1.Analyses of anxiety imagery characteristic factor structure in imagery dialogue psychological counseling techniques

Objective To explore the factor structure of the anxiety imagery characteristics questionnaire.Methods Through the literature review and expert group discussion,the initial anxiety imagery features questionnaire was created.The first questionnaire contained 49 items of anxiety imagery,which was answered by 106 imagery dialogue psychotherapists.After analysis of the items and exploratory factors,the formal questionnaire that contained 20 items of anxiety imagery characteristics was formed.The formal questionnaire was measured by 115 imagery dialogue psychotherapists,then,exploratory and confirmatory factor analyses were made.Results The exploratory factor analysis showed that four factors whose Eigen value were more than 1 were extracted from l0 items,including emotional panic,uneasy,tense muscles,and motor restlessness.Those four factors explained 54.39% of the total variance.Confirmatory analysis showed that x2/df was 2.142 ; root mean square error of approximation (RMSEA) was 0.059 ; and comparative fit index (CFI),goodness-of-fit index (GFI),non-normed fit index (NNFI) and RSMEA was 0.912,0.935,0.896,and 0.057,respectively.Conclusions The four-factor structure was constructed by 10 items of anxiety imagery characteristics questionnaire.
2.Carotid angioplasty and stenting for carotid stenosis in high-risk symptomatic NASCET-ineligible patients

Objective To investigate the efficacy and safety of carotid angioplasty and stenting for carotid stenosis in high-risk symptomatic NASCET-ineligible patients. Methods Twenty patients with symptomatic carotid stenosis at high risk ineligible for NASCET were treated with carotid angioplasty and stenting,12 of them were men,and 8 wre women,the patients ranged in age from 62 to76 years (mean age,69 years). Eleven patients had transient ischemic attack and 9 had cerebral infarction. Digital subtraction angiography showed that the degree of carotid stenosis in all patients was > 70% (NASCET criteria). Among them,9 patients had unilateral carotid artery severe stenosis (2 had restenosis after endarterectomy),6 had bilateral carotid artery severe stenosis,5 had unilateral carotid artery occlusion with contralateral severe sentoses (1 had undergone cervical radiotherapy for nasopharyngeal carcinoma). The embolic protection devices,predilation,and self-expandable stents were used in all patients. Results The success rate of the procedure was 100%. The residual stenosis rate was < 30%. The different levels of a transient decline in heart rates and blood pressure occurred in all the patients during the procedure. One patient was complicated with microembolic embolism. No ischemic stroke occurred in the remaining patients in the periprocedure. The postoperative examination with carotid ultrasound showed that the stenoses were improved significantly. No ipsilateral ischemic stroke and coronary ischemic events were observed at 1 and 3 months follow-up after the procedures. Conclusions Carotid artery stenting is less invasive,and the perioperative complications are fewer,The treatment of symptomatic carotid stenosis with high surgical risks is safe and effective.

Introduction:The 2010 targets of the China Hepatitis B Prevention Programme were a prevalence of hepatitis B surface antigen (HBsAg) less than 1.0% for children less than five years old and less than 6.0% for the total population. This survey assessed the prevalence of Hepatitis B infection in Lianyungang, Jiangsu province, China in 2009–2010.Methods:Multistage sampling was used with 2372 subjects among 17 selected villages. Blood specimen collection and testing by enzyme-linked immunosorbnet assay (ELISA) were completed using the following markers for hepatitis infection: HBsAg and antibody to HBsAg (anti-HBs); hepatitis B e antigen (HBeAg) and antibody to HBeAg (anti-HBe); and hepatitis B core antibody (total anti-HBc). The data were analysed with Epi Info, version 3.3.2.Results:The prevalence of HBsAg was 2.4% (95% Confidence Interval [CI]: 1.8–3.0; Adjusted Prevalence [AP] 2.9%); anti-HBs prevalence was 51.1% (95% CI: 49.1–53.1; AP 49.2%) and total anti-HBc prevalence was 41.7% (95% CI: 39.8–43.7; AP 45.5%). 6.Effect of danshensu and ligustrazine on related genes of myocardial hypertrophy induced by angiotensin II.

OBJECTIVETo study the effect of Danshensu (DSS) and Ligustrazine (TMZ), the extracts of Chinese herbs for promoting blood circulation, on angiotensin II (Ang II) induced myocardial hypertrophy and its related genes, and to explore the mechanisms of inhibitory effect.
METHODSAdopting one-step method, the total RNA of myocardial cells was extracted by TRIzol reagent. Then the expression of ANP and beta-actin mRNA, as symbol of myocardial cells, were detected by RT-PCR.
RESULTSMolecular biological research showed that Ang II could significantly increase the expression of ANP mRNA in myocardial cells (P < 0.01), which could be significantly inhibited by Losartan (P < 0.01), both DSS and TMZ had the inhibitory effect (P < 0.05). Ang II could increase beta-actin mRNA expression in myocardial cells simultaneously, Losartan, DSS and TMZ could also significantly inhibit it (P < 0.05).
CONCLUSIONThe effective ingredients of Chinese herbs for promoting blood circulation, DSS and TMZ, have the effect of inhibiting the hyper-expression of ANP and beta-actin induced by Ang II, and preventing myocardial hypertrophy, therefore, it could be used to prevent and treat cardiomegaly.

7.Analysis and quantitative assessment on the risks of diabetes at the community level in Chongqing

Objective To investigate the correlation risk factors of diabetes over 16-year-olds at the community level in Chongqing and to set a quantitative criteria for determining risk for diabetes and to identify persons having potential risk.Methods 1981 cases with 1:2 matched controls were chosen from five communities in Chongqing including Shapingba,Xiaolongkan,Tianxingqiao,Yubei Road,Ciqikou,which were interviewed with a uniformed questionnaire.The risk factors of diabetes mellitus were analyzed with logistic regression,and to calculate the odds ratios of risk factors for diabetes.Different levels of risk exposure factors were converted into a risk scores,using statistical models.Results An individual health risk appraisal model of diabetes was established,applicable to individuals of different sex,age,health behavior,disease,and family history,for men,13 risk factors including hyperlipidemia(14.995),coronary heart disease(6.689),family history of hypertension(4.005),smoking(3.111)etc.while for women,hyperlipidemia(12.426),family history of hypertension(3.986),stroke(2.714),liking sweets(1.244),about 15 risk factors,were entering the main effect model.The related risk scores were added to obtain a combined risk score to predict the individual's risk of diabetes in the future.Conclusion The incidence of diabetes could be effectively reduced by changing the unhealthy lifestyle and curing the patient with the disease.Evidence was provided to persuade people change their unhealthy lifestyles and behaviors through health education.The results could also be used in community to improve their health services.
8.Influence of aqueous extract of Aralia echinocaulis Hand.-Mazz on the expression of fracture healing-related factor receptors.

OBJECTIVETo investigate the influence of aqueous extract of Aralia echinocaulis Hand.-Mazz on the expression of fracture healing-ralated factor receptors.
METHODSSingle factor model was set up in SD rat. Selecting 14 and 28 days in the experiment. Immunohistochemistry was employed to determine the expression of Fibroblast growth factor receptor 2 (FGFR2), Fms-like tyrosine kinase (Flt-1) and Fetal licer kinase (Flk-1) at 14 and 28 days after model establishing.
RESULTSThe expression of Flt-1 and Flk-1 at 14 days (the latter was more remarkable) were obviously promoted in High dose group of aqueous extract of Aralia echinocaulis Hand.-Mazz, and higher than that in normal group and model group. The expression of FGFR2 in the high dose group of Aralia echinocaulis Hand -Mazz was also promoted visibility, close to that in the compare group (traditional Chinese medicine), but higher than than in the model group. There was no significant difference among them. At 28 days, the expression of FGFR2, Flt-1 and Flk-1 in all groups decreased except normal group, and got higher expression in model groups than each control groups.
CONCLUSIONAqueous extract of Aralia echinocaulis Hand.-Mazz can promote angiogenesis in fracture healing, improve the activity and aggregation of fibroblasts, osteoblasts and chondrocytes. It also helps to quicken ossification in the cartilage and promote fracture healing.

9.Repair of massive soft tissue defect in upper and lower extremities with free transmidline bi-lobed scapular skin flap.

OBJECTIVETo evaluate the result of free transmidline bi-lobed scapular skin flap transplantation to repair massive soft tissue defects in upper and lower extremities.
METHODSFree transmidline bi-lobed scapular skin flap was designed according to the characteristics of scapular skin and its vasculature, and they were used to repair massive soft tissue defects in upper and lower extremities as a result of hot crush injury and avulsion injury in 9 patients. The survival of the flap, the blood supply, the color and elasticity of the flap, as well as scar contraction and impairment in function were observed after operation.
RESULTSThe largest flap formed with the horizontal branch and descending branch circumflex scapular artery were 31 x 14 cm and 22 x 16 cm, respectively. The horizontal branch went across the spinal midline for 10 cm. All skin flaps survived with good elasticity and without necrosis. The grafted skin was excellent in the appearance and elasticity, with no scar contracture , and function of the injured extremities recovered well 2 to14 months after the operation.
CONCLUSIONFree transmidline bi-lobed scapular skin flap is an ideal procedure for the repair of massive soft tissue defects in upper and lower extremities.

10.Clinical observation on the paraumbilical skin flaps application in the repair of the wounds of 39 patients with severe burns.

OBJECTIVETo evaluate the effectiveness of paraumbilical skin flaps for the repair of the severe burn wounds.
METHODSPedicled, island, or free paraumbilical skin flaps, as well as combined transplantation of several kinds of flaps were used to repair the wounds of 39 patients with severe burns. The largest paraumbilical skin flap was 55 x 27 cm(2) in area, with the end approaching the armpit midline of the axilla on the same side, and extending 10cm from the abdominal midline. The survival, color, elasticity of the flaps, degree of deformity resulted from scar contracture, and the condition of donor area were observed.
RESULTSAll the paraumbilical skin flaps survived, without obvious infection subsequent to complete debridement of the wounds. The flaps exhibited good color and elasticity, without scar contracture after 1 to 46 months in 33 patients. No obvious scar formation was observed in the donor area.
CONCLUSIONParaumbilical skin flap is an ideal one for the early repair of deep burn wounds, because it can cover a large area, and can be transferred in different ways according to the area and the position of the wounds.
