Chinese Journal of Applied Clinical Pediatrics 2016;31(12):958-960
Renal interstitial fibrosis is the common pathway for a variety of chronic renal disease to final stage renal failure, the main feature of it is the large accumulation of extra cellular matrix (ECM).Myofibroblasts is the major effector cells which lead to the synthesis of ECM,an important source of myofibroblasts is epithelial-mesenchymal transition(EMT).Wnt/β-catenin signaling pathway take part in the regulation of EMT,which make it become a potential site of anti-fibrosis therapy.

AIM and METHODS:To investigate the effect of L-arginine -nitric oxide pathway on patients with essential hypertension via hemodynamics and neuroendocrinology. 24 essential hypertension patients were randomly divided into two groups, group I was given L-Arg, and groups Ⅱ was given normal saline as control. Blood pressure, heart rate, heart funtion, nitric oxide, angiotensinⅡ, endothelin, thromboxane A 2 and prostacyline were measure in all patients. RESULTS: In group Ⅰ arterial pressure decreased, heart rate increased, cardial output, systolic volume and eject fraction increased, total peripheral resistance decreased. NO and PGI 2 levels were inceased. But at 80 min , with NO concentration decreased, SBP,DBP were increased, TPR, FT and AngⅡ were also increased. While HR, CO, SV and EF were decreased. However TXA 2 and PGI 2 showed not much change. CONCLUSION: Exogenous L-arginine produced a vasodilatory effect by increasing NO production ,caused the change of other hemodynamic function . It also indirectly changed plasma ET, AngⅡlevels by negative feed-back and suppressed the accumulation of platelet.

Objective To survey the status of clinical isolates multi-drug resistant Enterobacteriaceae produ-cing KPC carbapenemase in Huizhou city of Guangdong Province .Methods 81 strains of multi-drug resistant Enter-obacteriaceae were collectded from 2009 to 2011 in Huizhou region.Modified Hodge test(MHT) was used to screen stains which may produce carbapenemase , modified three-dimensional test was used to detect ESBLs , AmpC and MBLs.Polymerase chain reaction (PCR) and gene sequencing methods were performed to confirm KPC genotypes . Results Of 81 strains multi-drug resistant enterobacteriaceae ,13 of which had passitive results were screened by modified Hodge test ,including 7 strains of Enterobacter cloacae ,4 strains of Klebsiella pneumonia and 2 strains of Escherichia coli;By modified three-dimensional test ,9 strains produced MBLs ,2 strains produced ESBLs ,1 strain pro-duced AmpC ,1 strain produced ESBLs and AmpC at the same time .KPC was amplified from a strain of Klebsiella pneumonia which produce ESBLs by PCR ,KPC-2 genetype was identified by gene sequencing .Conclusion Klebsiel-la pneumonia with KPC–2 carbapenemase has emerged in huizhou city ,but the production of metallo-beta-lactamase is the main cause that Enterobacteriaceae resist to carbapenemase drugs .

Chinese Journal of General Surgery 2013;(1):31-34
Objective To calculate the incidence of pathology-proved bilateral well-differentiated thyroid carcinoma (BWDTC) in patients undergoing total or near total thyroidectomy. Methods Clinical data were retrospectively reviewed in 121 patients who underwent total or near total thyroidectomy.Thirteen indexes selected from both the unilateral thyroid carcinoma group and the bilateral thyroid carcinoma group were adopted to undergo a univariate analysis and Logistic regression analysis. Results BWDTC was found in 33.06% patients after total or near total thyroidectomy.Logistic regression analysis showed that patient's age < 50,bilateral nodules based on ultrasound and the diameter of the tirst nodule > 2 cm were independent risk factors for BWDTC (P < 0.05).Conclusions In the premise of a malignant first nodule found in the preoperative examinations,BWDTC should be highly suspected if the patient is under 50 years old,has bilateral nodules found by preoperative ultrasound and the diameter of the first malignant nodule is over 2 cm.

Chinese Journal of Applied Physiology 2014;30(2):106-118
OBJECTIVETo explore the effects of high altitude on cognitive flexibility.
METHODSSimulated hypoxia at an altitude of 3 600 m was performed in a hypobaric chamber. Twenty-three volunteers without hypoxic experience were selected and the mean age was about 25.1 years. The physiological parameters (heart rate, blood pressure and oxygen saturation) were measured. Task switch paradigm was used to explore the cognitive flexibility in each phase, and the changing anxiety state was evaluated simultaneously.
RESULTSReaction time (RT) switch cost in hypoxia phase showed a significant increase compared with the baseline; anxiety level in hypoxia phase was higher than the adaptation phase; a remarkable negative correlation between anxiety level and RT switch cost was found in adaptation phase, whereas a positive correlation was found in landing phase.
CONCLUSIONHigh altitude (3 600 m) affects cognitive flexibility and anxiety state. Anxiety before the hypoxia exposure improves the cognitive flexibility performance, while anxiety after the hypoxia exposure hampers the performance because of the post-hypoxia effect.

Objective: To investigate the effect and safety of interleukin-2 combined with cisplatin in the treatment of malignant pleural effusion. Methods: Totally 86 patients with malignant pleural effusion were randomly divided into the observation group (44 cases) and the control group (42 cases). The control group was treated with cisplatin, and the observation group was treated with interleukin-2 additionally. The treatment course was 3 weeks. The ascites relief, quality of life, survival status and tumor markers before and after the treatment, and the adverse drug reactions were compared between the groups. Results: The total ascites remission rate and the effective rate in the observation group were significantly higher than those in the control group (P<0.05).Compared with those before the treatment, CEA, CA19-9 and CA450 decreased significantly after the treatment in both groups (P<0.05), those in the observation group were significantly lower than those in the control group (P<0.05).After the treatment, the physiological function, physiological occupation, emotional function, body pain and the other life quality scores in the observation group were significantly higher than those in the control group (P<0.05).The 24-month survival rate in the observation group was significantly higher than that in the control group (P<0.05).There were no significant differences in adverse drug reactions between the groups (P>0.05).Conclusion: Interleukin-2 combined with cisplatin can effectively relieve ascites and improve quality of life, which can also prolong the survival time of patients.
