1.Etiological Analysis and Differential Diagnosis of Syncope in Children
Journal of Applied Clinical Pediatrics 2006;0(13):-
Objective To study the feature of the etiology of pediatric syncope by cases study, and to discuss the feasibility on identification between the cardiac and neurally-mediated syncope with its inducements and symptoms. Methods One hundred and eleven patients were etiologically classified by standard diagnostic procedure to summarize the inducements, presymptoms and symptoms. The data about cardiac and neurally-mediated syncope were statistically treated to find their characteristic manifestation. Results Among these,60.4% was non-cardiovascular syncope,18.9% was postural hypotension,18.0% was vasovagal syncope and 9% was cardiac syncope. Most of them were non-cardiac syncope and neurally-mediated syncope was on the top, while cardiac syncope was rare in children.Prolonged standing and nausea was the feature of the neurally-mediated syncope, and the cardiac syncope is associated with physical exercise. Conclusions Most pediatric inpatients were non-cardiovascular syncope in which neurally-mediated syncope is first on the list. The movement-related syncope is the feature of the cardiac syncope. The prolonged standing and nausea are due to neurally-mediated syncope.

Objective To detect the expression level of miRNA-575 by pancreatic cancer stem cells. Methods Pancreatic cancer cell lines were cultured with serum-free medium. The cancer stem cells were obtained and their characteristics were observed with an invasion assay,a xenograft experiment, and flow cytometry analysis.Moreover,qRT-PCR was used to detect the expression of miR-575.Results A small subpopulation of cells in both ASPC-1 and PANC-1 cells survived and formed spheres in the absence of serum. The mean number of migrated cells of PANC-1 and ASPC-1 spheres was (147.3±18.6) cells and (113.2±12.9) cells. Therefore, the spheres have higher invasive ability compared with those of parental cells. As few as 5 × 105 cells in PANC-1 and ASPC-1 spheres could initiate a xenograft tumor in NOD/SCID mice, which suggested the tumorigenicity of the spheres was 100 times those of parental cell lines. The percentages of CD24+CD44+ in spheres were 0.38%-0.43% and 4.91%-5.21%,which showed increased expression compared with ASPC-1 and PANC-1 cell lines (P<0.05).The expression of miR-575 in spheres was 3.71 and 3.83 times its expression in cell lines ASPC-1 and PANC-1 respectively (P<0.05).Conclusions Pancreatic cancer cell spheres may be a source for collecting cancer stem cells in a serum-free medium culture, and MiR 575 may play an important role in regulating the biological characteristics of pancreatic cancer stem cells.

Objective To investigate the effects of glycyrrhizin on gene expression of transforming growth factor(TGF)-?signaling pathway of hepatic stellate cell(HSC)in mice by gene microarray tech- nique. Methods The HSCs were isolated from mice and cultured in vitro. Then the mice were divided into control group,TGF-?_1 group(5 ng/ml) or TGF-?_1(5 ng/ml)combined with glycyrrhizin(100?mol/L) group. The cells were collected after 10 hours to extract RNA.A cDNA microarray(GEArray~(TM) Q) targeting TGF-?/BMP signal transduction was used to screen the genes which showed significant changes in expression of TGF-?pathway of HSC by glycyrrhizin. Results The microarray analysis showed that 16 genes(16.7%),such as Smad2,Smad3,Smad7,CoL3A1,CoL1A2,and PAI-1,were upregulated by TGF-?_1 and then down-regulated by glycyrrhizin.Five genes(5.2%)(including BMP7,IGFbp3 and etc.)downregulated by TGF-?_1,were then up-regulated by glycyrrhizin.Finally,2 genes upregulated by TGF-?_1 were then up-regulated predominantly by glycyrrhizin in HSC(T?R2,betaglycan).Changes in some genes, such as Smad2 Smad3,Smad7,were further confirmed to be coincided with cDNA microarray by semi-quantitative RT-PCR.Conclusions The anti-fibrosis mechanisms of glycyrrhizin may be through to interference of TGF-?signaling pathway, decrease the synthesis and increase the degradation of collagen.

To explore the effect of advanced glycation end-products(AGEs)on cell viability and level of reactive oxygen species(ROS)in MIN6 cells. After intervention of various concentrations(100,200, and 400 mg/L)of AGEs for some time, cell viability was detected by MTT assay. 2', 7'-dichlorofluorescein diacetate(DCFH-DA)was used as a reactive oxygen species capture agent. The fluorescent intensity of 2', 7'-dichlorofluorescein(DCF), which was the product of cellular oxidation of DCFH-DA, was detected by flow cytometry. The level of ROS and insulin secretion was thus measured. Viability of MIN6 cells was inhibited by AGEs in a dose and time dependent manner(P<0.05).Intracellular fluorescent intensity of DCF was markedly elevated in the AGEs groups as compared with that in the control group(P<0.05).Insulin secretion was decreased in the AGEs groups than that in the control group(P>0.05). The results suggest that AGEs inhibit the viability and induce oxidative stress in MIN6 cells by overproduction of ROS.

Objective To study the therepeutic effect of interventional treatment on patients with primary liver carcinoma accompanied with hypoglycemia. Methods 10 cases liver carcinoma patients with hypoglycemia un- derwent TACE.Results Blood sugar of 10 patients was kept in normal range after TACE.Hypoglycemia recurred in 1 case half a month after interventional therapy.CT showed carcinoma was not completely controlled, and the blood supar was turned to normal after a second interventional therapy.2-month follow-up showed no recurrence of hypoglycemia in 10 patients. Conclusion Application of TACE had good therapeutic effect on liver carcinoma pa- tients accompanied with hypoglycemia.

Fermentation condition optimization of P. decumbens Ju-A10 for production of CMCase using three kinds of plant cellulosic wastes as carbon sources was made using RSA method. The result was that CMCase was the highest when the level of carbon source was 9. 77 % , 8. 69 % and 9. 97% , and liquid volume was 64. 7 mL, 54. 2 mL, 40. 8 mL for carbon sources of millet straw, wheat straw and paper sludge, respectively. The value of CMCase was 29. 26IU/mL, 29. 14 IU/mL, 29. 81 IU/mL, respectively, in the above cases. The value of R2 is 0. 9117 , 0. 9246, 0. 8655 , respectively. It could be concluded that the fermentation models were quite reliable. The method can be applied in optimization of fungi fermentation medium.
