2.Clinical analysis of 49 cases with Pseudo-Meigs' Syndrome: a report of two cases and a review of literatures over the last decade

Retrospective analyses were conducted for the clinical data of two cases with pseudoMeigs' syndrome at our hospital.Both had respiratory symptoms,such as cough and dyspnea.Radiological examinations revealed ascites,pleural effusion and pelvic mass.The definite pathological diagnosis was pelvic malignant tumor.After surgical tumor removal,ascites and pleural effusion disappeared without recurrence.Along with reviewed cases from PubMed in the last decade,a total of 49 cases had pseudoMeigs' syndrome.The age range was 11-73 years.Their clinical manifestations include dyspnea (78%),abdominal distension (69%),cough (14%),abdominal pain (12%),fatigue (10%),weight loss (6%) chest pain (4%),fever (4%),abdominal mass (4%),and oliguria (2%).CA125 was commonly elevated.The primary tumors in pelvic cavity accounted for 78%.And ovarian metastasis from colon cancer was one of the most common in metastatic tumor.
3.Volt-ampere characteristics of Yuan points of three yin channels of hand in women before, during and after menstruation

OBJECTIVE: To observe the changes of volt-ampere characteristics of Yuan-Primary acupoints in women before, during and after menstruation, and to find out the relationships between Qi-blood in human body and volt-ampere characteristics of acupoints, so as to develop a new method for quantitative analysis on changes of Qi-blood. METHODS: A high-sensitive detection system of volt-ampere characteristics of acupoint was applied to detecting the volt-ampere characteristics of Taiyuan (LU 9), Daling (PC 7) and Shenmen (HT 7) in healthy women before, during and after menstruation. RESULTS: The resistances on LU 9, PC 7 and HT 7 with some certain scan currents were quite different among the women before, during and after menstruation. For such changes, PC 7 was of the most obvious change, LU 9 was of the less and HT 7 was of the least. The total ratio of the scanning spots with significantly increasing changes of resistances on these three acupoints along with the menstrual process was 70.6%. CONCLUSION: The volt-ampere characteristics of acupoints vary with the menstrual process, which is of the acupoint specificity. The resistances on the acupoints increase with the menstrual process, and such results may be due to the blood loss during menstruation. The volt-ampere characteristics can be used as a quantitative index to study on the change of Qi and blood.
4.Expression and clinical application of the mRNA of PI3K and Akt mRNA in leukemia

Objective To study the expression of PI3K and Akt mRNA in acute leukemia(AL) and chronic myelocytic leukemia (CML) and try to find out a relationship between the prognostic significance of acute leukemia and the expression level of PI3K and Akt. Methods The samples were collected from 63 ALpatients, 7 relapsed AL patients, 14 CML patients and 10 AL patients in complete remission(CR) patients, 11samples of normal controls(NC). The expression of PI3K, Akt mRNA were measured by semi-quantity reverse transcription polymerase chain reaction(RT-PCR). Results The expression of PI3K mRNA in AL and relapse group were significantly higher than that in normal control. In CML, the P13K mRNA expression level was higher than that in NC and CR. The similar results of the expression of Akt mRNA were observed in CML, NC and CR. At the same time, the expression level of Akt mRNA in relapse group was significantly higher than that in NC. The complete remission rate in PI3K (+) was lower than that in PI3K (-). Similar result was obtained in Akt (+) and Akt (-). Conclusion The PI3K/Akt pathway may contribute to the occurrence of leukemia. The patients with positive expression of the members of this pathway had a lower remission rate.
5.Outcome of autologous peripheral blood stem cell transplantation for T cell lymphoma

Objective A retrospective analysis of patients with T-cell lymphoma (TCL) received autologous peripheral blood stem cell transplantation (APBSCT) was performed to evaluate the outcome of APBSCT.Methods A total of 22 patients who underwent APBSCT from September 2006 to December 2011 in Ruijin hospital were enrolled in the study,including 6 cases of lymphoblastic lymphoma and 16 of peripheral T-cell lymphoma (8 anaplastic large cell lymphoma, 4 PTCL-u, 1 subcutaneous panniculitis-like T-cell lymphoma, 2 nasal type extranodal NK/T and 1 primary cutaneous T-cell lymphoma). All patients were diagnosed based on the WHO Classification of Tumors of Hematopoietic and Lymphoid Tissues. Conditioning regimens were high-dose chemotherapies alone which include 13 cases with BEAM, 4 with ICE and 5 with CBV. The outcomes of the treatment were evaluated according to the revised International Working Group criteria.Results With a median follow-up of 13.1(1-60) months,the predicted 2-year overall survival (OS) and progression-free survival (PFS) after transplantation were (67.6±11.0) % and (71.1±11.1) %,respectively.A total of 6 patients experienced disease progression and 5 patients eventually died of disease. When all these patients based on the remission status before APBSCT (CR1 vs non-CR1) and chemosensitivity (sensitive vs refractory) were further classified, the PFS rates and OS rates were 100 % and 91.7 % respectively in CR1 or chemosensitive patients which were significantly higher than patients not in CR1 (42.6 % ) or with chemoresistant disease (19.0 % ). Conclusion Remission status and chemosensitivity at the time of transplantation significantly affect the outcome of APBSCT for TCL patients, thus it can be recommend to perform APBSCT for patients either in CR1 or early stage when the disease remain sensitive to chemotherapy.
6.Application of superimposed flaps in hand surgery

Objective To probe the application of superimposed flaps in hand surgery. Methods Form March 1998 to December 2003, we repaired 35 cases of skin defects with non trunk blood flaps and neurocutaneous vascular flaps. The area of flaps ranged from 1cm?1cm to 6cm?8cm. Results After operation, the flaps in 33 cases survived, but died in 2 cases due to unsuitable design. The follow ups ranging from 6 to 12 months showed that the quality and color of the survived flaps were similar to those of the normal tissue around. No swelling was found. Conclusion The superimposed flaps have reliable blood supply so that they are an ideal means to repair skin defects of hand.
7.Study on clinical and pathology of children with gastrointestinal bleeding

Objective To evaluate the etiologies of children with gastrointestinal bleeding(GIB), and the relationship between the pathological findings and clinical.Methods Gastroscopy or proctoscope was performed in 153 children with GIB. Pathological studies and tests as for helicobactor pylori (Hp) were undertaken.Results Among 153 children,140 cases(91.5 %) had definite diagnosis,including 74 children with upper gastrointestinal bleeding(UGIB) and 66 cases with lower gastrointestinal bleeding(LGIB). Gastric pathologic study was conducted in 56 cases. All patients had chronic superficial gastritis(CSFG). Hp test was positive in 33 patients. ObjectiveTo explore the prevalence and influencing factors of restless legs syndrome (RLS) in elderly maintenance hemodialysis (MHD) patients, to analyze the relationship between RLS and sleep quality, and to observe the efficacy and safety of hemoperfusion in improving RLS and sleep quality in elderly MHD patients with RLS. MethodsA total of 118 cases with maintenance bemodialysis were involved in this study. The RLS was diagnosed according to the criteria of the International Restless Legs Syndrome Study Group (IRLSSG). The subjects' quality of sleep was assessed by the Pittsburgh Sleep Quality Index (PSQI). The elderly MHD patients with RLS received hemoperfusion once a week for 3 months. The clinical and biochemical indexes were collected including age, gender, duration of hemodialysis, pre-dialysis blood concentrations of creatinine, hemoglobin, parathyroid hormone, β2-microglobulin, etc. T test, X2 test and binary logistic regression analysis were performed. ResultsRLS prevalence was found in 26.27% (11 men, 20 women, the ratio was 0. 55 : 1) of 118 elderly MHD patients. The duration of hemodialysis,sex ratio (female/male), the level of serum phosphorus, β2-microglobulin and parathyroid hormone in elderly MHD patients with RLS were significantly higher than those in elderly MHD patients withoutRLS (t=2. 332,X2=15. 343,t=4. 784,13. 124,10. 900;all P<0. 05). And there were no significant differences in age, dry weight, material of dialyzer membrane, and levels of serum creatinine, urea nitrogen, hemoglobin, hematocrit, calcium, ferritin and transferrin saturation between patients with RLS and without RLS(t=0. 341,0. 008, 1. 055,0. 051,0. 899,0. 912,0. 601,0. 192,0. 200,1. 094;all P>0.05). Logistic regression analysis revealed that serum parathyroid hormone and β2-microglobulinwere the important risk factors for RLS in elderly MHD patients. PSQI was higher in RLS group than in non-RLS group[(10. 7±2.7) vs. (4.9±2.5), t=10. 948,P=0. 000] . The manifestations of poor sleep were poor indexes of subjective sleep quality, sleep duration, habitual sleep efficiency and daytime dysfunction in MHD patients with RLS. And the RLS severity was correlated with PSQI (r=0. 839,P<0. 05). After the treatment with hemoperfusion, the level of serum phosphorus, serum parathyroid hormone, serum β2-microglobulin, RLS severity score and PSQI were significantly decreased in RLS patients compared with pre-hemoperfusion. The vital sign of RLS patients were stable during hemoperfusion, and there were no adverse events after the treatment with hemoperfusion. ConclusionsThe prevalence of RLS is high, and poor sleep quality is found in elderly MHD patients with RLS. Serum parathyroid hormone and β2-microglobulin are the important risk factors for RLS in elderly MHD patients. Hemoperfusion can effectively improve the RLS and sleep quality and is well tolerated.
9.Clinical efficacy observation of acupoint thread-embedding in treating obese patients with food addiction

Objective: To observe the effects of acupoint thread-embedding therapy and low-carbohydrate diet therapy on obese patients with food addiction. Methods: Sixty-five eligible patients were randomized into a thread-embedding group of 33 cases and a diet group of 32 cases to respectively receive 12-week treatment. Before treatment, after treatment and at 6-month follow-up, the two groups were observed and compared in terms of body mass (BM), waist circumference (WC), hip circumference (HC), waist-to-hip ratio (WHR), body mass index (BMI), body fat rate (BFR), basal metabolic rate (BMR) and Yale food addiction scale version 2.0 (YFAS 2.0). Results: At the end of treatment, there were no significant differences in the general efficacy, and the improvements in BM, BMI, WC, HC, WHR and BFR between the thread-embedding group and diet group (all P>0.05). At follow-up, the thread-embedding group showed more significant improvements in all the aforementioned indicators compared with the diet group except HC (all P<0.05). At the end of treatment and follow-up, BMR and YFSA 2.0 had more significant improvements in the thread-embedding group than in the diet group (all P<0.05). Conclusion: Acupoint thread-embedding therapy can produce significant efficacy in treating obese patients with food addiction; it can improve the food addiction state and work better in maintaining the efficacy compared with low-carbohydrate diet therapy.
10.Effect of therapeutic hypercapnia on type Ⅰ alveolar cells in transplanted lung in rats

Objective To investigate the effects of therapeutic hypercapnia on type Ⅱ alveolar cells (ATⅡ ) in the transplanted lung in rats.Methods Twenty-eight pathogen free adult male Wistar rats weighing 180-220 g were randomly divided into 2 groups (n= 14 each) : control group (group C) and therapeutic hypercapnia group (group T). The animals were anesthetized with intraperitoneal 3% pentobarbital 30 mg/kg, tracheostomized and mechanically ventilated with 50% O2-50% N2 (VT 10 ml/kg, RR 60 bpm, I∶ E1∶1). Left lung transplantation was performed. In group T starting from the beginning of reperiusion of the transplanted lung, the animals were ventilated with a mixture of 50% O2-N2 and C02(in appropriate concentrations) to keep PaCO2 between 80-100 mm Hg. After 90 min of reperfusion of the transplanted lung, blood samples were collected from pulmonary vein of the transplanted lung and blood gas analysis was performed. Oxygenation index was calculated.AT II cells were isolated from the transplanted lung and purified and examined with electronic microscope. The apoptosis rate of AT Ⅱ cells was detected by flow cytometry. Results Oxygenation index was significantly higher, the apoptotic rate of ATⅡ cells lower, the damage to ATⅡ cells was less in group T than in group C.Conclusion Therapeutic hypercapnia can protect the AT Ⅱ cells in the transplanted lung and improve the function of the trans planted lung.
