2.Clinical effect of laparoscopic ovarian endometriosis cyst resection and the influence of different operative methods on follicle loss

Objective To analyze the clinical effect of laparoscopic ovarian endometriosis (EMs) cyst resection and the influence of different operative methods on follicle loss.Methods Selected 86 EMs patients with laparoscopic ovarian cyst resection in our hospital from January 2013 to January 2016 and randomly divided them into two groups.The control group adopted traditional blunt tearing method,while the observation group adopted sharp isolating method.The specimen thickness along with the number,thickness and grading of specimen with ovarian cortex at hilus orarii,middle part of ovarian fossa and ovarian fossa were observed.Meanwhile,the influence on ovarian function and follicle loss of the two operation methods were compared.Results The specimen thickness close to the hilus orarii was more than that of the ovarian fossa and the middle part of ovarian fossa (t =4.474,t =3.520,P < 0.001).The thickness of specimen with ovarian cortex at ovarian fossa was significantly less than that of the middle part of ovarian fossa and the hilus orarii (t =1.705,t =4.428,P =0.046,P =0.000).The number of specimen ovarian cortex grading of 3 to 4 at hilus orarii was 20,which was significantly more than 2 at the ovarian fossa and 2 at the middle part(x2 =19.790,x2 =19.790,P < 0.001).The specimen thickness and ovarian cortical thickness of the middle part of the observation group were less than those of the control group (t =3.897,t =2.166,P =0.000,P =0.017).The specimen number of ovarian cortex at the hilus orarii was significantly less than the control group(x2 =5.512,P =0.019),and the specimen thickness and the ovarian cortex thickness were obviously less than those of the control group (t =4.992,t =2.349,P =0.000,P =0.011).After operation,the differences between FSH,LH,and E2 levels in the two groups were not statistically significant(P > 0.05),but the ovarian sinus follicle number and ovarian volume of the observation group were significantly more than those of the control group (t =7.181,t =10.921,P < 0.00 001).The pregnancy rate of the observation group was 60.0%,which was significantly higher than 43.75 % of the control group (x2 =5.289,P =0.021).Conclusion The laparoscopic ovarian EMs cyst resection can cause the great loss of follicle in the progress of resecting cyst at hilus orarii,and advanced sharp separation can obviously reduce the loss of normal ovarian follicles and cause smaller influence on ovarian function.
3.Clinical trial of magnesium sulfate injection combined with labetalol injection and nimodipine tablets in the treatment of pregnancy-induced hypertension

Objective To observe the clinical efficacy and safety of magnesium sulfate injection combined with labetalol injection and nimodipine tablets in the treatment of pregnancy-induced hypertension (PIH).Methods A total of 74 patients with PIH were randomly divided into control and treatment groups with 37 cases per group.Control group received 25% magnesium sulfate injection 20 mL each time,bid,intravenous drip + labetalol injection 50 mg each time,qd,intravenous drip;on the basis of the treatment of control group,the treatment group received nimodipine tablets 30mg each time,qd,orally.All patients were treated for 7 days.The clinical effect,changes of serum levels of cystatin C (Cys-C),blood pressure indexes,placental hemodynamic parameters,the pregnancy outcomes,and the occurrence of adverse drug reactions were compared between two groups.Results After treatment,the total effective rates of treatment and control groups were 94.59% (35 cases/37 cases) and 86.49% (32 cases/77 cases) with significant difference (P < O.05).After treatment,the main indexes in treatment and control groups were compared:the serum levels of Cys-C were (0.87 ±0.10) and (1.03 ±0.13)mg · L-1,cesarean section rates were 24.32% (9 cases/37 cases) and 54.05% (20 cases/37 cases),premature birth rates were 5.41% (2 cases/37 cases) and 21.62% (8 cases/37 cases),the differences were statistically significant (all P < 0.05).The adverse drug reactions of two groups were dizziness,hypotension and gastrointestinal discomfort,and the total incidence of adverse reactions of treatment group and control group were 13.51% and 16.22% without significant difference (P > 0.05).Conclusion Magnesium sulfate injection combined with labetalol injection and nimodipine tablets has a definitive clinical efficacy in the treatment of PIH,and also has positive significance for pregnancy outcome,which may be related to the regulation of Cys-C,without increasing the incidence of adverse drug reactions.
4.Treatment of Mild-to-moderate Bronchial Asthma (Heat Wheezing Syndrome) Patients at Acute Onset with Western Medicine Combined Danlong Oral Liquid: a Multi-center, Randomized Double- blinded, Positive-controlled Clinical Trial.

OBJECTIVETo observe the efficacy and safety of Danlong Oral Liquid (DOL) combined Western medicine (WM) in treating mild-to-moderate bronchial asthma patients (heat wheezing syndrome) at acute onset.
METHODSTotally 480 mild-to-moderate bronchial asthma patients (heat wheezing syndrome) at acute onset were randomly assigned to two groups in the ratio 3:1, the treatment group (360 cases) and the control group (120 cases). All patients received basic WM treatment. Patients in the treatment group took DOL, 10 mL each time, 3 times per day for 7 days in total, while those in the control group took Kechuanning Oral Liquid (KOL) , 10 mL each time, 3 times per day for 7 days in total. Efficacy for asthma symptoms, lung functions and scores of TCM syndrome and/or main symptoms were evaluated.
RESULTSThe percentage of clinical control and significant effectiveness of asthma symptoms in the treatment group was significantly higher than that of the control group (77.36% vs 56.07%, P < 0.01). The percentage of clinical control and significant effectiveness of lung functions in the treatment group was significantly higher than that of the control group (74.28% vs 50.00%, P < 0.01). The anterior-posterior difference in scores of TCM syndrome was significantly superior in the treatment group than in the control group (-11.26 ± 4.70 vs -9.21 ± 5.09, P < 0.01). The anterior-posterior difference in scores of main symptoms was significantly better in the treatment group than in the control group (-6.58 ± 3.08 vs -5.16 ± 3.45, P < 0.01). The incidence of adverse reactions was significantly lower in the treatment group than in the control group [1.73% (6/346 cases) vs 10.17% (12/118 cases) , P < 0.05].
CONCLUSIONDOL combined WM was superior to KOL in treating mild-to-moderate bronchial asthma patients (heat wheezing syndrome) at acute onset.

5.Transcriptional repression of hDaxx enhanced by adenovirus 12 E1B 55-kDa oncoprotein interacting with hDaxx.

BACKGROUNDDaxx has been identified as a nuclear protein that involves in apoptosis and transcriptional repression. Daxx co-localizes with the promyelocytic leukemia (PML) protein and regulates transcription. Human Daxx (hDaxx) is a protein that functions as a transcriptional regulation through its interaction with some DNA-associated proteins. The aim of this study was to explore the transcriptional regulatory effect of hDaxx interacting with adenovirus (Ad) 12 E1B (Ad12E1B) 55-kDa oncoprotein.
METHODSThe co-localization of hDaxx-Ad12E1B or hDaxx-PML protein in the nucleus was observed under a confocal microscope. Interaction of hDaxx and Ad12E1B was analyzed by yeast two-hybrid assay. Direct binding of hDaxx and Ad12E1B was analyzed using coimmunoprecipitation and Western blot in vivo and in vitro. The activity of a luciferase reporter gene, which was regulated by an hDaxx modulated thymidine kinase (TK) promoter, was detected in an automat luminometer.
RESULTSAd12E1B, which co-localized with hDaxx in the nuclei of G401-CC3 cells, disrupted the co-localization of hDaxx and PML in the PML oncogenic domains (PODs). hDaxx bound directly to Ad12E1B in vivo and in vitro. hDaxx interacted with Ad12E1B along its full length. Ad12E1B enhanced transcriptional repression activity of hDaxx.
CONCLUSIONAd12E1B disrupts the co-localization of hDaxx with PML in PODs and enhances transcriptional repression activity of hDaxx.

6.Mental health state of parents of children with autism.

OBJECTIVETo study the mental health state of parents of children with autism.
METHODSThe mental health state was evaluated by conducting the Symptom Checklist (SCL-90) on parents of 34 children with autism and of 35 healthy children.
RESULTSThe SCL-90 total scores in the fathers (162.5±34.0) and mothers of autistic children (175.1±51.0) were significantly higher than those in healthy children's parents (142.4±42.8 and 152.3±40.6, respectively) (P<0.05). The SCL-90 scores of obsessive-compulsive symptoms, depression, anxiety and paranoia in the fathers of autistic children were significantly higher than those in the fathers of healthy children (P<0.05). The SCL-90 scores of obsessive-compulsive symptoms, interpersonal sensitivity, depression, anxiety, paranoia, psychotic symptoms, hostility and sleep/diet were significantly higher in the mothers of autistic children than those in the mothers of healthy children (P<0.05). The mothers of autistic children presented higher SCL-90 factor scores in interpersonal sensitivity, anxiety and psychotic symptoms than the fathers (P<0.05).
CONCLUSIONSWe should pay more attention to the mental health of parents of autistic children.

7.An investigation on staff's opinions on community health services in Ganzhou City

Objective To investigate the knowledge and opinions of the staff in community health service centers.Methods Staff in Ganjiang,Jiefang and Nanwai community health service centers of Zhanggong District,Ganzhou City,Jiangxi Province were interviewed with questionnaire based on meeting by chance by trained interviewers about their opinions on 10 aspects of community health services.The data were evaluated by score method and factors influencing the score were studied with logistic regression analysis.Results Generally,the staff's evaluation for community health services was high,satisfying with colleagues' cooperation,professional training and service capacities in 95.96% ,90.91% and 88.88% of them,respectively.But their satisfaction with personal income and staff' s income was poor(24.24% and 43.43%).Staff for logistics were not so satisfied with their personal income.Staff for logistics and senior staff were not so active in their professional training.Conclusions The staff in community health service centers could better understand community health services with satisfaction.Incentive mechanism for the staff in community health services should be introduced with optimized management system to increase staff's income and sustain its stable development.
8.Clinical significance of protein Z alteration in patients with cardio-cerebral thrombotic diseases.

OBJECTIVETo study the alteration of protein Z (PZ) in patients with cardio-cerebral thrombotic diseases, its clinical significance and relations with FX.
METHODSPZ and FX:Ag were measured by ELISA, and plasma FX:C by first stage method. In 170 patients with acute ischemic stroke (AIS), 40 acute myocardial infarction (AMI) and 60 healthy adults as contrast, PZ, FX:C and FX:Ag were measured and compared between incipience and recurrence, different ages and genders.
RESULTSIn AIS and AMI groups, PZ levels decreased significantly to (940.02 +/- 229.82) microg/L and (1071.44 +/- 180.52) microg/L, respectively \[the contrast group was (2257.97 +/- 479.76) microg/L, P < 0.001\]. But FX:C and FX:Ag raised to (136.73 +/- 34.93)% and (135.54 +/- 54.39)% in AIS group; and to (139.53 +/- 29.18)%, (129.75 +/- 21.91)% in AMI group, respectively, while in the contrast group they were (94.33 +/- 22.00)% and (77.22 +/- 13.19)% (P < 0.001). In the comparative research between the AIS group, AMI group and the contrast group, PZ level was clearly found to negatively relate to the level of FX:C and FX:Ag (P < 0.001). Meanwhile, PZ level, FX:C and FX:Ag in recur-AIS group and recur-AMI group exhibited significant differences (P < 0.05) from those in the primary AIS and AMI groups, suggesting that the decrease of PZ levels reflected the pathological process of the disease. In addition, PZ level gradually decreased with the increase of age (P < 0.05), while FX:C and FX:Ag had no relations with age (P > 0.05). No correlation was found in sex with PZ level, FX:C, FX:Ag (P > 0.05).
CONCLUSIONPZ level was significantly decreased in AIS and AMI patients and was negatively related to FX:C and FX:Ag. The mechanism leading to FX increase may partially related with the decreased of PZ. PZ level was different in the primary and recurrent disease and was gradually decreased with the increase of age. Lack of PZ might be a etiological factor of cardio-cerebral thrombotic diseases.

9.Genetic diversity analysis of Penicillium marneffei isolated from AIDS patients in Guangdong, China using randomly amplified polymorphic DNA.

BACKGROUNDPenicillium marneffei (P. marneffei) is an emerging pathogenic fungus that can cause invasive mycosis in patients with AIDS. The epidemiological features of P. marneffei infection in AIDS patients in Guangdong province remain unclear so far. This study aimed to investigate the genetic diversity within a population of 163 P. marneffei isolates obtained from AIDS patients and search for the dominant clinical strains in Guangdong province.
METHODSOne hundred and sixty-three P. marneffei isolates obtained from AIDS patients in Guangdong province during January 2004 and December 2009 were studied by randomly amplified polymorphic DNA (RAPD) using two random primers (H2 and H22). The degree of similarity between samples was calculated through similarity coefficients from RAPD fragment data and the dendrogram was assessed using the unweighted pair group method with arithmetic mean (UPGMA).
RESULTSTwo primers showed a high degree of discrimination and good stability. Primer H2 yielded eight different patterns (H2-1 to H2-8) among 163 isolates with the discriminatory power being 0.413. Primer H22 identified seven types (H22-1 to H22-7) among 163 isolates with the discriminatory power being 0.467. Genetic similarity coefficients based on RAPD data among 163 P. marneffei isolates ranged from 0.681 to 0.957, 61.96% of which were no less than 0.83. The discriminatory power of the two primers was 0.524. One hundred and sixty-three P. marneffei isolates were clustered into nine distinct groups (groups I to IX) at the similarity coefficient value of 0.83 and group I was the most common, including 101 strains (61.96%).
CONCLUSIONThe RAPD analyses could provide important information as to the degree of genetic diversity and the relationship among clinical P. marneffei isolates, revealing genetic polymorphism and dominant strains.

10.Therapeutic effect of ossicular reconstruction with bioceramic or porous macromolecular polyethylene partial ossicular replacement prosthesis in patients with tympanosclerosis.

OBJECTIVETo evaluate the effect of ossicular reconstruction with partial ossicular replacement prosthesis (PORP) in patients with tympanosclerosis.
METHODSThe data of 31 cases of tympanosclerosis treated between 1992 and 2009 were reviewed. Of the 31 patients, 17 (17 ears) underwent ossicular reconstruction with porous macromolecular polyethylene PORP, and 14 (14 ears) with bioceramic PORP. All the patients were followed up for 3-24 months.
RESULTSSignificant improvement was found in postoperative speech frequency (500, 1000, 2000 Hz) pure tone average (PTA) and air-bone gap (ABG) (P < 0.05) after the treatments without statistically significant differences between the two groups (P > 0.05).
CONCLUSIONPorous macromolecular polyethylene and bioceramic are valuable ossicular prosthesis for tympanosclerosis.
