1.Clinical Progress in Platinum-based Chemotherapy for Esophageal Cancer

The esophagus carcinoma is one of the most common cancers in China. The therapy modes for esopha-gus carcinoma include surgery, radiotherapy and chemotherapy. Platinum-based combined chemotherapy has always been used in the treatment for esophagus carcinoma. In this article, we reviewed the application of platinum-based chemo-therapy in esophageal cancer in recent years. The efficacy of cisplatin in treating esophageal cancer has been proved. Cis-platin is used in neoadjuvant chemotherapy, adjuvant chemotherapy, concurrent chemo-radiotherapy and salvage chemo-therapy for esophageal cancer. Combined chemotherapy with 5-fluorouracil (5-FU) and cisplatin is regarded as the stan-dard regimen for esophageal cancer in concurrent chemoradiotherapy and salvage chemotherapy. Compared with cisplat-in, carboplatin causes lower rate of nephrotoxicity, reactions in the digestive tract and ototoxicity. But carboplatin does not have better effect. Patients with esopohageal adenocarcinoma show good clinical response to oxaliplatin and relatively sat-isfactory median survival. The rate of nephrotoxicity and reactions of digestive tract caused by cisplatin is lower. Oxaliplatin can lead to serious neurotoxicity. The therapeutic efficacy of nedaplatin is as good as that of cisplatin. Nedaplatin is used in concurrent chemoradiotherapy and salvage chemotherapy for esophageal squamous cell carcinoma. Compared with cispla-tin, nedaplatin can result in myelosuppression. Further research is warranted to explore whether nedaplatin can take place of cisplatin as the standard regimen. Lobaplatin combined with 5-fluorouracil (5-Fu) and leucovorin (CF) is effective and well tolerated for advanced esophagus carcinoma. The major toxicities noted are reversible bone marrow suppression and gastrointestinal tract reaction.
2.Bifunctional law in acupuncture theory.

Taking the basic theory of Chinese medicine as the instruction, through the bidirectional features of clinical manifestations, the relative etiology and pathology, acupoints and treatments, the bifunctional law in acupuncture theory is discussed. And it is held that after the confirmation of bidirectional symptoms of some diseases or syndromes, acupoints which have special functions in the bifunctional theory of acupuncture with a narrow meaning can be taken as major acupoints. And at the same time, other proper acupoints and flexible treating methods can be selected under the instruction of the bifunctional law of the broad meaning, so as to get the best therapeutic effect.

3.Observation of Clinical Efficacy of Puerarin Combined with β-aescine Sodium in the Treatment of Patients with Postoperative Edema of Hand Trauma

OBJECTIVE:To observe the clinical efficacy of puerarin combined with β-aescine sodium in the treatment of pa-tients with postoperative edema of hand trauma. METHODS:118 patients with postoperative edema of hand trauma were random-ly divided into observation group (n=59) and control group (n=59) by number. Both groups were taken infrared physiotherapy and appropriate physical treatment such as hand functional exercise;patients in control group was added β-aescine sodium 10 mg based on the physical treatment,iv,once a day;observation group was added puerarin 200 mg based on the treatment of control group,iv,once a day. Both groups were taken 10 days for one course of treatment. Compared the swelling scores,perimeter of suf-fered limb and edema scores,clinical efficacy and adverse reactions of patients between the two groups before and after treatment. RESULTS:The symptoms of hand limb swelling in 2 groups were obviously improved(P<0.01 or P<0.05),however,observa-tion group was better than that of control group,with significant difference(P<0.05);after treatment,the swelling,perimeter of suffered limb and swelling score in observation group were lower than control group,with significant differences(P<0.05);the to-tal effective rate in observation group was 94.91% and control group was 83.05%,with significant difference(P<0.05). No ADR was found in 2 groups. CONCLUSIONS:Puerarin combination with β-aescine sodium has obvious efficacy and safety in the treat-ment of postoperative edema of hand trauma.
4.Analysis of international discharge planning guidelines

Objective To provide a theoretical reference for building a discharge planning program suitable for China's national conditions based on a comprehensive content analysis of international discharge planning guidelines.Methods Discharge planning guidelines from 1995 to 2012 which were retrieved from PubMed,NGS (National Guideline Clearinghouse),google were analyzed by 5W1H method.Results A total of 11 discharge planning guidelines were collected from 21 through the full text screening.Conclusions This study shows that discharge planning should focus on patients with their needs as orientation,to provide continuous care through interdisciplinary and interagency coordination and cooperation,and an effective planning should be implemented standardized management and carried out early on admission.
5.Research progress in bone marrow mesenchymal stem cell in ophthalmology

Bone marrow mesenchymal stem cell (BMSC) is one kind of the adult stem cells.BMSCs have a strong capacity of self-renewal,the ability to differentiation into different cell types and low immunogenicity.The cells have been successfully differentiated into osteoblast,cartilage cells,adipose cells,cardiac muscle cells and vascular endothelial cells and have a bright future in the field of tissue engineering and cell transplantation.Resent years,BMSCs become the hot spot in ophthalmology due to their advantages.This paper summarizes the effects of BMSCs in the research of ophthalmology.

7.Analyzing of Depression and Nursing Strategy in Middle-aged Patients with Stroke

Objective To investigate the depression after stroke in middle-aged patients and discuss the mental nursing.Methods Self-Rating Depression Scale was used to find 30 depression patients from 52 patients with stroke.They were analyzed and accepted mental nursing.Results The rate of depression mental state patients in 52 patients is 57.69%.After accepting mental nursing,the number of unsociability,morose and fatigue decreased,while that of role adaptation and self-confident increased significantly(P<0.05).Conclusion There is high incidence of depression in middle-aged patients with stroke.Mental nursing is helpful improve their adaptation and quality of living.
9.Clinical Features of Cord and Artery Blood Gas Analysis in Neonate with Acid-Bas eImbalance

Objective To discuss clinical features and harmfulness of acid-base imbalance in newborn by investigating the changes in blood gas of cord and artery blood,and to establish more appropriate prevention measures. Methods One hundred and ninety-two cord blood and 113 peripheral artery blood of neonates were measured for blood gas analysis by automatic blood gas analysis instrument from America;meanwhile,the concentrations of K~+,Na~+ and Cl~- were detected by biochemical analyzer.Results Among 192 neonates,179 were normal labor and 13 neonates underwent acidosis of different degrees.The incidence was 79.9%.Among the 13 ca-(ses),2 cases had severe asphyxia and both of them got serious mixed acidosis.For all the neonates, 156 cases got acidosis, 84 neonates underwent respiratory acidosis (43.8%), 47 cases with metabolic acidosis (24.5%) and 25 cases with mixed acidosis (13.0%).The left 113 neonates were abnormal labor.Among them,99 cases had acid-base disturbance of different degrees (87.6%). Among the 99 acid-base disturbance cases, 50 cases were single metabolic acidosis (44.2%) and presented the most common changes; the second was single respiratory acidosis, totally 20 cases (17.7%).The third was metabolic acidosis combined with respiratory alkalosis, totally 16 neonates (14.2%).Eleven cases had mixed acidosis (9.7%) and 2 cases were respiratory alkalosis combined with metabolic alkalosis (1.8%).Conclusions While the normal labor neonates still got acidosis of different degrees,the acid-base imbalance of abnormal labor is much more prominent,and the critical neonates often got mixed acid-base imbalance.Serious acid-base imbalance can cause fatal pathological changes in body.The dangers of acidosis and alkalosis can not ignore either. The condition of acid-base imbalance can be judged more accurately by the combination of blood gas analysis and anion gap.The perinatal health cares are important measures to prevent acid-base imbalance.
10. Preparation of orally disintegrating tablets of scopolamine hydrobromide

Objective: To optimize the formula and the preparation technique of orally disintegrating tablets of scopolamine hydrobromide(SH). Methods: The wet granule compressing method and the direct compressing method were compared to choose the suitable preparation method. The orthogonal design was used to optimize the formula of SH tablets while taking the hardness, disintegrating time and dissolution rate of the tablets as indices, and the optimization was verified. Results: The wet granule compressing method was chosen for preparation. The optimized formula was composed of mannitol 40%, MCC 35%, lactose 0, and CMS-Na 10%. And the hardness, disintegrating time and T50 of the optimized tablets were 3.8 kg, 54 s, and 2.47 min, respectively. Conclusion: The obtained formula and preparation technique for orally disintegrating tablets of SH is satisfactory; the quality of the prepared tablets can be well controlled.
