1.Application foreground of IPv6 in telemedicine

This paper reviews the problem and shortage of IPv4 network which was unfolded during its use in telemedicine field and describes the features and advantage of IPv6, which is the next generation protocol of Internet by comparing it to IPv4. The application of IPv6 in telemedicine field is prospected. The work should be done and the problem may be met during the change from IPv4 to IPv6 network are also discussed.
2.Effect of montelukast on the secretion of mucus protein in human bronchial epithelial cells

ObjectiveTo investigate the effect of montelukast on the secretion of mucus protein in lipopolysaccharide (LPS) induced human bronchial epithelial cells.MethodsPrimary human bronchial epithelial cells were isolated and identiifed in vitro. LPS (1μg/mL) was used to induce cell inlfammatory response. Montelukast (50 μmol/L, 20μmol/L, 10μmol/L) was used as intervention. The concertration of mucin-5 subtype AC (MUC5AC) in cell supernatants was measured by ELISA. The expression levels of MUC5AC mRNA and protein were determined by RT-PCR and Western-blot. DCFH-DA lfuorescent probe was used to detect reactive oxygen species (ROS). To further elucidate the mechanism, NF-κB (p65)、IκBα、ERK1/2 phosphorylation be-fore and after montelukast intervention were determined by Western-blot.ResultsMontelukast decreases the expression levels of MUC5AC mRNA and protein in a dose-dependent manner in LPS induced human bronchial epithelial cells. Meanwhile, mon-teluskast suppresses ROS generation and NF-κB (p65)、IκBα、ERK1/2 phosphorylation.Conclusions In response to LPS in-duced inlfammation, montelukast decreases the expression level of MUC5AC in vitro, which may be related to NF-κB and ERK activation.
3.Research Progress in Identification Technology for Three Groups of Easily Confused Chinese Medicinal Materials

Many Chinese medicinal materials are difficult to be distinguished in characters resulting in easy confusion. In the pa-per, the basic concepts of three groups of easily confused Chinese medicinal materials ( schisandrae chinensis fructus and schisandrae sphenantherae fructus, phellodendri cortex and phellodendri amurensis cortex, akebiae caulis and clematidis armandii caulis) were illus-trated and the development of verification methods were reviewed in the respect of modern instrumental analysis technology. With the improvement of analysis technology, the identification of Chinese medicinal materials relies on experimental data instead of artificial ex-perience, which makes it more objective and accurate.
4.Analysis on TCM clinic syndrome of related index of metabolic syndrome in 1231 people

Objective:To probe the influence ofrelated index ofmetabolic syndrome on TCM clinical syndrome.Methods:A statistical factor analysis was made to related index ofmetabolic syndrome in 1 231 random people ofphysical examination.To probe the influence ofsyndrome differentiation offive zang on common factor.Results:The results indicated that there were close relationship between liver and lipid,heart and blood pressure, kidney and fasting plasma glucose.There were neither evident mutuality between abdominal circumference and syndromes offive zang,nor between related index ofmetabolic syndrome and the lung or the spleen.Conclusion:Regulation ofthe liver,heart and kidney in TCM may be the key to prevention and treatment ofmetabolic syndrome.
5.Study on the effectiveness of the self-management model for the aged patients with type 2 diabetes mellitus in communities

Objective To evaluate the effectiveness of chronic diseases self-management model for the aged patients with type 2 diabetes mellitus in communities.Methods Patients with T2DM aged over 60 years and over in two communities in Baoding were selected:one as intervention community and the other as control community.The intervention community underwent the chronic diseases self management theory based education by expert,face to face consultation and patient communication,while the control community received general community management.The scores of self-efficacy were collected before the sessions and 3 months after the end of the sessions,and the effectiveness of the intervention was evaluated by the changes of scores,fasting blood-glucose and hemoglobin A1c (HbA1c) before and after the session.The pure increment level of scores of diabetes self-efficacy was higher in the intervention community than in the control community [(16.9 ± 5.3)scores vs.(0.4±1.3) scores,t=21.317,P<0.01].Results All dimension values were also significantly higher in the intervention group than in control community (P< 0.01).The pure decrement level of fasting blood glucose (1.8 ± 1.7) % and HbA1c (0.7 ± 1.2) % after intervention was significantly higher in the intervention than in control community (t=6.096,P<0.01 ;t=2.930,P<0.05).Conclusions The chronic diseases self-management education may significantly improve patients' self-efficacy and decrease the level of blood-glucose and HbAlc.
6.Applying queuing theory to quantitative analysis on clinical dentists

Objective Queuing theory is the mathematical study of how waiting lines or queues are formed and dissipated o -ver time, which aims to work out the optimal design and optimal control of queuing systems based on the research of probability and regularity of various queuing systems .The aim of paper was to find out the maximum acceptable waiting time for outpatients in stomatol -ogy department and explore the rational allocation of dentists based on queuing theory model . Methods Questionnaires , worktime measurement and queuing theory model were applied to calculate indicators of queuing system in outpatient services of Stomatology De -partment , getting the maximum waiting time accpetable for patients and the reasonable number of dentists . Results The maximum acceptable waiting time for outpatients was 34.02 ±7.07 minutes, and it was reasonable to allocate 25 doctors in the morning and 16 doctors in the afternoon . Conclusion Applying queuing theory helps to optimize dentist number in outpatient services of Stomatology Department and provide scientific reference to improve medical efficiency .
7.The characteristics of cardiac systolic and diastolic function changes in human immunodeficiency virus-infected patients

Objective To understand the changes of cardiac systolic and diastolic function in human immunodeficiency virus (HIV)-infected patients without evidence of cardiac disease in China.Methods Forty-two HIV-infected patients who were followed up in the Department of Infectious Diseases at Peking Union Medical College Hospital without cardiac involvement were recruited.All the HIV-infected patients had received highly active antiroviral therapy (HAART) for more than 12 months with viral suppression.And 30 age and sex matched healthy subjects without cardiac disease manifestations were enrolled as controls.Every group members underwent transthoracic echocardiography evaluation.The indexes of cardiac systolic and diastolic function between HIV-infected patients and healthy controls were compared.Results Diastolic abnormality occurred in 20 cases in HIV-infected group and 6 cases in control group, with statistically significant difference (χ2=5.79, P=0.007).The E wave deceleration time (EDT) in HIV-infected patients were significantly decreased than healthy controls ([161.87±21.64] ms vs.[190.34±37.22], t=-3.20, P=0.002).There were no significant differences of E/A ratio ([1.16±0.35] vs.[1.19±0.26]), E/Ea ratio ([5.43±1.99] vs.[5.78±0.91]), isovolumic relaxation time (IVRT), ([93.18±20.34] ms vs.[93.57±18.55]ms), Ea ([10.18±2.80] cm/s vs.[11.45±2.75] cm/s) between HIV-infected patients and controls (t=1.13,1.53,0.67 and 0.29, respectively, all P>0.05).Among cardiac systolic function markers, left ventricular ejection fractions in HIV-infected patients and control group were (66.7±6.4)% and (68.7±4.2)%, respectively.And left ventricular shortening rates were (37.08±4.79)% and (38.17±3.96)%, respectively.Both showed no significant difference between the two groups (t=-1.51 and-1.00, respectively, both P>0.05).Conclusions Compared with control group, subclinical cardiac diastolic dysfunction is more frequently observed in HIV-infected patients.However, there are no significant differences of cardiac systolic function markers between HIV-infected patients and controls.
9.Maternal and neonatal perinatal outcomes in pregnancies after in vitro fertilization and natural pregnancy: a systematic: a meta analysis

Objective To systematically evaluate maternal perinatal complications and neonatal outcomes after in vitro fertili zation and embryo transplantation(IVF-ET).Methods The computer retrieval was performed on the databases of PubMed,Cochrane library,EMbase,CNKI,Wanfang and Chinese biomedical literature database.The articles on the comparison between IVF and natural pregnancy were included.The RevMan5.3 software was adopted to conduct the meta analysis.Results Twenty-three articles conforming to the requirements were included,including 1 312 651 cases,among them 113 044 cases were IVF and 1 199 607 cases were naturalpregnancy.IVF mother's age was larger than that of natural pregnancy,while the gestational weeks were smaller than those of natural pregnancy,the newborn birth weight was lighter than that of natural pregnancy(P<0.05);the probability of pregnancy-induced hypertension (PIH),placenta previa,postpartum hemorrhage and gestational diabetes mellitus (GDM) occurrence in IVF mother were higher than that of natural pregnancy (P<0.05);the rate of multiple and cesarean section in IVF mothers were higher than that of natural pregnancy (P<0.05);the occurrence rates of small for gestational age(SGA),low birth weight infants,stillbirth and premature in IVF were higher than those of natural pregnancy (P<0.05).Conclusion The maternal perinatal complications in IVF are higher than those of natural pregnancy,and the birth outcomes in IVF are worse than those of natural pregnancy.

China donation after Citizen's death (CDCD) has been organized since 2010,and now has been synchronized with the international organ transplantation.At present,liver transplantation has become the only safe and curative treatment for the end-stage liver diseases.Nevertheless,there is much restriction over further exploration of this technique.This article will mainly focus on donation after brain death (DBD),and summarize the four dominating injuries of donor liver,including the donor's primary injury,cut and perfusion injury,graft preservation injury,and ischemia-reperfusion injury.
