1.The diagnostic value of MCP-1 and RANTES in tuberculosis and malignant pleural effusion

Objective To determin and compare the significance of the expression of chemotactic cytokines in pleural effusions monocyte chemoattractant protein-1(MCP-1)and regulated on activation,normal,T-cell expressed and secreted(RANTES)levels in tuberculous and malignant pleural effusions.Methods The effusion and serum levels of MCP-1 and RANTES were measured in 37 patients with tuberculous pleural effusion and 35 patients with malignant pleural effusion,and selected 20 healthy people acting as control group.Results The effusion and serum levds of MCP-1 and RANTES in tuberculous effusion patients was significantly higher than that in malignant group(P<0.01);In tuberculous group,the content of MCP-1 in serum was higher than control group,the serum level of RANTES was no statistic significant difference between control group and tuberculous group.The contents of MCP-1 and RANTES were not correlated with pathologic type of lung.cancer.Conclusion MCP-1 and RANTES technique has an important clinical value in diagnosis and differential diagnosis of tuberculosis and malignant pleural effusion.
2.Function of ICU to supervise patients after comprehensive correction of facial skeletal contouring

Objective To investigate the function and necessity of ICU to supervise and cure the patients after comprehensive correction of facial skeletal contouring. Methods 178 patients were examined carefully and perfectly before operation to obviate taboo. After comprehensive correction of facial skeletal contouring operation, all the patients were transferred into ICU in order to be supervised and cured comprehensively. Results 26 (14. 61 %) patients had hypoxemia ( SPO2 ≤90 %),11(6. 18 %)kaliopenia (K+≤3. 5 mmol/L) and 18 (10. 11 %) hypertension. 21 patients appeared arhythmia. Among these 21 arhythmia patients, 15 (8.43 %) patients had pyknocardia (HR≥100/min), 3 (1. 69 %) atrial premature beat, 1 (0.56 %) fibrillation atrial, 2 (1. 12 %) premature ventricular contraction, 1 (0. 56 %) Ⅱ degree atrioventricular block. 8 patients had respiratory tract obstruction. 2 (1. 12 %) had hemorrhea of mandible and 1 transfused 600 ml blood. 17 (9. 55 %) patients had restlessness. 65 patients had nausea and vomitting. All patients in ICU having postoperative complications had been deal with corresponding management. There were no respiratory failure,hemorrhagic shock, cardiac arrest after operation in 178 patients. Conclusions ICU can offer timely, continuous, and systemic supervision and cure to patients after comprehensive correction of facial skeletal contouring,and reduce the postoperative emergency rate.
3.The advancement of cuproptosis in hematological tumors

Cuproptosis is a type of independent cell death form, that differs from apoptosis, necroptosis and ferroptosis. It is mediated by Copper (Cu), and mainly affects the lipoylation of proteases in the mitochondrial tricarboxylic acid (TCA) cycle and exhibits cytotoxicity through oligomerization; however, its specific mechanism, signal transduction process and regulation mode are still not clear. Mitochondria affect the sensitivity of cells to copper toxicity and play a central role in the occurrence and development of copper-related death. In recent years, though hematological tumors have achieved better remission through targeted therapy and immunotherapy, they are associated with high recurrence rates and poor prognoses. It is therefore imperative to find better prognostic indicators and new treatment ideas. This paper summarizes the interaction between Cu and mitochondria in the development of tumors and provides ideas for further exploration of the mechanism of copper death and coping with hematological tumors.
4.Meta-analysis of the effect of transitional care on quality of life in patients with pancreatitis

Objective:To evaluate the effect of transitional care on the quality of life of patients with pancreatitis.Methods:We searched the following databases such as PubMed, Web of Science, Cochrane Library, CNKI, CBM, and WanFang Data electronically. Other sources as applying were also retrieved. Comprehensive collection of continuing care on the quality of life in patients with pancreatitis related research. According to the evaluation method of Cochrane system, the quality evaluation and data extraction were carried out, and the RevMan 5.3 software was used for Meta analysis.Results:A total of 21 randomized controlled trials involving 1 818 patients (910/908) were included. SF-36 health survey scale, analysis of 8 Meta subgroups showed that all groups P<0.05, and the combined effect was statistically significant. The results of the intervention with telephone as the main outcome indicator were: maintaining a regular lifestyle [weight mean difference ( WMD) value was 4.33, 95% confidence interval ( CI)2.52-7.44, P<0.01], quitting smoking and drinking ( WMD value was 5.95, 95% CI 3.69-9.59, P<0.01), maintaining a healthy diet ( WMD value was 8.62, 95% CI 5.07-9.59, P<0.01). Continuous nursing based on telephone intervention can reduce the recurrence rate ( WMD value was 0.20, 95% CI 0.13-0.31, P<0.01). Meta-analysis based on the intervention of establishing archives for continuous nursing showed that WMD value was 0.25, 95% CI 0.12-0.52, P<0.01, and archives and telephone showed that WMD value was 0.24, 95% CI 0.09-0.63, P=0.004, and the combined effect was statistically significant. Conclusions:Continuous nursing can improve the quality of life of patients with pancreatitis, improve the compliance with medical advice, and reduce the recurrence rate, but it needs to be further verified by a large sample, high-quality, multi-centered randomized controlled trial.
5.Influencing factors on the death of HIV/AIDS patients treated with antiviral treatment in Butuo county, Liangshan Yi Autonomous Prefecture, 2010-2019

Objective:To understand influencing factors on the deaths of HIV/AIDS patients receiving antireviral treatment in Butuo county of Liangshan Yi Autonomous Prefecture (Liangshan) from 2010 to 2019, to provide data for drug replacement and sustainable antiviral treatment strategy.Methods:A matched case-control study was used to collect basic and follow-up information on AIDS death patients receiving antiviral treatment in Butuo county of Liangshan from 2010 to 2019. The control group was formed by sampling twice the number of cases. The logistic regression model was used to analyze the risk factors affecting mortality.Results:In 3 355 patients of HIV/AIDS treated with antiviral therapy, 1 179 cases in the death group and 2 176 cases in the control group. Including 81.34% were 30-49 years old, 69.09%males, 99.55% Yi nationality, 91.12% were married or cohabitated, 95.77% had junior high school education or below, and 88.41% peasants. Amultivariate logistic stepwise regression model showed that among the death risk factors, age ≥50 years old was 5.08 times (95% CI:3.05-8.48) that of the 18-29, female was 0.70 times (95% CI: 0.52-0.94) than male, the transmission rate of intravenous drug use was 1.43 times (95% CI: 1.06-1.91) that of heterosexual transmission, CD4 +T lymphocyte (CD4) count ≥350 cells/μl before treatment was 0.38 times (95% CI: 0.30-0.48) that of CD4 <200 cells/μl before treatment, the most recent antiviral treatment regimen containing LPV/r was 0.04 times (95% CI: 0.01-0.18) than that of stavudine (d4T) + lamivudine (3TC) + nevirapine (NVP)/efavirenz (EFV) regimen, drug resistance was 3.40 times (95% CI: 2.13-5.42) of non-drug resistance, non-viral load and non-drug resistance test results were 12.98 times (95% CI: 10.28-16.40) of non-drug resistance. Conclusions:Age, gender, transmission route, CD4 before treatment, the latest antiviral treatment program, and drug resistance test after antiviral therapy were the influencing factors of HIV/AIDS death in Butuo county. It is necessary to expand the coverage of viral load and drug resistance test to change the antiviral therapeutic schedule scientifically and carry out publicity and education on the compliance of patients with antiviral treatment and medical staff training in order to reduce the mortality of patients with antiviral treatment.
6.Effects of extended latissimusdorsi musculocutaneous flap combined with precision surgery for breast cancer in stage Ⅰ breast reconstruction

Objective:To evaluate the application value of extended latissimusdorsi musculocutaneous flap combined with breast precision hand in stage Ⅰ breast reconstruction.Methods:Twelve cases of extended latissimusdorsi musculocutaneous flap combined with stage Ⅰ breast reconstruction for breast cancer precision surgery were collected from January 2022 to February 2023 in our department, the postoperative complications of surgical techniques were analyzed, and the cosmetic effects of breast reconstruction were evaluated according to Harris standards.Results:All 12 cases underwent breast reconstruction with extended latissimusdorsi musculocutaneous flap, and the survival rate of the flap was 100%. There was local cutaneous margin necrosis in 1 case of back incision, partial ischemic necrosis of the nipple in 1 case, and effusion in the donor area of the back in 5 cases. Harris evaluation of cosmetic effect of breast reconstruction was excellent in 3 cases, good in 6 cases, and fair in 3 cases. Follow-up time was 4-16 months (median was 12 months), and no local recurrence or distant metastasis was observed during follow-up.Conclusions:The extended latissimusdorsi musculocutaneous flap combined with precision mastectomy stage Ⅰ reconstruction is a safe and feasible treatment method for breast cancer.
7.The research of chlorogenic acid content in Wudang Lonicerae Flos (Ⅰ)by different drying technology

Objective To study the different drying technology.on Wudang Lonicerae Flos (Ⅰ) in chlorogenic acid content.Methods Using HPLC method to research the difference in the contents of chlorogenic acid of Wudang Lonicerae Flos (Ⅰ) employing oven-drying method,air-dried method and steamed drying technologies.DIONEX C18 (4.6 mm×250 mm,5 μm) was used.The mobile phases consisted of acetonitrile-0.4% phosphoric acid aqueous solution (17 ∶ 83).The detection wavelengths of chlorogenic acid was 327nm.The flow rate was 0.4 ml/min.The column temperature was 30 ℃.Results The Separation of Chlorogenic acid content was satisfactory.The concentration and peak area showed a good linearity relationship with the range of 0.285-2.850 μg.Precision,repeatability and stability met the requirements.The average recovery was 100.26% with RSD 1.95%.Chlorogenic acid content in the steamed drying Wudang Honeysuckle (Ⅰ) was higher.Conclusions The method of steam drying is better than that of oven-drying method and drying.
