2.Development of radiation therapy for pancreatic carcinoma

Adenocarcinoma of exocrine pancreas is a highly malignant tumor with extremely low resectability.Radiotherapy,by inducing necrosis and apoptosis of tumor cells through irradiation effects on cellular DNA,in combination with chemotherapy,has made great contribution to multimodal treatment of this malignancy.The development of radiation therapy for pancreatic carcinoma in recent years was reviewed in neoadjuvant and adjuvant settings,and for locally advanced disease.Further evidence is required to show the impact of radiochemotherapy in the treatment of unresectable disease.
3.Effects of inhalation of different concentrations of sevoflurane combined anesthesia on somatosensory evoked potentials in adolescent patients undergoing scoliosis surgery

Objective To investigate the effects of inhalation of different concentrations of sevoflurane combined anesthesia on somatosensory evoked potentials (SEPs) in the adolescent patients undergoing scoliosis surgery.Methods Forty-five adolescent patients of both sexes,aged 12-17 yr,with body mass index of 15-23 kg/m2,undergoing scoliosis surgery under general anesthesia,were randomly divided into 3 groups ( n =15 each):low concentration sevoflurane group ( group S1 ),moderate concentration sevoflurane group ( group S2 ) and high concentration sevoflurane group (group S3 ).Anesthesia was induced with fentanyl 4 μg/kg,midazolam 0.03 mg/kg,propofol 2.0 mg/kg,and rocuronium 0.6 mg/kg.The patients were endotracheal intubated and mechanically ventilated.Anesthesia was maintained with propofol infusion at 8 mg· kg-1 · h-1 and remifentanil infusion at 0.25 mg· kg-1 ·min-1.After beginning of the surgery,1.8%,4.0% and 6.0% sevoflurane were inhaled for 10 min in groups S1-3 respectively,SEPs were measured by stimulating the posterior tibial nerve and significant SEP suppression was detected and recorded.The wave P40 latency,wave P40-N50 amplitude,and time for P40 waveform changes were recorded before sevoflurane inhalation (baseline) and at 10 min of sevoflurane inhalation,and the time for recovery of P40 waveform was recorded after sevoflurane inhalation was stopped.The prolonged percentage of P40 latency and decreased percentage of P40-N50 amplitude were calculated.Results The rates of significant SEP suppression were 80%,100% and 100% in groups S1,S2 and S3 respectively ( P > 0.05).Compared with group S1,the prolonged percentage of P40 latency and decreased percentage of P40-N50 amplitude were significantly increased,the time for P40 waveform changes was significantly shortened,and the time for recovery of P40 waveform was significantly prolonged in groups S2 and group S3 ( P < 0.05).Compared with group S2,the decreased percentage of P40-N50 amplitude was significantly increased,the time for P40 waveform changes was significantly shortened,and the time for recovery of P40 waveform was significantly prolonged in group S3 ( P < 0.05).Conclusion 1.8%,4.0% and 6.0% sevofiurane combined anesthesia can suppress SEPs in the adolescent patients undergoing scoliosis surgery,and they are not suitable for the surgeries requiring SEP monitoring.
5.Recombinant Human PLD2(rhPLD2)May Significantly Inhibit Expression of GPI-PLD of Guinea Pigs of Chronic Asthma in vivo

The effect of recombinant human phospholipase D2(rhPLD2)in vivo was investigated on the secretion of serum glycosyl phosphatidylinositol-specific phospholipase D(GPI-PLD)in guinea pigs of chronic asthma.Ater treating the guinea pigs attacked by chronic asthma with rhPLD2,the GPI-PLD activity detection was canrried out by phase separation of human placental alkaline phosphatase in Triton X-114.Compared with the healthy guinea pigs(NS group),the serum GPI-PLD in the guinea pigs of chronic asthma are much higher than that of control groups,P≤0.01.Our results showed that rhPLD2 could significantly reduce the secretion of GPI-PLD when the guinea pigs were attacked by chronic asthma.
6.Audiological Detection in the Diagnosis of Children with Acute Non-Suppurative Otitis Media (AN-SOM)

Objective To study the potential roles of acoustic immittance,distortion product otoacoustic e-mission (DPOAE),and the brainstem response audiometry (ABR)in the diagnosis of children with acute non-sup-purative otitis media (AN-SOM).Methods A retrospective analysis was administered to 182 cases with AN-SOM at the otolaryngology department between March 2014 and March 2015.We compared the results of the acous-tic immitance of 218 symptomatic ears and 146 asymptomatic ears,and also the proportion of DPOAE abnormity and ABR abnormity.We analyzed the specialty of symptomatic ears and asymptomatic ears in 3 terms of audiology test results.Results The proportions of abnormal tympanograms,DPOAE,ABR for symptomatic ear were 64.7%, 72.0%,and 57.8%,which were significantly higher than those of in 41.8%,39.7%,and 35.6% in asymptomatic ear.The difference were statistically significant(P<0.05).A further analysis of audiologic results showed that at least one abnormal result was as high as 196 ears (89.9%)in the asymptomatic ear,90 ears (61.6%)showed no symptoms.The differences were statistically significant(P<0.05).The results of audiology tests showed that tym-panograms in symptomatic ears showed diversity,that were 60.6% for type B or C,35.3% for type A,4.1% for the As.Conclusion The early detections by using acoustic immitance,DPOAE and ABR are very important to the diagnosis of children with acute non-suppurative otitis media.It can improve the sensitivity,and reduce the misdi-agnosis.

Objective To study the effects of Fluvastatin on the expression of intercellular adhesion molecule-1 (ICAM-1) in human umbilical vein endothelial cells (HUVECs) induced by C-reactive protein (CRP). Objective To investigate apoptosis of polymorphonuclear neutrophil and activity of caspase-3 in the peripheral blood and discuss their correlation with development of multiple organ dysfunc- tion syndrome (MODS) after multiple injury. Methods A total of 55 patients with multiple injury were included in the prospective study, and divided into two groups, ie, MODS group (multiple injury patients who developed MODS,) and non-MODS group (multiple injury patients who were free from MODS). The activity of caspase-3 was detected by flow cytometry and the serum levels of IL-6 and IL-10 were detected by ELISA. Then, we evaluated whether the neutrophil apoptesis was correlated with the ser- um levels of IL-6 and IL-10 in patients with MODS. Results Compared with non-MODS group, neu- trophil apoptesis was significantly reduced and activated caspase-3 decreased significantly in MODS group (P <0.05). In MODS group, serum IL-6 was increased significantly while serum IL-10 was decreased significantly compared with non-MODS group. The apoptosis of neutrophil in multiple injury patients with MODS had a negative correlation with IL-6 levels but a positive correlation with IL-10 levels. Conclu- sions The delayed apoptosis of neutrophil due to decreased activated caspase-3 may play partial roles in the development of MODS after multiple injury. IL-6 and IL-10 may contribute to the apoptotic changes.
10.Effect of cognitive-behavioral intervention on mental state and quality of life in elderly patients with coronary heart disease

Ninety elderly patients with coronary heart disease were randomly divided into 2 groups:in study group (n = 45) the cognitive-behavioral intervention as well as conventional medical treatment were performed and in control group ( n = 45 ) only conventional treatment was given. Short Form-36 ( SF-36),symptom checklist 90(SCL-90), Hamilton anxiety scale( HAMA), Hamilton depression scale( HAMD)were evaluated before and after intervention, the serum total cholesterol (TC), triglyceride (TG)and body mass index (BMI)were measured, the smoking and drinking behaviors were recorded. After 6-month of intervention the smoking [(0. 6 ± 0. 2)cigarettes/d] and drinking [(10. 1 ± 2. 3)g/d] behaviors, the scores of SF-36 (85 ± 16), SCL-90 (104 ±4), HAMA (10 ±4) and HAMD (12 ±3), the systolic blood pressure [( 123 ± 11 ) mm Hg ( 1 mm Hg = 0. 133 kPa)], diastolic blood pressure [(77 ± 5 ) mm Hg], the fasting blood glucose [(4. 2 ± 2. 6 ) mmol/L] and BMI ( 22.5 ± 0. 5 ) in study group were significantly improved compared to control group ( P < 0. 01 ). The results indicate that cognitive-behavioral intervention can improve the mental state and quality of life in elderly patients with coronary heart disease.
