1.Evaluation methods of anterior chamber and angle structures after laser peripheral iridotomy
Chinese Journal of Experimental Ophthalmology 2011;29(2):174-178
Laser peripheral iridotomy ( LPI) is one of the main therapy for glaucoma due to its reliable efficacy of widening peripheral angle and therefore decreasing intraocular pressure by partially relieving pupillary block. The parameters of the anterior chamber angle following the LPI are the main evaluation indexes. At present, the detecting and diagnosis technique for the anterior chamber angle is deeply advancing, including gonioscopy, ultrasound biomicroscopy, anterior segment optical coherence tomography and Pentacam. These methods offer some useful parameters and also present with some limits in the application respectively in the evaluation of anterior chamber and angle structure. This review focuses on the application of Methods mentioned above and morphologic changes of anterior chamber and angle structure following the LPI.

Objective To recognize CT signs of parapharyngeal space (PPS) changing in nasopharyngeal carcinoma (NPC) further. Methods 30 patients with proven NPC by biopsy were performed CT and MR scanning before radiotherapy. Results According to CT findings, four types of PPS display in 30 cases were found, each of which was observed and compared with MR findings and were as follows: (1)PPS ,which had no changing on CT, showed fat-filled triangular space continuously on MRI. (2) PPS, which became narrow and linear hypodense on CT,showed that fat intensity was pressed, but continuously on MRI; (3) PPS, which became narrow and linear hyperdense on CT,showed fat intensity, was pressed exceedingly but continuously either on MRI. (4)PPS,which was diminished on CT, showed the mass occupied PPS and lateral pterygoid muscles was pushed, medial fatty space of which was diminished, but intensity of medial or lateral pterygoid muscles was normal or abnormal on MRI. Conclusion (1)When PPS becomes narrow on CT,whether PPS shows hyperdense or hypodense, the mass makes a breakthrough PPS. (2) When PPS is diminished completely on CT,the mass steps PPS to the infratemporal fossa.

OBJECTIVETo investigate diagnosis and therapeutic effects of knee joint synovial chondromatosis with arthroscopic.
METHODSFrom March 1995 to July 2011, 28 patients with knee joint synovial chondromatosis were treated. Among them, 18 males and 10 females ranging age from 25 to 81 (mean 55.2) years, the course of disease ranged from 0.5 to 15 (mean 5.6) years. Clinical manifestation mainly included pain, swell and functional limitation of knee joint. Knee open surgery (17 cases) and laparoscopic surgery (10 cases) were respectively used. Clinical symptom, image data, pathological manifestation and effects under arthroscopy were observed, Lysholm scoring was used to evaluate effects.
RESULTSAll patients were followed up except one lost, the duration ranging from 6 to 24 months. Lysholm score in knee open surgery was increased from (41.89 +/- 6.81) preoperatively to (67.73 +/- 7.62) postoperatively;while in laparoscopic surgery it was increased from (40.78 +/- 7.54) preoperatively to (77.46 +/- 8.43) postoperatively.
CONCLUSIONArthroscopic surgery, which has no risk of rupture of incision, nonunion, earlier to exercise, is a good method to diagnosis and treat knee joint synovial chondromatosis.

Batch and continuous fermentation were adopted to investigate the effect of specific growth rate and amino acid components on RNA accumulation in Candida tropicalis ATCC 20408 in fermentation medium ( FM), yeast peptone dextrose medium (YPD), molasses fermentation medium ( MFM) and FM without corn steep liquor. The data showed that obvious differences in intracellular RNA accumulation were observed at different cell growth phases in bath fermentation prosess, and RNA level reached 11. 8% (g-RNA /g-DCW) during exponential phase, and only 6.9% during stationary phases. It was also found that intracellular RNA accumulation increased with the increase of specific growth rate in continue fermentation prosess, and the highest RNA level reached 15. 6% with the glucose conversion rate of 42. 8% at the dilution rate of 0. 5 h-1. Furthermore, the data showed that RNA lever was notably increased in batch fermentation process when amino acids or peptone was added into the fermentation medium containing no corn steep liquor. Taken together, it was reported for the first time that specific growth rate and amino acid components plays a leading role on the intracellular RNA accumulation in C. tropica lis, and specific growth rate is more important.

Objective: To investigate the mechanisms of electroacupuncture (EA) at Zusanli (ST 36), Liangmen (ST 21) and Sanyinjiao (SP 6) in intervening diabetic gastroparesis (DGP) based on calcium-activated chloride channel. Methods: Forty Sprague-Dawley rats were randomly divided into four groups, including a normal control group (group A), a model group (group B), an EA group (group C) and a metoclopramide group (group D), with 10 rats in each group. A single intraperitoneal injection of 2% streptozotocin (STZ) combined with 8-week high-glucose high-fat diet was used to establish a DGP rat model. After intervention, gastrointestinal propulsive rate was observed; the expression level of transmembrane protein 16A (TMEM16A) was examined by immunohistochemistry; the Ca2+ concentration in interstitial cells of Cajal (ICCs) was detected by immunofluorescence; and whole-cell patch-clamp technique was applied to detect the current intensity of calcium-activated chloride channel (ICaCC) in ICCs in gastric antrum. Results: After modeling, the blood glucose levels in group B, group C and group D were significantly increased compared with group A (all P<0.01); after intervention, compared with group B, the blood glucose levels in group C and group D were significantly decreased (P<0.05, P<0.01); the intra-group comparison of blood glucose level between after modeling and after intervention found significant difference only in group C (P<0.01). The gastrointestinal propulsive rates in group B, group C and group D were significantly different from that in group A (P<0.01 or P<0.05); the gastrointestinal propulsive rates were markedly higher in group C and group D than in group B (P<0.01, P<0.01). The expressions of TMEM16A in group B and group C were decreased compared with group A (P<0.01, P<0.05); the expressions of TMEM16A in group C and group D were increased compared with group B (P<0.01, P<0.05). The fluorescence intensity of Ca2+ was significantly lower in group B than in group A (P<0.01); the fluorescence intensity of Ca2+ was significantly higher in group C and group D than in group B (P<0.01, P<0.05). ICaCC in ICCs in group B was significantly decreased compared with group A; ICaCC in group C and group D were increased compared with group B. Conclusion: EA at Zusanli (ST 36), Liangmen (ST 21) and Sanyinjiao (SP 6) can significantly improve gastrointestinal motility in DGP rats by up-regulating the ICaCC in ICCs.

Objectives To put forward the modified Letournel classification of acetabular fractures, and evaluate the guiding role of the modified Letournel classification in clinical work. Methods A retrospective study of 170 patients (178 sides) with acetabular fractures treated at Nanfang Hospital between January 2006 and August 2018 was performed.Among them 129 cases were males and 41 were females. The average age was 40.2±14.2 years (range,14-82 years).According to the Letournel classification,unclassifiable/atypical acetabular fractures were found out with plain Ⅹ-ray and plain CT scan and three-dimensional CT,and then their unclassifiable reasons were analyzed.Based on anatomical landmarks, clear boundaries of acetabular walls were defined on the 3D printed pelvic model. The structure of each column is a trihedron. The wall's fracture and column's fracture were distinguished using a theory that the wall's fracture involved two surfaces of column with interruption of continuity and column's fracture involved three. When column's fracture associated with wall's fracture, lowercase a,p and q were used representing comminuted areas of corresponding anterior and posterior wall, quadrilateral plate respectively. When column's fracture didn't associates with wall's fracture,it is marked with number 0.Finally,all fractures were classified according to the modified Letournel classification. The relationship between fracture type and surgical approach was analyzed. Results There was 51.7% (92 sides) of acetabular fractures that couldn't be classified by the Letournel classification.Incomplete fracture lines (49 sides,53.3%),comminuted fractures (28 sides,30.4%),both of them (15 sides,16.3%) were their unclassifiable reasons. There were 8 types in the modified Letournel classification, including posterior wall fracture, posterior column fracture, anterior wall fracture, anterior column fracture, transverse fracture,T-shaped fracture, anterior column+posterior hemitransverse fracture, and both columns fracture.Posterior column+posterior wall fracture, transverse+posterior wall fracture had respectively become a form of posterior column fracture and of transverse fracture.According to the modified Letournel classification, the reclassification rate of 178 side fractures was 100%,which was significantly higher than 48.3% (86/178) of the traditional Letournel classification, and the difference was statistically significant (x2=124.06,P< 0.001).100% of posterior wall fracture and 80.00% of posterior column fracture were treated by posterior approach.100% of anterior column fracture and 73.68% of both columns fracture were treated by anterior approach. Conclusion All acetabular fractures can be classified by the modified Letournel classification. The orientation of the comminuted walls reflects in part severity of the injury, better helps orthopaedic surgeons understand the morphology of acetabular fractures and select appropriate surgical approach.

Objective To compare the sensitivity and specificity of venereal disease research laboratory (VDRL) test versus several other laboratory tests in the diagnosis of neurosyphilis. Methods Lumber puncture was conducted to obtain cerebrospinal fluid (CSF) from untreated outpatients with latent syphilis (LS) or serofast outpatients with LS. Then, VDRL test, rapid plasma regain (RPR) test, Treponema pallidum particle agglutination (TPPA) assay, fluorescent treponemal antibody-absorption (FTA-ABS) test and protein quantification were performed on these CSF samples. The sensitivity, specificity, positive predictive value and negative predictive value were compared between VDRL test and four other laboratory tests in the diagnosis of neurosyphilis. Results Totally, 61 cases of latent syphilis were included in this study. The sensitivity, specificity, positive predictive value and negative predictive value were 93.44% (57/61), 99.32%(293/295), 96.61%(57/59), 98.65% (293/297)for CSF-RPR, respectively, 91.80% (56/61), 82.71% (244/295), 52.34% (56/107),97.99 (244/249) for CSF-TPPA, respectively, 93.44% (57/61), 82.71% (244/295), 52.78%(57/108), 98.39%(244/248) for CSF-FTA-ABS, respectively, and 49.18%(30/61), 97.29% (287/295), 78.95% (30/38),90.25% (287/318) for CSF protein quantification, respectively. Conclusions CSF-VDRL cannot be replaced by CSF-RPR, -TPPA, -FTA-ABS, or CSF protein quantification in the diagnosis of neurosyphilis. CSF-RPR shows a high sensitivity and specificity in the diagnosis of neurosyphilis, with an increased diagnostic capability (area under the receiver operating characteristic curve) compared with CSF-TPPA, CSF-FTA-ABS or CSF protein quantification.
