Bis-(3′,5′) cyclic di-guanylate (c-di-GMP) is an almost ubiquitous intracellular second messenger in bacteria. Now it is known to regulate complex physiological processes, including mobility, adhesion, virulence and biofilm formation. The level of c-di-GMP is regulated by diguanylate cyclases (DGCs) containing GGDEF domains and phosphodiesterases (PDEs) containing EAL or HD-GYP domains.Recent studies have demonstrated that HD-GYP domain protein is a novel phosphodiesterase, which is also involved in the regulation of c-di-GMP degradation. This review highlights recent advances in the structure and biochemical functions of HD-GYP domain proteins, which might help to further clarify the mechanism of c-di-GMP signal system.

Human enterovirus 71 (EV71) is one of the major etiological agents for the hand, foot, and month disease (HFMD) and is causing frequent, widespread occurrence in the mainland of China. The single positive-stranded RNA genome of EV71 is translated into a single polyprotein which is autocleavaged into structural and nonstructural proteins. The functions of many nonstructural proteins characterized in the life cycle of virus are potential targets for blocking viral replication. This article reviews the studies of the structures and functions of nonstructural proteins of EV71 and the anti-enterovirus 71 drugs targeting on these nonstructural proteins.

For the detection of steatosis, quantitative ultrasound imaging techniques have achieved great progress in past years. Magnetic resonance imaging proton density fat fraction is currently the most accurate test to detect hepatic steatosis. Some blood biomarkers correlate with non-alcoholic steatohepatitis, but the accuracy is modest. Regarding liver fibrosis, liver stiffness measurement by transient elastography (TE) has high accuracy and is widely used across the world. Magnetic resonance elastography is marginally better than TE but is limited by its cost and availability. Several blood biomarkers of fibrosis have been used in clinical trials and hold promise for selecting patients for treatment and monitoring treatment response. This article reviews new developments in the non-invasive assessment of non-alcoholic fatty liver disease (NAFLD). Accumulating evidence suggests that various non-invasive tests can be used to diagnose NAFLD, assess its severity, and predict the prognosis. Further studies are needed to determine the role of the tests as monitoring tools. We cannot overemphasize the importance of context in selecting appropriate tests.

Objective To evaluate the feasibility and clinical efficacy of amputation in situ secondary braches of splenic pedicle for laparoscopic splenectomy(LS)in patients of idiopathic thrombocytopenic purpura(ITP).Methods LS was performed in 41 ITP patients hospitalized in our department from January 2007 to November 2008 by dissecting secondary braches of splenic pedicle. The serosa on the spleen pedicle was opened by using a harmonic scalpel. Then secondary structures of the splenic arteries and veins, one by one, were disconnected from downward to upward, and then double ligated with the Hem-O-lok clips. The ligated section was cut off with the harmonic scalpe1,and then the spleen was resected. Results LS was successfully completed with no conversion to open surgery in all the 41 cases. The operative time was 75 to 180 min,and the estimated intra-operative blood loss was 50 to 800 ml.There were 3 cases of vice spleen and the vice spleen removed. Two cases were converted to traditional LS with Endo-GIA.The period of hospitalization was 4 to 9 d after operation. There was no severe postoperative complication.Platelet counts began to increase significantly after operation, reached to the peak in 5 to 7 d after operation.During the follow-up of 3 to 20 months to all 41 ITP patients, the total effective rate was 85.4%(35/41,including 22 cases of complete response and 13 cases of partial response). Conclusion LS with amputation in situ secondary braches of splenic pedicle is a safe, effective, minimally invasive and low complication in treatment of ITP.It is worth be recommended because of its lower costs in comparison with common LS.

Objective To investigate the effects of 10 mg and 20 mg atorvastatin and 10 mg rosuvastatin on inflammatory factors in patients with acute coronary syndrome (ACS).Methods 66 patients with ACS were randomly divided into three groups:the 10 mg atorvastatin group, the 20 mg atorvastatin group and the 10 mg rosuvastatin group(n=22 for each group). The levels of blood lipids, serum matrix metalloproteinases-9 (MMP-9)and plasminogen activator inhibitor-1 (PAI-1)were measured before and after two-week treatment.19 patients with normal coronary angiography were assigned to the control group. Results The concentration of serum MMP-9 and PAI-1 was higher significantly in patients with ACS than those in control subjects(P<0.05 or P<0.01).After two weeks'treatment, the serum MMP-9 and PAI-1 levels were lowered significantly (P<0.01),which were much better in groups of 20mg atorvastatin and of 10mg rosuvastatin than those in group of 10mg atorvastatin (P<0.05 or P<0.01 ).No relationship was observed between the levels of above inflammatory markers and serum hpids levels(P>0.05).Conclusion 10 mg Rosuvastatin can greatly reduce the serum level of MMP-9 and PAI-1 as compared to 10 mg atorvastatin in patients with ACS ,equivalent to the effect of 20 mg atorvastatin, suggesting that the anti-inflammatory effect is independent of lipid-lowering action.
