1.Nutrition risk screening of ICU patients

It's known that ICU patients is in critical condition and might deteriorate rapidly,so it's important for them to get appropriate nutritional support in order to recover.We must make proper nutrition treatment plan for patients according to their different nutritional condition and potential risk.Nutritional risk screening can not only assess the nutritional status of children,is more likely to predict its potential nutritional risk,to guide nutritional support treatment plan,and ultimately improve the prognosis.Looking out for comprehensive,simple,fast,and easy nutritional risk screening tools as soon as possible and appropriate nutrition risk screening is significantly for growth and development as well as prognosis of ICU patients.
2.Expression and characteristic distribution of gamma-aminobutyric acid and its receptors in submandibular gland of the rat

Objective:To detect the expression of gamma-aminobutyric acid(GABA)and GABA receptors in the epithelial cells of rat submandibular gland. Methods:Immunocytochemical expression of GABA and GABA receptors in submandibular gland of SD rat was investigated by EnVision techniques.Furthermore,the distribution pattern and the functional implications of GABA system were assessed.Results:Immunoreactive GABA was detected in both acinar and ductal cells in the submandibular gland of SD rat.Strong staining for GABA was localized mainly in the cytoplasm near the basement side of epithelial cells of the excretory and striated ducts.GABA-A receptor a1 subunit was positively expressed in the acinar and ductal cells,while GABA-B receptor R1 and R2 subunit was only slight expressed in the epithelial cells of the striated ducts.However,no staining for GABA-A receptor b2 subunit was found in the submandibular gland of SD rat. Conclusion:The expression of GABA and its receptors in the epithelial cells of rat submandibular gland suggests that GABA system might be involved in the secretion and/or excretion of saliva in the rat.
3.Efficacy of 23 - gauge vitrectomy cutter replaeing scissors in conventional 20 -gauge pars plana vitrectomy for severe PDR

AIM: To determine whether the 23 - gauge ( 23G ) vitrecomy cutter could replace scissors in conventional 20-gauge ( 20G ) pars plana vitrectomy for treating severe proliferative diabetic retinopathy ( PDR) .
METHODS:Non-comparative interventional case series. Totally 27 eyes of 27 patients with PDR stageⅥ confirmed by funduscopy and B-ultrasound scan were enrolled. They underwent 20G vitrectomy, in which 23G vitrectomy cutter replaced scissors to remove neuvascular membrane. All 27 eyes received complete panretinal photocoagulation, 17 eyes received no tamponade, 6 eyes were 12% C3 F8 tamponade, 4 eyes were filled with silicone oil. The follow up time was 3mo. The operation duration time, iatrogenic retinal tear and retinal bleeding need electric coagulation, best corrected visual acuity ( BCVA) , retinal reattachment were analyzed.
RESULTS: The operative time was 35- 120 ( average 79-19±29. 82) min; intraoperative iatrogenic retinal breaks were detected in 2 eyes (7%). At the end of 3mo follow up, BCVA>0. 1 were in 9 eyes, from 0. 05-0. 1 in 10 eyes,<0. 05 in 8 eyes. Retinal reattached in 25 eyes (93%), still detached in other 2 eyes with silicone oil.
CONCLUSION: The 23G vitrectomy cutter could replace scissors in conventional 20G pars plana vitrectomy for treating severe PDR.
6.Determinants of plasma B-type natriuretic peptide levels in patients with chronic atrial fibrillation

ObjectiveTo assess the factors influencing plasma B-type natriuretic peptide(BNP) levelsin patients with chronic atrial fibrillation (AF).MethodsClinical and echocardiographic information weredetected or collected.Level of plasma BNP were measured through immunofluorescence quantitified method in222 CAF patients.The patients were divided into two groups according to left ventricular ejection fraction(LVEF) :LVEF ≥50% (normal group) and LVEF < 50% (case groups) .LVEF was detected by transthoracicechocardiography.Fasting Blood Glucose, total cholesterol, triglycerides, highdensity lipoprotein cholesterol, low density lipoprotein cholesterol, creatinine, blood urea nitrogen, uric acid, were measured, and history ofhypertension, diabetes mellitus and smoking, were recorded.ResultsSerum levels of BNP in patients of chronicAF with LVEF < 50% were significantly higher than those with LVEF≥50% (Mean Rank, 158.00 and 87.78 ,Z=-7.705, P < 0.001) .In multiple logistic regression analysis, male gender (OR: 13.115,95% CI 3.382 to50.860,P <0.001),history of hypertension(OR:3.710,95% CI 1.104 to 12.472 ,P = 0.034) ,lower TC(OR: 0.397,95% CI 0.214 to 0.735, P = 0.003) , LVEF(OR :0.791,95 % CI 0.723 to 0.867, P < 0.001) , history ofsmoking(OR: 4.660,95% CI 1.256 to 17.281, P = 0.021)were significant and independent determinants ofBNP elevation.ConclusionLower LVEF, male gender, history of hypertension and smoking, lower plasma TCare independent predictors of plasma BNP levels in patients with chronic AF.
7.Association of TNF-? gene polymorphism with Graves′ ophthalmopathy

Objective:To investigate the association of tumour necrosis factor- ? (TNFs-?) gene polymorphism with Graves ophthalmopathy (GO)Chinese in Shandong . Methods: Allele and genotype frequencies were determined by polymerase chain reaction with sequence specific primers(PCR-SSP). Results: Allele and genotype frequencies of TNF-? +488、-308 were not different significantly between Graves′ disease(GD) and control, or GD subgroups with and without GO. There were also no significant differences between GO subgroups with different severity. Conclusion: There are no associations of TNF-? +488、–308 polymorphisms with GO susceptibility in Shandong Chinese.
8.The tention-free repair of abdominal incision hernia with polypropylene mesh

Objective To evaluate the effect of the tention free repair on patient with abdominal insicion hernia. Methods 15 cases of abdominal insicion hernia were reviewed, all of them were repaired with polypropylene mesh. The mesh was overlaid on the defect, continuously monofilament nonabsorbable suture was used to fix the mesh on the edge of the defect and the mesh must be overlapped 2 cm on fascia abdominal layer and fixed by suture. Prophylactic antibiotics was used routinely in single dose. Result No relapse was found in follow up(rang 15~38 month). Conclusion The tension free hernia repair with polypropylene mesh in patient with abdominal incision hernia is a safe and simple operation with minimal postoperative pain.
9.A Clinical Observation on Treating 17 Cases with Osteoporosis by Yi Shen Zhuang Cu He Ji

51 cases of osteoporosis were put on clinical trial in our OPD and randomly assigned to TCM treatment group (N = 17), TCM control group and blank group. The total effective rate in the TCM treatment group obtained was 94. 1% and the significantly effective rate achieved 88. 2%. After the treatment, the density of the osseous tissue increased by 3-4%, main symptoms improved markedly, the serum calcium increased and the urine pyridine phenol decreased. All the findings were superior than that in the rest two groups. This composite is effective and safe (no side reactions) and worth put into large sale production.
10.The distant occult peritoneal metastasis in patients with operable gastric cancer

Objective To evaluate the existence of occult peritoneal metastasis,Douglas′ pouch biopsy was undertaken intraoperatively in patients with gastric cancer. Methods From April 1998 to August 1999, Douglas′ pouch biopsy was performed in 16 cases of gastric cancer during laparotomy in which obvious distant peritoneal metastasis was not found by inspection or palpation.[WT5”HZ] Results Occult peritoneal metastasis in Douglas′s pouch was established in 5 out of the 16 cases,while the invasion depth of primary tumor were all under S stage category and the numbers of regional lymph node metastasis were 7 and up.Conclusion[WT5”BZ] At stages when gastric cancer invaded the visceral serosa, peritoneal biopsy in Douglas′ pouch must be performed as routine procedure in order to detect occult distant peritoneal metastasis.
