1.Application of standard swallowing function assessment scale on extubation time for patients with nasal feeding

Objective To explore the effect of the standardized swallowing assessment (SSA) on extubation time for patients with nasal feeding. Methods One hundred patients with nasal feeding in our department from January 2013 to December 2014 were divided into the observation group and control group randomly with 50 in each group. The control group was treated with education on aspiratioin prevention and the right time for extubation was decided following doctor′s order. SSA was used in the observation group with weekly SSA and swallowing function, then the patients took pertinent functional exercises till a right time for extubation was decided. The two groups were compared in terms of incidences of re-intubation and aspiration pneumonia. Result The observation group had significantly lower incidences of re-intubation and aspiration pneumonia than the control one (P<0.01). Conclusions SSA is used to assess the swallowing function as well as the aspiration risks , which helps patients to take functional exercise and helps nurses to decide the right time for extubation. It can effectively lower the incidences of re-intubation and aspiration pneumonia and improve patients′prognosis.
2.Expression of CD_64 in Neonatal Infection Disease and Its Clinical Significance

Objective To study CD64 expression in neutrophilic granulocyte and its clinical effect in neonatal infection disease. Methods CD64 was detected among 59 neonatal patients(septicemia group 34 patients, local infection group 25 patients)hospitalized in our neonatal department diagnosed as neonatal infection disease in 48 h after hospitalized,2 weeks after therapy, then the results were compared with 27 patients as non - infection disease during the same period. Results CD64 in septicemia group was (6156. 21?3643. 32) molecula per cell,in local infection group was (2176.19 ? 946. 32)molecula per cell, in non- infection group was (2176. 19 ? 946. 32) molecula per cell.There were significant differences among three groups (all P0.05). Conclusions CD64 expression increases in bacterium infection disease. It is more obvious in widespread infection desease.and it can be the mark in early diagnosis of neonatal infection disease.

Through study on Bianque Shenying Zhenjiu Yulong Jing (Bianque's Classic of Effective Acupuncture Prescriptions), acupuncture experiences and ideology of WANG Guo-rui, an acupuncture physician of the Yuan Dynasty (1271-1368), is discussed in this article. Inherited and carried forward DOU's acupuncture theory, WANG Guo-rui encouraged to use experiential points and to expand extra points. Penetrating needling method was created, and the application of acupoints according to needling opportunities and seasonal factors were enriched by him. Great importance was also attached on treatment according to differentiation of syndromes, which including the simultaneous acupuncture and moxibustion, simultaneous reinforcing and reducing, reducing after reinforcing, reinforcing after reducing, predomination of reducing and predomination of reinforcing. The flexible treating methods are still extensively adopted in clinic nowadays. WANG Guo-rui is a master of clinical acupuncture who made great contribution to the development of acupuncture and moxibustion.

4.Effect of ulinastatin on oxidative stress injury to myocardial ceils in diabetic rats in vitro

Objective To evaluate the effect of ulinastatin on oxidative stress injury to myocardial cells in diabetic rats in vitro.Methods The H9c2 cells were cultured in DMEM culture medium and the cells at the logarithmic growth phase were seeded in 96-well plates (density 1 × 104 cells/ml,200 μl/well) or in 6-well plates (density 1× 105 cells/m1,2 ml/well).The cells were randomly divided into 4 groups (n=18 each) using a random number table:normal control group (group C),high-glucose group (group HG),high-glucose + oxidative stress group (group HG+OS),ulinastatin +high-glucose+oxidative stress group (group U+HG+OS).The cells were cultured in high-glucose DMEM culture medium (25.0 mmol/L) for 48 h in group HG.After the cells were cultured in high-glucose DMEM culture medium for 24 h,H2O2 with the final concentration of 500 μmol/L was added to the high-glucose culture medium,and the cells were continuously cultured for 24 h in HG+OS and U+HG+OS groups.In group U+HG+OS,ulinastatin 400 U/ml was added to the high-glucose culture medium.The cells were collected for determination of cell viability,H9c2 apoptosis,activity of superoxide dismutase (SOD) and contents of malonadehyde (MDA).Apoptosis rate was calculated.The cell culture supernatant was collected for detection of lactate dehydrogenase (LDH) activity.Results Compared with group C,the cell viability and SOD activity were significantly decreased,and the apoptosis rate,MDA content and LDH activity were increased in the other groups.Compared with HG group,the cell viability and SOD activity were significantly decreased,and the apoptosis rate,MDA content and LDH activity were increased in HG+OS and U+HG+OS groups.Compared with group HG+OS,the cell viability and SOD activity were significantly increased,and the apoptosis rate,MDA content and LDH activity were decreased in group U + HG+ OS.Conclusion Ulinastatin can mitigate oxidative stress injury to myocardial cells in diabetic rats,and inhibited cell apoptosis is involved in the mechanism.

8.Effect of pretreatment with dexmedetomidine alone or in combination with sufentanil on myocardial ischemia-reperfusion injury in rats

Objective To investigate the effect of pretreatment with dexmedetomidine alone or in combination with sufentanil on myocardial ischemia-reperfusion (I/R) injury in rats.Methods Fifty healthy male Sprague-Dawley rats,weighing 250-300 g,were anesthetized with intraperitoneal pentobarbital sodium 60 mg/kg.Myocardial I/R was induced by occlusion of anterior descending branch of left coronary artery for 30 min followed by 120 min of reperfusion.The rats were then randomly divided into 5 groups (n =10 each):sham operation group (group S),group I/R,dexmedetomidine pretreatment group (group DP),sufentanil pretreatment group (group SP),and dexmedetomidine + sufentanil pretreatment group (group DS).In group S the anterior descending branch was only exposed but not ligated.Dexmedetomidine 0.5μg/kg and sufentanil 0.1μg/kg were injected intraperitoneally 30 min before ischemia in groups DP and SP,respectively.Dexmedetomidine 0.5 μg/kg and sufentanil 0.1 μg/kg were injected intraperitoneally 30 min bbefore ischemia in group DS.Arterial blood samples were collected at 120 min of reperfusion for determination of serum creatine kinase (CK) and lactic dehydrogenase (LDH)concentrations.The rats were sacrificed at 120 min of reperfusion and hearts were removed for microscopic examination.Myocardial infarct size was calculated.The malondialdehyde (MDA) content and superoxide dismutase (SOD) activity in myocardial tissues were measured.Results Compared with group S,the serum CK and LDH concentrations were significantly increased,the myocardial infarct size was enlarged,and SOD activity was decreased in the other groups,MDA content was significantly increased in groups I/R,DP and SP (P < 0.05 or 0.01).Compared with group I/R,the serum CK and LDH concentrations,MDA content and myocardial infarct size were significantly decreased,and SOD activity was increased in groups DP,SP and DS (P < 0.05).Compared with group DS,the serum CK concentration was significantly increased,the myocardial infarct size was enlarged,and MDA content was increased in groups DP and SP,and LDH concentration was significantly increased and SOD activity was decreased in group DP (P < 0.05).The pathological changes were significantly attenuated in groups DP and SP compared with group DS.Conclusion Dexmedetomidine pretreatment can reduce myocardial I/ R injury in rats,dexmedetomidine combined with sufentanil pretreatment provides better efficacy than either alone,and inhibition of lipid peroxidation is involved in the mechanism.
9.Effect of fresnel press-on prisms combined with mental image network training on binocular visual function of the postoperative concomitant strabismus

AIM: To discuss the effect of fresnel press-on prisms combined with mental image network training on binocular visual function recovery of the postoperative concomitant strabismus(CS).
METHODS: Totally 120 postoperative children with CS were selected from January 2015 to January 2018 in our hospital. According to the random digital table method, they were divided into the combination group and the prism group, 60 cases in each group, the prism group was given fresnel press-on prisms treatment, the combination group was given mental image network training on the basis, the visual function recovery of the two groups was compared.
RESULTS: After 6mo treatment, binocular visual machine visual function and Titmus near stereopsis visual function in both groups were significantly higher than before, and the binocular visual machine visual function and Titmus near stereopsis visual function in the combination group were significantly higher than those in the single press group, the difference was statistically significant(all P<0.05). The long distance fusion function normal rates in the combination group and the prism group were significantly higher than those before treatment, the long distance fusion function normal rate in the combination group was significantly higher than that in the prism press group, the difference was statistically significant(93.3% vs 70.0%, P<0.05).
CONCLUSION: Fresnel press-on prisms combined with mental image network training can effectively promote the binocular visual function recovery of the postoperative children with CS, which is worth for further clinical promotion.
10.Protective effect of intravitreal injection of antioxidant NAC on retina in early diabetic rats

AIM: To investigate the protective effect of the antioxidant N-acetylcysteine(NAC)on retina in early diabetic rats.
METHODS: Thirty healthy adult male SD rats were randomly assigned to the normal control group(CON group, n=10)and the diabetes group(DM group, n=20). After fasting for 12h, the DM group was injected with 1% streptozotocin(STZ)solution, according to 60mg/kg disposable left lower abdominal injection. After 72h, blood was taken from the rat tail vein to detect blood glucose, diabetic model animals were defined as ≥16.7mmol/L. Model rats were randomly divided into diabetes control group(group D)and NAC treatment group(group N). After the model was established, N group of rats were injected with 4μL 1.6μg/μL NAC through the vitreous cavity every week. Rats in CON group and D group were injected with 4μL 0.01mmol/L phosphate buffer saline. All the rats no diet water, group feeding. Body mass and blood glucose were recorded weekly. After the diabetes was modeled, 2mo killed the experimental animals. The thickness of the inner layer of the retina of rats in each group was determined by HE staining. The number of retinal ganglion cells and the level of pigment epithelial derived factor in the retina were measured by immunofluorescence.
RESULTS: The thickness of retinal kernel layer increased in group N compared with group D(P<0.01), and there was no difference between group CON and group(P>0.05). Compared with CON group, the number of retinal ganglion cells decreased in group D(P<0.01), and decreased slightly in group N(P>0.05). Retinal ganglion cells decreased in group D compared with group N(P<0.01). Compared with CON group, PEDF expression decreased in group D(P<0.01), and decreased slightly in group N(P>0.05). The expression of PEDF in group D decreased compared with group N(P<0.01).
CONCLUSION: The protective effect of antioxidant NAC on retinal tissue in early diabetic rats may be due to the up-regulation of PEDF levels in the retina.
