2.Role of Fas/Fas ligand apoptotic pathway of peripheral blood lymphocytes in the development of viral myocarditis

Objective To investigate the role of peripheral blood lymphocyte apoptosis through Fas/ Fas ligand pathway in the development of viral myocarditis. Methods Twenty-five viral myocarditis (VMC) and 25 healthy children were chosen for study. The apoptosis percentage in peripheral blood iymphocytes was quantitatively examined, flow cytometry with Annexin V/PI double parameter was used. And the expressions of Fas and Fasl protein in peripheral blood lymphocytes were analyzed with flow cytometry. Results The percentage of apoptosis in peripheral blood lymphocytes in children with VMC [(0. 6976 ± 0. 1109)%] was significantly higher than that of healthy children [(0. 1288 ± 0. 1069) %] (t = 18.459, P < 0.001). Moreover, the apoptosis cell percentage showed a significant positive correlation with the serum levels of myocardium enzyme (creatine kinase-MB, lactate dehydrogenase, creatine kinase, hydroxybutyrate dehydrogenase)(r =0. 817 ,P <0. 01 ;r =0. 785 ,P <0.01 ;r =0. 726 ,P <0. 01 ;r =0. 712 ,P <0. 01). The expression of Fas and Fasl protein in VMC patients(2. 8804 ± 0. 3020,4. 4496 ± 0. 4836) were higher than those of health children(2. 1300 ±0. 6200,0. 1238 ±0. 0384) ,which had significant difference(t = 17. 321 ,P <0. 001 ;t' =44. 577, P < 0. 001). The apoptosis percentage of peripheral blood lymphocytes in children with VMC showed a significant positive correlation with the Fas and Fasl protein expression in peripheral blood lymphocytes(r =0. 972, P < 0. 01; r = 0. 958, P < 0. 01). Conclusion Apoptosis percentage of peripheral blood lymphocytes increases among children with VMC, probably because immune response decreased or it is related with the degree of myocardium damaged. Fas and Fasl play an important role in apoptosis of peripheral blood lymphocytes in children with VMC.

4.Multilevel analysis on influencing factors regarding blood level among elderly hypertensive patients at the community level

Objective To explore the factors affecting blood level of elderly hypertensive patients, using a multilevel analysis model. Methods 927 elderly hypertensive patients from 23 communities were studied, through a multi-stage random sampling method. The influencing factors on systolic blood pressure (SBP) and diastolic blood pressure (DBP) were analyzed through a two-level linear multilevel model, respectively. Results The average blood pressure of subjects appeared as: SBP (139.2±11.7) mm Hg ( 1 mm Hg=0.133 kPa), DBP (85.6±8.6) mm Hg. Ratio of physician versus patients was the factor affecting blood level of subjects from the community level. SBP and DBP of the subjects from the higher physician/patient ratio communities were 3.86 mm Hg and 2.51 mm Hg, lower than the subjects in the lower ratio communities, after controlling the other related factors. Age, gender,overweight/obesity were the individual risk factors of hypertension, while factors as regularl medicine taking, reducing salt intake and related self-efficacy to manage disease could reduce the blood pressure.Reducing salt intake could lower the SBP for 2.44 mm Hg and DBP for 2.03 mm Hg, after controlling the other factors. Conclusion Multilevel analysis model could effectively analyze the hierarchically structured data while both factors from the community and individual levels could affect the blood level among elderly patients with hypertension.
5.Correlations of ABCA1 with foam cells formation in renal interstitium, tubulointerstitial lesion and serum cholesterol

Objective To investigate the correlations of ATP-binding cassette transporter A1 (ABCA1) protein expression in renal interstitium with foam cells formation,tubulointerstitial lesion and serum cholesterol in glomerulonephropathy.Methods Five patients with Alport syndrome,28 patients with membranous proliferative glomerular nephritis (MPGN),35 patients with focal segmental glomerulosclerosis (FSGS),36 patients with idiopathic membranous nephropathy (IMN) and 34 patients with IgA nephropathy (IgAN) were enrolled in the study.Expression of ABCA1 protein was detected by immunohistochemical method.The relative area of foam cells in renal interstitium was semi-quantified and the correlations of ABCA1 protein expression in renal interstitium (except for renal tubules) with the relative area of foam cells,integration of tubulointerstitial lesion (TIL) and serum cholesterol were examined using t test and linear correlation analysis.Results In renal interstitium,monocyte-macrophages,foam cells and renal tubular epithelial cells expressed ABCA1 protein.ABCA1 protein expression in renal interstitium with foam cells infiltration was significantly higher as compared to that without foam cells infiltration (P< 0.05),but there was no correlation between ABCA1 expression and foam cells area (P>0.05).In MPGN,FSGS,IMN patients,there was a positive correlation between ABCA1 protein expression in renal interstitium and integration of TIL (P<0.05).There was a positive correlation between serum cholesterol level and ABCA1 protein expression in renal interstitium (P<0.05).Conclusions In glomerulonephropathy,cholesterol may promote the ABCA1 expression in renal interstitium.ABCA1 may have double-effects on transportation of cholesterol.ABCA1 may take part in the process of tubulointerstitial lesion.
6.Significance of renal interstitial foam cells in patients with glomerular disease

Purpose To observe the pathologic types of glomerular diseases which have high renal interstitial foam cells infiltration and to evaluate the relationship between infiltration of foam cells (FC) in renal interstitial tissue and pathologic parameters.Methods A total of 2 862 patients who had received renal biopsy were enrolled in this study. Patients with Alport syndrome (AS,n=5), membranous proliferative glomerular nephritis (MPGN,n=28), focal segmental glomerulosclerosis (FSGS,n=144), idiopathic membranous nephropathy (IMN,n=132), IgA nephropathy (IgAN,n=893) were divided into two groups:FC+ group with foam cells and FC- group without foam cells.Results Infiltration of foam cells in renal interstitial tissue was commonly seen in AS.The frequency of interstitial foam cells was 46.43% in MPGN, 20.14% in FSGS, 13.64% in IMN, and 6.27% in IgAN. It was found that the segmental glomerular sclerosis and interstitial fibrosis were more severe in FC+ group than that in FC- group.Conclusions The renal interstitial foam cells are most common in patients with AS, but also seen in patients with MPGN, FSGS, IMN and IgAN. There might be a relationship between glomerular sclerosis, interstitial fibrosis and infiltration of foam cells. The present of foam cells in the renal interstitial tissue may contribute to the progression of renal diseases.

9.Application of the calculated electrophysiological parameters in early diagnosis of carpal tunnel syndrome

Objective: To investigate the application of the calculated electrophysiological parameters in early
diagnosis of carpal tunnel syndrome (CTS). Methods: 44 patients (60 hands) with a diagnosis of
CTS and 31 healthy volunteers (44 hands) were enrolled. Acquired indicators include median nerve
distal motor latency (DML), complex muscle action potential (CMAP) amplitude, conduction velocity
(MCV), median nerve sensory nerve action potential (SNAP) amplitude, and conduction velocity
(SCV). Then the terminal latency index (TLI), the residual latency (RL), and the difference in peak
sensory latencies between the median and ulnar nerves (ΔPSL) were calculated. Results: The two
groups were matched in age and gender distribution. The CTS group showed significant difference
in SCV, DML, SNAP, and CMAP compared with the control group. The sensitivity, specificity, cutoff value, Youden index, and area under the curve of each indicator are respectively as follows: TLI
(0.733, 0.932, 0.622, 0.629, and 0.877), RL (0.750, 0.977, 1.334, 0.727, and 0.907), ΔPSL (0.950,
0.841, 0.150, 0.791, and 0.942), SCV (0.950, 0.796, 56.5, 0.746, and 0.946), DML (0.867, 0.932,
3.55, 0.799, and 0.930), SNAP (0.683, 0.932, 21.68, 0.615, and 0.844), and CMAP (0.683, 0.773,
8.76, 0.456, and 0.758).
Conclusion: The calculated electrophysiological parameters have higher sensitivities and specificities
relative to a single electrophysiological parameter, which could greatly improve the accuracy of early
diagnosis of CTS.
10.Application and prospect of mitotherapy in mitochondria-related diseases

Mitochondria is an important organelle in mammalian cells with multiple functions,such as ener-gy production and cell homeostasis maintaining. It is known that hundreds of diseases are associated with mi-tochondrial defects. The studies show that the exoge-nous mitochondria can directly enter mammalian cells in vitro, and they also can quickly transform into ani-mal tissues by local or intravenous injection. Current-ly, it has raised a new therapeutic strategy for mito-chondrial diseases, called mitotherapy, which trans-plants exogenous functional mitochondria into mito-chondria-defective cells. The mitochondria in recipient cells play their own roles, including energy produc-tion,maintaining free radical balance,and cell viabili-ty recovery. Since there is no effective method for mito-chondria-related diseases up to now, the mitotherapy will provide a new approach for the prevention and treatment of these diseases.
