Chinese Journal of Endocrinology and Metabolism 1986;0(03):-
The clearance of proteins differing in charge and/or size (IgG4, total IgG and albumin) was evaluated in 95 diabetic patients. Urinary IgG4 values increased in group mi-croalbuminuria. The increased urinary IgG4 excretion correlated significantly with the degree of albuminuria and diabetic retinopathy. Urinary total IgG levels increased obviously only in group with massive macroalbuminuria. The IgG4/total IgG ratio was highest in group mi-croalbuminuria, and its change had a special pattern. Urinary IgG4 and IgG4/total IgG ratio may be sensitive parameters in assessing early protein charge-selectivity impairment and in detecting incipient diabetic nephropathy.

OBJECTIVETo investigate the regulatory effect of miR-126 on epidermal growth factor-like domain 7 (EGFL7) in ECV-304 cells.
METHODSThe miR-126-expressing plasmid targeting EGFL7 (plegfp-N1-miR-126) was constructed and transiently transfected into ECV-304 cells via liposome. The changes in the mRNA and protein expressions of EGFL7 in the transfected cells were analyzed by fluorescence quantitative RT-PCR and Western blotting.
RESULTSTransfection with the recombinant plasmid plegfp-N1/miR-126 resulted in decreased EGFL7 expression with the passage of time, and the expression reached the lowest level at 48 h after the transfection. The expression of EGFL7 protein was reduced by 67% following the transfection in comparison with the control level, while the transfection with the empty vector resulted in a reduction only by 6.5% relative to the control level.
CONCLUSIONSmiR-126 can downregulate EGFL7 expression at the protein level in ECV-304 cells.

Objective To evaluate the therapeutic effects of adult neural stem cells derived from the brain, adipose tissue and bone marrow on spinal cord injuries in adult rats. Methods The brain subventricular zone (SVZ), adipose tissue and bone marrow from the same rat were obtained to induce the neural stem cells (NSCs). In 72 SD rats with spinal contusive injury, the NSCs from the 3 origins were grafted into the injured spinal cord one week after the injury, with 24 rats as the saline control group and another 24 as the sham-operated group. The locomotor recovery of the rats was evaluated according to the BBB scores, and the cell survival, distribution, migration and differentiation in the injured spinal cord were evaluated by immunohistochemistry. Results Compared with the rats in sham-operated and saline groups, the rats receiving transplantation of NSCs of different origins all showed significantly increased BBB scores. At 9 weeks after the transplantation, the rats receiving brain SVZ-derived NSCs (SVZ-NSs) exhibited significantly improved locomotor function compared with those grafted with the other two NSCs (P<0.05). The SVZ-NSs showed significantly higher Brdu/nestin+cell percentage than bone marrow-derived NSCs(BM-NSs) and adipose-derived NSCs(AD-NSs) at 4 weeks after grafting, but till 8 week after the grafting, only a few positive cells were found in the injured spinal cord in the 3 groups, without significant difference between them (P>0.05). Conclusion Grafting of the NSCs derived form the brain SVZ, adipose tissue and bone marrow all help improve the locomotor recovery of the rats following spinal cord injury, and the SVZ-NSs has the most obvious effect. But AD-NSs may seem a better option than those of other origins for repairing the injured spinal cord due to their abundant sources and strong proliferation ability.s

Objective:To explore the genetic etiology and the value of early diagnosis of early onset epileptic encephalopathy (EOEE) with unknown etiology. Methods:A total of 60 children with EOEE of unknown etiology were prospectively enrolled in the outpatient and inpatient departments of Fujian Provincial Hospital from January 2018 to January 2021.Peripheral blood was collected prospectively for whole-exome sequencing and copy number variation (CNV) detection to analyze the clinical characteristics and genetic sequencing results of the children.Results:Twenty-four patients with EOEE-related pathogenic or suspected pathogenic mutations were detected, including infantile spasms (10 cases), Dravet syndrome (3 cases), pyridoxine-dependent epilepsy (1 case) and ohtahara syndrome (1 case), and unknown epileptic encephalopathy (9 cases). The onset age of EOEE-related patients ranged from 1 day to 11 months (median age was 4.2 months), the treatment age ranged from 2 days to 4 years (median age was 10 months), and the age of diagnosis was controlled within 1 month after treatment. There were 20 cases (33.3%) single gene variants and 4 cases (6.7%) CNV variants.A total of 13 genes were involved: KCNQ2, SCN1A, SCN8A, CACNA1E, CDKL5, PPP3CA, PCDH19, TSC1, TSC2, ZEB2, ALDH7A1, DCX and HNRNPU.The 4 CNV abnormalities were 17p13.3 deletion, 11q23.3q25 deletion, 1q36.31-p36.33 deletion, 1q43-1q44 deletion and Xp22.33 duplication, respectively.Totally, 20 mutations were new loci reported for the first time at home and abroad; 11q23.3q25 deletion that resulted in infantile spasm was first reported at home and abroad.Infantile spasm caused by ZEB2 mutation and epileptic encephalopathy caused by PPP3CA gene were both reported for the first time in China. Conclusions:Gene and CNV are important potential causes of children suffering from EOEE.When the etiology is unclear, the combination of whole-exome sequencing and CNV sequencing technology can improve the diagnosis level of genetic etiology of children with EOEE.The early genetic detection of these children can early diagnose and accurately treat epilepsy.

OBJECTIVETo investigate the medium-term curative effects of locking proximal humerus plate for the treatment of comminuted fractures of proximal humerus, and provide evidences for the clinical practice.
METHODSFrom August 2005 and April 2008, 23 patients with comminuted fractures of proximal humerus were treated with locking plates, including 12 males and 11 females, aged 27 to 76 years old (averaged 51.5 years old). There were 18 cases of traffic accident injuries, 4 cases of falls injuries, and 1 case injured after heavy pressure. According to Neer classification, 11 cases were three-part fractures, and 12 cases were four-part fractures. Outcomes were assessed with radiography and the Constant-Murley (C-M) shoulder evaluation.
RESULTSAll the patients got primary healing of incisions. Twenty-three patients were followed up, and the duration ranged from 17 to 49 months, with an average of 35.25 months. Twenty patients had fracture healing during 4 to 7 months after operation. There was no significant differences among 3, 6 and 12 months after operation in C-M scoring. The average C-M score was (79.85 +/- 17.23) points (38 to 100 points) at the 12th month after operation, 8 cases got an excellent result, 8 good, 5 fair, and 2 poor. In the LPHP plus bone graft group 6 cases got an excellent result, 4 good, 3 fair, and 1 poor; in LPHP fixation group 2 excellent, 4 good, 2 fair, and 1 poor.
CONCLUSIONThe medium-term curative effect of the locking proximal humerus plate in the treatment of proximal humeral fractures is significant. For the comminuted fractures of proximal humerus combined with osteoporosis and bone defects, bone graft should be performed routinely.

BACKGROUND:
Although endovascular therapy has been widely used for focal aortoiliac occlusive disease (AIOD), its performance for extensive AIOD (EAIOD) is not fully evaluated. We aimed to demonstrate the long-term results of EAIOD treated by endovascular therapy and to identify the potential risk factors for the loss of primary patency.
METHODS:
Between January 2008 and June 2018, patients with a clinical diagnosis of the 2007 TransAtlantic Inter-Society Consensus II (TASC II) C and D AIOD lesions who underwent endovascular treatment in our institution were enrolled. Demographic, diagnosis, procedure characteristics, and follow-up information were reviewed. Univariate analysis was used to identify the correlation between the variables and the primary patency. A multivariate logistic regression model was used to identify the independent risk factors associated with primary patency. Five- and 10-year primary and secondary patency, as well as survival rates, were calculated by Kaplan-Meier analysis.
RESULTS:
A total of 148 patients underwent endovascular treatment in our center. Of these, 39.2% were classified as having TASC II C lesions and 60.8% as having TASC II D lesions. The technical success rate was 88.5%. The mean follow-up time was 79.2 ± 29.2 months. Primary and secondary patency was 82.1% and 89.4% at 5 years, and 74.8% and 83.1% at 10 years, respectively. The 5-year survival rate was 84.2%. Compared with patients without loss of primary patency, patients with this condition showed significant differences in age, TASC II classification, infrainguinal lesions, critical limb ischemia (CLI), and smoking. Multivariate logistic regression analysis showed age <61 years (adjusted odds ratio [aOR]: 6.47; 95% CI: 1.47-28.36; P = 0.01), CLI (aOR: 7.81; 95% CI: 1.92-31.89; P = 0.04), and smoking (aOR: 10.15; 95% CI: 2.79-36.90; P < 0.01) were independent risk factors for the loss of primary patency.
CONCLUSION
Endovascular therapy was an effective treatment for EAIOD with encouraging patency and survival rate. Age <61 years, CLI, and smoking were independent risk factors for the loss of primary patency.
