1.Relationship between ways of nutritional support and immune function in patients with malignant obstructive jaundice after PTCD

Objective To investigate the clinical effect of different nutritional therapies on the immune function of patients with malignant obstructive jaundice after percutaneous transhepatic cholangiodrainage (PTCD).Methods A total of 50 patients with malignant obstructive jaundice who were admitted to our hospital from January 2009 to March 2013 were randomly divided into two groups according to the admis-sion order.The patients in group A (n=25 )received enteral nutritional support after PTCD,and those in group B (n=25 )received total parenteral nutritional support after PTCD.Intra-group and inter-group comparisons were made in terms of jaundice clearance,nutritional indices,and body’s immune function on preoperative day 1 and postoperative day 7;comparison between the two groups was made by t test. Results Among the 50 patients who underwent PTCD,39 (78%)had good drainage,while 1 1 (22%)did not reach the expectation,of which,5 (10%)were in group A and 6 (12%)in group B.In both groups,the nutritional indices on postoperative day 7 were significantly higher than those on preoperative day 1(P<0.05),but no significant difference in these indices was observed between group A and group B on postoperative day 7 (P>0.05).The immune function of patients in both groups was significantly improved following PTCD and nutrition-al support (P<0.05),but there was no significant difference between the two groups (P>0.05).Although the same scheme of nutritional support was used,there were 1 1 patients who did not achieve the expected jaundice clearance after PTCD and had limited improvement in immune function compared with those who had complete jaundice clearance (all P<0.05).Conclusion Jaundice clearance is closely re-lated to PTCD in patients with malignant obstructive jaundice,but not markedly associated with the ways of nutritional support.
2.IL-12 signaling pathway through Lck/ p38 /c-jun in splenocytes from systemic lupus erythematosus mouse

Aim To investigate whether there is IL-12 signaling pathway through lck/P38/c-jun in splenic cells obtained from lupus mouse and its effect on splenic cells. Methods Mice with graft versus host disease were used as lupus nephritis model. Activity of Lck tyrosine kinase, p38 phosphorylation and mRNA expression of c-jun in splenic cells were determined by autoradiography, Western blot and Northern blot, respectively. Results There were higher levels of Lck activity, p38 phosphorylation and c-jun expression of IL-12-stimulated splenic cells from lupus model when compared with that observed in similarly treated splenic cells from normal control. The Lck activity and p38 phosphorylation were almost inhibited by Lck inhibitor PP1, on the other hand, p35 specific inhibitor SB203580 decreased phosphorylation of p38. In addition, expression of c-Jun was also inhibited by PP1 or SB203580 although splenic cells were stimulated with IL-12. Conclusion Aberrant murine splenic cell, IL-12-me-diated intracelluar signaling pathway through lck/p38/c-Jun were involved in immunologic damage.
3.Effect of cognitive-behavioral intervention on mental state and quality of life in elderly patients with coronary heart disease

Ninety elderly patients with coronary heart disease were randomly divided into 2 groups:in study group (n = 45) the cognitive-behavioral intervention as well as conventional medical treatment were performed and in control group ( n = 45 ) only conventional treatment was given. Short Form-36 ( SF-36),symptom checklist 90(SCL-90), Hamilton anxiety scale( HAMA), Hamilton depression scale( HAMD)were evaluated before and after intervention, the serum total cholesterol (TC), triglyceride (TG)and body mass index (BMI)were measured, the smoking and drinking behaviors were recorded. After 6-month of intervention the smoking [(0. 6 ± 0. 2)cigarettes/d] and drinking [(10. 1 ± 2. 3)g/d] behaviors, the scores of SF-36 (85 ± 16), SCL-90 (104 ±4), HAMA (10 ±4) and HAMD (12 ±3), the systolic blood pressure [( 123 ± 11 ) mm Hg ( 1 mm Hg = 0. 133 kPa)], diastolic blood pressure [(77 ± 5 ) mm Hg], the fasting blood glucose [(4. 2 ± 2. 6 ) mmol/L] and BMI ( 22.5 ± 0. 5 ) in study group were significantly improved compared to control group ( P < 0. 01 ). The results indicate that cognitive-behavioral intervention can improve the mental state and quality of life in elderly patients with coronary heart disease.
4.A study of deformable image registration of high-dose-rate brachytherapy CT image to intensity modulated radiation therapy CT image

Objective To study a novel method for the high-dose-rate brachytherapy (HDR) CT image to the intensity modulated radiation therapy (IMRT) CT image deformable image registration and dose accumulation.Methods The applicator in the HDR CT image is first segmented and removed,then a deflation step is performed on the applicator-free HDR CT image by solving the Navier-Stokes equation.Demons algorithm is utilized to register the deflated HDR CT image to the IMRT CT image,along with the HDR dose.The deformed HDR dose is then added on the IMRT dose and yield the final accumulated dose.Results The HDR CT image and IMRT CT image,as well as the corresponding dose distribution,from five cervical cancer patients are used for evaluation of the proposed algorithm,the results show that the proposed method can effectively get rid of the influence of the applicator and produce an accurate accumulated dose.Conclusions Dose accumulation and supervision is an important step in adaptive radiotherapy for accurate dose delivery and treatment plan re-optimization.The proposed method in this study can effectively accumulate the HDR dose to the IMRT dose domain,and the accuracy is proved to be sufficient for clinical needs.
5.Sol-gel preparation of ultrathin nano-hydroxyapatite coating and its characterization.

Present study used dip-coating techniques to fabricate ultrathin nano-HA coating on titanium in organic sol-gel of Ca (NO3)2. 4H2O and PO(CH3)3 and inorganic sol-gel of Ca (NO3)2. 4H2O and (NH4)2HPO4. Scanning electron microscope (SEM) and grazing-incidence X-ray diffraction (XRD) were used to observe the morphology and distribution of crystallite size (D) and lattice strain (epsilon) of ultrathin nano-HA coating. After heated at 400 degrees C, the apatite structure of coatings on titanium began to appear. At heating temperature of 400 degrees C-600 degrees C, the effect of heating temperature on D and epsilon of both coatings was obvious. Precursor types significantly affected the particle diameters of nano-HA coatings, which were 25-40 nm for organic sol-gel and about 100 nm for inorganic sol. The thickness of ultrathin nano-HA coatings was 2.5 microm for organic sol-gel and 5 microm for inorganic sol and morphology of interfaces between coating and titanium was intact and homogenous.

6.Uncertainty Analysis of Quercetin Determination in Sicao Tongmai Capsules by HPLC

Objective: To analyze the uncertainty of quercetin concentration determination in Sicao Tongmai capsules by HPLC. Methods: The source of uncertainty was confirmed by analyzing the HPLC determination process. The uncertainty components were quantified by statistics, and the extended uncertainty and confidence level were finally obtained. Results: The extended uncertainty of the measurement results was 0. 19 μg · g-1 . Quercetin concentration in Sicao Tongmai capsules was (353. 65 ± 0. 19)μg·g-1 . Conclusion: The uncertainty analysis method is suitable for the standard limit formulation for Sicao Tongmai capsules, and it is important to establish uncertainty analysis methods for traditional Chinese medicines.
7.Digital signal processing of a novel neuron discharge model stimulation strategy for cochlear implants.

OBJECTIVETo apply the classic leakage integrate-and-fire models, based on the mechanism of the generation of physiological auditory stimulation, in the information processing coding of cochlear implants to improve the auditory result.
METHODSThe results of algorithm simulation in digital signal processor (DSP) were imported into Matlab for a comparative analysis.
RESULTSCompared with CIS coding, the algorithm of membrane potential integrate-and-fire (MPIF) allowed more natural pulse discharge in a pseudo-random manner to better fit the physiological structures.
CONCLUSIONThe MPIF algorithm can effectively solve the problem of the dynamic structure of the delivered auditory information sequence issued in the auditory center and allowed integration of the stimulating pulses and time coding to ensure the coherence and relevance of the stimulating pulse time.

8.Group psychotherapy on anxiety and depression symptoms in patients with alcohol dependence

Objective:To explore the clinical effect of group psychotherapy on relieving anxiety and depression symptoms in patients with alcohol dependence.Methods:Totally 117 patients with alcohol dependence accompanied with anxiety and depression symptoms were selected from the Second People's Hospital of Puer City from 2014 January 1 to December 31.The anxiety and depression symptoms were assessed with the Self-Rating Anxiety Scale (SAS),Self-Rating Depression Scale (SDS) and Brief Psychiatric Rating Scale (BPRS).They were divided into drug combined group (intervention group,n =89) and single drug treatment group (control group,n =88).The two groups were administrated diazepam replacement therapy,and the intervention group combined group psychotherapy.Eight weeks later,treatment effect of the two groups were evaluated with the Clinical Global Impression Scale (CGI).Results:After intervention,the scores of SAS,SDS,BPRS,and CGI in intervention group decreased [(21.1 ±8.3) vs.(65.3± 10.2),(35.1 ±3.5) vs.(51.7 ±5.6),(4.3 ± 1) vs.(5.4 ±0.6),Ps <0.05].There was no significant difference in above scores between before and after treatment in the control group (Ps > 0.05).Conclusion:The effect of group psychotherapy on the basis of medicine may be better than that of single drug therapy on relieving anxiety and depression in patients with alcohol dependence.
9.Cone beam CT image iterative reconstruction based on Split-Bregman method.

OBJECTIVEWe propose a new iterative reconstruction method based on split-Bregman method with tight frame regularization for effective and accurate reconstruction of the sparse-view cone beam CT image.
METHODSA tight frame was chosen as the regularization term for the objective function, so that the image reconstruction involves only the minimization of an objective function according to the compressed sensing theory. We utilized the split-Bregman method to tackle the task of minimization in three steps: (1) a fast calculation of the forward projection matrix; (2) introducing an intermediate variable to transform the non-differentiated L1 regularization term into the differentiated L2 regularization problem, and solving the target function using conjugate-gradient method; (3) updating the intermediate variable using shrinkage formula from Bregman method.
RESULTSDigital and physical phantom experimental results suggested that our new approach had great advantages in terms of image quality, reconstruction time, and applicability.
CONCLUSIONThe proposed method can accurately reconstruct CBCT image with limited data to lower the X-ray dose and accelerate the calculation speed in comparison with the POCS method.

10. Analysis on the epidemiology and etiologic characteristics of first imported severe fever with thrombocytopenia syndrome case in Shenzhen

Objective:
To study the epidemiology and the etiology characteristics of first imported severe fever with thrombocytopenia syndrome (SFTS) case reported in Shenzhen city in 2017.
Methods:
Data on descriptive epidemiology was collected to study the characteristics to the epidemic. The serum sample collected from the suspect SFTS case was detected for IgM, IgG by ELISA and severe fever with thrombocytopenia syndrome bunyavirus (SFTSV) nucleic acid by real-time RT-PCR. The samples were further inoculated in Vero cell for virus isolation. The partial fragements of L and S gene were amplified by RT-PCR and sequenced to construct homology comparison and phylogenetic tree with the strains isolated from other areas.
Results:
The case was laboratory confirmed imported SFTS case in Shenzhen on May 2017. IgM antibody and RNA of SFTSV were detected in the serum sample. SFTSV named GDSZ01/2017/China was successfully isolated from the serum sample. The high nucleotide homology of L and S genome segments were found at 95.3%-98.2% and 93.8%-98.8% with other representative strains from the popular provinces, respectively. The phylogenetic tree indicated that GDSZ01 was most close to SDTA_3 strain, next to strains in Hubei procince. The isolated SFTSV belonged to genotype C3 with HB29, HB154.
Conclusions
The virological, serological and molecular features showed that the imported case of SFTS in 2017 was caused by SFTSV C3 genotype.
