2.Effect of fetal-maternal immune tolerance on pregnancy-induced hypertension

Objective:To analyze the levels of immune cytokine in peripheral blood(PB) and the expressions of immune related transcription factors in placenta cytotrophoblast,which in patients with pregnancy-induced hypertension(PIH).And to explore the effect of fetal-maternal immune tolerance on pregnancy-induced hypertension.Methods:50 cases patients with PIH were selected as the observation group,and 40 healthy people were chosen as the control group.Enzyme-linked immunoassay(ELISA) was performed to detect the levels of IFN-γ,IL-4,IL-17 and TGF-β in peripheral blood.Quantitative Real-time PCR(RT-qPCR) and Western blot were used to analyze the mRNA expressions and protein levels of T-bet,GATA-3,RORC and Foxp3 in placenta cytotrophoblast,respectively.Results:①Compared with the control group,the IFN-γ and IL-17 levels in peripheral blood in the observation group were obviously higher and the TGF-β level was greatly lower(P<0.05).There was no significant difference of IL-4 level between the two groups(P>0.05),but the ratios of IFN-γ/ IL-4 and IL-17/TGF-β in the observation group were larger than that in the control group(P<0.05).②Compared with the control group,the mRNA expressios and protein levels of T-bet and RORC in the observation group were increased significantly(P<0.05),but that of GATA-3 and Foxp3 were decreased greatly(P<0.05).Thus,the ratios of T-bet/GATA-3 and RORC/Foxp3 in the observation group were significantly larger than that in the control group(P<0.05).Conclusion:PIH is occurred with the imbalance of Th1/Th2 and Th17/Treg,and it might be associated with the mechanism of fetal-maternal immune tolerance disorder that induced by Th cells.
3.Risk factors of positive margins and residual lesions following LEEP conization in CIN3 patients

The clinical data of 380 patients undergoing loop electrosurgical excision procedure(LEEP) conization for cervical intraepithelial neoplasia grade 3(CIN3) at the Affiliated Hospital of Xuzhou Medical University from January 2017 to December 2019 were retrospectively analyzed. Univariate and multivariate logistic regression analyses were used to determine the risk factors of positive margin and residual lesions following conization. One hundred and twelve cases (29.5%) had positive margins, and the risk factors for positive margins were positive high-risk human papillomavirus ( OR=4.92, 95% CI: 1.81-13.36), high-grade lesions on thin cytologic test (TCT)screening ( OR=3.95, 95% CI: 2.42-6.42), and glandular involvement ( OR=3.58, 95% CI: 1.93-6.63). The hysteretomy was performed following conization in 169 hundred and sixty-nine patients who had difficulty in follow-up or no fertility intension, among whom 51 (30.2%) had residual lesions; logistic regression analysis showed that positive margins on LEEP were a risk factor for residual lesions at the time of hysterectomy ( OR: 2.83, 95% CI: 1.44-5.59). The study indicates that positive HR-HPV, high-grade TCT lesions, and lesions involving glands are risk factors for positive margins in CIN3 patients undergoing LEEP conization; and positive margins is risk factor for residual lesions following conization,to which more attention should be paid in clinical practice.
4.Influence of ovarian artery blood flow index on pregnancy outcomes of patients undergoing in vitro fertilization-embryo transfer

Objective To investigate the influence of ovarian arterial blood flow index on pregnancy outcomes in patients undergoing in vitro fertilization and embryo transfer (IVF-ET).Methods Totally 136 infertile patients undergoing IVF-ET from February 2014 to April 2016 were divided into pregnant group and non-pregnant group.The monitoring through transvaginal B of two groups for chorionic gonadotropin (HCG) day and ovarian artery resistance index (RI),pulsatility index (PI) and umbilical artery diastolic flow velocity height ratio (S/D) on 2nd day of menstruation (M2) was conducted.The difference of blood flow indexes in two groups was analyzed.Results Compared with those in non-pregnant group,RI,PI and S/D in pregnant group were decreased on M2,and the difference was statistically significant (P < 0.05).RI and PI were also decreased significantly on HCG day,and the difference was statistically significant (P < 0.05),but S/D showed no significant changes (P > 0.05).RI and PI decreased significantly on HCG day in two groups (P < 0.05) when compared with those on M2,but in non-pregnant group,the comparison of S/D on HCG day showed no statistical significance (P > 0.05).Conclusions There is close relationship among RI and PI of ovarian artery and ovarian reserve function.RI and PI are effective indexes to predict outcomes in IVF-ET,and provide scientific basis for the preparation and treatment for IVF-ET.
5.Relationship between tumor metastasis-related genes and adenomyosis

AIM: To study the tumor metastasis-related genes expression in adenomyosis and normal endometrium in order to investigate the pathogenesis of adenomyosis. METHODS: 43 specimens of adenomyosis, 22 specimens of controls (normal endometrium) were studied. The expressions of nm23-H1, MMP-2, MMP-9, MT1-MMP, and TIMP-1 in adenomyosis and controls were detected by immunohistochemical method. RESULTS: The expression levels of MMP-2, MMP-9, and MT1-MMP in adenomyosis were significantly higher than those in controls ( P 0 05). CONCLUSION: MMP-2, MMP-9, especially MT1-MMP, maybe play an important role in the pathogenesis of adenomyosis.
6.Multicenter randomized controlled clinical study on levornidazole and sodium chloride injection in the treatment of pelvic anaerobic infections

Objective To evaluate clinical efficacy and safety of levornidazole in the treatment of pelvic anaerobic infections. Methods A multicenter randomized controlled clinical study was conducted to evaluate clinical efficacy and safety of levornidazole. One hundred and fourty-three patients with pelvic anaerobic bacteria infection were classified into 70 cases treated by levornidazole in study group and 73 cases treated by Ornidazole in control group. Those patients in two groups were both administered at a dose of 0. 5 g twice daily for 5 - 7 days. The rate of clinical efficacy, bacteria clearance and adverse effect were recorded and compared between two groups. Results At the endpoint, the rate of clinical efficacy were 80% (56/70) in study group and 81% (59/73) in control group, which did not reach significant difference (P>0. 05). The rate of bacteria clearance were 97% (36/37) in study group and 92% (22/24) in control group, which also did not reach significant difference(P >0. 05). The rate of adverse reaction of 3% (20/70) in study group was significantly lower than 22% ( 16/73 ) in control group ( P < 0. 05 ). Conclusion It is effective and safe to treat pelvic anaerobic infections with levornidazole and sodium chloride injection.
7.Effect of BRL37344 on cardiac fibroblast phosphatidylinositol 3-kinase-protein kinase B signal transduction pathway

Objective To explore the effects of the β3 adrenergic receptor (β3-AR) agonist BRL37344 on cardiac fibroblast proliferation and collagen fiber hyperplasia in Wistar rats by promoting the phosphaticlylinositol 3 kinase-protein kinase B(PI3K-Akt) signal transduction pathway. Method Cardiac fibroblasts(CFbs) were isolated from 1 - 3 day-old Wistar rats under the sterile environment in the laboratory of the First Affiliated Hospital of Hasbin Medical University, by using enzyme digestion and an modified technique of differential anchoring velocity.The cultured myocardial cells were randomly (random number) divided into five groups. ① In blank group, rats were not treated with drug; ②in BRL group, rats were treated with BRL37344; ③in LY group, rats were treated with LY294002(PI3K antagonist) for one hour before treated with BRL37344;④in Akt-Ⅰ group,rats were treated with Akt-Ⅰ (Akt antagonist) for one hour before treated with BRL37344;⑤in L-A group, rats were treated with LY294002 and Akt-Ⅰ for one hour before treated with BRL37344. MTT colorimetric method and RT-PCR were used to observe the role of β3-AR agonist with or without PI3K antagonist and/or Akt antagonist in cardiac fibroblast proliferation (CFP) and collagen fiber hyperplasia (CFH). Comparisons between groups were made by one-way ANOVA and Tukey test. Results ①β3-AR was present in the CFbs. ②Compared with BRL group, the CFP and CFH in LY and Akt-Ⅰ groups were lower (P <0.01) and those in L-A group were lowest (P < 0.01). ③Compared with blank group,the expressions of type Ⅰ and type Ⅲ fiber mRNA obliviously increased in BRL group (P < 0.01),and at 48 hours,the expressionrs reached peak. At 48 hours,compared with BRL group,the expressions in LY and Akt-Ⅰ groups were lower, and were lowest in L-A group ( P <0.01). Conclusions BRL37344 promotes cardiac fibroblast proliferation and expressions of type Ⅰ and Ⅲ collagen fiber mRNA by activating the PI3K-Akt signal transduction pathway.
8.One case report of lipoprotein glomerulopathy with literature review

Objective To discuss the clinical characteristics and prognosis of lipoprotein glomerulopathy (LPG) in chil-dren. Method Clinical data of one pediatric LPG patient were retrospectively analyzed. The clinical features and prognosis of childhood LPG were summarized based on literature review. Results A nine years old girl presented with frequent urination. The ifrst urine test revealed hematuria and proteinuria. After one week anti-infection treatment, the hematuria and proteinuria were continued. The serum albumin was slightly reduced. The hyperlipidemia and mild anemia were emerged. Kidney biopsy showed that enlarged glomeruli, with dilated capillary loops and weak eosinophilic lipoprotein thrombi in the capillary lumina under the light microscope;layered or tuftedemboluscontaining particulated lipid vacuoles under electron microscope. Gene sequencing identified APOE Tokyo (Leu141-Lys143→0). The diagnosis of LPG was confirmed. The lipid-lowering therapy was administrated and the disease was alleviated. Conclusion LPG is a rare disease in children. The level of blood lipid was signiifcantly increased, and the hormone therapy was ineffective. Kidney biopsy is the main basis for diagnosis. The genetic testing can prompt the genetic background. Lipid lowering therapy can relieve the progress of the disease.
9.Molecular imaging for tracking transplanted embryonic stem cells in the treatment of acute liver injury

BACKGROUND:Embryonic stem cel s have the capacity of multi-differentiation potential, and have been utilized for the therapy of acute liver injury. However, the migration and proliferation of embryonic stem cel s after transplantation remains not wel characterized.
OBJECTIVE:To track the transplanted embryonic stem cel s in repairing acute liver injury by bioluminescence imaging technology.
METHODS:Murine embryonic stem cel s (D3) were transducted with a construct composed of firefly luciferase, monomeric red fluorescence protein and herpes simplex virus truncated thymidine kinase triple fusion reporter genes by lentivirus system. Stable D3 embryonic stem cel s integrating three report genes were screened. The undifferentiated embryonic stem cel s or differentiated embryonic stem cel s from the 6-day-old embryoid body were transplanted into acute liver injury model of SV129 mouse through spleen, and the transplanted cel s were monitored by bioluminescence imaging technology.
RESULTS AND CONCLUSION:Reverse transcription PCR results showed that the expression level of Oct-4 and Nanog was not affected in embryonic stem cel s transducted with triple fusion reporter gene compared with wild-type embryonic stem cel s. The migration process of transplanted cel s was visualized by bioluminescence imaging technology. Teratomas were found in both triple fusion-embryonic stem cel s treatment group and triple fusion-embryoid body cel s treatment group at liver, and the teratoma formation could be suppressed by ganciclovir administration because ganciclovir can react with herpes simplex virus truncated thymidine kinase and trigger cel necrosis process. Histological analysis showed that teratomas comprised tissues from al three germ layers. These results demonstrate that triple gene fusion does not affect differentiation potential of embryonic stem cel s and it is risky to utilize embryonic stem cel s for cel therapy, because it affects repair of liver injury. The therapy strategy requires further improvement and real-time visualizing of embryonic stem cel s in vivo is absolutely necessary.
10.Application of the Mandarin Tone Identification Test (MTIT) in children with hearing impairment.

OBJECTIVEThe present study aimed to evaluate the Mandarin tone identification ability in children with hearing impairment (HI) in quiet and in noise using the new developed Mandarin Tone Identification Test (MTIT), and to evaluate the practical application of MTIT.
METHODSubjects were Mandarin-speaking children with moderate, severe and profound HI and aged from 5 to 12 years (n = 81, Male = 37, Female = 44). The MTIT was administered in five test conditions: in quiet and in -10, -5, 0 and +5 dB signal-to-noise ratios (S/N).
RESULTSThe degree of HI significantly influenced the tone identification ability. The tone identification performance of children with moderate to severe HI was significantly better than that of children with profound HI. However, when compared with children with normal hearing, the performance of children with moderate to severe HI was not satisfactory. Among the six tone contrasts, the Tone 1/Tone 2 and Tone 2/Tone 3 contrasts were the most difficult tone contrasts for children with profound HI and the Tone 2/Tone 3 contrast was the most difficult for children with moderate to severe HI in quiet.
CONCLUSIONSMTIT could be used to examine tone perception ability of children with hearing aids. In quiet, the Tone 1/Tone 2 and Tone 2/Tone 3 contrasts are difficult to identify, while six tone contrasts are all difficult to identify for children with HI in noise. In future, the MTIT can also be used to evaluate the tone identification ability on children cochlear implants.
