1.Erythropoietin signal pathway is upregulated in mesenchymal stem cell during hypoxia

The multi-potent differentiation capability of mesenchymal stem cells (MSCs) implies potential to achieve patient-specific regenerative therapy for myocardial infarction. However, it is evident that transplanted stem cells do not survive well in the harsh ischemic microenvironment. Erythropoietin (EPO) has been shown to be a potent inhibitor of neuronal and myocardial apoptosis. Here, we investigate the cell viability and the change of erythropoietin signal pathway of MSCs challenged by hypoxia treatment.Methods MSCs were derived from the bone marrow of 2-week-old Wistar rats and expanded. Hypoxia treatment of cells was performed using a ProOx-C-chamber with the oxygen concentration set to 0.5% for the times required.Results The rate of trypan blue staining was 3.5% ± 0.4% in control group, and 3.9% ± 0.2%, 5.0% ±0.5%, 7.1% ± 0.5% in the groups treated with hypoxia for 24, 48, 72, 96 hours. Western blot analysis suggested the expression of EPO in MSCs was significantly upregulated after 48-hour treatment with hypoxia, which was further confirmed by immunofluorescence staining. There was higher and earlier expression of EPOR. And the expression level of total ERK remained constant during the hypoxic treatment. However, the expression of HIF-1α phosphorylated ERK was significantly upregulated after 24-hour treatment with hypoxia and peaked at 72 hours.Conclusions MSCs is not sensitive to hypoxia insult alone. The components in EPO signal pathway (e. g.EPO, EPOR and P-ERK) is upregulated in MSCs after hypoxic treatment, which suggests that EPO signal pathway plays an important role in the hypoxia-tolerance and paracrine protecting capability of MSCs.

4.Clinical analysis of oral and maxillofacial spaces infections in diabetic patients:a report of 31 cases

The characterization and management of oral and maxillofacial spaces infections in diabetic patients were studied in order to determine the pattern of this clinical condition and formulate a management plan.There were 31 cases with average age of 61 years(s=9);the mean hospitalization time was 14 days(s=6);the average fasting blood glucose level on admission was 10.4 mmol/L.Of the 31 patients 20 were multiple-space infections and 11 were single-space infections.13 patients had major complications during admission.Odontogenic infection was the most common cause of the space infections.Streptococcus viridians and staphylococcus aureus were common organisms(5/19,4/19)identified through pus and/or blood cultures.Early surgical incision and drainage,perfect blood glucose control,intravenous antimicrobial therapy,preventing asphyxia and managing major complications are necessary and effective approaches for the management plan.
5.CD4+CD25+FoxP3+Treg in the immune mechanism of severe mycoplasma pneumoniae pneumonia in children

Objective To investigate the role of CD4+CD25+FoxP3+ in severe Mycoplasma pneumonia among children.Methods One hundred and forty children with M.pneumoniae pneumonia (65 severe and 75 non-severe) who were hospitalized were enrolled along with forty other children as controls.X-ray was assessed.The proportions of peripheral blood CD4+CD25+FoxP3+cells were determined by flow cytometry.Results Both severe and non-severe children had decreased CD4+CD25+FoxP3+cells as compared with control subjects in acute phase (0.87 ± 0.66% vs.3.88 ± 2.00%,P < 0.01 and 1.17 ± 0.70% vs.3.88 ±2.00%,P <0.01,respectively).The levels of CD4+CD25+FoxP3+cells in severe children were lower than those in non-severe children in acute phase and recovery phase (0.87 ±0.66% vs.1.17 ±0.70%,P <0.05 and 1.66 ±0.85% vs.3.61 ± 1.45%,P<0.01,respectively).Both severe children and non-severe children expressed higher CD4+CD25+FoxP3+cells in recovery phase than in acute phase (1.66 ± 0.85 % vs.0.87 ± 0.66%,P <0.01 and 3.61 ± 1.45% vs.1.17 ±0.70%,P <0.01,respectively).Conclusion The expression of CD4+CD25+FoxP3+Tregs may play a role in the onset of severity of mycoplasma pneumonia and the low express of CD4+CD25+FoxP3+Tregs in children infected with M.pneumonia may increase the susceptibility to severe mycoplasma pneumonia.
6.Study on Anesthesia Method Improvement for Branchofiberoscope

OBJECTIVE:To compare anesthesia effects of 3 anesthesia methods,to investigate anesthesia improvement method for bronch fiber oscope. METHODS:315 patients undergoing bronch fiber oscope were randomly divided into group A(106 cas-es),group B(104 cases)and group C(105 cases). Group A was treated with traditional bronch fiber oscope lidocaine atomization local anesthesia,group B was treated with modified bronch fiber oscope pethidine combined with lidocaine atomization local anes-thesia and group C was treated with three-limb tube connected with laryngeal mask and target-controlled propofol and remifentanil general anesthesia. SaO2,SBP,DBP,heart rate,breathing rate,excellent anesthesia rate,compliance rate,the rate of bodymove-ment and choking,pain recalling rate were compared among 3 groups before and during examination,and ADR was observed. RE-SULTS:Compared with before examination,SaO2 of group A was decreased significantly during examination,with statistical sig-nificance(P<0.05);that of group B and C had no obvious change before and during examination. Blood pressure,heart rate and breathing rate of 3 groups had no obvious change before and during examination. The excellent anesthesia rate and compliance rate of group A were significantly lower than those of group B and C,but the rate of bodymovement cough and pain recalling rate were significantly higher than group B and C,with statistical significantly(P<0.05). The excellent anesthesia rate and compliance rate of group B were significantly lower than those of group C,but the rate of bodymovement cough and pain recalling rate were signifi-cantly higher than group C,with statistical significance (P<0.05). No ADR was found in 3 groups. CONCLUSIONS:Modified bronchoscope atomization local anesthesia and modified painless bronchoscope are better than traditional bronch fiber oscope atomi-zation local anesthesia in anesthesia effect,safety,degree of comfort and acceptability. Modified bronchoscope atomization local an-esthesia was best but most expensive,so these methods can be chosen according to patient’s condition.
7.The protective effects of tumor necrosis factor receptor 1 deletion on hindlimb ischemia

Objective Signals in the tumor necrosis factor α TNFα pathway, including TNFα and tumor necrosis factor α receptor 1TNFR1, are upregulated after acute hindlimh ischemia. It is unclear, however, whether this upregulation is apathophysiological consequence or a compensatory response. In the present study, the effect of TNFR1 deletion in a mouse model of hindlimb ischemia was investigated.Methods Hindlimb ischemia was produced through ligation of the proximal and distal left femoral artery and its branches in TNFR1 knockout TNFR1-/- and wild type WT mice. Laser Doppler blood flow measurement showed that limb perfusion was significantly higher in TNFR1-/- mice than that in the WT mice at 1 day after operation.Results The calculated ischemic scores were 3.67 ± 0.52 in WT group and 1.83 ± 0.41 in TNFR1-/- group P <0.05. The rate of amputation was 50% in WT mice versus 0% in TNFR1-/- mice. There were less TUNEL positive cells in the calf muscle of TNFR1-/- mice than that of WT mice. Furthermore, DNA fragmentation was more obvious in WT mice.Western blot showed less expression of Bax and cleaved caspase 3 in TNFR1-/- mice 1 day after ischemia, suggesting a reduced apoptotic process in the absence of TNFR1.Conclusion In mice with hindlimb ischemia, knockout of TNFR1 prevents the activation of death-related proteins down streaming to TNF α and attenuated cell death including apoptosis. Systemic block of the TNFR1 might be a new interventional methods-to preserve the limbs from acute ischemic attack in patients with peripheral arterial obstructive disease.
8.Clinical observation on governor vessel Daoqi method for treatment of dyssomnia in the patient of depression.

OBJECTIVETo explore clinical therapeutic effects of Governor Vessel Daoqi needling method combined with antidepressants on dyssomnia in the patient of depression.
METHODSdepression with dyssomnia were randomly divided into a treatment group (n=23) and a control group (n=22). The treatment group were treated with Governor Vessel Daoqi needling method and oral administration of antidepressants, with Shenting (GV 24), Baihui (GV 20), Dazhui (GV 14), Shendao (GV 11) and Zhiyang (GV 9) selected as main acupoints; and the control group with simple antidepressants. They were treated for 4 weeks. Changes of scores for Hamilton Depression Rating Scale (HAMD) and Pittsburgh Sleep Quality Index (PSQI) were investigated in the two groups.
RESULTSSignificant differences before and after treatment in the scores for HAMD (P < 0.01) in both the two groups, and a significant difference before and after treatment in the score for PSQI only in the treatment group (P < 0.01) were found; after treatment, there were significant differences between the two groups in the scores of HAMD and PSQI (P < 0.01).
CONCLUSIONThe combined therapy of Governor Vessel Daoqi needling method and antidepressants can significantly improve dyssomnia in the patient of depression.

9.Expressions of PPAR γ protein during the course of pancreatic fibrosis of chronic pancreatitis in Wistar rats

ObjectiveTo observe the expressions of PPAR-γ protein during the course of pancreatic fibrosis of chronic pancreatitis (CP) in Wistar rats and its significance.Methods Bibutyltin dichloride ( DBTC,8 mg/kg body weight) in a volume of 200 ml solvent was injected into the tail vein to establish the CP rat's model.Wistar rats were randomly divided into control group and 1,3,5,7,14,42 d group according to weights.Pancreatic tissue underwent routine pathological examination,and collagen accumulation in pancreatic sections was determined by staining for Sirius Red.Pancreatic myeloperoxidase (MPO)activity was determined.Expressions of α-SMA and PPAR-γ proteins were assessed by immunohistochemical method.Results Light microscopy showed signs of acute pancreatitis with interstitial edema and infiltration of neutrophilic granulocytes 7 d after DBTC injection.Acinar cells necrosis,atrophy,lymphocytes and monocytes infiltration,fibrosis within lobule and peri-lobule as well as pancreatic duct changes were found,which was in accord with the course from AP to CP.One days after induction,the activity of MPO,expressions of α-SMA was significantly increased[ (0.78 ±0.71) vs (0.15 ±0.05)U/g,6.67 ±3.14 vs 0,P<0.05],then it did not increase with time of induction.Seven days after induction,collagen level reached the peak [ (45.42 ±15.99)% ],which was significantly higher than that in control group [ (10.87 ± 2.28 )%,P < 0.05 ].Collagen fibers accumulated from periductal to intra-acinar and/or inter-acinar areas.In control rats,the expression of PPAR-γ protein was positive only in vessel walls,and the expression level was 0.17±0.41 and increased with time of induction,then reached the peak of 4.83 ± 2.71 at 42 d.ConclusionsDuring the course of pancreatic fibrosis in rats,the expression of PPAR-γ protein is gradually increased,and plays a limited anti-inflammation and anti-fibrosis role.
