1.Evaluation on the effect of Qinshan nuclear power station in normal operational condition on the environmental radioactivity in Shanghai city

Objective To study the effect on the environmental radioactivity level and the health of the residents in Shanghai when Qinshan Nuclear Power Station(QNPS) has been in normal operational condition for 17 years. Methods The field monitoring and the sampling analytical method were used for monitoring the radioactivity level in outdoor environment, drinking water, food and soil. Results The outdoor natural radiation level and the radioactivity level in the sample of the environmental media were on the normal background level. There was no distinct change in the environmental radiation level of Jinshan Area when QNPS was in normal operational condition. The radioactivity level in the environmental media was far lower than the limit values specified by the national standard GB. The effective dose of the outdoor natural external exposure for the residents is about 0.102 mSv/a and the effective dose of the internal exposure from the artificial radionuclides 90Sr and 137Cs in food for the residents is about 0.488 μSv/a. Conclusions There is no effect of the radioactive contamination on the environmental radioactivity level and the health of the residents in Shanghai when QNPS has been in normal operational condition for 17 years.
2.DDA and BCG polysaccharide nucleic acid improved the immunogenicity and protective efficacy of tuberculosis subunit vaccine against Mycobacterium tuberculosis infection in mice

Objective To investigate the adjuvant effect of dimo-thylidioctyl ammonium bromide (DDA) and/or DDA-BCG polysaccharide nucleic acid( BCG-PSN), which was combined with a Mycobacterium tuberculosis fusion protein AMM ( Ag 8 5 B - MPT64190-198 - Mtb8.4 ) to boost BCG primed immunization. Methods DDA with or without BCG PSN was mixed with the fusion protein AMM to construct the boosting vaccine. Mice were immunized with BCG and then boosted twice with AMM formulated with the adjuvant DDA with or without BCG-PSN. PBS or BCG vaccination without boosting was used as control. The humoral and cell-mediated immune responses were analyzed by ELISA and ELISPOT. Moreover, the protective efficacy of BCG prime-AMM subunit vaccine boosting against Mycobacterium tuberculosis infection was analyzed. Results With in vitro stimulation of Ag85B and PPD( purified protein derivative) antigen, the number of IFN-γ secreting cells from the mice boosted twice by AMM/DDA/BCG-PSN and AMM/DDA were higher than BCG and PBS group (P <0.05). The CFU in lungs of mice boosted with AMM/DDA/BCG-PSN was less than that of PBS group(P <0.05), while the CFU of AMM/DDA-boosted mice was less than that of BCG and PBS group(P < 0.05).However, fewer lesions were seen in lungs of mice immunized with BCG alone or BCG-prime-AMM/DDA/BCG-PSN boosting than the other groups. Conclusion DDA is an idea adjuvant for tuberculosis subunit vaccine;BCG-PSN might play a role in alleviating the immunity-mediated pathology.
3.Epigallocatechin gallate suppresses 1-methy1-4-phenyl-pyridinium-induced the damage of oxidative stress in PC12 cells by activating nuclear factor-related factor 2

Objective To investigate the relationship between the neuroprotective effect of epigallocatechin gallate ( EGCG ) for PC12 cells induced by 1-methyl-4-phenyl-pyridinium ( MPP+ ) and activating nuclear factor-related factor 2 ( NRF2 ).Methods Well differentiated PC12 cells treated with MPP+ were used as the in vitro cell models,and PC12 cells were pretreated with different concentrations of EGCG.MTT assay was used to investigate the cell viability.Western blot was used to observe the expression of NRF2 in cells and distribution in the nucleus and the cytoplasm.Real-time PCR was used to observe the antioxidant enzymes,HO-1 and NQO1 mRNA expression.Results Pretreatment of PC12 cells with different concentrations of EG CG for an hour could restore cell viability.Western blot showed that expression of NRF2 in cells treated with MPP+ for 24 hours was increased 148% +5% compared with the control group (t =6.102,P <0.01 ).The level of NRF2 in EGCG pretreated group was 188% + 6% compared with the control group(t =11.172,P <0.01 ).Moreover the NRF2 protein level in the nuclear was also increased.Western blot showed that the NRF2 protein level in the nuclear was 258% +2% compared with the control group (t =21.995,P < 0.01 ).Further research found U 120,an inhibitor of ERK,could inhibit the effect of EGCG.The levels of NRF2 in both samples were 148% ± 15% and 158% ± 1% compared with their respective control groups(t =6.118,8.058,both P <0.01 ).In accordance with the NRF2 data,real-time PCR indicated that the levels of HO-1 and NQO1 mRNA expression increased obviously in the group pretreated with EGCG.Likewise,U120 could also inhibit HO-1 and NQO1 mRNA expression induced by EGCG.Conclusions EGCG can repair oxidative damage to PC12 cells induced by MPP+.The protective effect may be related through the ways to activate ERK-NRF2 and induce downstream of antioxidant enzyme expression,such as HO-1 and NQO1.
4.Value of ~(18)Fluorodexyglucose PET/CT for gastric carcinoma

Objective: To investigate the value of positron emission tomography/computerized tomography(PET/CT) with fluorine-18-labeled fluorodeoxyglucose(18FDG) for gastric carcinoma.Methods: Thirty-two patients(25 males,7 females,aged 31-82 years) suspected of gastric carcinoma underwent whole-body PET/CT after taking in 600 ml of water to distend the gastral cavity.The maximal standard uptake value(SUVmax) of the region of interest(ROI) in PET and the maximum width of the gastric wall in CT were analyzed.Pathological specimens were obtained from all the patients during surgery or gastroscopy.Results: 18FDG PET/CT found gastric carcinoma in 24 of the patients.The rates of positive and negative prediction and the accuracy of PET/CT in the diagnosis of the disease were 92.3%,100% and 93.8%.SUVmax was positively correlated with the maximum width of the gastric wall,but they exhibited no statistically significant differences between the patients with involved lymph nodes and those without.Based on the PET/CT findings,the 24 gastric carcinoma patients were clinically classified as follows: 9 in stage Ⅰ,1 in stage Ⅰ-Ⅱ,3 in stage Ⅱ,1 in stage Ⅲ and 10 in stage Ⅳ.Conclusion: 18FDG PET/CT is highly valuable for gastric carcinoma in its diagnosis,the evaluation of its biological behavior and determination of its treatment strategies.
5.Hypertension control in communities:analysis of implementation of non-pharmaceutical management

Objective To assess effectiveness of implementation of non-pharmaceutical treatment in a standardized community-based hypertension management program in China.MethodsA protocol of standardized community-based hypertension management was developed based on current guidelines for prevention and treatment of hypertension in China.Physicians in community health-care service centers at the grassroots across China were trained in a standardized way using this protocol during 2005 to 2008,and then the trainees were required to manage hypertensive patients according to the protocol.Hypertensive patients eligible for criteria of inclusion and under management for more than one year were analyzed to observe changes in their behavioral risk factors,pharmaceutical and non-pharmaceutical treatment,and effectiveness of blood pressure lowering.ResultsBy the end of 2008,a total of 29 411 hypertensive patients had been managed for one year according to the protocol and 20 077 patients with complete data (47.1% for male)with mean age of 61±11 years were eligible for analysis.After standardized management of one year,prevalence of smoking,alcohol drinking and proportion of patients who preferred salty diet decreased from 17.7 percent,18.1 percent and 32.8 percent to 8.9 percent,8.7 percent and 15.8 percent,respectively(P<0.01).Srstolic blood pressure(BP)decreased by 11.6[95%confidence interval(CI)10.8-12.4]mm Hg(1 mm Hg=0.133 kPa),13.7(95%CI 13.3-14.1)mm Hg and 15.2(95%CI14.8-15.6)mmHg in the patients with non-pharmaceutical therapy,pharmaceutical therapy and both,respectively;and diastolic BP decreased by 7.5(95%CI6.9-8.1)mm Hg,8.3(95%CI8.1-8.5)mmHg and 8.0(95%CI7.8-8.2)mm Hg in the three groups,respectively.During the one-year period of management,proportion of the patients with appropriate non-pharmaceutical therapy increased continuously.By the end of one-year management,59.5 percent of smoked patients were required to quit their smoking,55.5 percent of alcohol drinkers were required to limit their drinking,52.3 percent of overweight and obese patients were required to reduce their weight and 47.3 percent of patients with physical exercises less than three times a week were required to increase their regular physical activity.However,certain proportion of the patients with risk factors did not receive appropriate non-pharmaceutical therapy.Conclusion Guideline-oriented hypertension management succeeded in lifestyle changes among the patients,but it is a gradual process to its full implementation.
6.CT-guided percutaneous intervention combined with rehabilitation integration treatment for lumbar disc herniation

BACKGROUND:CT-guided percutaneous intervention and rehabilitation techniques are both classic programs for diagnosis and treatment of lumbar disc herniation. Is the combination of CT-guided percutaneous intervention and rehabilitation techniques preferential y used for treatment of lumbar disc herniation?
OBJECTIVE:To evaluate the curative effect of CT-guided interventional injection combined with rehabilitation integration treatment for lumbar disc herniation and to analyze prognostic factors.
METHODS:Eighty-eight patients with lumbar disc herniation were subjected to CT-guided interventional injection combined with rehabilitation integration treatment from May 2010 to May 2013. Injection medicine consisted of betamethasone, tanshinone Ⅱ A sulfonate, neurotropin, normal saline and iohexol. Rehabilitation integration treatment included traction, manipulation, acupuncture, transcutaneous electrical nerve stimulation and thermal magnon. Macnab criteria and Chinese version of Oswestry low back pain disability questionnaire were used to assess the curative effects in comparison with the 112 patients receiving only CT-guided interventional injection that were reported previously. The prognostic factors, such as age, disease course time and herniation type of target segment were testified by correlation analysis.
RESULTS AND CONCLUSION:Total y 77 patients were completely fol owed up for 1 year. There were excellent outcomes in 64 cases, while favorable outcomes in 7 cases, fair outcomes in 5 case, poor outcome in 1 case, with a better outcome rate of 92%.There was a significantly decreased trend in Oswestry disability Index scores at 1, 6, 12 months during the fol ow-ups (P<0.01). No severe complications occurred in al the included patients. The curative effects were improved in term of better outcomes rate compared with the 112 patients receiving only CT-guided interventional injection, but there was no significant difference (P>0.05). The young group (≤ 45 years) had better outcomes than the older group (>45 years;P<0.01). In addition, disease course time and herniation type of target segment were not statistical y significant risk factors predicting clinical results (P>0.05). These findings indicate that CT-guided interventional injection combined with rehabilitation integration treatment could relieve lower back pain and radical leg pain effectively and decrease life disability level.
7.Prevalence and Risk Factors of Mitral Regurgitation in the Population Aged ≥ 35 Years

Objective: To explore the prevalence and risk factors of mitral regurgitation (MR) in the population ≥35 years in China in order to provide prevention reference for high risk crowd. Methods: The residents ≥35 years were taken by a stratified multistage sampling method. General information of crowd was collected by predesigned questionnaire and physical examination including life style, disease history, body weight and height. Echocardiography was conducted, fasting blood glucose and blood lipid levels were measured. Results: A total of 28814 subjects were enrolled. The overall MR detection rate was 18.4%, the detection rate in male and female were both 18.4%. The detection rates of moderate and severe MR were 0.3% in the paticipants at (35-50) years, 0.9% at (51-64) years and 2.2% at ≥65 years. MR prevalence showed an increasing trend with aging. Multivariate Logistic regression analysis indicated that age, systolic blood pressure, urban and rural, district, left atrial front and back diameter, left ventricular end diastolic front and back diameter, left ventricular ejection fraction, stroke, atrial fibrillation and heart failure were the risk factors for MR occurrence. Conclusion: MR detection rate was rather high in China. Specific prevention action should be taken for those with theabove risk factors.
8.Changes of cerebral glucose metabolism assessed by positron emission tomography before and after carotid artery stenting in asymptomatic carotid stenosis

Objective To evaluate the changes of cerebral glucose metabolism before and after carotid artery stenting (CAS) in patients with asymptomatic carotid stenosis.Methods Eleven patients,admitted to and performed CAS in our hospital from March 2010 to December 2010,were enrolled.The patients received PET-CT examination before and 3 months after CAS (tracking by 18F-FDG).The max standardized uptake value (SUVmax) of each artery supplement area was recorded by region of interest (ROI) method.The metabolism of affected carotid artery area was compared before and after CAS,and the affected carotid area and the unaffected one.Results The SUVmax before CAS in the affected carotid area (4.99±0.96) was significantly lower than that of the unaffected one (5.67±1.27,P<0.05); follow-up SUVmax in the affected carotid area (5.83 ±1.10) was significantly increased as compared with that before CAS (P<0.05); follow-up SUVmax in the affected carotid area showed no significant difference as compared with that in the unaffected one after CAS (5.85±1.08,P>0.05).Conclusion The cerebral glucose metabolism of the affected carotid area decreases significantly,which could be improved after CAS.
9.Detection Rates and Influencing Factors of Atrial Septal Defect and Ventricular Septal Defect Among Live Births in Two Cities of East China

Objective: To investigate the detection rates and inlfuencing factors of atrial septal defect (ASD) and ventricular septal defect (VSD) among neonates in two cities of East China and to provide scientiifc basis for the prevention, diagnosis, treatment and monitor of ASD and VSD. Methods: 2100 newborns with gestational age of at least 28 weeks were recruited consecutively from each city between 2013-09 and 2014-11. Data related to ASD and VSD were collected by questionnaires and echocardiographic screening was conducted within 7 days after birth. Results: A total of 4152 neonateswere examined with gestational age of (39.03 ± 1.29) weeks, among whom 2189 were male infants (52.72%), and age of mother was (26.32 ± 4.10) years old. Detection rates of ASD and VSD were 60.5‰ and 12.8‰ respectively, showing no significant difference between genders (P>0.05). Multivariate logistic regression analysis showed that maternal pre-pregnancy BMI and home decoration were the inlfuencing factors of ASD and maternal drug use in early pregnancy was the inlfuencing factor of VSD among newborns. Conclusions: Detection rates of ASD and VSD among neonates were relatively high in two cities of East China. Early screening is importtant to reduce the incidence of ASD and VSD and improve the prognosis.
10. Multiple free homologous superficial peroneal artery perforator flaps of crus for repair of multiple hand wounds

Objective:
To explore the effects of multiple free homologous superficial peroneal artery perforator flaps of crus for repair of multiple hand wounds.
Methods:
From November 2017 to December 2018, eight cases with eighteen hand wounds were hospitalized in our unit. Among them, wounds were distributed in the forefinger and middle finger in four cases, wounds were distributed in the middle finger and ring finger in two cases, wounds were distributed in the forefinger, middle finger, and ring finger in one case, and wounds were distributed in the middle finger, ring finger, and little finger in one case. The area of skin defect ranged from 1.5 cm×0.8 cm to 4.0 cm×3.0 cm. There were 4 males and 4 females, aged 34-62 years. Wounds of six cases were repaired by two free superficial peroneal artery perforator flaps from homolateral crus, and those of two cases were repaired by three free superficial peroneal artery perforator flaps from homolateral crus. Superficial peroneal artery and its accompanying vein of flap were anastomosed by end to end with digital artery and palmar or dorsal subcutaneous vein of recipient site during the operation. The area of flap ranged from 2.5 cm×1.2 cm to 5.0 cm×4.0 cm. No nerve was harvested during the operation, and donor site was sutured directly. The survival of the flaps and the healing of donor sites were recorded. During follow-up, the recovery of donor and recipient sites was observed.
Results:
All flaps survived well, donor site healed well. No vascular crisis occurred. Follow-up for 4 to 12 months showed that the appearance of flap was satisfactory with good color, texture, elasticity, and function. Protective sensation of recipient site was recovered. Five months after operation, flap of finger pulp in one case was swollen slightly with two-points discrimination of 10 mm, which received the thinning surgery. Obvious scar formation was not observed in donor site of crus. The appearance of the donor site was good without functional damage.
Conclusions
The application of multiple free homologous superficial peroneal artery perforator flaps of crus to repair the multiple hand wounds has advantages of easy acquisition, easy operation, little effect on donor sites, and satisfactory clinical effects.
