Aim To investigate the lipid-lowering mechanism of Chinese Medicine curcumin.A sterol regulatory report system of LDLR gene expression in HEK-293 cell line was established.Method A green fluorescent protein(GFP) gene was constructed downstream of the sterol regulatory element-1(SRE-1) and the constructed plasmid was introduced into HEK-293 cell line.This SRE-GFP transfected 293 cells were treated with different concentration of curcumin for 2 days.The expression of GFP in the cells was measured by a fluorescence microscope and Flow Cytometry.Results The expression of GFP in SRE-GFP transfected 293 cells was obviously induced by curcumin.Conclusion Curcumin may positively affect the expression of LDLR gene by its influence on SRE-1.
2.The Effects of Noise from the Subway Cars on the Preferred Listening Volumes and Noise Exposure Using Earphones

Objective To investigate the effects of noise from the subway cars on the preferred listening volumes using different kinds of earphones.Methods A total of 75 subjects with normal hearing were investigated to listen the same song wearing 4 kinds of earphones: insert earphones, intra-concha earphones, supra-concha earphones, and circumaural earphones, in the semi-anechoic background and inside the Line 7 subway-cars of the Beijing subway (the mean of LAeq,1s is 80.3 dB A and then selected personally preferred listening volumes.The effects of noise in the subway cars and earphone types on the preferred listening volumes were then analyzed.Results Inside subway-cars, the preferred listening levels using insert earphones, intra-concha earphones, supra-concha earphones, circumaural earphones were 76.6±4.7, 84.3±5.0, 82.3±4.2, and 80.8±4.2 dB A, respectively.The levels were respectively 65.5±6.8, 71.9±7.1, 69.5±7.5, 67.9±7.3 dB A for those in the semi-anechoic background.The preferred listening levels increase more than 10 dB averagely when stepped into noisy subway-cars background from silent background.According to the threshold levels stipulated in the WHO, to avoid hearing impairment, the daily noise exposure duration should not exceed 134 mins when exposed to subway-cars noise and the duration should not exceed 315, 54, 85, and 120 mins respectively when wearing those 4 kinds of earphones in subway-cars.Conclusion The noise and earphones types have great effects on the preferred listening volumes.The noise exposure levels in the ear canal wearing earphones with good sound insulation and listening at highly increased volume may be lower than that without earphones.
3.Genetic analysis of neonatal diabetes mellitus and its influence on treatment outcome

Fourteen neonatal diabetes mellitus(NDM)patients were recruited. 9 patients were treated with glyburide and the other 5 with insulin. ABCC8, KCNJ11, and INS genes were sequenced in 6 of them. Gene mutations were found in 2, 1, and 1 cases in these genes, respectively. One case with 6q24 hypomethylation and another without known mutation were also found. 8 out of 9 patients treated with glyburide reached euglycemia(88.9%). The other 5 patients with insulin therapy either died or lost contact. The results suggest that glyburide therapy is effective in neonatal diabetes mellitus, while insulin therapy may contribute to poor compliance.
4. Advances in the research of basic study and clinical application of adipose-derived mesenchymal stem cells

Since the discovery of adipose-derived mesenchymal stem cell (ADSC) in more than ten years, a great progress has been made from its basic research to clinical application. Compared with bone marrow mesenchymal stem cells, ADSCs are more abundant in reserve, easier to obtain with fewer injuries and less complications. These cells have multiple differentiation potential and can differentiate into adipocytes, chondrocytes and osteoblasts with the influence of different inducing factors. Early studies of ADSCs mainly focused on the ability of multi-directional differentiation, espe-cially on the regeneration of bone defects and cartilage tissue. At present, the researches mainly focus on immunoregulation and paracrine function of ADSCs. Although ADSCs have made a great progress in clinical application, the cell preparation, use pattern, and mechanisms in clinical treatment are not clear. This paper elaborates on these issues.
5. Transplantation of compound tissue flap of toe to reconstruct the thumb with necrosis caused by electric burns in four patients

From January 2010 to December 2017, 4 patients of thumb with necrosis caused by electric burns (all male, aged from 31 to 58 years) were admitted to our hospital, with 1 patient of second degree injury of right thumb, 2 patients of third degree injury of right thumb, and 1 patient of third degree injury of left thumb. Routine debridement under general anesthesia was performed within 7 days after injury. The compound tissue flap of contralateral second toe was transplanted to reconstruct the thumb with third degree defect, and compound tissue flap of ipsilateral distal hallex was transplanted to reconstruct the thumb with second degree defect. Dorsalis pedics artery was anastomosed with radial artery, saphenous vein or dorsalis pedics vein was anastomosed with cephalic vein. The donor site was transplanted with split-thickness skin graft from autologous thigh. All the tissue flaps and skin grafts survived in 2 weeks after surgery. Within 1 year of follow-up, the reconstructed thumbs can achieve radial abduction and palmar abduction with good function. Reconstruction of thumb with free transplantation of compound tissue flap of toe is a good method to repair thumb with necrosis caused by electric burn.
6.Experimentation of effect of PD-1 inhibitor on myocardial inflammation microenvironment and radiation-induced injury

Objective:To explore the potential mechanism of PD-1 inhibitor P on RIMI from the perspective of immune microenvironment.Methods:To establish a mouse model of radiation-induced myocardial injury (RIMI), twenty C57BL/6 mice were randomly divided into 4 groups, 5 in each group. Group A was the healthy control group; Group B was the PD-1 inhibitor group; Group C was the simple irradiation group, with a heart irradiation of 15 Gy; Group D was the irradiation+ PD-1 inhibitor group. One month after irradiation, the mice were anesthetized and sacrificed. The morphological changes of myocardial tissues were observed by HE staining. The myocardial fibrosis was assessed by Masson staining. CD 3+ , CD 3+ CD 4+ , CD 3+ CD 8 lymphocyte subsets and cytokines (IL-4, IL-6, IL-17A, TNF-α, TGF-β 1 and INF-γ) levels were determined by flow cytometry. The apoptosis rate of myocardial cells was detected by TUNE. Results:One month after irradiation, there was no obvious myocardial fibrosis in group B, and collagen fibers were distributed in the interstitium of myocardial cells in groups C and D. Semi-quantitative analysis results showed that the myocardial collagen volume fraction (CVF) of groups A, B, C and D were (1.97±0.36)%, (2.83±1.03)%, (5.39±0.77)% and (7.72±1.43)%, respectively. The CVF between group A and group B was similar ( P=0.314), and the differences in CVF between the other groups were statistically significant (all P<0.05). Compared with group A, the absolute value and percentage of CD 3+ T lymphocytes were significantly increased in groups B, C and D (all P<0.01). The values in group D were significantly higher than those in group B and group C (all P<0.01); The absolute value and percentage of CD 3+ CD4 T lymphocytes were similar among four groups (all P>0.05); The absolute value and percentage of CD 3+ CD 8 T lymphocytes in group D were significantly higher than those in groups A, B and C (all P<0.001). The expression levels of IL-6, IL-17A, and TGF-β 1 in group D were significantly higher compared with those in groups A, B and C (all P<0.001). The apoptotic index was gradually increased in four groups, and the differences in apoptotic index among four groups were statistically significant (all P<0.001). Conclusion:PD-1 inhibitors can aggravate RIMI by promoting myocardial immune inflammatory response.

8.Clinical efficacy and safety of randomized phase 2 trial of pemetrexed-cisplatin or docetaxel-cisplatin plus thoracic intensity-modulated radiation therapy in patients with stage IV lung adenocarcinoma

Objective To evaluate the clinical efficacy and toxicity of concurrent pemetrexed-cisplatin (PP) or docetaxel-cisplatin (DP) with intensity-modulated radiation therapy (IMRT) in patients with stageⅠV lung adenocarcinoma. Methods Stage IV lung adenocarcinoma patients with unknown EGFR mutation status or wild-type admitted to Guizhou Cancer Hospital from 2011 to 2016 were randomly assigned into the PP (n=50) and DP groups (n=51).All patients received concurrent IMRT of the chest at a prescription dose of 60-70 Gy. Primary endpoint was 1-year survival rate, and secondary endpoint was acute toxicity. Results The overall response rate was 68. 0% and 72. 5% in the PP and DP groups (χ2=0. 250, P=0. 617) . The median survival time was 19. 6 months ( 95%CI 13. 9-25. 3) versus 12. 1 months ( 95%CI 10. 7-13. 5) in the PP and DP groups. The 1-, 2-and 3-year overall survival rates were 72. 0% versus 52. 9%, 28. 0% versus 17. 6%, and 16. 0% versus 13. 7%, respectively in the PP and DP groups ( P=0. 049) . In the PP and DP groups, the incidence of grade 3-4 leukopenia was declined by 48% and 63%( P=0. 098) , and the incidence of grade 3-4 neutropenia was decreased by 34% and 65%( P=0. 002) , the incidence of grade 3-4 anemia was reduced by 38% and 10%(P=0. 024), and the incidence of grade 3-4 thrombocytopenia was declined by 40% and 14%( P=0. 003) . The incidence rate of grade 2 pneumonitis ( P=0. 625) and grade 3 esophagitis ( P=0. 484) were similar in both groups. No patients experienced ≥grade 3 pneumonitis or ≥ grade 4 radiation esophagitis. Conclusions Pemetrexed-cisplatin combined with chemoradiotherapy yields higher clinical efficacy compared with docetaxel-cisplatin plus concurrent chemoradiation in the treatment of stageⅠV lung adenocarcinoma. The incidence of radiation pneumonitis and esophagitis is similar. The incidence and severity of hematological toxicity does not significantly differ between two groups.Treatment-related toxicity is tolerable in both groups.
