1.Comparision of compound radix scutellariae gargle with compound chlorhexidine gargle in the treatment of chronic gingivitis

Objective:To compare the clinical effects of gargle fluid containing scutellaria baicalensis with chlorhexidine in the treatment of chronic gingivitis and to observe TNF-α expression in gingival crevicular fluid(GCF) before and after treatment.Methods:90 cases of chronic gingivitis were divided into 3 groups (n =10) based on random number table and treated by scaling and mouth wash with normal saline(group A),chlorhexidine(group B) and scutellaria baicalensis(group C) respectively.PI and GI were measured,TNF-α level in GCF was examined after initial therapy,1 week,2 week and 1 month after treatment.Results:Comprehensive analysis of the whole treatment process showed that there was difference of PI among 3 groups(F =3.506,P =0.034).PI in group B was lower than that in group A and C (P < 0.05;A vs C,P > 0.05);there was difference of GI among the 3 groups (F =6.359,P =0.003).GI in group A was higher than that in group B and C(P <0.05;B vs C,P >0.05),there was difference of TNF-α concentration in GCF among the 3 groups(F =3.780,P =0.027);TNF-α in group A was higher than that in group B and C(P <0.05,B vs C,P >0.05).Conclusion:Radix scutellariae and chlorhexidine gargle fluid are equally effective in the treatment of chronic gigivitis.
2.The antagonistic effect of notoginsenoside-Rg_1 injected in striatum on immunoinflammatory injury in rat model of Parkinson's disease

Objective:To study the effect of notoginsenoside-Rg 1 on immunoinflammatory injury in rat model of Parkinson′s disease.Methods:The rat model of Parkinson′s disease was established and treated with notoginsenoside-Rg 1 by injecting in the striatum.The change of rotational behavior,levels cytokines in striatum and splenocyte proliferation were measured.Results:After treatment with notoginsenoside-Rg 1 the rotational behavior and levels of TNF-?,IL-1?,IL-6 were decreased.The splenocyte proliferation was increased.Conclusion:Notoginsenoside-Rg 1 can regulate the immunological function and reduce the pathological injury to dopaminergic neuron caused by immunoinflammatory reaction.
3.Clinical efficacy of botulinum toxin type A injected by BellaVita in treatment of focal hyperhidrosis

Objective To explore the new way of administration and clinical effect of botulinum toxin A in the treatment of focal hyperhidrosis.Methods The clinical efficacy was observed in 132 sites of 28 patients with focal hyperhidrosis,and the degree and range of focal hyperhidrosis were determined by the minor iodine-starch test.50 U of botulinum toxin A was injected in unilateral axillary,palms and soles with BellaVita instrument and 30 U for forehead.Each patient was followed-up in 1 week,2 weeks and every month after injection for 8 months.According to the results of the minor iodine-starch test the objective effect and evaluation score were obtained,and the comprehensive effect evaluation score was calculated with the objective effect evaluation score and the subjective effect evaluation score in each follow-up.Results The comprehensive effect evaluation score before injection of botulinum toxin A was 1.34±3.94,and that after injection was 23.21±9.44 for 1 week,92.41±11.95 for 1 month,98.21±5.60 for 2 months,95.98±5.94 for 3 months,and 86.61±10.17 for 4 months,respectively.Compared with that before injection,the difference was statistically significant (P <0.05).The effect decreased slowly after 4 months of injection,and the efficacy was maintained for 8 months (4.46±6.98);compared with that before injection,the difference of the clinical efficacy was not statistically significant (P >0.05).Based on the comprehensive effect evaluation scores,the differ ence of the clinical efficacy was not statistically significant between 1 week and 6 months after injection (P>0.05).Conclusions The clinical effect of botulinum toxin A injected by BellaVita is prompt and effective for focal hyperhidrosis.
4.Study on the proficiency testing program of pharmaceutical preparation analysis.

This proficiency testing program is established to evaluate the pharmaceutical preparation analysis capacity of laboratories recommended by 18 countries and economies. It was authorized by Asia Pacific Laboratory Accreditation Cooperation (APLAC), and organized by Shanghai Institute for Food and Drug Control (SIFDC) and China National Accreditation Service for Conformity Assessment (CNAS). The 0.3sigma test is used to evaluate the homogeneity and stability of the proficiency testing sample. The results of the laboratories were assessed by Z-score. The robust average and the robust standard deviation of the participants' results were calculated as assigned value and standard deviation for performance assessment of hydrochlorothiazide and captopril using robust statistics. Thirty-three of 38 laboratories recommended by 18 countries and economies sent their results back. Twenty-four laboratories' results were observed as satisfactory. Five laboratories were identified as having reported at least one questionable result. Four laboratories were identified as having reported at least one unsatisfactory result.
5.Application of percutaneous transhepatic cholangial drainage and percutaneous transhepatic cholangial stent implantion in treating malignant biliary obstruction in elderly patients

Objective:To evaluate the clinical value of percutaneous transhepatic cholangial drainage (PTCD) and percutaneous transhepatic cholangial stent(PTCS)implantation therapy for malignant biliary obstruction in the elderly patients.Methods:Fifty three patients over 60 years-old were treated with PTCS implantation (n=35),PTCD for internal-external drainage(n=11),or single external drainage(n=7). The imaging examination was performed for all the patients one week after surgery. The serum levels of total bilirubin (TBIL) and alanine aminotransferase (ALT) were detected before and after surgery.Rusults:Metallic stents were successfully implanted in 32 patients during the surgery, and 3 patients were implanted 5 to 7 days later after PTCS. Both total serum levels of TBIL and ALT were significantly decreased at one week after surgery (P<0.05). The mean survival periods were 11.5 months for stent implantation patients and 5.5 months for PTCD patients. Conclusion:PTCD and PTCS are easy to perform and safe and effective in the treatment of malignant biliary obstruction of elderly patients.
6.Association of CD44 gene rs13347 polymorphism with occurrence and metastasis of breast cancer

Objective By exploring the relationship between the gene polymorphism of tumor related gene CD44 and the occurrence of breast cancer,the pathogenic gene or genetic predisposing gene of breast cancer was studied so as to provide theoretical basis and technical support for the screening,diagnosis and treatment of breast cancer.Methods Samples from 265 cases of breast cancer and 290 healthy female were collected. Multiple polymerase chain reaction (PCR) amplification and two generation sequencing were used to analyze the genetic polymorphism of 4 loci of CD44 rs11607862,rs13347,rs7116432 and rs8193.T he size and metasta-sis of 235 patients were compared.Results There was no significant difference in the expression of four sitesloci in breast cancer patients.However,in the analysis of cancer patients,the GG genotype of rs13347 lo-cus was lower than that of CC/CG type,and the lymph node metastasis rate was lower.Conclusion There is a certain correlation between the rs13347 loci of CD44 and the metastasis of breast cancer.The correlation be-tween the genetic polymorphisms of 3 loci of rs11607862,rs7116432 and rs8193 and the development of breast cancer needs more clinical samples and further research.
7.Status quo and influencing factors of self-management in readmitted patients with myocardial infarction

Objective? To investigate the correlation between self-efficacy, discharge readiness and self-management in readmitted patients with myocardial infarction, to explore the influencing factors to self-management and to provide a basis for intervention strategies in clinical practice. Methods? Totally 198 readmitted patients with myocardial infarction from First Hospital of Shanxi Medical University were selected using convenient sampling from December 2017 to May 2018 and investigated with the general informational questionnaire, Coronary Heart Disease Self-Efficacy Questionnaire (CHDSEQ), Discharge Readiness Scale (DRS), and Coronary Heart Disease Self-Management Scale (CHDSMS). Pearson correlation analysis and multivariate linear multilayer regression analysis were used to explore the influencing factors to self-management behavior. Results? The CHDSMS score of the 198 patients was (75.21±9.31), with a 63.12% scoring metrics;the CHDSEQ score was (50.91±10.43); the DRS score was (161.53±26.76). Correlation analysis showed that self-efficacy was positively correlated with self-management (r=0.537, P<0.01), and that discharge readiness was positively correlated with self-management (r=0.416, P< 0.01). Multilayer correlation analysis revealed that the two factors could account for 14.1% of the variance of self-management. Conclusions? Medical and nursing workers should evaluate the discharge readiness of readmitted patients with myocardial infarction accurately, improve patients' self-efficacy, and provide effective discharge guidance for different groups according to relevant influencing factors, thus improving patients' self-management.
8.Development and reliability and validity testing of a rehabilitation needs questionnaire for stroke patients

Objective:To develop the rehabilitation needs questionnaire for stroke patients, so as to provide a tool for medical staff to implement continuous rehabilitation services.Methods:Based on the conceptual framework of the International Classification of Function, Disability and Health, and on the basis of literature review and qualitative research, the questionnaire items were preliminatively established after two rounds of Delphi expert consultation, and 130 stroke patients admitted to the Department of Neurology and Rehabilitation of the First Hospital of Shanxi Medical University from April to December 2021 were selected to test the reliability and validity of the questionnaire. And 200 stroke patients were selected for confirmatory factor analysis to form a formal scale.Results:Exploratory factor analysis extracted a total of 4 common factors: physiological function rehabilitation needs, social rehabilitation environmental support needs, emotional/psychological support needs, rehabilitation knowledge/information needs. After project analysis and 3 exploratory factor analysis, a final questionnaire containing 16 items was formed.The Cronbach α of the questionnaire was 0.935, with a broken half reliability of 0.824. The fitting index of confirmatory factor analysis was within the standard range. The χ2/ df was 2.979, the incremental fitting index was 0.907, the comparative fitting index was 0.906, and the root mean square error of approximation was 0.100. Conclusions:The reliability and validity of the rehabilitation needs questionnaire for stroke patients are good, which can preliminarily assess the rehabilitation needs of stroke patients.
9.Temozolomide and whole brain radiotherapy for leptomeningeal metastases from non-small cell lung cancer

Objective To evaluate the clinical efficacy oftemozolomide (TMZ) and whole brain radiotherapy (WBRT) in the treatment of leptomeningeal metastases (LM) from non-small cell lung cancer (NSCLC).Methods The clinical data were retrospectively analyzed of the 19 patients with LM from NSCLC who had been treated from October 2007 to June 2016 at Guangdong Sanjiu Brain Hospital.Of them,10 were treated by a combination of TMZ+WBRT,and 9 by other therapies.The survival rate was determined by the Kaplan-Meier method and analyzed using the log-rank test.Results After treatment for 2 weeks,the illness was alleviated in 8,stable in 2 and progressive in 0 of the 10 patients receiving TMZ+WBRT,yielding a remission rate of 80%;the illness was alleviated in 5,stable in 3 and progressive in one of the 9 patients receiving other therapies,yielding a remission rate of 55.6%.The median overall survival was 8 months,the survival rate was 56.3% at 6 months and 33.8% at one year for those receiving TMZ+WBRT;the median overall survival was 7 months,the survival rate was 55.6% at 6 months and 14.8% at one year for those receiving other therapies.Conclusion Temozolomide and whole brain radiotherapy may prolong the survival time and improve the prognosis of patients with LM from NSCLC.
10.Mechanism of aggravated severity in hypertriglyceridemia-associated acute pancreatitis:insights from the pathogenesis of"fat-turbidity-toxic heat"

Hypertriglyceridemia-associated acute pancreatitis is an inflammatory disorder of exocrine pancreas caused by metabolism disturbances of triglyceride-rich lipoproteins.Currently,hypertriglyceridemia-associated acute pancreatitis is characterized by an escalating incidence rate,a tendency for more severe cases,and a lack of therapeutic drugs.Traditional Chinese medicine has distinct advantages in treating this disease,but its theoretical framework has not yet been established.Hypertriglyceridemia-associated acute pancreatitis manifests itself as a febrile disease,aberrant accumulation of fat and turbidity may stem from dietary imbalances and visceral dysfunction in ordinary individuals.The prolonged accumulation of fat and turbidity can transform into turbid pathogen,subsequently engendering heat,constituting a pivotal pathogenic factor.Throughout the progression of the disease,the fiery pathogen consumes the fat and turbidity,resulting in the generation of toxic heat,which is a crucial mechanism in the exacerbation of the disease severity.Thus,this article posits therapeutic principles aimed at averting the transformation of fat and turbidity into turbid pathogen and counteracting toxic heat in this disease.This article reviews two key theories from traditional Chinese medicine classics relevant to hypertriglyceridemia-associated acute pancreatitis:the theory of fat-turbidity associated with hypertriglyceridemia and the febrile disease related to acute pancreatitis.Combining these traditional theories with modern research on the mechanisms that intensify hypertriglyceridemia-associated acute pancreatitis and the corresponding targets of traditional Chinese medicine,it suggests that the pathogenesis of"fat-turbidity-toxic heat"serves as the theoretical basis of traditional Chinese medicine for the aggravated severity of hypertriglyceridemia-associated acute pancreatitis.The article aims to offer new insights for the treatment of hypertriglyceridemia-associated acute pancreatitis.
