evidence for the relationship between inflammation and depression.Methods Rats in LPS group were given a single dose of 3.5 mg/kg LPS.while the rats in control group were given the same dosage of saline.The dialysis in ventro-hippocampus were collected by microdialysis within 8 hours and then the TRP,KYN and KA were detected by LC-MS/MS.And the expression of indoleamine 2,3-dioxygenase was detected by Western-blot.Results The level of TRP((550.15± 107.96) pmol/L) and KYN ((337.95±62.73) pmol/L) showed a time-dependent increase after administration LPS 4 h compared with the control group(TRP (368.38±59.31) pmol/L,KYN (172.80±43.96) pmol/L),while KA level in the circulation exhibited a trend to decrease,especially at 7 h ((3.47±0.62) pmol/L,P<0.05).The ratio of KYN/TRP significantly increased at about 5 h (0.69±0.11,P< 0.05),and an ratio of KA/KYN (0.02±0.00) was dramatically decreased after administering LPS 4 h compared with the control group (0.05±0.01)(P<0.05).Most of the analytes showed more dramatic changes around 4 h to 8 h.LPS group(1.48±0.37) increased the protein expression of indoleamine 2,3-dioxygenase compared with the control group(1.00±0.24) (P<0.01).Conclusions LPS may cause tryptophan metabolic abnormalities and accelerate the way of kynurenine metabolism,leading to decreased the kynurenic acid status.
2.Preliminary exploration on interactive follow-up mode for rehabilitation ofpostoperative breast cancer patietns in new medical environment

Objective To evaluate the effectiveness and prospect of innovative interactive follow-up mode for rehabilitation ofpostopera-tive breast cancer patietns in new medical environment. Methods The novel patient-centered medical care was promoted,and Pink Ribbon Clubs was established in different regions. Meanwhile,the patients’ self-support groups were formed, and a specialist for out-patients follow-up of outpatients by telephone or on the internet was arranged to provide psychological assessment,with timely and effective response. All data concerning to the follow-up were recorded and the patients were required to complete the relevant questionnaires. Results Compared with the same period in 2013,the follow-up rate was significantly increased to 80. 25%,and the patients showed higher satisfaction. The incidence of lymphedema in the patients was significantly reduced from 13% in 2013 to 7%. The quality of life was significantly improved and self-rat-ing score on depression was decreased from (51. 90 ± 8. 55) to (35. 81 ± 6. 21). Conclusion The kind of“Trinity” interactive mode is an effective follow-up mode with the clinical benefit in patients’ rehabilitation of physical and mental health,which is worthy of further applica-tion.
3.A meta analysis of gemcitabine plus platinum chemotherapy compared with single-agent chemotherapy in the treatment of non-small cell lung cancer.

BACKGROUND AND OBJECTIVEWhether gemcitabine plus platinum chemotherapy is superior to gemcitabine or platinum single-agent chemotherapy for patients with non-small cell lung cancer (NSCLC) is still in dispute, and the aim of this study is to evaluate the efficacy and safety of gemcitabine combining platinum chemotherapy for patients with NSCLC.
METHODSWe searched relevant randomized controlled trials (RCTs) from VIP, CBM, CNKI, the Cochrane library, PUBMED and EMBASE. We traced the related references and experts in this field and communicated with other authors to obtain the information that has not been found. We made quality assessment of qualified RCTs assessed by the exclusion and inclusion criteria and used RevMan 5.0 provided by the Cochrane Collaboration to perform meta-analysis.
RESULTSFour RCTs were eligible and included 984 patients. Meta analysis results suggested that: compared with gecitabine single-agent chemotherapy, the combination had a statistically significant benefit in increasing the response rate (OR = 3.29, 95% CI: 1.79-6.05, P = 0.000 1) and 2-year survival rate (OR = 3.22, 95% CI: 1.45-7.12, P = 0.004) while increased the risk of the incidence of adverse reactions, especially the grade 3-4 thrombocytopenia (RR = 8.16, 95% CI: 1.71-39.07, P = 0.009); compared with cisplatin single-agent chemotherapy, the combination had a statistically significant benefit in increasing the response rate (OR = 3.51, 95% CI: 2.20-5.60, P < 0.01) and 1-year survival rate (OR = 1.67, 95% CI: 1.16-2.41, P = 0.006) while increased the risk of the incidence of adverse reactions, especially the grade 3-4 thrombocytopenia (OR = 28.55, 95% CI: 14.06-57.04, P < 0.01).
CONCLUSIONCompared with single-agent chemotherapy, the combining can significantly improve the efficiency and survival rate while increase the toxicity rare. The results still need to be proved by high quality RCTs.

4.Peroxisomal Fitness: A Potential Protective Mechanism of Fenofibrate against High Fat Diet-Induced Non-Alcoholic Fatty Liver Disease in Mice

Background:
Non-alcoholic fatty liver disease (NAFLD) has been increasing in association with the epidemic of obesity and diabetes. Peroxisomes are single membrane-enclosed organelles that play a role in the metabolism of lipid and reactive oxygen species. The present study examined the role of peroxisomes in high-fat diet (HFD)-induced NAFLD using fenofibrate, a peroxisome proliferator-activated receptor α (PPARα) agonist.
Methods:
Eight-week-old male C57BL/6J mice were fed either a normal diet or HFD for 12 weeks, and fenofibrate (50 mg/kg/day) was orally administered along with the initiation of HFD.
Results:
HFD-induced liver injury as measured by increased alanine aminotransferase, inflammation, oxidative stress, and lipid accumulation was effectively prevented by fenofibrate. Fenofibrate significantly increased the expression of peroxisomal genes and proteins involved in peroxisomal biogenesis and function. HFD-induced attenuation of peroxisomal fatty acid oxidation was also significantly restored by fenofibrate, demonstrating the functional significance of peroxisomal fatty acid oxidation. In Ppara deficient mice, fenofibrate failed to maintain peroxisomal biogenesis and function in HFD-induced liver injury.
Conclusion
The present data highlight the importance of PPARα-mediated peroxisomal fitness in the protective effect of fenofibrate against NAFLD.
5.Construction of the theoretical knowledge system of stroke emergency nurses based on Delphi consultation

[Abstract]? Objective? To construct a theoretical knowledge system for stroke emergency nurses and to provide reference for the establishment of the theoretical knowledge training course for stroke emergency nurses. Methods? Through literature review and referring to relevant guidelines, the preliminary items of theoretical knowledge of stroke emergency nurses were formed. Through Delphi method, 20 experts were inquired for two rounds, and the main points of theoretical knowledge were established. Results? The positive coefficients of the two rounds of expert inquiries were 100%, and the positive coefficients of experts were high. The judgment basis, familiarity and authority coefficients of expert consultation were 0.925, 0.844 and 0.885, respectively, all> 0.700, and the reliability of consultation results was high. The Kendall Harmony Coefficient of the secondary indicators was 0.462 and 0.293 respectively (P<0.01). The theoretical knowledge system of stroke emergency nurses included 8 primary indicators, 24 secondary indicators and 45 key points of knowledge. Conclusions? The consultation experts' opinions are concentrated and the results are credible. The establishment of the theoretical knowledge system of the stroke emergency nurses can provide a reference for setting up the theoretical training courses for stroke emergency nurses.
6.Optimization of in-hospital treatment mode among patients with acute ischemic stroke from medical staff's perspective:a qualitative study

[Abstract]? Objective? To explore medical staff's attitude towards in-hospital treatment mode among patients with acute ischemic stroke(AIS),and the optimizable factors of the mode,so as to provide reference for further optimization of the treatment mode. Methods? From November of 2017 to January of 2018, 15 medical staff from the AIS team in Changhai Hospital Affiliated to the Second Military Medical University were selected by purposive sampling and interviewed by semi-structured interviewing method. Data was collected in the way of handwriting and recording. Colaizzi data analysis was used to analyze the materials. Results? The experience of medical staff in optimizing the treatment mode of AIS hospital included five themes: (1)the treatment mode of AIS hospital was basically reasonable; (2)the need for early warning of neurointerventional treatment team;(3)the need for the construction of specialist nursing team for the emergency treatment of stroke; (4)the need for supervision of the treatment process of AIS by information technology; (5)the need for pre-hospital warning. Conclusions? It is necessary to make preparations in advance for invasive treatment team, construct the specialist stroke first aid nursing team for the whole process guiding and quality control, promote the integration of pre-hospital and in-hospital treatment so as to activate the in-hospital treatment system as soon as possible, improve the construction of AIS treatment informatization in order to assist the supervision of treatment efficiency, and further shorten the time of in-hospital treatment of AIS.
7.Diagnostic value of nucleic acid matrix-assisted laser desorption ionization-time of flight mass spectrometry in sputum smear-negative patients with nontuberculous Mycobacterial pulmonary disease

Objective:To investigate the diagnostic value of nucleic acid matrix-assisted laser desorption ionization-time of flight mass spectrometry(MALDI-TOF MS)in sputum smear-negative patients with nontuberculous Mycobacterial(NTM)pulmonary disease.Methods:Clinical data of 123 patients suspected of NTM pulmonary disease admitted in Public Health Clinical Center Affiliated to Shandong University between July 2022 and November 2023 were retrospectively analyzed. Bronchoalveolar lavage fluid(BALF)specimens were collected for MALDI-TOF MS assay and MGIT 960 culture. The diagnostic efficacy of MALDI-TOF MS for NTM pulmonary disease in patients with negative sputum smears for acid-fast bacilli was evaluated with receiver operating characteristic(ROC)curve. Statistical analysis was performed using SPSS 26.0 software and MedCalc statistical software.Results:Diagnosis of NTM pulmonary disease was finally confirmed in 66 out of the 123 suspected patients. It took 8 to 24 h for MALDI-TOF MS to identify NTM species and resistance. By MALDI-TOF MS,72 NTM strains were identified,with the Mycobacterium avium complex being the most prevalent(34 strains,47.22%),followed by the Mycobacterium abscessus complex(13 strains,18.06%);resistance to macrolides was detected in 6 cases,while no resistance to aminoglycosides was found. It took 9 to 45 days for BALF MGIT 960 culture to identify NTM,and took 7 to 15 days for NTM typing and drug sensitivity testing. By BALF MGIT 960 culture,28 NTM strains were identified;and 1 case was found to be resistant to macrolides. Using confirmed diagnosis as the gold standard,MALDI-TOF MS demonstrated higher sensitivity,negative predictive value,and agreement rate compared to MGIT 960 culture(84.85% vs. 42.42%,81.13% vs. 56.32%,80.49% vs. 62.60%, χ2=25.667,8.998,9.664, P<0.05 or <0.01). The area under ROC curve(AUC)for MALDI-TOF MS was significantly higher than that of MGIT 960 culture(0.801 vs. 0.642, Z=3.300, P=0.001). Conclusion:Compared to MGIT 960 culture,MALDI-TOF MS exhibits superior diagnostic efficiency in detecting NTM pulmonary disease in patients with acid-fast bacilli smear-negative sputum,with advantage of rapidly identifying NTM species and resistance.
8.Helicobacter pylori may participate in the development of inflammatory bowel disease by modulating the intestinal microbiota.

Inflammatory bowel disease (IBD) is a non-specific inflammatory disease of the gastrointestinal (GI) tract that is generally accepted to be closely related to intestinal dysbiosis in the host. GI infections contribute a key role in the pathogenesis of IBD; however, although the results of recent clinical studies have revealed an inverse correlation between Helicobacter pylori (H. pylori) infection and IBD, the exact mechanism underlying the development of IBD remains unclear. H. pylori, as a star microorganism, has been a focus for decades, and recent preclinical and real-world studies have demonstrated that H. pylori not only affects the changes in the gastric microbiota and microenvironment but also influences the intestinal microbiota, indicating a potential correlation with IBD. Detailed analysis revealed that H. pylori infection increased the diversity of the intestinal microbiota, reduced the abundance of Bacteroidetes, augmented the abundance of Firmicutes, and produced short-chain fatty acid-producing bacteria such as Akkermansia. All these factors may decrease vulnerability to IBD. Further studies investigating the H. pylori-intestinal microbiota metabolite axis should be performed to understand the mechanism underlying the development of IBD.
