1.Clinical significance of autoimmune antibody in patients with lupus nephritis

Objective To investigate the clinical significance autoimmune antibody in patients with lupus nephritis(LN).Methods ELISA assay was performed to examine anti-C1q antibody and anti-nucleus antibody;immunoblot assay was performed to examine anti-dsDNA antibody,anti-nucleosome antibody and anti-Sm antibody in sera of 38 patients with LN and 22 patients of system lupus erythemalosus without renal damage.Standard methods were uaed for laboratory testing.Results The positive rate of anti-C1q antibody,anti-dsDNA antibody,anti-nucleosome antibody,anti-Sm antibody and anti-nucleus antibody were significantly higher in LN than in controls(P<0.01).AntiC1q antibody and anti-nucleus antibody were significantty higher in LN than in controls(P<0.01,P<0.05).Serum levels of anti-C1q antibody was positively correlated with SLEDAI score and the level of anti-nucleus antibody,but was negatively oorrelated with C3、 C4.Conclusion Anti-C1q antibody,anti-dsDNA antibody,anti-nucleosome antibody and anti-nucleus antibody are suggested to play a role in the pathogenesis of LN and can be used as diagnostic tools and disease activity markers.

Objective Surgical treatment is the first choice for treatment in pituitary adenomas except for prolactinomas?In part of patients after operation would occur hypopituitarism,which with acute adrenal crisis is the most serious, and chronic adrenal insufficiency is different degree affects the patient's quality of life?Therefore,it is very important to accurately assess the hypothalamic pituitary adrenal( HPA) axis in patients with pituitary adenoma?But there is still a lack of reliable methods for evaluating adrenal axis?We reviewed the harm of adrenal insufficiency,HPA axis evaluation method of the new progress and postoperative glucocorticoid replacement treatment of new ideas, cleared the importance of reassessment and regular follow?up and the development of individual programs.
4.Rediscovery the Effect of Intensive Insulin Therapy on Stress Hyperglycemia in Critical Trauma Patients

Objective To investigate the effect and safety of intensive insulin therapy on patients who had stress hy-perglycemia induced by critical trauma, when different blood glucose was aimed in surgery intensive care unit (SICU). Methods We retrospectively analyzed SICU patients who were admitted between 2010.1-2012.7 with admission blood glucose and 30 minutes blood glucose both over 11.1 mmol/L ,and without known history of diabetes. In total, 153 pa-tients were set into three groups according to their target blood glucose:intensive insulin therapy (IIT) group with target blood glucose of 4.0-6.1 mmol/L;NICE SUGAR (NST) group with target blood glucose of 6.2-8.3 mmol/L;conventional in-sulin therapy(CIT)group with target blood glucose of 9.9-11.1 mmol/L. Each group had 51 patients. To collect data from these three groups of patients,we compared daily insulin doses , hypoglycemia incidence, final blood glucose, APACHEⅡscores upon discharging from ICU, time of the ventilatory support, length of staying in ICU, morbidity and mortality rate. Results Comparing these three groups, daily insulin dosage and hypoglycemia incidence, were significantly lower in NST and CIT group than in IIT group. Daily insulin dosage was higher in NST group than in CIT group;no significant difference of hypoglycemia incidence was shown between NST group and CIT group. The final blood glucose was lowest in IIT group and highest in CIT group (P<0.05). APACHE II score was 9.3±7.5 upon discharge from ICU in NST group. Ventilation time and duration of ICU admission both were shortest in NST group but show no significant difference between IIT and CIT group . No significant difference of complicate incidence and mortality rate was indicated among all three groups. Conclusion In patient with stress hyperglycemia induced by critical trauma,maintaining the patients’final blood glucose between 6.2-8.3 mmol/L can effectively control the stress hyperglycemia,improve prognosis and reduce the mortality of hypoglycemia.
5.Influencing factors of heart failure among patients with continuous ambulatory peritoneal dialysis

Objective To explore the influencing factors of heart failure among patients with continuous ambulatory peritoneal dialysis(CAPD).Method Clinical data were collected from the histories of 158 patients receiving CAPD to investigate the factors inducing heart failure for the purpose of finding out the potential independent influencing factors using t test or chi-square test for univariate analyses and logistic regression in multivariate analyses.Results The incidence of heart failure was 36.71%among the enrolled CAPD patients.Single factor analysis showed that level of volume management,age,hemoglobin,serum pre-albumin, cholesterol and c-reactive protein were related to heart failure.Analysis of multiple variables logistic regression revealed that levels of volume management,serum pre-albumin and blood urea nitrogen were related to heart failure.Conclusions It is an easy,effective and cheaper way for nurses to increase patients’ability of volume control.The strategy including remedying malnutrition and decreasing the level of uremic toxins might be effective for delayed exacerbation of cardiac function among CAPD patients.
6.Clinical analysis of lower uterine segment pregnancy

Objective To discuss the diagnosis and management for lower uterine segment pregnancy.Methods Clinical data of 6 cases of lower uterine segment pregnancy between January 2005 and February 2006 were retrospectively analyzed.Results Five patients had a history of caesarean section,while another one had undergone repeated pregnancy.Uterine artery embolization(UAE) in combination with methotrexate(MTX) injection was carried out in 2 cases,and uterine artery embolization only,MTX injection only,and total hysterectomy were employed in 1 case,1 case,and 2 cases,respectively.Conclusions Previous caesarean section and repeated pregnancy are risk factors of the occurrence of lower uterine segment pregnancy.Uterine artery embolization in combination with methotrexate injection is a safe and reliable management option.
7.Effects of cornel iridoid glycoside on inflammatory reaction in the brain of traumatic brain injury rat model

AIM:To observe the effects of cornus iridoid glycosides(CIG)on inflammatory reaction especially the inflammatory cytokines in the brain after traumatic brain injury,and to explore the possible mechanisms of its neuroprotective effect.METHODS:SD rats were intragastrically administered with different doses of CIG(30,60 and 120 mg?kg-1?d-1)for 7 d.The traumatic brain injury rat model was induced by improved Feeney's fall weight method,and the brains were taken out 24 h and 72 h after brain injury,respectively.The morphological changes were observed by HE staining in the cerebral cortex.The expressions of inflammatory cytokine tumor necrosis factor-?(TNF-?)and interleukin-1?(IL-1?)were detected by immunohistochemical method.The image processing and statistical analysis were used to measure the number and the area of immunoreactive cells.RESULTS:HE staining showed the pathological changes were serious in the cerebral cortex of model group,and compared with the model group,the pathological changes were obviously reduced in CIG group.The positive immunoreactive cells of TNF-? and IL-1? were mainly distributed around the foci of contusion,the expressions of TNF-? and IL-1? in the model group were significantly higher than those in sham operated group,and the high expressions were sustained from 24 h to 72 h after brain injury.Compared with the model group,the levels of TNF-? and IL-1? in the brain of CIG treatment groups were obviously decreased in a dose-dependent manner and the inhibitory effects of TNF-? and IL-1? were more significant at 72 h after brain injury.CONCLUSION:CIG may have neuroprotective effect on traumatic brain injury through inhibiting the expression of inflammatory cytokines and reducing the inflammatory reaction.

Positive growth factors play an active role in improving host metabolism, and then promote the ability to receive the operation and chemotherapy. However, there are many problems concerning their security. They are able to keep tumor cells survive and proliferate which can increase the number of s-phase cells and the expression of some cancer genes (c-myc, etc). They probably, in theory, enhance the tumor’s response to chemotherapy by increasing the number of proliferating tumor cell. They offer a new insights into tumor’s therapy.
