1.Acoustics Analysis to Phonation of Patients of Lung Qi Deficiency

Objective:To observe vox acoustics features of patients of lung Qi deficiency,understand the influence of different extents of lung Qi deficiency on phonation,and expound the relationship between lung Qi state and phonation. Methods:Apply computer vox analysis system to test the vox sustainable vowel signal(i) of 65 cases of lung Qi deficiency and 20 cases of healthy persons respectively,observe and analyse their acoustics parameters(basic frequency,micro bother of frequency,slight bother of swing and standard yawp energy) of natural sound lowest bourdon and comfort sound and natural sound most alt. Results:For lung Qi deficiency patients,the basic frequency of 3 different phonations is lower than healthy persons,whose lowering trend increases with degrees of lung Qi deficiency,among which,under lowest bourdon of natural sound,there's marked difference of basic frequency between middle and severe lung Qi deficiency and healthy adults(P
2.The efficacy and safety evaluation of three-year specific immunotherapy to allergic rhinitis in children

Objective To evaluate the efficacy and safety of specific immunotherapy (SIT) to allergic rhinitis in children.Methods Sixty-four patients with mite allergy allergic rhinitis in children,were divided into two groups by random digits table:treatment group and control group,each group with 32 cases.Treatment group was given SIT with standardized allergen vaccine for 3 years on the basis of symptomatic therapy,control group only received symptomatic therapy.Observation indexs included rhinitis symptoms score,drug score,skin index (SI),serum specificity IgE (sIgE),peripheral eosinophil (Eos) counting,development of asthma and the new sensitization.Results The Eos counting,SI after treatment 3 years in treatment group were significantly better than those before treatment and those in control group after treatment 3 years[(0.14 ± 0.12) × 109/L vs.(0.74 ± 0.18) × 109/L,(0.78 ± 0.36) × 109/L and 1.03 ± 0.13 vs.1.51 ± 0.32,1.51 ± 0.37] (P < 0.01).There was no statistically significant difference in sIgE between two groups (P >0.05).The rhinitis symptoms score,drug score in two groups after treatment 1,2,3 years were significantly better than those before treatment (P < 0.01).The rhinitis symptoms score,drug score in treatment group after treatment 1,2,3 years were significantly better than those in control group(P < 0.01).The rate of new sensitization in treatment group was significantly lower than that in control group [3.1% (1/32)vs.34.4% (11/32),P < 0.01].Conclusion Keeping long-term SIT is effective and safe for children's allergic rhinitis induced by mite,it also prevents new allergen appeared and allergic rhinitis development for asthma.
3.Efficacy evaluation of standardized dust mite allergen specific immunotherapy to patients of allergic rhinitis.

OBJECTIVE:
To evaluate the efficacy of mite allergen specific immunotherapy (SIT) to patients of allergic rhinitis.
METHOD:
A total of 102 patients with mite allergy were recruited into the study. They were randomly divided into two groups: SIT group (n = 51) and ST (symptomatic therapy) group (n = 51). They were given SIT with standardized allergen vaccine for 3 years or only symptomatic therapy respectively. Observation items include: rhinitis symptom scores, drug score, skin prick test result, serum specificity IgE (sIgE), peripheral eosinophil counting. The development of asthma and new allergens sensitization was also assessed.
RESULT:
The blood eosinophil numbers, skin test index, rhinitis symptom scores and drug scores were all decreased significantly after the treatment with SIT for 3 years compared to that of ST group (P < 0.01). Although the level of serum slgE was decreased, no statistic diferences were found. No patients developed asthma in SIT group, and only 2.1% of patients had new allergen sensitization; 17.4% of those in ST group developed asthma, 32.6% had new sensitization. No severe adverse events occurred.
CONCLUSION
Keeping long-term SIT is effective and safe for patients with allergic rhinitis induced by mite, which can also prevent new allergen sensitization and development for asthma.

4.Clinical application of sural nerve nutritional vessel axial flap pedicle with the lowest perforating branch of the peroneal artery

Objective To report the improved operative technique and clinical results of sural nerve nutritional vessel axial flap Methods Based on anatomical studies,the sural nerve nutritional vessel flap pedicle with the lowest perforating branch of the peroneal artery were performed to repair the soft tissue defects of the lower leg,the ankle and the foot Results The modified flaps were applied in 7 cases,all flaps survived completely The size of the flap ranged from 30 cm?10 cm to 23 cm?8 cm The length of pedicles were 1 6~3 0 cm The colour and texture of flaps were excellent,the outline and functional results were satisfactory after 6~12 months follow up Conclusions This new procedure can enlarge the survival and repairing area of the flap,it can also solve the distortion problem of fasciocutaneous pedicle The blood supply of this flap is reliable Flap elevation is easy and quick It is very useful in repairing large skin defects of the lower leg,the ankle and the foot

6.Effect of integrin expression on procollagen synthesis of fibroblasts in wound healing

Objective To investigate the effect of integrin expression on procollagen synthesis of fibroblasts in wound healing. Methods Antisense oligonucleotides were used to intercept the expression of integrin ? 5 or ? 1 subunit on fibroblasts respectively in wound healing, then the changes of procollagen mRNA were detected by RT-PCR. Results The antisense oligoncleotdes could specifically inhibit the expression of integrin subunit. Decreased expression of integrin?5 or ?1 subunit on fibroblasts significantly lowered mRNA of procollagen ? 1(Ⅰ) and ? 1(Ⅲ). Conclusion Reduction of integrin expression on fibroblasts in wound may inhibit procollagen synthesis in the level of transcription. These results provide the experimental gists on studying of scar reduction in wound healing.
7.Clinical Features of Acute Leukemia with Positive Mixed Lineage Leukemia Fusion Gene in Children

Objective To analyze the clinical features of acute leukemia(AL) with positive mixed lineage leukemia(MLL)fusion gene in children,and explore their treatment protocols,prognosis factors,and so on.Methods Clinical features,treatment protocols,and prognosis factors were studied retrospectively among 51 AL patients with MLL fusion gene.MLL fusion gene was detected by morphology immunology,cytogenetics,molecul arbiology and reverse transcrption polymerase chain reaction(RT-PCR).Results Fifty-one AL patients with MLL fusion gene positive,included 37 cases of acute lymphoblastic leukemia(ALL) and 14 cases of acute myelocytic leukemia(AML).Forty-two patients exhibited abnormal clonal chromosome 11.MLL fusion gene rearrangements and MLL fusion gene partial tandem duplication were found among 36 cases and 15 cases,respectively.Thirty-two cases who received regular chemotherapy were followed up.Twenty-four cases including 19 cases of ALL and 5 cases of AML had achieved complete remission(CR).Six cases including 5 cases of ALL and 1 cases of AML had achieved more than 2 years CR.Sixteen cases were alive update including 12 cases of ALL and 4 cases of AML.Ten cases of positive MLL fusion gene were turning negative.Up to now,6 cases relapsed and 6 cases were dead.Conclusions The incidence of AL children with positive MLL fusion gene is low.It has some features,such as,high replapse rate and poor prognosis.A few patients sensitive to chemotherapies can achieve CR.They live with constant negative MLL fusion gene.
8.The relationship between clinical skill training and medical students cultivation

Objective This research approaches the methods of intensifying the medical clinical skills training to develop students’medical abilities in clinical practice.Methods We chose 129 seven-year medical students.They were devided into two groups in random.The con-trol group entered the clinical practice directly,while the experimental group accepted the clini-cal skill training before they entered the clinical practice.Results The examination record and qualification number of experimental group were higher than the control group obviously.After the clinical skill training,the examination record became higher than before.Conclusion The experiment shows that receiving clinical skill training before entering clinical practice is the ef-fective method of elevating medical students’clinical abilities and practical skill level.
9. Apoptosis of Tca8113 cell induced by antitumor component-I from Agkistrodon acutus venom through CHOP/Caspase-12 pathway

AIM: To investigate the effect of endoplasmic reticulum stress pathway on the apoptosis of tongue squamous cancer Tca8113 cells induced by antitumor component-I from Agkistrodon acutus venom (AAVC-I). METHODS: The in vitro experiments were performed on subculture tongue squamous cancer Tca8113 cells in their growth period. A normal control group, a DL-dithiothreitol (DTT) positive control group and different AAVC-I concentrations were set according to the experiment objective. MTT assay was used to detect the proliferation inhibition of Tca8113 cells after been treated with different concentrations of DTT and AAVC-I for 24 h. The results were used to choose appropriate concentrations of DTT and AAVC-I in DTT positive control group and AAVC-I treated group, respectively. HE staining and Annexin V-FITC/PI double fluorescence staining were used to monitor the apoptosis of Tca8113 cells. Western blot was used to identify the expression levels of apoptosis-related proteins including endoplasmic reticulum stress glucose-regulatory protein 78 (GRP78), enhance-binding protein-homologousprotein (CHOP), cysteine-containing aspartate specific protease-12 (Caspase-12), cysteine-containing aspartate specific protease-9 (Caspase-9) and cysteine-containing aspartate specific protease-3 (Caspase-3).RESULTS:The proliferation inhibition of Tca8113 cells increased with an increased concentration of AAVC-I concentration (P<0.05), causing cell shrinkage, increased cell gaps, cytonuclear condensation, cell fragmentation, the appearance of apoptotic bodies, and increased rate of apoptosis (P<0.05). In addition, the expression level of GRP78 protein, CHOP protein, proteins of Caspase-12, Caspase-9 and Caspase-3 were increased (P<0.05).CONCLUSION: Endoplasmic reticulum stress CHOP/Caspase-12 pathway plays an important role in AAVC-I induced Tca8113 cells apoptosis.
10.Atorvastatin inhibits atherogenesis by RXRα-mediated depressing oxida-tive stress in STZ-induced diabetic ApoE-/-mice with fat-rich diet

AIM:To explore the effects of atorvastatin (Atorv) on atherosclerosis in streptozotocin (STZ)-in-duced diabetic apolipoprotein E knockout ( ApoE-/-) mice with fat-rich diet and the possible mechanism .METHODS:C57 mice served as control.ApoE-/-mice (n=34) fed with high-fat diet were randomly divided into ApoE-/-group, STZ-ApoE-/-group and STZ-ApoE-/-+Atorv group.Intraperitoneal injection of streptozotocin was performed to create di-abetic animal model .Blood glucose was determined by glucose oxidase method .Blood lipid levels were detected by enzymic method or selective homogeneous method .The plaque area in the thoracic aorta was measured by HE staining .The protein level of nicotinamide-adenine dinucleotide phosphate ( NADPH) oxidase subunit gp91phox in the thoracic aorta was deter-mined by Western blotting .The levels of reactive oxygen species ( ROS) in blood and thoracic aorta homogenates were de-tected by Fenton reaction and Griess reagent .Human umbilical vein endothelial cells ( HUVECs ) were isolated from healthy umbilical cords by collagenase I and cultured .ROS production was detected by flow cytometry .NADPH oxidase ac-tivity was measured using lucigenin assay .Effects of retinoid X receptor α( RXRα) on inhibition of oxidative stress by ator-vastatin were evaluated by RNA interference and plasmid transfection .RESULTS: (1) Compared with C57 group, the plaque areas of the thoracic aorta in ApoE-/-group were increased .No difference of the fasting glucose between the 2 groups was observed.The levels of triglyceride (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), thoracic aorta gp91phox protein and ROS in blood and thoracic aorta homogenates were higher in ApoE-/-group than those in C57 group.(2) Compared with ApoE-/-group, the plaque areas of the thoracic aorta in STZ-ApoE-/-group were further enlarged [(314.13 ±35.72) μm2 vs (215.88 ±34.19) μm2, P<0.05].The levels of blood glucose, TG, TC and LDL-C, thoracic aorta gp91phox protein and ROS in blood and thoracic aorta homogenates were higher in STZ-ApoE-/-group than those in ApoE-/-group (P<0.05).(3) Compared with STZ-ApoE-/-group, the plaque areas of the thoracic aorta in STZ-ApoE-/-+Atorv group were reduced [(217.47 ±24.56) μm2 vs (314.13 ±35.72) μm2, P<0.05].The levels of blood glucose , LDL-C, TC, HDL-C and TG showed no significant difference between the 2 groups.Thoracic aorta gp91phox protein level and ROS production in blood and thoracic aorta homogenates were lower in STZ -ApoE-/-+Atorv group than those in STZ-ApoE-/-group (P<0.05).(4) High glucose-induced increases in NADPH oxidase activity and gp91phox expression were significantly inhibited by atorvastatin (10-6 mol/L) in HUVECs.The inhibitory effects of atorvasta-tin on high glucose-induced ROS production and NADPH oxidase activation were largely impaired when the cells were trans -fected with RXRαsiRNA.However , the effect of atorvastatin was significantly strengthened when RXRαwas over-expressed in the HUVECs transfected with RXRαplasmid.CONCLUSION: Atorvastatin inhibits atherogenesis by depressing high glucose-induced oxidative stress in diabetic ApoE-/-mice with fat-rich diet.The anti-oxidative stress effect of atorvastatin is mediated by RXRα.
