Purpose:To analyze the differences between prem enopausal and postmenopausal female patients with T1 primary breast cancer as to pathological classification, rate of lymphatic metastasis and some relative rec eptors, and to discuss the appropriate mode of operation to T1 breast cancer. Objective To investigate the relationship between postoperative cognitive dysfunc-tion and plasma cortisol levels in aged patients undergoing total hip replacement surgery. Methods Ninety patients aged 65-75 years old,scheduled for total hip replacement surgery under general anesthesia were enrolled in the present study.Cognitive state was tested by mini-mental state examination (MMSE).The patients were divided into two groups,POCD group and non-POCD group.Perioperative plasma levels of cortisol were determined on 2 d preoperative,2 d postoperative and 7 d postoperative.Results Incidence of POCD after 7 d in aged patients undergoing total hip re-placement surgery was 37.8%.Compared with day 2 d preoperative,plasma cortisol levels in POCD patients significantly increased on 2 d and 7 d post operation (P <0.01 or P <0.05).Compared with non-POCD patients,cortisol levels on the 2 d and 7 d postoperative day significantly increased in POCD patients (P <0.05).Plasma cortisol levels showed a negative correlation with MMSE scores on postoperative 2 d and 7 d(P <0.01).Conclusion Elderly patients have a relatively higher incidence of POCD after hip replacement surgery.POCD may be caused by high plasma cortisol levels in elderly patients following hip replacement surgery under general anesthesia.
5.Clinical Efficacy of Transcatheter Induced Closure in Patients With Small Patent Ductus Arteriosus

Objective: To explore the methodology and efifcacy of transcatheter induced closure in patients with small patent ductus arteriosus (PDA). Methods: A total of 717 PDA patients treated in our hospital from 2005-11 to 2014-08 were summarized and there were 8 patients with small PDA were treated by transcatheter induced closure method including 3 male and 5 female from (1-6) years of age at the mean of (3.9±1.4) years with the body weight of (10-21) kg at the mean of (15.2±3.7) kg. The procedures were performed under local or general anesthesia with right cardiac catheterization and descending aortic arch angiography to observe PDA morphology and to measure PDA diameter at aortic and pulmonary aterial lateral and ductus length. Right catheter along the guide wear was pushed to the narrowest part of PDA and the motion was repeated for several times to stimulate the local area and then, the catheter was kept at PDA aortic lateral about 20 minutes thereafter. Results: Descending aortic arch angiography indicated that no residual shunt at 20 min after catheter partial blockage in all 8 patients, the immediate closure rate was 100%. No patient suffering from re-canalization by 1 year follow-up examination. Conclusion: Transcatheter induced closure of small PDA has minor trauma, no foreign material implantation, with low cost and good effect. It provides a new method for treating such particular type of PDA patients in clinical practice.
6. Processing of origin integration method and technology of Rehmanniae Radix

Objective To study the processing of origin integration method and technology of Rehmanniae Radix (RR) and prove the feasibility of this new and innovative way. Methods The processing of origin integration technology of RR was selected with appearance and character as indexes, and thinking of appearance traits, content of catalpol, acteoside, rehmaionoside A, D and leonuride as an index, adopting comprehensive index scoring method to select the new processing of origin integration technology of RR. Results The optimized technology of the processing of origin integration of RR was as follows: out 2—6 cm diameters of fresh RR, baked completely at 75 ℃, piling up and sweating for 12 h, cutting 4—5 mm thick slices, dried at 75 ℃ up to 4—5 h, taking out and packing after cooling. Conclusion Adopting the processing of origin integration technology directly processes fresh RR into RR decoction pieces, which is simple as well as convenient, reducing the repeated processing and storage steps and achieving the aim of reducing the processing cost on Chinese herbal medicines and improving the quality of Chinese medicine decoction pieces.
7.Correlation between high-risk human papillomavirus viral load and severity of cervical lesion

Objective To explore the correlation between high-risk human papillomavirus(hr-HPV) viral load and severity of cervical lesion. Methods One thousand eight hundred and six women undergoing both hr-HPV DNA test by hybrid capture Ⅱ(HCⅡ) and colposcopic biopsy for histologic results were enrolled in this study.Correlation between hr-HPV viral load and pathological findings was investigated. Results Of 1 806 cases,641(35.5%) patients were positive for HPV DNA.23.2%(301/1299) of women with normal diagnosis had HPV infection,significantly lower than cervical lesion including cervical intraepithelial neoplasia(CIN) and cervical cancer(P=0.000).Associations(odds ratio) among severity of cervical lesion and viral load were calculated.The significantly increased risk only existed between cervical cancer and CIN1 on high viral load(odds ratio,8.5;95% confidence interval,1.0-71.4;P=0.049).Furthermore,viral load values in CIN1,2,3 and cervical cancer were calculated and median analysis revealed non-significant difference(P=0.712). Conclusion The hr-HPV viral load can distinguish cervical lesion as CIN and cervical cancer from normal one,while the prediction of the severity of cervical lesion may be inadequate and need further investigations.
8.Thoracoscopy combined with laparoscopy in treatment of esophageal cancer radical operation (40 cases)

Objective To explore and analyze the application and problems of esophageal cancer radical opera-tion by thoracoscopy combined with laparoscopy. Methods From July, 2011 to December, 2014, clinical data of 40 patients undergoing esophageal cancer radical operation by thoracoscopy combined with laparoscopy were analyzed retrospectively. Results Except 2 cases being relayed to assisted small thoracic incision because of 1 case with clear tumor invading to trachea and 1 case with obvious tumor invasion to thoracic aortic artery, the remaining 38 cases were successfully completed operation. There were 2 cases with postoperative anastomotic fistula in the neck, 2 cases of anastomotic stenosis and 1 case of hoarseness, there was no perioperative death. Follow-up was within 36 months, 2 patients died 10 months and 24 months after surgery respectively, due to tumor local recurrence. 1 patient with supraclavicular lymph node metastasis underwent local radiation therapy. The remaining was all survival with re-lapse-free. Conclusions Esophageal cancer radical operation by thoracoscopy combined with laparoscopy is a good method with safety and reliability, minimal invasion, fast recovering, and satisfied recent curative effect. It is worthy of applying clinically.
9.Clinical study on changes of indexes of hemorheology and inflammatory factors of fasudil intervention on acute myocardial infarction

Objective To investigate the possible mechanisms of fasudil on acute myocardial infarction (AMI)by observing the changes of hemorheology and inflammatory factors.Methods 120 patients diagnosed AMI in our hospital were randomly divided into the two groups:60 cases in the control group were given the conventional treat-ment and the other 60 cases in the treatment group were added fasudil for 2 weeks based on that.Then we analyzed the changes of hemorheology and inflammatory factors.Results 2 weeks later,the whole blood restore viscosity,plasma viscosity,CRP,IL -1β,IL -6 and TNF -βin the two groups were all obviously decreased than those of before(P <0.05),while,packed cell volume in the treatment group was obviously decreased as well (P <0.05).Furthermore, the changes in the treatment group were more significant compared with the controlled group (P <0.05).Conclusion Fasudil intervention on AMI can improve the situation of hemorheology and relieve the inflammation more effectively than conventional treatment.
10.Study on relations between transient receptor potential vanilloid 1 and pungent property of traditional Chinese medicines.

The five-flavor theory of traditional Chinese medicines (TCM) and the flavor efficacy generation mechanism has long been focuses and difficulties in studies on traditional Chinese medicinal properties. In this paper, by using the pharmacophore-based virtual screening technique, the authors discussed the relations between the pungent property and transient receptor potential vanilloid 1 (TRPV1) by studying the TCM components' role in regulating TRPV1 ion channel. The results showed that the matching relationship between TRPV1 agonist pharmacophore model and TCM chemical components could identify the active ingredients from pungent herbs. Therefore, the authors proposed that TRPV1 is one of the potential targets for efficient pungent herbs. The pungent property of TCMs is decided by its chemical components, and consistent with the inherited and additive characteristics.
