Objective To investigate the influence of family nursing intervention on daily life ability of senile dementia patients? Methods Eighty senile dementia patients were equally randomized into two groups: the control group and the observation group? The controls received conventional home nursing instruction and health education and those in the latter group were managed with home nursing intervention? The activity of daily living scale(ADL)was used to assess their daily life ability? Result After nursing intervention,the daily life ability of observation group was better than control group(Z = 18?914,P < 0?05)? Conclusions The family nursing intervention is effective in directing the dementia patients with exercises of daily living ability? Thus it may improve their ability in daily life?
2.Medium and long-term follow-up outcome after arthroscopic repair of type II SLAP lesions with two Loops

Objective To determine the medium-long term functional outcome of arthroscopic repair of type II SLAP lesions with two loops.Methods According to retrospective study,29 cases patients with arthroscopic repairs of isolated type II SLAP lesions were collected from March 2011 to June 2013,the mean patient age at the time of surgery was 31.8 years(range17 to 47 years).All patients with ground anchor suture,and through double loop knot technology to fix SLAP injury.Visual analogue scale(VAS),Kerlan-Jobe Orthopaedic Clinic(KJOC),Roche Shouder function Score(ROWE) were used to determine the function of shoulder joint.The shoulderjoint range of motion before treatment and post treatment were recorded,shoulder joint mobility was recorded,satisfaction survey was done,compared the preoperative and postoperative data to evaluate its clinical effect.Results All patients were available for follow-up of 2.7-5.2 years.Overall,functional outcome was improved from baseline compared with final follow-up for VAS scores((4.3±1.2) points vs.(1.5±0.8) points,t=5.435,P<0.05),KJOC scores((72.1±4.9) points vs.(92.8±5.4) points,t=5.921,P<0.01),and ROWE scores((54.1±12.4) points vs.(84.3±10.2) points,t=7.326,P<0.01).Only a minority of patients feel pain when they did srenuous exercise.According to satisfaction investigation of patients,patient satisfaction was reported in 89.7%,3 cases had not good results,they could not return to previous level of play,including 2 cases were athletes,1 case was not athlete.Conclusion There is no more persistent pain and dysfunction after surgery,a good or excellent medium-long term functional outcome can be anticipated after arthroscopic repair of type II SLAP lesions with two loops.
4.Efficacy of integrated swallowing function rehabilitation training in patients with nasal feeding during acute ischemic stroke

Objective To investigate the effect of integrated swallowing function rehabilitation training on nasal feeding patients with pseudobulbar palsy or bulbar palsy after acute ischemic stroke. Methods A total of 54 patients met the diagnosis criteria were enrolled prospectively. They were divided into either an integrated group (n=31)or a control group (n=23)using a random number table method. The patients of the control group received routine treatment of stroke,including brain protection therapy,improvement of cerebral circulation,dehydration,regulating blood pressure,and symptomatic treatment,etc;in addition to conventional treatment,those of the integrated group also received integrated swallowing function rehabilitation training,including neuromuscular electrical stimulation plus swallowing reflex facilitation technique,and oral facial glossopharyngeal function training,etc. The oral intake function in patients of the integration and pseudobulbar palsy or bulbar palsy (according to MRI and signs)was compared between the two groups. The evaluation index was the Functional Oral Intake Scale (FOIS)score. Results (1)The FOIS score (median 4)of the integrated group was higher than that of the control group (median 2). There was significant difference between the two groups (P<0. 01). There was also significant difference before and after comparison between the two groups. (2)The FOIS score of pseudobulbar palsy in the integrated group was higher than that of the control group (median scores 4 and 2 ). There was significant difference (P<0.01);There was no significant difference in the FOIS scores of bulbar palsy between the two groups,but the FOIS score of the integrated group was higher than that of the control group (median scores 4 and 3). (3)The median differences between the integrated group and the control group were 2 and 1 respectively. The increased amplitude of FOIS of the integrated group was higher than that of the control group. There was significant difference (P<0. 01). Conclusion The integrated swallowing function rehabilitation training can improve the oral intake function,significantly reduce irritating cough,increase the capacity of oral intake,and increase the types of food. The effect of the improvement of pseudobulbar palsy is more significant in the integrated group.
5.Bcr-abl gene positive essential thrombocythemia treated with imatinib: one case and literatures review

Objective To observe the efficacy of imatinib on the treatment of bcr-abl positive essential thrombocythemia (ET). Methods A case of bcr-abl positive ET resistant to hydroxyurea (HU) treating with imatinib (200~400 mg/d) was reported and related literatures were reviewed. Results A case of bcr-abl positive ET was initially treated with 1.5~2.0 g/d HU, the platelet count decreased to 562x109/L after 4 weeks; however, the platelet count increased to (1020~1330)×109/L treating with same dose of HU 16 months later. With the elevation of HU to 3.0 g/d, platelet count was still high(1290~1780)x109/L companied with the very low white blood cell count(0.3~0.9)×109/L. While treating with imatinib (400 mg/d) for 1 month,the platelet count decreased to 390×109/L and white blood cell count was 0.5×109/L; Furthermore, treating with 200×300 mg/d of imatinib, the platelet and white blood cell count recovered in normal after 1 month,and bcr-abl fusion gene negative 2 months later. Conclusion Imatinib may be the effective targeting drug for the bcr-abl positive ET, and the bcr-abl positive ET is sensitive to low dose imatinib.
6.Clinical analysis of rhytidoplasty on 1 028 facial parts by injecting Sunmax

Objective To investigate the short and long term effects of Sunmax injection for the rhytidoplasty on different parts of the fa-cial wrinkles. Methods 1 028 face lift patients were collected from January 2007 to December 2011 who admitted to our plastic surgery de-partment,236 cases of male,792 cases of female,with an average age of 48. 7 (21~72)years old. According to different drug injection sites, all the patients were divided into 5 groups:wrinkles between eyebrows,the forehead wrinkles,nasolabial fold wrinkles, crow's feet,nose wrin-kles. The rhytidoplasty effects of 5 groups a day,1 month,3 months,6 months,9 months after injection were observed and compared. Further-more,the short and long term effects of the Sunmax injections were integrated to analyze. Results After Sunmax injection,a variety of skin wrinkles and sags disappeared;1 month after injection,wrinkles did not appear;3~6 months after injection,the effects was still as the same as the day after injection. 9 months after injection,the effects for the forehead wrinkles and crow's feet was slightly diminished. Conclusion Sunmax injection is an emerging technology to remove facial wrinkles,which has been widelymused inmour department for severalmyears and got patients' postoperative satisfaction. It is worth spreading for rhytidoplasty.
8.Modified trabeculectomy in primary acute angle-closure with high intraocular pressure and extremely dilated pupil

ObjectiveTo determined the outcome of modified trabeculecotomy (with paracentesis,mitomycin C,scleral flap adjustable suture and formation of anterior chamber at the end of the surgery)in the management of primary acute angle-closure (PAAC) with high intraocular pressure (IOP) and extremely dilated pupil. MethodsTwenty-one eyes of 21 successive cases with PAAC with high lOP(IOP > 30mmHg) and large pupil (maximum vertical diameter > 5 mm) were evaluated prospectively. They underwent modified trabeculecotomy between January 2005 and March 2009.The operative success was defined as IOP ≤ 20 mmHg (± medical therapy) without the necessity of further surgery for glaucoma. ResultsSuccess was achieved in all of the eyes(100%)at the postoperative first week.The mean preoperative IOP was (48.25 ± 3.14) mmHg under a mean number of 3.35 antiglaucomotous medications, but it reduced to (10.47 ± 1.15,P < 0.01) mmHg without medication at the first week, (13.86 ± 0.93,P < 0.01) mmHg at postoperative 3rd month respectively.Only 1 eye needed 1 kind of antiglaucomotous eye drop from the 2nd month postoperatively. The mean vertical diameter of pupils was (5.81±0.23) mm preoperatively, (5.92 ±0.21 ) mm at the first week(P > 0.05). No case received section iridectomy. No serious complication was observed.ConclusionsModified trabeculectomy provides reduction of IOP and protection of pupil in cases with PAAC with high IOP and large pupil,and the procedures such as releasing aqueous humor gradually,appliance of scleral flap adjustable sutures and formation of anterior chamber at the end of the surgery can effectively reduce the risk of serious complications.
9.Relationship between EGFR and KRAS mutations and clinicopathologic features of non-small cell lung cancers

Purpose To explore the relationship between the mutations of epidermal growth factor receptor ( EGFR) and KRAS genes and clinicopathological characteristics in patients with non-small cell lung cancers (NSCLC). Methods Clinical samples from 431 NSCLC patients were obtained for EGFR and KRAS gene analysis. PCR based direct DNA sequencing was used to investigate mutations in exon 18-21 of EGFR gene and codon 12 and 13 of exon 2 of KRAS gene. Results The overall EGFR mutation rate of primary NSCLC was 53. 6% (231/431) in this study cohort and eight cases showed double EGFR mutations. Mutation rates in female and male were 65. 2% (122/187) and 46. 9% (98/209), respectively. The mutation rate was higher in patients with non-smokers and adeno-carcinoma and adenosquamous carcinoma subtypes than in their counterparts (P<0. 05), with the percentage of 57. 2% (124/216), 60. 3% (199/330), 42. 9% (6/14), respectively. In squamous cell carcinomas and other subtypes, EGFR mutation rates were 11. 6% (5/43) and 11. 1% (1/9), respectively. The EGFR mutation types included exon 18 point mutations (4. 0%, 9/227), exon 19 deletion mutations (4. 5%, 101/227), exon 20 insert or point mutations (9. 7%, 22/227) and exon 21 point mutations (41. 4%, 94/227). Activating mutations of KRAS gene were detected in 7. 8%(31/396) of NSCLC. Twenty-eight patients showed codon 12 mutations ( G>T, G>A, G>C) , and three patients had codon 13 mutations ( G>A, G>T) . Most of these mutations were G to T transversion (64. 5%, 20/31). Conclusion Polymerase chain reaction-direct sequencing is a reliable and effective method for the detection of the EGFR and KRAS gene mutation in NSCLC patients. The mutation rate of EGFR is higher in Chinese patients, especial-ly in non-smoking female patients with adenocarcinoma.
10.Dietary intake level of elements with Kashin-Beck disease areas in parts of Aba Autonomous Prefecture of Sichuan Province

Objective To investigate and determine the dietary intake level and food source of elements in Kashin-Beck disease(KBD) and non-endemic KBD areas in parts of Aba Autonomous Prefecture , Sichuan Province. Methods Three KBD villages were selected from each county including the counties of Songpan , Jiuzhaigou and Ruoergai in Aba Autonomous Prefecture with multi stage random sampling method in 2010 , and 122 KBD patients who were diagnosed at county-level or higher-level hospitals were selected from villages. One non-KBD village was selected randomly from each county and 43 controls without KBD or other bone and joint disease were selected in proportion to the number of investigated KBD patients. Dietary intake of each object was recorded 24 hours before the survey using a 24 hour dietary survey method and intake of each kind of food(cooked food) was recorded. Three day dietary intake of each surveyed object was recorded in continuous and the average value was calculated. The local resident representative food samples were collected and 20 kinds of elements in these samples were determined with inductively coupled plasma mass spectrometry ( ICP-MS ) and inductively coupled plasma atomic emission spectrometry(ICP-AES), and daily elements intake was calculated and compared with the relevant intake standards. Results Daily intakes of vegetables and meat were 126.72, 28.25 g, respectively, in patients with KBD. More than 3 times of brick tea was drank by KBD patients than non-KBD people (P<0.05). Per capita daily intakes of Ca, Fe, Mg, K, Na, Zn, I, Cu and Co were less than recommended nutrient intake or allowable daily intake in both groups. More than 80% patients with KBD overtook more Al and Pb than tolerable upper intake levels. Conclusion There is a single dietary structure and a serious shortage of a variety of essential elements intake in parts of Aba Autonomous Prefecture, and the dietary intake level of elements is different between two groups.
