1.Clinical study on partial pulpotomy in the treatment of fracture of crown of permanent incisors with pulp exposure

Objective To investigate the clinical effect of par ti al pulpotomy in the dreatment of complicated crown fracture of permanent inciso rs with pulp exposure. Methods 44 soldiers suffering from compl icated crown fracture of the incisors with exposure of pulp as a result of injur y 46 during training were enrolled for study. Altogetber teeth were treated with partial pulpotomy, and they were followed-up for 1 to 5 years. Result One of the teeth was found fo have pulp necrosis one year after treatm ent dut to falling off of the filling. The treatment failed in other toeth dut t o polpitis 2 to 4 years later. The treatment was successful in 42 teeth (91.3%) . Conclusion Partial pulpotomy was an effective treatment for c omplicated crown fracture of permanent incisors with exposureof pulp.
2.Changes of gastric myoelectrical activity of chronic superficial gastritis after zhizhu capsule treatment

Objective To explore the changes of gastric myoelectrical activity of chronic superficial gastritis after zhizhu capsule treatment.Methods Sixty chronic superficial gastritis patients with dyspepsia were divided into two groups; the control group (n =26) was treated with pantoprazole and mosapride and the treatment group (n =34)with the same drugs combined with ZhizhuKuanzhong capsules for 2 weeks.Gastric myoelectrical activities of all patients were recorded by electrogastrography before and after treatment.The symptoms were scored in 60 chronic superficial gastritis patients according to themodified glasgow method.Results The glasgow scores before and after treatment were(6.12 ± 0.82) and (0.72 ± 0.56),respectively; they were (6.87 ±0.58) and(2.80 ± 1.10) in the control group,respectively.The glasgow scores of the treatment group were decreased more obviously than the control group (t =9.58,P < 0.05).The number of cases in the treatment and control groups which the main power ratio was more than 1 before and after meals was 7 (20.59%),29 (85.29%) and 5 (19.23%),19 (73.08%),respectively.In two groups,there was significantly statistical difference in the percentage of which main power ratio was more than 1 between before and after treatment (x2 =28.57,15.17,P < 0.05).After treatment,there was statistically significant difference in the percentage of normal slow wave and bradygastria before and after meal in two groups[treatment group:before meal:(63.22 ± 10.05)%,(20.35 ± 11.23)% ;after meal:(68.52 ±13.03) %,(18.62 ± 10.03) % ; control group:before meal:(50.1 ± 16.22) %,(28.38 ± 17.25) % ; after meal:(54.57 ± 18.32) %,(27.67 ± 16.27) %] (t =2.18 ~ 3.84,P < 0.05).There was no statistical differences in the percentage of tachygastria before and after meal in two groups (P > 0.05).Conclusions The dyspepsia symptoms of chronic superficial gastritis might be related to the disorder of elactrogastrographic frequency.ZhizhuKuanzhong capsules can effectively improve the gastric myoelectrical activity and relieve the dyspeptic symptom of chronic superficial gastritis patients.
3.Clinical and pathological evaluation of conservative surgery for tubal pregnancy

Objective To conduct the clinical and pathological evaluation on the conservative surgery in tubal pregnancy . Methods 513 cases of tubal pregnancy in this hospital from January 2006 to December 2012 were divide into the conservative sur-gery group(A ,314 cases) ,samlpintectomy group(B ,43 cases) and the medication conservative treatment group (C ,156 cases) .The hospitalization days ,HCG negative-conversion time ,cure rate ,re-pregnancy outcome ,pregnancy during 1-year follow-up were com-pared between the group A and C .In group B ,43 cases of tubal pregnancy and accomplishing fertility were firstly performed the tubal linear incision in the pregnant site ,then the biopsy was conducted after stopping bleeding and finally the salpingectomy in af-fected side was performed .The tubal electric injury degree by the unipolar or bipolar electrocoagulation and the pathological changes under light microscope were observed .Results The hospitalization days ,symptom relief rate ,HCG negative-conversion time ,cure rate and re-pregnancy outcome had statistical differences between the group A and C (P<0 .05) .Conclusion The tubal conserva-tive surgery has short hospitalization time ,high symptom relief rate ,high cure rate and few complications .The pathological observa-tion shows the localized electrocoagulation injury .The pregnancy rate after surgery is high .

5.Analysis of antibiotk resistance and plasmid profile of Gram-negative bacilli isolated form a urologic unit

and each of the resistant strains was resistant at least to 5 antibiotics.13 strains out of the 28 harbored plasmids ranging from 1 to 6 in number and 105 Md to 2.7 Md in molecular weight.The plasmid with 50 Md was the most popular one in this study,which may be the epidemic plasmid in the urologic unit.It was also found that 2 different species of bacteria isloated from the patients with infection of the urinary tract showed identical plasmid profile,which indicates that these 2 patients may be associated with each other.
6.Effects of dexmedetomidine on inflammatory responses and insulin resistance during cardiac operations with car-diopulmonary bypass

Objective To observe the effects of dexmedetomidine on inflammatory responses and insulin resistance during cardiac operations with cardiopulmonary bypass(CPB).Methods Fifty patients scheduled for elective cardiac valve replacement with CPB were equally and randomly divided into two groups using a random number table:observation group and control group(n =25).A loading dose of dexmedetomidine 1.0 μg/kg was injected intravenously over 1 5 min after induction,followed by continuous infusion at 0.4 μg·kg-1 ·h-1 until the end of CPB in doservation group.While the e-qual volume of normal saline was given in control group.After induction(T1 ),at 30 min after the be-ginning of CPB(T2 ),at the end of CPB(T3 ),and at 2 h after the end of CPB(T4 ),the jugular venous blood samples were taken for determination of serum concentrations of TNF-a,IL-6,insulin and blood glucose.The insulin sensitivity index (ISI ) = 1/(glucose × insulin )was obtained. Results Compared with control group,the serum TNF-a and IL-6 concentrations at T2-T4 were sig-nificantly decreased in observation group(P <0.05),the serum insulin and blood glucose concentra-tions at T2 ,T3 were significantly decreased in observation group(P <0.05 ),the ISI at T2 ,T3 were significantly increased in observation group(P <0.05).Conclusion Dexmedetomidine can reduce in-flammatory responses,thus reducing insulin resistance and blood glucose during cardiac operations with CPB.

Objective To study the formulation and preparation factors influencing in vitro release mechanism of drug from?-carrageenan/konjac glucomannan hydrophilic matrix tablets.Methods The matrix tablets containing sinomenine hydrochloride as a model drug were obtained by direct compression method or wet granulation technique.The effects of the ratios of?-carrageenan/konjac glucomannan,the amount of matrix materials,the kinds of diluent agent,tablet size,preparation method,and compressing pressure on release mechanism of the matrix tablets were studied by evaluating the n value in the Peppas equation. Results The release mechanism of the matrix tablets was non-Fickian release that coupled diffusion and erosion modle.With a decrease in the ratio of?-carrageenan/konjac glucomannan,a decrease in the amount of matrix materials,a decrease in the water-solubility of diluent agents,an increase in tablet size,and a decrease in compressing pressure,the ratio of drug diffusion was increased.Preparation method had little influence on drug release mechanism.Conclusion Drug could be slowly released from the matrix tablets containing?-carrageenan/konjac glucomannan as matrix material,the main factors influencing drug release mechanism are the ratio of?-carrageenan/konjac glucomannan,the amount of matrix materials, the kinds of diluent agent,tablet size,and compressing pressure.

AIM: The drug release characteristics of ?-carrageenan/konjac glucomannan matrix tablet were elvated by in vitro dissolution experiment. METHODS: Sinomenine hydrochloride was used as model drug,the matrix tablet containing ?-carrageenan and konjac glucomannan as matrix material were prepared by wet granulation technique. Drug release and tablet erosion in different pH values,ionic strength of media and rotation speeds were studied by in vitro dissolution experiment. 10.Effect of transcutaneous electrical acupoint stimulation on sedative efficacy during induction of anesthesia with propofol

Objective To evaluate the effect of transcutaneous electrical acupoint stimulation (TEAS) on sedative efficacy during induction of anesthesia with propofol.Methods Patients,of ASA physical status Ⅰ or Ⅱ,aged 21-69 yr,scheduled for elective thyroid surgery,were randomly divided into 2 groups using a random number table:control group (group C) and TEAS group.TEAS was applied to bilateral Hegu and Neiguan acupoints for 30 min.The frequency was 2/100 Hz,wave length was 0.2-0.6 ms and the intensity was maintained at about 8-12 mA according to the current that could be tolerated.Induction of anesthesia was started at the end of TEAS.Sequential method was used to determine the median effective target plasma concentration (EC50) of propofol causing loss of consciousness in each group.The initial target plasma concentration of propofol was 3 μg/ml.BIS value was recorded every 30 s within 5-8 min after loss of consciousness and the average BIS value was calculated.It was considered to be positive response when the average BIS value was below 50.The target concentration of propofol was decreased/increased by 0.3 μg/ml in the next patient.Results EC50 of propofol causing loss of consciousness was 3.08 μg/ml in group TEAS,and 3.70 μg/ml in group C,and there was significant difference between the two groups.Conclusion TEAS can enhance the sedative efficacy during induction of anesthesia with propofol in the patients.
