1.Effect of Xuebijing injection on systemic inflammatory response and immune function of patients with acute exacerbation of severe chronic obstructive pulmonary disease

Objective To investigate the impacts of Xuebijing injection on systemic inflammatory response and immune function of patients with acute exacerbation of severe chronic obstructive pulmonary disease (AECOPD). Methods A prospective randomized controlled trial (RCT) was conducted. Forty patients with severe AECOPD were divided into control group and Xuebijing group in accordance with the random number table, each group 20 cases. Both groups were treated by routine conventional basic therapy of severe AECOPD including anti-infection, phlegm-expelling formula, bronchodilators and mechanical ventilation, etc, while in Xuebijing group, intravenous drip of Xuebijing (100 mg, twice a day for 5 days) was additionally used. The changes of data of arterial blood gas analysis, blood routine examination, C-reactive protein (CRP), procalcitonin (PCT), immune function, and acute physiology and chronic health evaluation system Ⅱ (APACHE Ⅱ) score before and after therapy were observed and compared between the two groups. The length of stay in hospital, duration of mechanical ventilation, and prognosis were also compared between the two groups. Besides, according to the difference in APACHE Ⅱ score, all the patients were divided into APACHEⅡscore≥15 score group (18 cases) and APACHEⅡscore<15 score group (22 cases), and the immune function were compared between the two groups. Results ①The immune function was descended and disordered in patients with severe AECOPD. Compared with the < 15 score group, the expressions of CD45+, CD3+, CD4+, CD4+/CD8+ ratio, B-lymphocyte, natural killer cell (NK cell) in ≥ 15 score group were significantly lowered [CD45+(×106/L):663.92±100.61 vs. 1 289.92±169.38, CD3+(×106/L):342.05±108.93 vs. 882.37±172.56, CD4+(×106/L):205.96±63.97 vs. 486.24±108.64, CD4+/CD8+ratio:0.76±0.49 vs. 1.32±0.57, B-lymphocyte (×106/L):124.77±32.72 vs. 166.06±48.02, NK cells (×106/L): 186.47±39.57 vs. 243.51±44.72, all P < 0.05]. There was no statistically significant difference in the expression of CD8+ between the ≥ 15 score group and < 15 score group (P > 0.05). ② Compared with those before therapy, the pH value, oxygenation index, arterial partial pressure of carbon dioxide (PaCO2), white blood cell count (WBC), CRP, PCT were significantly improved after therapy in both control and Xuebijing groups. Compared with those in the control group, WBC, CRP, PCT were significantly lowered in Xuebijing group [WBC (×109/L): 10.29±3.83 vs. 12.69±3.42, CRP (mg/L): 9.06±4.19 vs. 15.26±4.22, PCT (ng/L): 0.18±0.21 vs. 0.42±0.24, all P < 0.05]. There were no statistically significant differences in pH value, oxygenation index and PaCO2 between two groups (all P>0.05). The degrees of improvement of CD45+, CD3+, CD4+, CD8+ and CD4+/CD8+ ratio were more remarkable in Xuebijing group after treatment than those in control group [CD45+ (×106/L): 1 079.38±153.86 vs. 1 015.63±157.11, CD3+ (×106/L): 652.05±100.05 vs. 596.81±106.85, CD4+ (×106/L): 358.92±67.53 vs. 329.99±72.61, CD8+ (×106/L): 205.73±35.19 vs. 230.41±39.74, CD4+/CD8+ratio: 2.16±0.63 vs. 1.52±0.54, all P < 0.05]. The B-lymphocyte and NK cell before treatment and after treatment showed no statistical significant differences between the two groups (all P > 0.05). Compared with the control group, the duration of mechanical ventilation (hours: 56.25±22.87 vs. 69.45±26.59) and the length of stay in hospital (days: 11.00±0.74 vs. 14.00±2.06) were shortened, and APACHE Ⅱ score was significantly lowered in Xuebijing group (8.21±2.97 vs. 12.08±3.12, P < 0.05). The numbers of multiple organ failure and dead patients in Xuebijing group were less than those of control group, but no statistical significant differences were found (both P > 0.05). Conclusion Xuebijing injection for treatment of patients with severe AECOPD can ameliorate inflammatory response, improve immune function, shorten the duration of mechanical ventilation and the length of stay in hospital, and decrease the risk of death.
2.Analysis on clinical significance of Acinetobacter baumannii isolated from patients′sputum samples in cerebral surgery ICU

Objective To understand the clinical distribution and drug resistance situation of112 strains of Acinetobacter Bau-mannii isolated from the patients′sputum samples in cerebral surgery ICU in order to provide the basis for the rational use of anti-bacterial drugs in clinical therapy.Methods 112 strains of Acinetobacter Baumannii were performed the retrospective analysis.The VITEK2-Compact automatic microbiological analyzer was adopted to conduct the bacterial identification and the drug susceptibility test and the K-B method was combined as the supplement control.The Whonet 5.6 software was used for conducting the statistical analysis.Results Among 112 strains of Acinetobacter baumannii,75 strains(67.0%)were multi-drug resistant,8 strains(7.1%) were pan-resistant.The antimicrobial susceptibility tests showed that the resistant rates of Acinetobacter baumannii to mezlocillin, piperacillin and nitrofurantoin reached up to 100%.Acinetobacter Baumannii had the lower resistant rate to polymyxin B (2.7%), the resistant rates to minomycin and ceperazone/sulbactam were 4.5% and 10.7% respectively.Conclusion The drug resistance phenomena of Acinetobacter Baumannii in the cerebral surgery department ICU is serious.Minomycin,polymyxin B and ceperazone/sulbactam have the better antibacterial activities to clinically isolated Acinetobacter Baumannii and may be the alternative drugs for therapy.Clinic should strengthen the monitoring and rationally use the antibacterial drugs.
3.Knowledge Mapping Analysis of Tai Chi Chuan Based on Web of Science

Tai Chi Chuanis a traditional Chinese method of regimen and health care, which has attracted wide attention from countries in the world. This article was aimed to analyze the current situation and hot spots about Tai Chi Chuan for future research. A total of 1349 articles were retrieved from Web of Science from 1981 to 2013. CiteSpace was used to map knowledge domain of 1349 articles in order to explore the current situation and hot spots aboutTai Chi Chuan internationally. The results showed that the research onTai Chi Chuan showed a rising trend. The main research regions were in the United States and China. The research fields were extended from medicine and sport science to psychology, engineering and computer science. The hot spots were focused on physical control, psychology and cardiovascular diseases. It was concluded thatTai Chi Chuan was a method benefit to both physical and mental health. The focus of future Tai Chi Chuan research was on basic theory, action mechanism and strict evidences.
4.Clinical Observations on Electroacupuncture plus Pelvic Floor Muscle Training for the Treatment of Postoperative Retention

Objective To investigate the clinical efficacy and safety of electroacupuncture plus pelvic floor muscle training in treating postoperative retention of urine. Method Patients were allocated, according to admission date, to a treatment group of 33 cases and a control group of 34 cases. The treatment group received electroacupuncture at point Ciliao (BL32) plus pelvic floor muscle training and the control group, intermittent catheterization. The overall health degree was assessed using the therapeutic effect evaluation, first voiding time, residual urine volume and the number of urinary tract infection patients before and after treatment. Result The improvement rate was higher in the treatment group than in the control group and there was a statistically significant difference (P<0.05). Voiding recovery time was shorter and residual urine volume was smaller in the treatment group and there were statistically significant differences compared with the control group (P<0.01). Conclusion Electroacupuncture plus pelvic floor muscle training can effectively treat postoperative retention of urine. It is a safe and effective therapeutic method.
5.Clinical effect of oxaliplatin,calcium tolinate and tegafur in the chemotherapy of rectal cancer patients

Objective To observe the effect and toxicity of oxaliplatin combined with calcium folinate and tegafur in the treatment of rectal cancer patients,in order to provide a safe and effective clinical program for rectal cancer treatment.Methods 117 rectal cancer patients conforming to the selection criteria were randomly divided into treatment group 59 cases and control group 58 cases by using random number method,treatment group were received chemotherapy of Oxaliplatin,calcium folinate and tegafur;control group were received FOLFOX4 program.After imple-mentation of two treatment plans,solid tumor curative effect evaluation standard from WHO was referenced to evaluate the curative effect.And toxicity classification was reached according to the WHO cancer drug toxicity assessment standard.Results Total effective rate of treatment group was 27.1%,total effective rate of control group was 29.3%,the comparative difference of two rates was not statistically significant (χ2 =0.069,P >0.05).The main symptoms of adverse reactions of two treatment plans were decreased of leukopenia,hemoglobin and thrombocytopenia of the blood system;nausea,vomiting,weak and abnormal liver function of the digestive system;,and limb numbness or pain of the nervous system.Adverse reactions difference in two groups was not significant (χ2 =0.106,0.158, 0.000,0.563,0.001,0.284,0.068,0.000,all P >0.05).After treatment,the median surial time was 15.5 months in treatment group and 16.5 months in control group,the difference of the median surial time in two groups was not statistically significant (P =0.781,P >0.05).Conclusion Combination of Oxaliplatin,calcium fluoride and tegafur is another safe and effective plan after the FOLFOX4 program in the treatment of rectal cancer drug application.
6.Thoughts About the Location of Point Zusanli (ST36)

ObjectiveTo explore a more perfect method for acupoint location by taking point Zusanli (ST36) as an example. MethodPoint Zusanli was located during extension and flexion of the knee joint. The selected points were not at the same place. Its reason was explained. The method of locating point Zusanli was exploredin flexion of the knee.ResultDuring extension and flexion of the knee joint, point Dubi(ST35)shifted superiorly and inferiorly. Therefore, the influence of this factor on the location of point Zusanli was considered when the acupoint was located usingpoint Dubi as a reference point.ConclusionBecause different body positions are often required when acupoints are clinically located, attention should be paid to the problem of how to correctly locate acupoints in different body positions. Again becausethe positions of acupuncture points are changeable and relative, the location of acupoints should be finally determined by acupuncturist using the method of acupoint examination under the direction of various location methods. Therefore, the relationshipbetween locating methods such as bone-length measurements and actual positions of acupoints is worthy to be considered again. Only not abandoning but also not depending on measurements is a more perfect method for acupoint location.
7.The study of low doses 131 I on serum levels of ghrelin and related serological indexes in patients with grave′s disease

Objective To investigate the low doses of 131 iodine on serum Ghrelin levels and serological related indexes in pa‐tients with hyperthyroid .Methods A randomized controlled trials ,96 cases of hyperthyroidism in our hospital were randomly divid‐ed into two groups (group A and group B)according to the ratio of 1∶1 from June 2011 to June 2012 ,48 cases in each group ,and at the same time ,42 cases health people as health control group .The group A were treated with 131 iodine solution 3 .69 -4 .40 MBq/g reatment ,group B was given 131 iodine solution 2 .59-2 .96 MBq/g treatment ,and the serum Ghrelin levels and serological related indicators of two groups patients were statistically analyzed after treatment .Followed up for 2 years ,review of the serum levels of Ghrelin and serological related indexes and compare the incidence of hypothyroidism .Results The serum FT3 ,FT4 and FPG levels of hyperthyroid patients were significantly higher than the health control group (P< 0 .01);conversely ,the serum S‐TSH and Ghrelin levels of hyperthyroid patients were significantly lower than the health control group (P<0 .01) .3 months later after treatment ,the serum FT3 ,FT4 and FPG levels of the hyperthyroidism group B were significantly lower than hyperthyroidism group A(P<0 .05);on the contrary ,the serum Ghrelin and S‐TSH levels were significantly higher and hyperthyroidism group A (P<0 .05);2‐year follow‐up after treatment ,the incidence of hypothyroidism of hyperthyroidism group A were higher and group B ,but without statistical significance(P>0 .05) .Conclusion A low dose of oral 131 iodine solution (2 .59‐2 .96 MBq/g) ,could get a better short‐term efficacy in patients with hyperthyroidism ,high security .

A disintegrin and metalloproteinase 10 is a zinc dependent transmembrane type Ⅰ multi-domain of protein which is highly conserved in evolution and mediates protein maturation and ectodomain shedding, with protein hydrolysis, cellular adhesion, cellular fusion and signal transmission.This article reviews its structure and function, the substrate and relative signaling pathways, the factors influencing its expression and catalytic activity,the research progress in urinary tract diseases.
9.The clinical study of efficacy of cyclophosphamide plus MP for patients with multiple myeloma and its influ-ence on serum CRP,LDH,Alb and β2 -MG

Objective To explore the therapeutic efficacy and safety of cyclophosphamide combined with MP for patients with multiple myeloma (MM),as well as its influence on serum CRP,LDH,albumin (Alb)and β2 -microglobulin (β2 -MG).Methods 6 1 patients diagnosed MM in our hospital were divided into two groups random-ly:32 cases in the treatment group were given the regimen of cyclophosphamide combined with MP,and 29 cases in the controlled group were given the regimen of MP.After four periods of treatment,the curative effects and side effects of two groups were evaluated,besides,the changes of serum CRP,LDH,Alb and β2 -MG were compared.Results In the treatment group,the ORR was 90.63%,CR was 37.50%,VGPR was 25.00%,PR was 28.13%.And in the controlled group,the ORR was 62.06%,CR was 10.34%,VGPR was 17.24%,PR was 34.48%.The further studies shows that the CR and ORR between the two groups were significantly different (χ2 =6.050,7.015,P=0.013, 0.008).Moreover,the side effects included hematological toxicities and non-hematological toxicities which were not different notably (P>0.05).In addition,the serum CRP,LDH andβ2 -MG were decreased after the treatment nota-bly (t=7.321,4.972,7.668,5.174,4.231,2.379,all P<0.05),especially in the treatment group (t=-2.097,-2.409,-2.112,all P<0.05);the Alb was increased compared to precious notably (t=-4.170,-2.805,all P<0.05),especially in the treatment group (t=1.710,P<0.05).Conclusion The regimen of cyclophosphamide com-bined with MP is an effective and safe therapy for multiple myeloma which seems significantly superior to MP regimen with higher CR and ORR and which can also lower CRP,LDH,β2 -MG and raise Alb to cut down the burden of tumor.
