Juvenile polyps are the most common colorectal polyps in children,usually present with painless,intermittent rectal bleeding.Colonoscopy is used to diagnose and juvenile polyps should be removed once found.The etiology and pathogenesis of juvenile polyps is still unclear.Histologically,the typical juvenile polyp is hamartoma with a prominent lamina propria,infiltration with inflammatory cells and cystic glands.This article reviewed the recent progress in the study of the clinical manifestation,histological characteristics,biological characteristics,diagnosis,treatment,pathogeny and molecular mechanisms of colorectal juvenile polyps in children.

Through the analysis of evaluation indexes, efficiency analysis of a hospital introducting large medical equipment beyond millions yuan in recent 3 years,suggestions of hospital equipment management are proposed.

OBJECTIVE:To establish a method of contents determination of 17 amino acids in Fritillariae Pallidiflorae Bulbus produced in Xinjiang. METHODS:Cation-exchange column was used to separate 17 kinds of amino acids;post-column derivatiza-tion liquid chromatography was used to determine the contents of amino acids:the column was strong sulfonic acid cation exchange resin LCAK06/Na with mobile phase A(weighing trisodium citrate 11.9 g,citric acid 6 g,phenol 1 g,dissolving by water,then adding 65 mL of methanol and 6 mL of concentrated hydrochloric acid,bringing the volume with tap water,adjusting pH to 3.4), mobile phase B(weighing trisodium citrate 11.9 g,NaOH 2.8 g,phenol 1 g,boric acid 5.0 g,adding water to dissolve,adjustingpH to 10.8),gradient elution,flow rate was 0.45 mL/min for A pump and 0.25 mL/min for M pump,the detection wavelength was 440 nm for proline and 570 nm for the remaining amino acids,the injection volume was 50 μL. RESULTS:The linear range were 20~400 nmol/mL of aspartic acid,threonine,serine,glutamic acid,glycine,alanine,valine,methionine,isoleucine,leucine,ty-rosine,phenylalanine,histidine,lysine,arginine,proline,10-200 nmol/mL of cystine(r were higher than 0.9890);RSDs of preci-sion,stability and reproducibility tests were lower than 2.0%;limits of detection were 0.16 nmol/mL except for cystine(0.08 nmol/mL);recovery was 98.5%-99.5%(RSD was 0.06%-0.21%,n=6). CONCLUSIONS:The method is simple with good precision, stability and reproducibility,and can be used for the simultaneous determination of amino acids in Fritillariae Pallidiflorae Bulbus produced in Xinjiang.

BACKGROUND: Myasthenia gravis is a spectrum of disease that there are varied options of treatment. A consensus guideline is conceived by the Neuromuscular Council of the Philippine Neurological Association to summarize all the possible options of treatment in generalized autoimmune myasthenia gravis.
METHODS: Articles were retrieved from 1966 to 2012 from the internet, appraised, summarized, and consolidated to come up with recommendations in this treatment guideline. The American Academy of Neurology classification of evidence (Class I,II,III,IV) and levels of recommendations (Level A, B, C. U) were used.
RESULTS: New insights were found on the pathophysiology of autoimmune myasthenia gravis; reviews and outcome of treatment groups as well as the newest immunosuppressant which is rituximab as added to the armamentarium of medical immunosuppression. There are only a few small population randomized controlled trials on some immunosuppressant drugs like cyclosporine, tacrolimus and mycophenolate. Most studies are retrospective or open labelled studies. Thus, most recommendation grade is at Level C.
CONCLUSION: This guideline summarizes the following: Pyridostigmine is the initial treatment for the mild stage or beginning of myasthenia symptoms. For rapid progressive illness, the option of thymectomy is introduced followed by prednisone followed by azathioprine . For poor responders, the following are additional options: cyclosphosphamide, cyclosporine, mycophenolate, tacrolimus, methotrexate and rituximab. Management of MG crisis is mechanical ventilation, plasma exchangeor IVIg with prednisone and/or azathioprine. IV cyclophosphamide may be given for resistant severe crisis of MG patients.

Objective:By cocculture with mesenchymal stem cells as feeder cells,we observe whether we can isolate and culture embryonic stem cells(ES cells)which maintain undifferentiated and multipotential. Methods :tissues and cell types were used in basical research to cure disease. The blastocysts of 3.5 d from Kunming species mice were collected and cultured on mesenchymal stem cells layers. The embryonic stem cell colonies were preliminarily identified by light microscope AKP staining and 0ct-4 staining in morphology and biological character. Results:The embryonic stem cell colonies by cocculture with mesenchymal stem cells as feeder cells were identified. The results showed that: 1. all the ES cells had its typical morphological characteristics such as nestlike colonies,round or elliptic,with clear edge and smooth surface,strong refraction ,significant swelling growth,compactness of cell aggragation and indistinction between two edges of neighbour cells within a colony,relatively bigger nuclear compared with little cytoplasm,i.e.,high ratio of the nuclear- cytoplasm; 2.after the AKP staining,all of the ES cells showed strong AKP activity as same as the ES cells of F0,and on the contrary,the embryonic fibroblast cells had no expression of AKP showing the lost of staining;3. after the Oct-4 staining,all of the ES cells showed strong Occt-4 activity. Conclusion:By cocculture with mesenchymal stem cells as cells,we can isolate and culture embryonic stem cells (ES cells) which maintain undifferentiated and multipotential.

Impulse oscillometry system(IOS) is a new pulmonary function test in recent years, IOS has been developed as a patient-friendly lung function test that minimizes demands on the patient, a noninvasive method without pain, the results is objective and comprehensive, suitable for all patients, especially for the elder ly, children, coma patients, pregnant women and critically ill patients, for better detection, diagnosis, and treat ment of different respiratory diseases.This article is to determine the reference values of IOS, its utility in children, the existing questions and its development prospect.

Besides being able to specially target tumor tissues, anticancer peptides have other advantages such as easy synthesis, low immunogenicity, high potency, low toxicity, and easy access. These peptides exert their anti-tumor effects through mechanisms such as modulating immune system, killing tumor cells directly, and/or inhibiting tumor metastasis.

Objective To study the relationship between B-type natriuretic peptide(BNP) and type 2 diabetes mellitus(DM) patients with congestive heart failure(CHF).Methods 78 patients with DM were divided into pure DM group(n=37) and DM+CHF group(n=41) ,the level of plasma BNP in patients with DM(n=78) and normal control group(n=20) was.measured.Results The level of plasma BNP in grade Ⅱ ,Ⅲ～Ⅳgroup of cardiac function were significantly higher than that in pure DM group and control group(P<0.05),grade Ⅲ～Ⅳ group were significantly higher than that in grade II group(P<0.05),LVEF in Ⅱ,Ⅲ～Ⅳgroup of cardiac function were significantly lower than that in pure DM group and control group(P<0.05),grade Ⅲ～Ⅳ group were significantly lower than that in grade Ⅱ group(P<0.05) ;The level of plasma BNP was positively related to LVEF(r= -0.713,P<0.05).Conclusion BNP was an objective indicator to evaluate the severity of DM with heart failure.

Objective To study the effects of trimetazidine on cardiac function in type 2 diabetes mellitus patients with heart failure. Methods Ninty type 2 diabetes mellitus patients with heart failure were randomly divided into two groups (45 patients in control group,45 patients in treatment group). All patients in both two groups received routine therapy in order to have blood glucose under control,the treatment group was treated with trimetazidine 20mg po Tid ,treatment for 6 months. Before and after treatment ,two groups were detected left ventricular end-diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD) and left ventricular ejection fraction (LVEF), and the clinical efficacy were evaluated. Results Ater treatment, the rate of total efficiency was 68. 9% in control group, and it was 86. 7% in treatment group. The clinical efficacy of treatment group was significantly better than control group(P＜0.05) ;Ater treatment,LVEDD,LVESD were significantly reduced than those before treatment(P ＜0.05) ,LVEF was significantly higher than that before treatment (P ＜0.05);After treatment,LVEDD, LVESD in treatment group was significantly reduced than those in control group, LVEF was significantly higher than control group (P ＜ 0.05).Conclusion Trimetazidine can significantly improve the cardiac function in type 2 diabetes mellitus patients with heart failure.

The highly down-loaded 175 papers from 7 journals of ophthalmology covered in Scopus database in 2011 were analyzed,showing that the most concerned subjects were rintinopathy,macular edema,glaucoma, neovasculariza-tion,Ranibizumab, retinal blood vessel obstruction, OCT,cornea, artificial lens, diabetic retinopathy, nanometer medicine and technology,the research has extended to retro-ocular segment from anterior-ocular segment,the papers of multiple authors accounted for 92%, the reviews accounted for 51.42%and were highly down-loaded.
