4.Effect of Administrative HDL-C Level on Adverse Cardiovascular Events After Discharge in Elder Patients With ST-segment Elevation Myocardial Infarction

Objective: To investigate the effect of administrative HDL-C (high density lipoprotein cholesterol) level on adverse cardiovascular events after discharge in elder patients with ST-segment elevation myocardial infarction (STEMI).
Methods: A total of 325 STEMI patients treated in our hospital from 2010-04 to 2012-07 were retrospectively studied. According to administrative HDL-C level, the patients were divided into 2 groups as High HDL-C group, n=139 and Low HDL-C group, n=186. The basic and clinical conditions, in-hospital treatment, death and the medication, adverse cardiovascular events during 6 and 12 months follow-up period were compared between 2 groups.
Results: The patients with in-hospital reperfusion therapy was higher in Low HDL-C group, and Low HDL-C group had the higher ratio of emergent PCI treatment, all P<0.05. During follow-up period, High HDL-C group had less patients with β-receptor blocker treatment, less non- fatal MI, adverse cardiovascular events and re-hospitalization, all P<0.05. Cox regression analysis showed that low HDL-C level (HR=0.558, P=0.024) and diabetes mellitus (HR=1.528, P=0.040) were the risk factors of adverse cardiovascular events, while emergent PCI (HR=0.47, P=0.001) was the protective factor in STEMI patients after discharge.
Conclusion: The lower administrative HDL-C level in elder patients with STEMI had the higher risk of adverse cardiovascular events after discharge.
6.Instant effect of moxibustion at different points on electrogastrogram of patients with functional dyspepsia

Objective:To observe different effects of moxibustion at Zusanli (ST 36) and Yanglingquan (GB 34) on electrogastrogram (EGG) of patients with functional dyspepsia (FD). Methods:A total of 65 cases were randomized into a Zusanli (ST 36) group (n=33) and a Yanglingquan (GB 34) group (n=32) by random serial number, to receive moxibustion therapy to the corresponding point respectively. The 8-lead EGG was used before and after treatment to collect EGG parameter of the gastric body, antrum of stomach, lesser curvature and greater curvature, including the dominant frequency, slow wave percentage, average frequency, response area and motility index. Results:After treatment, the dominant frequency and slow wave percentage were improved in nearly the whole stomach area in Zusanli (ST 36) group, showing statistical significances (bothP<0.05); the slow wave percentage and average frequency were improved in stomach body and antrum of stomach in Yanglingquan (GB 34) group, showing statistical significances (bothP<0.05); moxibustion at Zusanli (ST 36) had a significantly higher influence on EGG than Yanglingquan (GB 34) , in which the lifting of slow wave percentage was significantly higher than Yanglingquan (GB 34) group (P<0.05). Conclusion: Moxibustion at Zusanli (ST 36) and Yanglingquan (GB 34) has the effect of adjusting abnormal EGG pattern in FD patients. Compared with Yanglingquan (GB 34), Zusanli (ST 36) has a superior effect in improving EGG in stomach area.
7.Retrospective Analysis on Serological Detection of Hepatitis Viruses in Physical Examination Population in Xi’an During 2009~2014

Objective To investigate retrospective analysis on serological detection of hepatitis viruses in physical examination population.Methods Hepatitis A virus (HAV)IgM,HBsAg,HCV IgG,HDV IgM,HEV IgM and HEV IgG were detected by ELISA assay in physical examination population during 2009~2014.Results The positive rate of HAV IgM was 0.15%(2/1 315)in 1 315 physical examinees and that of HBsAg was 2.83% (3 360/109 965)in 109 965 ones during 2009~2014. The positive rates of HBsAg decreased trend in the last 6 years in general (χ2 =63.070,P =0.001).There was no signifi-cant difference for HBsAg positive rate among male physical examinees in last six years (χ2 =4.804,P =0.441),but the positive rate of HBsAg in female population decreased trend in the last 6 years in general (χ2 =18.046,P =0.003),moreo-ver the positive rate of HBsAg in male popolation was significantly higher than that in female ones (χ2 =126.9,P =0.000). The positive rate of HCV IgG was 0.79% (333/41 898)in 41 898 physical examinees during 2009~2014 and that decreased trend in last 6 years (χ2 = 18.380,P =0.003).There was no HDV IgM detected in 1 079 physical examinees serum.The positive rate of HEV IgM and IgG were 0.25% (3/1 195)and 3.93% (47/1 195)respectively in 1 195 physical examinees. Conculsion The physical examination population during 2009~2014 were mainly infected by HBV and HCV and the infec-tion rate decreased trendly in last 6 years,which provided important data for epidemiology of serological hepatitis viruses in Shaanxi provice.
8.Progression of moderate stenosis of carotid atherosclerosis:an analysis of influencing factors

Objective To assess the influencing factors of the progression of carotid atherosclerotic stenosis using color Doppler flow imaging (CDFI). Methods From January 2009 to December 2014, a total of data 279 consecutive patients first assessed by CDFI as moderate stenosis of carotid atherosclerosis (stenosis rate 50 -69%)and regularly reexamined with CDFI at 12,24 and 36 months after initial examination were enrolled retrospectively. The residual diameter of vascular lesions and the changes of hemodynamic parameters were documented,and they were divided into either a progression group (n = 40)or a non-progression group (n = 239,and the non-progression group was divided into steady group[n = 210]and improved group [n = 29])according to whether the degree of stenosis progressed into severe stenosis (stenosis rate 70 -99%)or occlusion. The effects of the risk factors for common cerebrovascular disease and taking lipid lowering drugs (atorvastatin 20 mg/ d)on stenosis progression were compared in patients between the 2 groups. There were significant differences in hypertension,smoking and the regular use of atorvastatin . The effects of those factors on the progression of carotid stenosis were compared further through Logistic regression analysis. Results The residual vascular diameters of stenosis at 24,and 36 months were reduced obviously in the progression group compared with those of the non-progression group. There was significant difference (all P < 0. 05),and both the stenotic sites and distal peak systolic flow velocity ratio were significantly higher than those of the steady group and improved group (all P < 0. 05). Among the risk factors for cerebrovascular disease,hypertension (OR,2. 686,95% CI 1. 120 -6. 442,P = 0. 027)and smoking (OR,2. 265,95% CI 1. 081 -4. 746,P = 0. 030)were the major risk factors for affecting the progression of carotid stenosis. Regularly taking atorvastatin was a protective factor of delaying the progression of carotid stenosis (OR,0. 383,95% CI 0. 178 -0. 827,P = 0. 015). Conclusions CDFI may objectively evaluate the progression of carotid stenosis. Smoking and hypertension are the independent risk factors for affecting the progression of carotid stenosis,and regularly taking atorvastatin contributes to delay the progression of carotid stenosis.
9.Content Determination of Phosphatidylcholine in Eustachian Tube Lavage Fluid of Patients with Secretory Otitis Media by HPLC

OBJECTIVE:To establish a method for content determination of phosphatidylcholine in eustachian tube lavage fluid of patients with secretory otitis media. METHODS:HPLC was used. The samples were pretreated by liquid-liquid extraction. It was performed on the column of Hypersil CN with mobile phase of acetoneitril-methanol-phosphoric acid (100∶10∶0.6,V/V/V)at the flow rate of 1.8 ml/min,the detection wavelength was 205 nm,temperature was 30 ℃ and volume was 20 μl. RESULTS:The lin-ear range of phosphatidylcholine was 11.99-119.9 μg/ml(r=0.999 6);RSDs of precision tests of intra-day and inter-day were no more than 15%;average recovery was 97.54%(RSD=9.36%,n=9);the average content of phosphatidylcholine in eustachian tube lavage fluid of patients was(24.43±3.61)μg/ml. CONCLUSIONS:The method is simple and accurate,and can be used for the content determination of phosphatidylcholine in eustachian tube lavage fluid of patients with secretory otitis media.
10.Correlations of ABCA1 with foam cells formation in renal interstitium, tubulointerstitial lesion and serum cholesterol

Objective To investigate the correlations of ATP-binding cassette transporter A1 (ABCA1) protein expression in renal interstitium with foam cells formation,tubulointerstitial lesion and serum cholesterol in glomerulonephropathy.Methods Five patients with Alport syndrome,28 patients with membranous proliferative glomerular nephritis (MPGN),35 patients with focal segmental glomerulosclerosis (FSGS),36 patients with idiopathic membranous nephropathy (IMN) and 34 patients with IgA nephropathy (IgAN) were enrolled in the study.Expression of ABCA1 protein was detected by immunohistochemical method.The relative area of foam cells in renal interstitium was semi-quantified and the correlations of ABCA1 protein expression in renal interstitium (except for renal tubules) with the relative area of foam cells,integration of tubulointerstitial lesion (TIL) and serum cholesterol were examined using t test and linear correlation analysis.Results In renal interstitium,monocyte-macrophages,foam cells and renal tubular epithelial cells expressed ABCA1 protein.ABCA1 protein expression in renal interstitium with foam cells infiltration was significantly higher as compared to that without foam cells infiltration (P< 0.05),but there was no correlation between ABCA1 expression and foam cells area (P>0.05).In MPGN,FSGS,IMN patients,there was a positive correlation between ABCA1 protein expression in renal interstitium and integration of TIL (P<0.05).There was a positive correlation between serum cholesterol level and ABCA1 protein expression in renal interstitium (P<0.05).Conclusions In glomerulonephropathy,cholesterol may promote the ABCA1 expression in renal interstitium.ABCA1 may have double-effects on transportation of cholesterol.ABCA1 may take part in the process of tubulointerstitial lesion.
