Objective To analyze the laboratory detection findings in infectious mononucleosis(IM) and mucocutaneous lymphnode syndrome(MCLS)and find the main points of the differential diagnosis and direct the treatment. Methods Five hundreds and twenty-five children were in-patient department over these past 10 years .These patients were divided into 2 groups,IM group (225 patients) and MCLS group(280 patients). EB-virus antibody,Paul-Bunnell test, the circumference blood numeration ,classification of leucocytes, the numeration of atypical lymphocytes were detected and compared,as well as blood platelet of these patients. Results The positive rate of EB-virus antibody in IM and MCLS were high, and had no discrepancy. However, the positive rate of Paul-Bunnell test and numeration of atypical lymphocyte in IM group were significantly higher than those in MCLS group. The total circumference blood leucocyte numeration of the groups increased, but the numeration neutrophil increased significantly in MCLS group, the rising numeration of lymphocyte in IM; the blood platelet numeration was elevated significantly in MCLS group and continued when the total circumference blood leucocyte numeration tended normally. Conclusions The causes of IM and MCLS are related to the infection of EB-virus. The laboratory characteristics of IM and MCLS are different, understanding the main points of the differential diagnosis contributes to the clear diagnose and early treatment.

Objective:To investigate the influence of free position delivery on maternal labor pain, labor control, maternal and infant outcomes and pelvic floor muscle function.Methods:Forty-eight pregnant women who took delivery in free position in the Third People′s Hospital of Bengbu from April to December 2018 were selected as the study group, and 48 pregnant women who took sitting position delivery at the same period were selected as the control group at a 1∶1 ratio. The pregnancy outcomes of the two groups were compared. The duration of labor time and the amount of bleeding were observed. Simplified McGill Pain Questionnaire (SF-MPQ) and labor agentry cale (LAS) were used to evaluate the pain and the labor control of mothers 2 h after delivery. The neonatal asphyxia was evaluated by neonatal Apgar score. The Hamilton anxiety scale (HAMA) and the Hamilton depression scale (HAMD) were used to evaluate the anxiety and depression status of mothers in the two groups 6 weeks after the delivery. The pelvic floor muscle function of the pregnant women in the two groups was compared 6 weeks after the delivery. Results:The rate of maternal transfer to cesarean section in the study group was lower than that in the control group [4.17%(2/48) vs. 16.67%(8/48)] , and the difference was statistically significant ( χ2 = 4.019, P = 0.045). The time of the first and the second stage of labor, the total duration of labor, and the amount of bleeding in the study group were lower than those in the control group [(472.69 ± 55.24) min vs. (515.25 ± 67.26) min, (43.26 ± 5.23) min vs. (52.34 ± 5.56) min, (535.24 ± 38.22) min vs. (582.68 ± 40.16) min, (179.12 ± 25.65) ml vs. (195.34 ± 24.31) ml], and the differences were statistically significant ( P<0.05). There was no statistically significant difference in the time of the third stage of labor between the two groups ( P>0.05). After the delivery, the scores of verbal response scale (VRS), visual analogue scale (VAS), present pain intensity scale (PPI) and HAMA, HAMD in the study group were lower than those of the control group [(6.24 ± 1.21) scores vs. (7.68 ± 1.32) scores, (21.25 ± 2.98) scores vs.(27.95 ± 3.75) scores, (2.78 ± 0.29) scores vs. (3.49 ± 0.85) scores, (6.35 ± 0.18) scores vs. (7.54 ± 0.16) scores, (4.38 ± 0.12)scores vs.(5.54 ± 0.25) scores], the score LAS in the study group was higher than that of the control group [(175.18 ± 15.64) scores vs. (122.65 ± 14.85) scores], and the differences were statistically significant ( P<0.05). The Apgar scores of neonates in the study group was higher than that of the control group at 1 min and 5 min after the delivery, but there was no statistically significant difference ( P>0.05). There was no statistically significant difference in abnormal pelvic floor muscle strength, muscle strength fatigue and abnormal vaginal pressure between the two groups ( P>0.05). The incidence of adverse outcomes in the study group was lower than that in the control group [13.04%(6/46) vs. 35.00%(14/40)] ( P<0.05). Conclusions:Free posture delivery can reduce the rate of transfer to cesarean section, shorten the delivery process, and reduce the amount of bleeding. It also can alleviate the anxiety, pain of mothers, increase the labor control, and reduce the incidence of adverse labor outcomes, but its impact on maternal pelvic floor muscle function needs to be further explored.

Objective Speckle is the inherent problem exists in B-Mode ultrasound image, and effective speckle noise reduction will improve the image quality and contribute greatly to clinical doctors to diagnose, especially in fine ophthalmic examination with high frequency ultrasound. Methods This paper proposed a new speckle reduction method based on the Laplacian pyramid and multiscale analysis. In the Laplacian pyramid, true clinical features were separated from noise, according to the different bandpass ultrasound image characteristics in each layer, and the advanced eight directions speckle reduction anisotropic diffusion (SRAD)was adapted to suppressed the speckle noise, and the identified noise and the extracted features were selectively emphasized by suitable edge, coherence and contrast enhancement filtering from fine to coarse scales. The performance of the proposed method was compared to the traditional SRAD method and coherence-enhancing diffusion method by measuring the equivalent number of looks (ENL) and Time cost. Results The ENL and Time cost value of the proposed method were higher compared to the SRAD and the cedif method,i.e 1.172 3 vs 1.122 3,0.929 3 and 0.864 0 vs 1.396 0,1.468 3.Conclusions In summary, the proposed method can more effectively remove the speckle noise while preserving the edge and details of the images.

Journal of Practical Stomatology 2009;25(4):467-470
Objective: To evaluate bond strength and interface of various dentin adhesives by a modified interface processing method. Methods:Nine extracted human molars were ground to expose fresh dentin surface. Three dentin adhesives (Single Bond, Clearfil SE Bond and Clearfil S3 Bond) were applied respectively and a block of composite resin was built up. The specimens were then stored in distilled water for 24 h at 37 ℃ and sectioned into beams of 0.9 mm×0.9 mm cross-sectional area. The microtensile bond strength was determined with Microtensile Tester(n=15). In addition, the bonding specimens were conditioned with 6 mol/L HCl, 5% NaOCl and 0.08 mg/ml hyaluronidase to observe the adhesive interfaces by SEM. Results: Significant differences in bond strength were observed among the three adhesives(P<0.05): SE Bond (45.06±5.29) MPa> Single Bond (35.50±6.40) MPa> S3 Bond (30.46±3.82) MPa. Single Bond showed resin tags about 9-12 μm long and gaps between hybrid and adhesive layers. No obvious boundary between hybrid layer and adhesive were observed for both SE Bond and S3 Bond, while the length of resin tags were 9-14 μm and 4-7 μm for SE Bond and S3 Bond respectively. Conclusion:The morphology of the bond interface of dentin adhesive has a relationship with the bond strength.

Objective To determine the regulation of electroacupuncture(EA)at Zusanli acupoint for the expression of estrogen receptors mRNA in the brain of ovariectomized rats.MethodsFemale Wistar rats were randomly divided into 3 groups:intact group(INT),ovariectomized group(OVX),and ovariectomy and acupuncture group(OVX+EA).Radioimmunoassay(RIA)was used to measure the estradiol(E2)and testosterone(T)levels in plasma. The mRNA levels of estrogen receptor(ER)α and ERβ in the brain of rats were measured with RT-PCR.ResultsThe levels of endogenous estrogen increased significantly in OVX+EA rats compared with OVX rats(P<0.01),while the plasma T levels were lower in the OVX+EA group than those of the OVX group(P<0.01).Eelectroacupuncture can up-regulate ERα expression in the brain of ovariectomized rats(P<0.01).However,it was shown to have a down-regulatory effect on the levels of ERβ expression(P<0.01).ConclusionThere is an intimate relationship between the expressions of estrogen receptors mRNA and the endogenous hormone levels, such as E2 and T.Electroacupuncture can regulate the expression of estrogen receptors mRNA in the brain and the E2 and T levels in plasma of ovariectomized rats, which might,at least in part, explain the mechanisms of acupuncture adjusting the neuroendocrine system.

Objective To investigate whether cold or heat stress affected signal transduction of p38 mitogen-activated protein kinase (MAPK) in the spinal cords and ganglia. Methods The rat models of acute cold and heat stresses were established, and the immunohistochemistry and Western blotting analysis were used. Results The phospho-p38 (p-p38) immuno-positive cells were observed in the Rexed Ⅱ layer of normal thoracic spinal cord, with low cell density and brown nucleus staining, and without staining in Rexed Ⅲ~Ⅹ layers. In the spinal ganglia, the p-p38 immunoreaction was mainly located in the nuclei of a few small-sized neurons. Acute heat stress (10 min & 20 min) reduced the number of positive cells in Rexed Ⅱ layer, with body-temperature (38℃) stress (20 min) having no effect. No immunostaining was changed in the spinal ganglia. Acute cold stress (10 min & 20 min) slightly reduced the positive cells in the Rexed Ⅱ layer, with unchanged immunostaining in spinal ganglia. Western immunoblot analysis showed that activation level of p-p38 was differently downregulated by heat and cold stresses compared to the normal groups and body temperature (38℃)-bathed groups. Conclusion Constitutive activation of p38 can be observed in the normal spinal cord and ganglia; heat stress and cold stress significantly and gentlely inhibite the phosphorylation level of p38, respectively.
