1.A Practical Approach to the Patient with Obesity

The prevalence of obesity and obesity-related comorbidities is rising. Primary care physicians are the frontline of healthcare and play a central role in the management of obesity. In this article, we discuss the 5As framework (Ask, Assess, Advise, Agree, and Assist) as a practical framework for obesity counselling, focusing on initiating the conversation and assessing the person with obesity. The assessment includes taking a weight history, excluding secondary causes, understanding lifestyle factors contributing to weight gain and assessing for complications of obesity. This assessment then makes it possible for subsequent patient engagement, including advising, agreeing (goal setting), and assisting the patient on an individualised care plan.
3.Dual Energy CT and its use in neuroangiography.

The dual energy CT (DECT) technology has been recently employed in the form of two X-ray sources of different energies to enhance the contrast between adjacent structures. Its use in the cardiac arena has been widely highlighted due to the higher temporal resolution. However, it may also be used in the craniocervical and peripheral vasculature for better differentiation between contrast-enhanced vascular lumina and calcified plaques, in the characterisation of ureteric stones, and in the evaluation of hepatic lesions. The objective of this paper is to revisit DECT physics, review the literature and discuss its use in CT neuroangiography with case illustrations from our institution, and impact on dose savings.

5.Wide range of normality in deep tendon reflexes in the normal population

Background and objective: Deep tendon reflexes are important physical signs in neurological
examination. Despite being an established technique clinically, there is lack of normality data in the
healthy population, especially among the elderly. This study aims to determine the range of normality
in deep tendon reflexes among the adults. Methods: The study subjects consisted of 176 healthy
volunteers. They were examined by trained assessors using standardized protocol. Results: Among the
commonly elicited deep tendon reflexes, isolated absent reflexes were found in up to 34.4% (supinator)
in the older group, and 12.5% (triceps) in the young adults, significantly higher in the older group.
Symmetrical absent reflexes was seen in up to 26.3% of supinator in the older group. Absent reflexes
of the entire limbs both sides were however, much less common particularly among young adults,
with 6.3% of the older group having absent reflexes in the entire lower limbs, and 2.5% in the entire
upper limbs. Isolated asymmetry in reflex was present in up to 17.0% (triceps). However, only 6.3%
had asymmetrical reflexes in the contiguous anatomical region.
Conclusion: Isolated absent or asymmetry deep tendon reflexes were common particularly in the
elderly.
6.Unravelling the mystery in a case of persistent ACTH-independent Cushing's syndrome.

INTRODUCTIONWe present a rare variety of adrenocorticotrophic hormone (ACTH)-independent Cushing's syndrome known as primary pigmented nodular adrenocortical disease (PPNAD).
CLINICAL PICTUREThe patient initially underwent unilateral adrenalectomy for what was thought to be a left adrenal adenoma.
OUTCOMEPartial resolution of symptoms and demonstrable persistent hypercortisolism after surgery prompted further evaluation with findings leading to the diagnosis of Carney complex. A review of the adrenal histology was consistent with PPNAD.
CONCLUSIONThis entity of PPNAD, which has rarely been reported in Asians, forms part of the Carney complex. The diagnosis may not be simple and straightforward, as illustrated in this patient.

7.Factors influencing late stage of breast cancer at presentation in a district Hospital - Segamat Hospital, Johor

Introduction: In Malaysia, late stage presentation of breast
cancer (stage III or IV) has been a healthcare problem that
varies geographically throughout the country. This study
aims to understand the factors influencing late stage of
breast cancer at presentation among Malaysian women in
Segamat Hospital, Johor, which is a district hospital.
Methods: A retrospective descriptive study was conducted
on secondary data of all newly diagnosed breast cancer
women from 1st August 2011 to 28th February 2014.
Secondary data includes age, ethnicity, marital status, family
history, education level, occupation, presenting symptom,
duration of symptom, tumour size, tumour pathology,
tumour grading, oestrogen, progesterone and HER-2
receptor status were collected and analysed using SPSS
version 20.0.0.
Result: In total, data from 52 women was analysed and two
women were excluded for incompleteness as these women
defaulted. Late stage at presentation was 59.6% of all new
cases (17.3%, stage III and 42.3%, stage IV). The commonest
age group of all women diagnosed with breast cancer was in
the 5th decade. Majority of them were Malay, married and
housewives with no family history of breast cancer. The
statistically significant factors associated with late stage at
presentation include Malay ethnicity (p=0.019), presenting
symptoms other than breast lump (p=0.047), and duration of
breast lump more than 3 months (p=0.009).
Discussion/Conclusion: The study demonstrated presentation
at late stage of breast cancer is a major health concern
among Malaysian women in district hospital. This may be
attributed to different sociocultural beliefs, strong belief in
complementary and alternative medicine, lack of awareness,
and difficult accessibility to healthcare services.

8.Assessment Effects Of Maintenance Therapy On Quality Of Life Of Opiate Abusers

Objective: This study was conducted to assess the effects of Methadone Maintenance Therapy (MMT) and buprenorphine-naloxone Maintenance Therapy (BNX) on the Quality of life (QoL) of opiate abusers. Methods: The QoL status of opioid-dependent patients was assessed using the WHOQOL-BREF
questionnaire. It is a cross-sectional study involving a total of 108 patients who received MMT or BNX therapy in Malaysia from May 2011 to September 2011. Results: A statistically significant difference in the overall QoL and
psychological aspect among patients on MMT was observed. On the contrary, the scores of overall QoL and quality of social relationship for BNX group were higher in patients with lower dosage. Conclusion: The comparison between
patients on high dose MMT and high dose BNX exhibited significant difference in the overall QoL especially in psychological, social relationship and environment domains, with the high dose MMT group having better mean score.

Important bioactive molecules are molecules
that are pharmacologically active derived from natural
sources and through chemical synthesis. Over the years
many of such molecules have been discovered through
bioprospective endeavours. The discovery of taxol from
the pacific yew tree bark that has the ability in stabilising
cellular microtubules represents one of the hallmarks of
success of such endeavours. In recent years, the discovery
process has been aided by the rapid development
of techniques and technologies in chemistry and
biotechnology. The progress in advanced genetics and
computational biology has also transformed the way
hypotheses are formulated as well as the strategies for drug
discovery. Of equal importance is the use of advanced
drug delivery vehicles in enhancing the efficacy and
bioavailability of bioactive molecules. The availability of
suitable animal models for testing and validation is yet
another major determinant in increasing the prospect for
clinical trials of bioactive molecules.
10.Optimizing seizure detection by quantitative EEG in paediatric refractory status epilepticus

Background & Objective: Continuous electroencephalography (cEEG) is valuable in the diagnosis
and management of refractory status epilepticus (RSE) but requires intensive skilled interpretation.
Density spectral array (DSA) is a quantitative analytic tool used to screen cEEG recordings for seizures.
This study aims to determine the optimal amplitude setting and to compare the use of single-averagetrendgraph display and eight-trendgraph display in seizure detection with DSA. Methods: Five excerpts
from pediatric cEEG recordings with RSE were identified. In Phase 1 of the study, each of 4 readers
determined the DSA amplitude setting one most preferred for each excerpt, and marked all seizures
in one excerpt using one’s preferred setting. Inter-rater agreement in seizure detection was measured.
In Phase 2, readers marked all seizures in all excerpts, first using single-average-trendgraph display,
and then using eight-trendgraph display after a wash-out period. Intra-rater agreement in seizure
detection between the two display methods was calculated. Results: In Phase 1, DSA readers’ choice
of preferred amplitude settings varied widely but inter-rater agreement in seizure detection was high.
In Phase 2, seizure detection using single- and eight-trendgraph displays showed high agreement with
each other and, where they disagreed, single-average-trendgraph was more sensitive. Additionally,
low seizure-to-background amplitude ratio in EEG recordings was associated with worse detection
sensitivity/specificity.
Conclusions: DSA amplitude settings do not affect seizure detection. Single-trendgraph display is
comparable to eight-trendgraph display in screening cEEG for seizures. Seizure detection with DSA
performs better in cEEG recordings with high seizure-to-background amplitude ratio.
