2.Tolerability and safety of enteral nutritionin in critical ill patients with hemodynamic instability

In patients with a functional gut,enteral nutrition is the preferred route of nutrition support.The favorable effects of enteral nutrition include prevention of mucosal atrophy,maintaining of the integrity of gut flora,and improvement of immunocompetence.The guidlines recommend EN be withheld in patients requiring significant hemodynamic support,because of highdose catecholamine agents which could reduce EN tolerance.Splanchnic perfusion is reduced in sepsis shock,yet vasoactive agents have demonstrated both improved and diminished perfusion.Inadequate perfusion increases the risk of experiencing rare but serious adverse events.This study summarizes the tolerability and safety of enteral nutritionin in critically iH patients with hemodynamic instability and provides theoretical basis for the early administration of EN.
4.Effect of preoperative regional artery chemotherapy on gastric carcinoma

Objective To evaluate the effects of preoperative regional artery chemotherapy (PRACT) on gastric carcinoma. Methods Celiac arteriography was performed by transfemoral artery catheter according to Seldinger's method and superselective catheterization proceeded to the supplying artery of focus of lesion. Antineoplastic agents of Fluorodeoxyuridine (FUDR)(0.8 g/m 2), epirubicin(40 mg/m 2), oxaliplatin(80 mg/m 2) were infused into regional artery by a single administration. The specimens from 14 cases of gastric carcinoma(GC) who received PRACT were separated into pre operation group and post operation group. Both microscopic study to observe the necrosis of cancer cell and TUNEL(TdT mediated dUTP nick end labeling) method to detect the state of apoptosis were performed respectively in the two groups. Correlation of apoptosis index (AI) between the two groups, as well as correlation between the situation of necrotic tumor cell and the blood vessel, was analyzed. 5.Progress and prospect in the clinical translation of cancer nanomedicine

Nanotechnology has shown broad application prospects in the diagnosis and treatment of cancer. Currently, nearly 80 cancer nanomedicines are under clinical investigation, and many have been approved with enhanced anti-tumor efficacy and decreased side effects. However, the presence of various barriers in related basic research, process control and clinical trials lead to extremely low translation rate. From the perspective of clinical commercialization, we summarized the progress, clinical status, challenges and opportunities of cancer nanomedicine, and presented a cutting-edge prospect on the rational design of nanomedicine and clinical trial strategies.
6.Effect of different sedation anesthesia depth toperioperative hemodynamic in closed-loop target controlled infusion

Objective To investigate the effect of different sedation depth of anesthesia the BIS settings (BISis 45 and 55) to perioperative hemodynamic. Methods Thirty-two patients were randomly divided into two groups. Through closed-loop target controlled infusion technology, depth of sedation anesthesia in A:the BIS=45, and in B: the BIS=55.12 time points(i.e. before operation, tracheal intubation instant, operation beginning, 20, 40,60,80,100 and 120 minutes after operation beginning, operation accomplishment point, tracheal catheter pulling out point, before patient′s carried out of operation room)perioperativenoninvasivemean arterial blood pressureandheart rate wererecorded. Results Themean arterial blood pressurebetween the two groups at time point T2,T11 was significantly different. Within group B,compared with T1, there wassignificantdifferenceat T2,T11. Heart ratein group B at T5and T10increasedsignificantly compared with A.Within group B,heart rate at T5,T6 increased significantly compared with that at T1. Conclusion Compared with BIS 55, the sedation anesthesia depth of BIS 45 can better keep stable of blood pressureandheart rate.
7.Influence of psychological factors on aesthetic satisfaction of all-ceramic restorative patients

ObjectiveTo analyze the relationship between the patients' psychological factors and esthetics satisfaction of all-ceramic restoration of anterior teeth.MethodsThe N scales of Eysenck personality questionnaires (EPQ) were filled at the first visit,and the total scores of N scales were calculated.After restorative treatments,the satisfaction questionnaire was used to evaluate the patients' aesthetic satisfaction on material quality,color,geometry,profile,self-confidence,etc,and to analyze the relation between psychological factors and satisfaction of the patients.ResultsThe scores of EPQ N scale of the patients in this study were as follows:Nmax=23,Nmin =3,Navg =9.35±5.714.The patients had high satisfaction with restoration.Aesthetic satisfaction,color and self-confidence were in relation to the N scales scores,satisfaction of color was in relation to sickness suspicion,satisfaction of profile was in relation to enthrallment extent and guilty consciousness,and satisfaction of self-confidence improvement was in relation to anxiety and self-determination.Conclusions The psychologic factor of the patients has some influence on the esthetics satisfaction of all-ceramic restoration of anterior teeth.
10.A comparative effectiveness research on operative methods of femoral neck fractures in the elderly

Objective To compare the effect of compression hollow screw fixation,artificial femoral head replacement and total hip replacement in the elderly.Methods From August 2007 to April 2009,152 cases of femoral neck fracture in the elderly were divided into three groups: compression hollow screw fixation group(54 patients,24 males,with an average age of 59.8±5.3 years);artificial femoral head replacement group(48 patients,16 males,with an average age of 65.3±6.7 years);and total hip replacement group(50 patients,18 males,with an average age of 77.3±6.5 years).The evaluation indicators,including the average sugical time,the average amount of bleeding,the average length of stay and the average postopererative bed time,were compared respectively among the three groups.The postoperative evaluation was made according to Harris Score.Results Harris score in the three groups were 21 excellent and 23 good in the compression hollow screw fixation group,with an average score of 80.52±2.70;26 excellent and 14 good in the artificial femoral head replacement group,with an average score of 86.57±1.90;and 42 excellent and 3 good in the total hip replacement group,with an average score of 96.04±2.10.The total hip replacement group was significantly better than the other two groups(H=0.589,F=12.151,Ps<0.05).The rate of postoperative complications were 25.93%(14/54) in the compression hollow screw fixation group,18.75%(9/48) in the artificial femoral head replacement group and 4.00%(2/50) in the total hip replacement group(H=1.291,P<0.05).Conclusion Compared among the three surgery,total hip replacement could resume function and improve the quality of life better,with lower rate of complications.However,the surgical injury and the amount of bleeding were largest.Therefore,we suggest selecting appropriate surgery with considering the patient′s age,physical condition,displacement degree of the fracture,et al.in old femoral neck fractures patients.
