1.The prevalence and metabolic risk factors of nonalcoholic fatty liver disease among middle-aged and aged people

Objective To investigate the prevalence of nonalcoholic fatty liver disease in a population of routine check-up,and analyze the correlation between non-alcoholic fatty liver disease and metabolic risk factors.Methods Physical examination data of 2 064 routine check-up (aged 45 or more) in the Third People's Hospital of Chengdu from January 2014 to December 2015 were randomly selected.HbA1c,fasting blood glucose,blood lipids and other indicators were measured to calculate the prevalence of NAFLD,and to analyze the correlation between NAFLD and metabolic risk factors.Results Among 2 064 cases underwent physical examination,the overall prevalence of NAFLD was 16.09% (332/2 064).The male prevalence rate of NAFLD was significantly higher than that in female (18.51% vs.12.87%;x2 =11.913,P =0.001).In four age groups (< 50 years old,50 to 59 years old,60 to 69 years old,≥ 70 years old),the NAFLD prevalence rate was 16.26%,15.63%,16.97% and 14.72% respectively.No statistical significance (x2 =0.900,P =0.826) was found in NAFLD prevalence rate difference in all age groups of has.In 50 ~ 59 years old and ≥70 years old group,the NAFLD prevalence rates in male were higher than that in female,the difference was statistically significant (both P < 0.05).BMI,waist circumference,systolic blood pressure,diastolic blood pressure (DBP),HbA1c,fasting blood glucose,TC,TG,LDL-C,uric acid levels in NAFLD patients were higher than the normal population.HDL-C levels in patients with NAFLD is lower than that of the normal,and the difference is statistically significant (both P < 0.05).Logistic regression analysis showed that BMI,hypertension,diabetes,and high TG were the independent risk factors of NAFLD;HDL-C and NAFLD were the protective factors.Conclusions In the Chengdu area of Sichuan Province,the prevalence rate of NAFLD was high,the incidence rate of NAFLD in male was higher than that in female.BMI,hypertension,diabetes mellitus and high TG were the independent risk factors of NAFLD,and HDL-C was the protective factor.
2.Effects of very early hyperbaric oxygen treatment on patients with malignant brain edema after surgical operation for severe traumatic brain injury

Objective To investigate effects of very early hyperbaric oxygen treatment (HBOT) on patients with malignant brain edema after surgical operation for treatment of severe traumatic brain injury (TBI).Methods A total of 146 patients who suffered from malignant brain edema after last surgical operation for severe TBI were enrolled for this study.According to the intervention time of HBOT,they were randomized into a very early group (HBOT within 3 days after operation,n =55),an ordinary group (HBOT at 4 to 10 days after operation,n =65) and a control group (non-HBOT,n =26).Mortality rate of the 3 groups were recorded within 4 weeks after operation,and GCS (Glasgow Coma Scale) score were assessed in 1,2,3 and 4 weeks after operation.Dynamic head CT scan were performed for detecting brain status and for determine the duration of brain edema.Results The mortality of the very early group,the ordinary group and the control group were 10.9%,7.7% and 11.5%,respectively,and no statistically significant difference was revealed among the groups (P > 0.05).In very early group,the GCS scores of 1,2,3 and 4 weeks after operation were (8.837 ±3.350),(10.755 ± 3.388),(11.633 ± 3.408) and (12.367 ± 3.408),respectively,with significant difference between the time points 1 week and 2,3 and 4 weeks as well as 2 and 4 weeks after surgery (P <0.05),but not between 2 and 3 as well as 3 and 4 weeks after surgery (P > 0.05).In ordinary group,the GCS scores at 1,2,3 and 4 weeks after surgery were (8.509 ±3.042),(9.458 ±3.115),(10.186 ±3.203) and (10.627 ±3.439),respectively,with significant difference between 1 week and 2,3 and 4 weeks after operation (P < 0.05).In control group,the GCS scores at 1,2,3 and4 weeks after surgery were (8.042 ±2.881),(8.417 ±2.962),(8.542 ±3.02) and (8.958 ± 3.043),with no statistical difference among different time points (P > 0.05).When compared with the very early group,the GCS sores of the ordinary group and the control group were significantly lower after intervention (P < 0.05),and the GCS of control group was lower than that of the ordinary group (P < 0.05).As for brain edema duration,the very early group was the shortest among the 3 groups (P > 0.05) Conclusion Very early hyperbaric oxygen treatment could improve consciousness state and alleviate malignant brain edema after surgical operation in TBI patients.
3.Efifcacy of the consolidation chemotherapy after autologous stem cell transplantation in refractory/relapse lymphoma

Background and purpose: High-dose chemotherapy followed by autologous stem cell transplantation (auto-HSCT) is considered as the ifrst line treatment for patients with relapse/refractory lymphoma after conventional chemotherapy. However, most of these patients still relapse the second time. The purpose of this study was to analyze the efifcacy of the consolidation chemotherapy after autologous stem cell transplantation (HSCT) refractory/relapse lymphoma in high risk. Methods:A total of 38 patients with relapsed/refractory lymphoma including Hodgkin lymphoma (HL) and non-Hodgkin lymphoma (NHL) were included, who were underwent auto-HSCT in our transplan-tation department from Jan. 2010 to Dec. 2013. In treatment group, 19 patients received 2 courses of consolidation che-motherapy after auto-HSCT every 2 to 3 months, with the regimen of mini-BEAM or modiifed mini-CBV. Another 19 patients had no chemotherapy after auto-HSCT as control group. Results:The median follow-up duration was 17.2 and 7.5 months in the treatment and control group respectively. The follow-up data demonstrated prolonged progression-free survival (PFS) in the treatment group than the control group [24.7 months vs 7.8 months, P=0.029 under intend-to-treat analysis ITT;24.7 months vs 5.2 months, P=0.01 under per protocol analysis(pp)]. There is also a trend of improved overall survival (OS) in the treatment group (P=0.055, ITT). Conclusion:Consolidation chemotherapy after auto-HSCT for refractory/relapsed lymphoma patients delay the relapse and tend to improve the overall relapse rate.
4.Exploration on the Feasibility of Priority Use of Essential Medicines Based on the Investigation Results in Two Third-level Grade-A Hospitals in Beijing

OBJECTIVE:To provide reference for promoting the priority use of essential medicines. METHODS:The use amount ratio of essential medicines in 2 third-level grade-A hospitals in Beijing from 2011 to 2013 was statistically investigated. And the literatures related to policies of essential medicines and application were retrieved for in-depth analysis. RESULTS:The use amount ratios of essential medicines in the outpatient and emergency departments in Beijing Hospital of Ministry of Health from 2011 to 2013 were respectively 24.04%,23.10% and 22.76%;and the use amount ratios of essential medicines of inpatients were respectively 16.56%,14.52% and 12.04%. The use amount ratios of essential medicines in the outpatient and emergency depart-ments in Beijing Friendship Hospital(a pilot hospitals of cancelling drug addition medical reform)in the 3 years were respectively 21.59%,19.85% and 22.93%;the use amount ratios of essential medicines of inpatients were respectively 17.70%,17.62% and 15.89%. The use amount ratios of essential medicines did not meet 25%-30% of the requirements. CONCLUSIONS:Cancelling the drug addition and adjusting the types and quantity of essential medicine list had no obvious effects on the use amount ratios of gen-eral third-level grade-A hospitals. It is suggested to promote the priority use of essential medicines by systems of medical insurance total amount prepayment,single disease payment or diagnosis related groups-based prepayment and the free policy of essential medi-cines,rather than administrative order.
5.Influence of history of oral bisphosphonates on the incidence rate of fever after intra-venous injection of zoledronic acid in patients with osteoporosis

Objective:To investigate the prevalence of fever after intravenous injection of for the first time in osteoporotic patients who have or haven’t taken bisphosphonates orally.Methods:Bone mineral density measurement was conducted of the osteoporotic patients using dual-energy X-ray absorptiometry method,and according to the WHO diagnostic criteria for osteoporosis,the patient whose T score was less than -2.5 was defined as osteoporosis.The data of 113 osteoporotic patients who received intravenous injection of zoledronic acid in our hospital for the first time were used for analysis.Among them,35 who had the history of oral bisphosphonates were defined as group A,and 78 who didn’t have the history of oral bisphosphonates were defined as group B (control group).Moreover,we also compared the preva-lences of fever between the patients.Pearson chi-square test was used to investigate the association be-tween oral bisphosphonates therapy and prevalence of fever,and binary logistic regression was used to an-alyze the impact factors of fever after intravenous injection of zoledronic acid.Results:Among the 35 pa-tients who had the history of oral bisphosphonates,nobody had high fever,and 2 patients had low or mo-derate degree of fever (5.71%).Among the 78 patients who didn’t have the history of oral bisphospho-nates,33 patients had fever (42.3%),15 with low fever,and 14 with moderate degree of fever,and 4 with high fever.Pearson chi-square test showed that the prevalence of fever between group A and group B had significant difference,and that differences of prevalence of fever between the outpatients and the inpa-tients was not statistically significant.Binary Logistic regression analysis showed that oral bisphosphonates medication history was a protective factor of fever,and with the increasing of previous oral bisphosphonates for one month,there was a reduction of fever for 23.1%.Conclusion:Patients who have the history of oral nitrogenous bisphosphonates alendronate have a significant reduction in prevalence of fever after intravenous injection of zoledronic acid for the first time.That is,oral nitrogenous bisphosphonates alendronate history is a protective factor of fever for patients with intravenous injection of zoledronic acid for the first time.
6.Design of electronically controlled hydraulic damper for prosthetic knee

OBJECTIVE: To design an electronically controlled hydraulic damper cylinder as a key part of the computer-controlled intelligent knee joint.METHODS: According to the characteristics of hydraulic fluids and processing technology, new type of micro-structure of the digital throttle and unique structure of the two-cylinder piston had been applied to the damper. It could control extension and flexion of the knee joint in two-way automatically, which improved the gait symmetry of artificial limb. Finally, a special self-made testing device was used to evaluate the performance of the hydraulic damper. The damping force adjustment was simulated in test by controlling the two needle valves with micro-computer. The kinematics speed was detected under different state of simulating the driving moment of hip joint and the damping moment of knee joint. The testing results indirectly built the dynamics relationship among the inertial force, damping force and the swing speed of the knee joint. RESULTS: When the position value X of the needle valve was reduced by the step motor driving, the resistance of the hydraulic cylinder was increased which resulted in faster speed of the cylinder rod. The sensitivity of the speed to the driving force decreased with the opening of needle valve increasing. When the stepping motor controlled the needle valve closer, the damping moment increased which led to a faster swing speed.CONCLUSION: The design of hydraulic damper cylinder meets the requirements of prosthetic knee dynamics control.

Objective To determine the significance of renin angiotensin system (RAS) expressed on placenta in pregnancy induced hypertension (PIH). ObjectiveTo investigate the effect of Puerarin on expression of inductible nitric oxide synthase (iNOS) in RAW264.7 macrophage cells induced by lipopolysaccharide (LPS). MethodsThe protein and mRNA levels of lipopolysaccharide (LPS)-induced iNOS was determined in RAW264.7 macrophage cells using Western blotting and RT-PCR respectively. ResultsPuerarin inhibited expression of iNOS concentration dependently (P<0.01). ConclusionPuerarin may reduce iNOS level, result in prevention of inflammation.
9.Bone marrow-derived mesenchymal stem cells injection for liver failure in New Zealand white rabbits

BACKGROUND:Hepatocyte transplantation as an effective method for liver failure has been confirmed by animal models and clinical application.However,limited source and poor proliferation of hepatocyte graft limit its development.Studies have shown that bone marrow mesenchymal stem cells(MSCs)have potentials to differentiate into hepatocyte and bile epithelial celts,with strong proliferation.OBJECTIVE:To explore the therapeutic effect of bone marrow MSCs transplantation on liver failure of New Zealand white rabbits.METHODS:Adult male New Zealand rabbits were treated with D-galactosamine,and 3 mL hepatocyte suspension(1×109/L)was injected into the liver of transplantation group,but the control group was injected with the same volume of culture solution with no bone marrow MSCs.Alanine aminotransferase(ALT)and aspartate aminotransferase(AST)activity was detected 48,72 hours,1,4 weeks following transplantation,and pathological detection was performed at 4 weeks.RESULTS AND CONCLUSION:The liver functional index following transplantation of bone marrow-derived MSCs transplantation was significantly decreased,and ALT and AST activity at 4 weeks was significantly less than the control group(P < 0.05).At 4,the transplantation group displayed disorderly hepatic cord,hepatocyte swollen and degeneration,necrosis,accompanied by bleeding and inflammatory cell infiltration.In addition,the hepatic lobule structure was detectable,and regenerative hepatocyte increased among necrotic hepatocyte;small cells with large ratio of nucleus and cytoplasm at header,central vein and surrouding necrosis focus extended to the liver tissues.
10.Associated factors for HIV stigma among the patients on antiretroviral therapy in Wenshan Prefecture

Objective:
To learn HIV related stigma and its associated factors among the patients on antiretroviral therapy ( ART ) in Wenshan Prefecture, Yunnan Province, so as to provide evidence for eliminating HIV discrimination.
Methods:
A total of 419 subjects were recruited by convenience sampling from three ART clinics in Wenshan City and Maguan County between October 2017 and January 2018. HIV/AIDS Related Stigma and Discrimination Scale developed by Li Xianhong et al was employed. The multivariate linear regression model were used to explore the influencing factors for HIV stigma.
Results:
The median scores of disclosure concern, public rejection, family stigma, internalized stigma, health service providers' stigma were 24.00, 6.00, 10.00, 20.00, 2.00, respectively, and the overall was 68.00. The multivariate linear regression analysis showed that female patients ( standardized β=0.135 ) , patients with opportunistic infection ( standardized β=0.120 ), patients had no HIV infected family member ( standardized β=-0.128 ) , patients without family support ( standardized β=-0.175 ) , patients received gift from ART clinics ( standardized β=0.124 ) , patients scored lower in ART knowledge ( standardized β=-0.117 ) were likely to scored higher in HIV stigma.
Conclusions
The stigma on disclosure concern and internalized stigma dimensions are grievous among ART patients in Wenshan Prefecture. Gender, opportunistic infection, HIV infection in family, family support, receiving incentive gifts from clinics and awareness of ART are associated with HIV stigma.
