1.The curative effect observation of Danhong injection combined with atorvastatin calcium in the treatment of lacunar cerebral infarction of the elderly

Objective To observe the clinical curative effect of Danhong injection combined with atorvastatin calcium in the treatment of lacunar cerebral infarction of the elderly.. Methods 125 elderly patients with lacunar cerebral infarction were randomly recruited into two groups, a control group(n=60 cases) was treated with atorvastatin calcium on the basis of conventional symptomatic treatment, and an observation group (n=65 cases) was treated with Danhong injection combined with atorvastatin calcium on the basis of conventional symptomatic treatment, the courses of two groups were 14 d. The clinical curative effect, the degree of nerve function defect score, the changes of hemorheology of two groups before and after the treatment were compared. Results The total effective rate of the observation group(89.2%)was significantly higher than that of the control group(71.7%)(P<0.05). The degree of nerve function defect score(16.24±4.63)of observation group after treatment improved significantly better than that of control group(22.17±7.38), P<0.05, and the whole blood viscosity(high), whole blood viscosity(low), plasma viscosity and fibrinogen [(3.62±0.64)mPa?s,(8.07±2.45)mPa?s, (1.61±0.30)mPa?s, (2.76±0.28)g/L]decreased significantly than that of the control group[(4.73±0.85)mPa?s, (9.86±3.26)mPa?s, (2.05±0.33)mPa?s,(3.94±0.35)g/L], P<0.05. Conclusion The treatment of Danhong injection combined with atorvastatin calcium for elderly lacunar cerebral infarction had a good therapeutic effect. It could improve nerve function defect degree and blood rheology indexes.
2.Comparison of Preventive Effect of Maternal-Infant Hepatitis B Virus Vertical Transmission by Symphysial Immunoprophylaxis

Objective To investigate the efficacy by inoculated with hepatitis B immunoglobulin(HBIG) and hepatitis B vaccine in dif-ferent doses and time points to prevent maternal-infant vertical transmission of hepatitis B virus(HBV).Methods Gravidas positive for HBV were selected and measured HBV DNA,according to difference HBV DNA degrees divided into A,B,C groups.Every group was randomly divided into 4 groups using a variety of combined immunity methods to compare its efficacies.Results There were no significant differences in the positive rate of HBV and HBsAb in A,B groups by statistics analysis.There were significant differences in the positive rate of HBV and HBsAb in group C by statistics analysis.Conclusion According to the different HBV DNA take different combined immunodeficiency approach to prevent maternal-infant vertical transmission.
3.High-resolution melting analysis for detection of K-ras mutation in colorectal cancer and its clinical significance

Objective The K-ras gene plays a major role in the development , progression, and drug selection for the target treatment of colorectal cancer .The aim of this study was to evaluate the feasibility of detecting K-ras mutation in colorectal cancer by high-resolution melting ( HRM) analysis and to investigate the relationship between K-ras mutation and the clinicopathological parame-ters of colorectal cancer. Methods We collected the tissue samples of colorectal cancer from 179 patients, detected the mutations in codons 12 and 13 of the K-ras gene, and analyzed the relationship between K-ras mutation and the clinicopathological parameters of the patients. Results In the 179 cases of colorectal cancer, K-ras mutation was found in 77 (43.02%), significantly higher in those aged ≥60 years than in those aged <60 years (55.17% vs 33.70%, P<0.05).Multivariate logistic regression analysis showed a significant influence of age on K-ras mutation (OR=1.506, 95%CI:1.028-2.011, P<0.05). Conclusion HRM analysis is a rapid, sensitive, and inexpensive diagnostic tool for the detection of K-ras mutation, and K-ras mutation is associated with the age of colorectal cancer patients .
5.Application of laparoscopy in assisted reproduction techniques

Objective To investigate the clinical significance of gamete intrafallopian transfer (GIFT) under laparoscope. Methods The gonadotropin was adopted to induce superovulation and the spermatic fluid was managed with the swim-up method. After oviums were dislodged by puncture under laparoscope, gametes were transplanted by intubating the fallopian tube. Results Among a total of 19 patients, 4 were pregnant, the pregnancy rate being 21.1% (4/19). Three women had a delivery at term and 1 had an abortion. No serious complications were seen. Of the 15 unfertilized patients, normal pregnancy was observed within 6 months after surgery in 5 patients, the circle pregnancy rate being 5.6% (5/89). Conclusions GIFT under laparoscope is a practical assisted reproductive technique (ART) that may achieve a relatively high pregnancy rate. Some other pelvic diseases resulting in infertility can be treated simultaneously, which contributes to natural fertilization in the future.
6.Emergency repair method and its effect in treatment of large avulsion injury of lower extremity in elderly patients

Objective To evaluate the emergency repair method and its effect for treatment of extensive avulsion injury of the lower extremity in the elderly patients.Methods From May 2008 to February 2015,11 patients aged over 60 years sustaining large avulsion injury (across the knee or ankle joint) of the lower limb in traffic crushes were treated by emergency debridement and one-stage repair with the procedures of autologous skin replantation,arteriovenous anastomosis and vacuum sealing drainage (VSD).Two patients sustained injury to both lower limbs,and nine patients sustained a unilateral injury.Effect of wound repair was evaluated with skin flap survival area.Active and passive motility of the knee and ankle were recorded at postoperative 24 h.Results One patient with multiple fracture and extensive muscular inactivity due to severe crush injury to the lower limbs had poor blood supply after the replantation and was amputated at postoperative 24 h.The remaining ten patients with large skin avulsion showed 90% of the avulsed skin survived after operation.At the follow-up of 3-24 months,the ten patients showed no obvious pigmentation or scar tissue,were satisfied with the function recovery in knee (106°-110° in flexion and 0°-5°in extension) and ankle joint (15°-20°in dorsiflexion and 30°-35° in plantar flexion),and could walk freely,including excellence outcomes in 3 patients and good outcomes in 6 patients.Conclusion The emergency repair methods including autologous skin replantation,arteriovenous anastomosis and VSD combined with one-stage in situ repair can attain satisfactory clinical effect.
7.Risk factors for acute myeloid leukemia in adults: a hospital-based case-control study

Objective To determine risk factors of environment and lifestyles involved in acute hematology departments of 29 secondary-care and tertiary-care hospitals in Shanghai. There were 206 hospitalized patients with de novo AML during June 2003 to April 2007 meeting the diagnostic criteria for AML by the World Health Organization, excluding those with definite history of radiotherapy or chemotherapy, and 412 age- and gender-matched controls with non-hematological other diseases from the same hospitals. All the participants were interviewed face-to-face using a standard structured questionnaire,including personal medical history, family medical history, history of medications, traditional Chinese herb medicines, exposure to occupational hazards, lifestyle factors, etc. in the past five years prior to onset of the disease. Conditional univariate and multivariate logistic regression analyses were performed to identify risk factors for AML. Results Logistic regression analysis showed that taking traditional Chinese herb medicine might be a protective factor for AML, with an odds ratio (OR) of 0. 39 and its 95% confidence interval (CI) of 0. 16-0. 98. Potential risk factors for AML included history of diabetes and taking hypoglycemic agents ( OR: 1.39, 95% CI: 0.67 -2. 87), occupational exposure to metal ( OR: 1.73, 95% CI: 0. 77 -3. 91 ), glue, paint and other coating materials, etc. ( OR: 1.72, 95% CI: 0. 93 - 3.2), raising animals (OR: 1.33, 95% CI: 0. 79 -2. 24). No association of living within a distance of 100 meters from high voltage power transmission lines, home renovation and decoration in the past year, hair dyeing, smoking,alcohol drinking with AML in adults was found. Conclusions Factors mentioned above such as occupational exposure to metal, glue, paint and other coating materials potentially increase risk for AML in adults,however taking traditional Chinese herb medicine is possibly a protective factor for it.
8.Evidence summary of health workers′ self-protection during aerosol-generating procedures in patients with acute respiratory infectious disease

Objective:To evaluate and summarize the best evidence of health workers respiratory protection during aerosol-generating procedures in patients with acute respiratory infectious diseases.Methods:We searched EBM Guidelines, Essential Evidence Plus, Dynamed, UpToDate, JBI, BMJ, Clinical Key, Cochrane Library, NICE, AARC, PubMed, EMbase, CKNI, Wanfang to collect related literature including guidelines,evidence summary, recommended practices, standards, consensus and systematic reviews.Results:A total of 20 articles were included, including 10 guidelines, 1 consensus, 2 standards, 2 evidence summaries, and 5 systematic reviews. 24 best evidences including aerosol-generating procedures, training of protective equipment using, principles of protective equipment using, protective equipment using, aerosol operating environment control and medical staff self-monitoring were summarized.Conclusion:The study integrated the best evidence of healthcare workers respiratory protection during aerosol-generating procedures in patients with acute respiratory infectious disease. It is recommended to combine the current status of institutional protection resources and clinical practice experience to promote the conversion of the best evidence to clinical practice.
9.Effect of the Traditional Chinese Medicine Combined with Traction and Physical Therapeutic on Lumbar Intervertebral Discprolapse

ObjectiveTo observe the curative effect of the Chinese traditional medicine combined with traction and physical therapeutic on lumbar intervertebral discprolapse (LIDP).Methods95 LIDP patients were randomly divided into the treatment group (50 cases) and control group (45 cases). Patients of the treatment group were treated by Chinese traditional medicine, traction and physical therapy. Patients of the control group were treated by traction and physical therapy only.ResultsThe total clinical effective rate of the treatment group was 100%, but that of the control group was 75.6%, there was a significant difference between two groups (P<0.05).ConclusionChinese traditional medicine therapeutics is an effective non-surgical therapeutics for LIDP.
