1.Exposure to environmental hexachlorocyclohexane (HCH) and dichlorodiphenyltrichloroethane (DDT) among rural children in north eastern China.

OBJECTIVETo assess HCH and DDT exposure levels and associated risk factors among 262 children aged 6-10 years in a northeastern rural area of China between April and May of 2008.
METHODSEight HCH and DDT metabolites in serum samples were monitored by gas chromatography. A questionnaire was administered to identify the sources of pesticides in children' serum samples.
RESULTSAt least one pesticide metabolite was detected in 81.7% of the tested children. Higher amounts of pp'DDD were detected in 50% of them. Children's age and their father's occupation as farmers, together with not changing work clothes after work, were the main risk factors for HCH and DDT exposure among them.
CONCLUSIONChildren living in rural areas are experiencing multiple sources of organochlorine pesticide exposure. These pesticides may have been retained in the environment for a long period of time.

2.Susceptibility of cockroaches (Blattella germanica Linneaus) to various insecticides.

During the period from May to October 1969, the authors carried out a series of tests with the aim to ascertain the present status of susceptibility of adult cockraoches to various insecticides. The tests on the susceptibility of cockroaches to dieldrin, D.D.V.P., Diazinon; Dibrom, Malathion and lindane were performed with the topical application and the residual film methods under the conditions of 26 degrees C and 80% of relative humidity. The insects tested were collected from Seoul City and were reared at the laboratory. Some of the important results so far obtained can be summarized as follows: The median lethal doses (LD(50)) of the five insecticides against female adult cockroaches were: 0.4 ug/roach or 4.02 ug/body weight (gm) by Dieldrin, 0.29/roach or 2.88 ug/body weight (gm) by D.D.V.P., 0.29 ug/roach or 2.96 ug/body weight (gm) by Diazinon, 0.54 ug/roach or 5.37 ug/body weight (gm) by Dibrom, and 2.96 ug/roach or 29.60 ug/body weight (gm) by Malathion. And those against male adult cockroaches were: 0.18 ug/roach or 3.29 ug/body weight (gm) by Dieldrin, 0.27 ug/roach or 4.89 ug/body weight (gm) by D.D.V.P., 0.16 ug/roach or 2.91 ug/body weight (gm) by Diazinon, 0.49 ug/roach or 8.97 ug/body weight (gm) by Dibrom, and 1.32 ug/roach or 24.19 ug/body weight (gm) by Malathion. The level of susceptibility of adult cockroaches to the insecticides tested was 1.0-1.6 times higher as compared with the results obtained on the Japanese Takashi strain. The median knockdown times (LT(50)) of the female adult cockroaches by insectcides at 1% concentration of 3 mg per square centimetre were 1.82 hours by Malathion, 1.36 hours by Diazinon, 0.56 hours by Lindane, 0.50 hours by Dibrom, and 2.56 hours by D.D.V.P. A marked difference was seen to exist between the males and the females. The female cockroaches were more resistant than the male to Dieldrin, Diazinon and Malathion.

3.The mortality effect of insecticides to Bulimus striatulus.

Ever since the first intermediate host of Clonorchis sinensis was identified with Bulimus striatulus, it has been place to an important position in epidemics. One way to prevent Clonorchis sinensis is to exterminate Bulimus, which is itself the first intermediate host and there by to separate the life cycle of Clonorchis. In killing B. striatulus, nicotine sulfate and lindane have been chosen from insecticides which are widely used in farming areas. And then nicotine sulfate and lindane have been used to check their effectiveness in killing B. striatulus. In this experiment, the resistance of cercaria which parasites to B. striatulus has also been studied. Dipping method was used in the study. Nicotine sulfate and lindane have been used to check the mortality effect. Nicotine sulfate was used to check percentage of mortality in varied times. The existance of cercaria was tried only in nicotine sulfate. In the experiment of mortality effect of insecticides to B. striatulus and cercaria, the difference in killing rate and the resistance in different concentration and different length of time have been researched. It resulted as following: In the experiment with nicotine sulfate, the mortality increased with thicker concentration in the constant length of time. When compared the necessary liquid of nicotine sulfate and lindane in LD 50, nicotine sulfate was less used than lindane, but the mortality proved high. In the treatment to the same concentration of nicotine sulfate in different length of time, it proved that the longer period showed higher mortality. The mortality time required in LD 90 of nicotine sulfate was dependent on the concentration. And thicker concentration showed quicker effect. In the resistance of cercaria and B. striatulus to nicotine sulfate, the resistance of cercaria was proved to be stronger than that of B. striatulus.

4.Scabies in a 2-month-old Infant Successfully Treated with Lindane.

Diagnosis of scabies in young children can be challenging since the morphology and distribution of skin lesions may differ from adults. Therefore, clinicians should keep scabies in mind in their differential diagnosis in a child who presents with severe pruritic, polymorphic skin lesions. Regarding the treatment of scabies, the reported clinical experience with gamma benzene hexachloride (lindane) in young children is quite limited because of its neurotoxicity. However, a recent review suggests that lindane is an excellent alternative drug with minimal risk. We report the case of a 2-month-old male infant with pruritic, erythematous macules, papules, nodules, vesicles, and pustules from the top of the head to the tip of the toes. Initially, he was thought to have impetigo and antibiotics were prescribed. After obtaining a careful history and with the use of skin scraping, he was diagnosed with scabies. He was successfully treated with lindane with no adverse reactions.

5.A survey on the resistance of Vietnam fleas to some insecticides.

The results of the tests carried out in 1971 strongly suggested that a considerable portion of the flea population in MR 2 had developed resistance not only against DDT but also malathion and dieldrin. There is also the possibility of the fleas developing a resistance to diazinon and lindane. Thus careful consideration should be given to the continuous use of diazinon. In areas where fleas had developed a resistance to malathion, further tests should be conducted continuously to confirm the possible development of resistance. Other insecticides which have not been used in Vietnam for any purpose should be ready to be substituted, if the development of resistance is confirmed.

6.In vitro pediculicidal efficacy and acute dermal irritation tests of hair cream from the methanolic extract of the stem of Tinospora crispa (L.) hook. f. et Thoms.

OBJECTIVE: To determine the in vitro pediculicidal efficacy and acute dermal irritation in rabbits of hair cream prepared from the methanolic extract of Tinospora crispa (L.) Hook. f. et Thoms. known in the Philippines as makabuhay.
METHODS: Several concentrations of methanolic extract, an aqueous and alcoholic extract, from makabuhay were tested for in vitro pediculicidal efficacy based on methods established by earlier studies. The 60% concentration of the methanolic extract, found to be the most efficacious, was made into a hair cream and was tested for in vitro pediculicidal efficacy against Kwell® Reformulated Shampoo (Permethrin 1%). The same product was evaluated for acute dermal irritation in rabbits.
RESULTS: The 60% concentration of the methanolic extract formulated into a hair cream showed in vitro pediculicidal efficacy based on stringent criteria of 100%, 96.4%, 85.7%, 78.6%, 67.9%, 64.3% and 64.3% after 30 minutes, 1 hour, 2 hours, 3 hours, 4 hours, 5 hours, and 6 hours of observation, respectively, as compared with the 100% efficacy of Kwell® Reformulated Shampoo (Permethrin 1%). The hair cream passed the dermal irritation test on abraded and intact skin of rabbits.
CONCLUSION: The hair cream from the 60% methanolic extract of makabuhay was shown to possess some in vitro pediculicidal efficacy and was found to be non-irritating. This herbal pediculicide may be an alternative treatment for lice infestation.

7.Studies on the molluscicidal and cercariacidal effects of various pesticides in liver fluke.

Clonorchiasis for which no specific therapy is available has been recognized as one of the serious public health problems in terms of waste of human productivity in Korea today and an urgent consideration must be given to its prevention and control. For past several years, the author has been finding that the infection rate of liver fluke in fish from the Kum-Ho river which has long been recognized as the world's notorious prevalent area of the disease, has been decreasing gradually. Here the author formulated the hypothesis that the decreasing of infection rate might be influenced by the common use of pesticide in plants along the river. Basing this hypothesis, the effective use of pesticide may contribute to the control of the disease by cutting the life cycle of liver fluke. To test above hypothesis, molluscicidal effect on Parafossarulus manchouricus and cercariacidal effect of various pesticides in liver fluke have been determined and the following results were obtained. In molluscicidal effect, the most potent agent in low concentration was Bayer 73 which is 3 to 70 times stronger than other pesticides. When compared with NaPCP which has been widely used for the eradication of Onchomelania, Bayer 73 shows 5 to 10 times stronger effect. The order of pesticide in molluscicidal effect was: Bayer 73, Nicotin, NaPCP, Sevin, Parathion, Copper sulfate and Lindane. In cercariacidal effect, the most potent agent was Parathion which had 2 to 5 times stronger effect than Bayer 73 or NaPCP. The order of pesticide in the cercariacidal effect was: Parathion, Bayer73, NaPCP, Lindane, Nicotin,Sevin, and Copper sulfate. By applying the common used concentration of pesticides considering the toxicity to plants, Bayer 73 is most potent in molluscicidal effect and then Nicotin, NaPCP, Copper sulfate, Sevin, Parathion and Lindane in order. In cercariacidal effect, Parathion is the most potent and NaPCP, Bayer 73, Lindane, and Copper sulfate are followed. Considering with the toxicity to human body the most safe and effective pesticides were Bayer 73, Seven, NaPCP, and Copper sulfate but among those agents, Bayer 73 and NaPCP show relatively stronger effect in both Parafossarulus manchouricus and cercaria of liver fluke.

8.Crotamiton Resistant Scabies.

A case of crotamiton resistant scabies is described in a 21-year-old male patient The scrapings using mineral oil from lesions on the wrist and finger webs showed mites and eggs, typical of scabies. Ten percent crotamitn cream was applied 10 times in one month without any effect. We could not find any source of reinfection in the patient. After above treatment the scrapings still showed mites. and eggs. Retreatment with 1% gamma benzene hexachloride cream was effective in clearing of the skin rashes and itching sensation.

9.Establishment and application of median serum markers for second trimester screening in Qingdao region.

OBJECTIVETo establish the median of serum markers for second trimester screening in Qingdao region and to assess the influence of median correction on the performance of screening.
METHODSMaternal serum alpha-fetoproteins (AFP), human chorionic gonadotrophin, free beta subunit (β -HCG) and unconjugated oestriol (uE3) were assayed for prenatal screening of 18 188 singleton pregnancies at 15-20(+ 6) weeks gestation from January 2009 to July 2010. The median of serum markers was calculated based on above results and applied for risk estimation in screening for fetal aneuploidy from August 2010 to March 2011. The screening performance, specified in terms of detection rates (DRs), false positive rates (FPRs) and odds of being affected given a positive result (OAPR) were compared between the two groups. The risks of 45 affected pregnancies detected during the study were estimated with both Caucasian and corrected medians.
RESULTSThe average level of AFP in local pregnancies was similar to that of the Caucasian population, whilst β -HCG and uE3 were respectively 11% and 33% higher than those of Caucasians. The multiple of median (MoM) value was between 0.94 and 1.02 for the dataset based on the corrected median. At a cut-off of l in 270, FPR has decreased from 5.2% to 4.9%, and DR of Down syndrome has increased from 60% to 69.2%, and OAPR has increased from 1:79 to 1:59 when evaluating risk based on the corrected median. For the 45 affected pregnancies, three Down syndrome pregnancies could be missed because their risk estimates were lower than the cut-off level based on Caucasian median.
CONCLUSIONIt is useful to establish and apply population and laboratory-specific medians in order to improve the performance of prenatal screening and diagnosis.

10.Reemerging skin disease caused by arthropods II: louse.

The aim of this article is to provide guidelines to diagnosis and therapeutics for HLI in Korea. Head lice infestation (HLI) caused by Pediculus humanus capitis is still common in Korean children in spite of an improvement since the 1980s in the country's economic status and the concepts of hygiene. HLI is troublesome and, if not treated, can lead to secondary bacterial infection as a result of scratching of the irritated scalp induced by an inflammatory reaction to injected saliva or anticoagulant. In addition, HLI causes social and psychological problems as well as physical symptoms. Confirmation of the HLI depends on identification of adult lice, nymphs, or viable nits. Nits are more commonly found at the nape of the neck and retroauricular areas. The differential diagnosis includes hair casts, dandruff, and debris by hair spray. Few chemical shampoos such as those containing lindane or pyrethrin are currently available in Korea. Although HLI is an important public health issue in Korea, little information is available on HLI. Medical doctors, school nurses, and teachers should be acquainted with HLI for the successful control of this disease, which is distressing a number of children covertly.
