1.CT findings of desmoplastic small round cell tumor occurred in abdomen and pelvis

Objective To investigate the CT features of desmoplastic small round cell tumor (DSRCT)occurred in abdomen and pelvis.Methods The CT images of 4 patients with pathologically proven DSRCT occurred in abdomen and pelvis were retrospectively analyzed.Results The CT features common to the 4 DSRCT in abdomen and pelvis were as follows:the lobulated soft-tissue masses were very large;the masses often involved omental and serosal surfaces of abdomen and pelvis organ and had close relationship to the surrounding organs;the originating site of the tumors were not apparent;intratumoral hemorrhage or necrosis were often present;and contrast-enhanced CT scan showed mild to medium heterogeneous enhancement.Conclusion The CT features of DSRCT in abdomen and pelvis are rather specific and can provide helpful information to the clinical diagnosis.
2.Therapeutic Effects on Single and Repeated - Dose Administration of Clonazepam for Seizure in Children

Objective To explore the change of degrade of clonazepam in serum with single and repeated - dose administration in children with seizure, and find a reasonable method for using the clonazepam. Methods Children with seizures were divided into single - dose paradigam, repeated - dose paradigam, and decreased - dose paradigam. The concentration of CZP in serum was determined by high performance liquid chromatography( HPLC). Results The serum concentrations of clanazepain in single - dose paradigam were (101.9?12.1),(76.9 ? 5.8),(50.7?2.9),(30.9?5.4),(21.5?6.8)?g/L,the time point that the blood samples collected were 15,30,60,120 and 480 min. The serum concentrations in repeated - dose paradigam were (97. 2 ? 6. 1),(130.4? 13. 4), (99. 4 ? 9.8),(79.6?2.4)?g/L,in decreased-dose paradigam were( 101.1 ?13.1),(123.1 ?6. 6), (99.4 ?9. 8), (79. 3 ? 2. 2)?g/L,in these two groups,the time point were 15,45,60 and 120 min. Conclusion Repeated administration of CZP with decreased dose may increase its effectiveness in treatment without substantially increasing toxicity.

Objective To study the influence of colon hydrotherapy on premature infants with jaundice.Methods Based on intravenously infusing yinzhihuang and albumin and enzyme inducer,the warmed normal saline was poured into colon by 10 mL/kg per 8 hours in 40 preterm infants,and compared with 44 cases of control group.Serum bilirubin of all patients were measured at 24,72,120,168,216 hours,respectively.The days of meconiumbeing eliminated completely were recorded.Results The bilirubinemia level in trial group were lower in trial group than that in control group at every time and the rate of hyperbilirubinemia of the test group was lower(?~2=4.073 P

5.Utilization Investigation and Rationality Evaluation of Vancomycin during the First Half Year of 2015 in A Hospital

OBJECTIVE:To investigate the utilization of vancomycin in a hospital,and provide reference for the rational use of vancomycin. METHODS:Retrospective survey was conducted for the medical records of 180 inpatients using vancomycin from Jan. to Jun.,2015 in the hospital. The general information,usage and dosage and pathogenic examination were summarized,do-mestic vancomycin rational use evaluation criteria was conducted to evaluate the medication rationality,summarize and analyze cor-responding inappropriate questions. RESULTS:The vancomycin drug utilization index(DUI)of 180 cases was 0.88(mainly elder-ly patients);the microbial test sample submission rate was 84.44%(152 cases);there were 74 cases (41.11%) about the drug combination;32 cases(17.87%)about unauthorized use;37 cases(20.56%)about inappropriate use,in which inappropriate sol-vent(12 cases,32.43%)and inappropriate drug combination(11 cases,29.73%)were the main types. CONCLUSIONS:The clin-ical application of vancomycin is good with no abusing phenomenon,and the microbial inspection rate is qualified in the hospital. But there still exists some problems or shortcomings when using vancomycin,it is necessary to further standardize the medication monitoring of vancomycin to ensure patients’safety.
6.Effect of adenovirus-mediated bcl-2 transfer on regeneration and repair of peripheral nerves in rats

Objective To explore the effect of adenovirus-mediated bcl-2 transfer on the regeneration and repair of peripheral nerves in rats. Methods The sciatic nerve-transected model was selected and sutured end to end with epineurium. Then, adenovirus/sense-B-lymphoma/leukemia-2 (Ad/s-bcl-2), adenovirus/anti-sense-B-lymphoma/leukemia-2 (Ad/as-bcl-2), adenovirus/?-galactosidase (Ad/LacZ) and normal saline (NS) were injected into the sciatic nerve 0.5 cm away from the sutured point respectively. The rats were perfused with 4% paraformaldehyde at the 48th hour, 7th, 15th and 30th days respectively after operation. The spinal cords L 4-6 were harvested, frozen sectioned and given x-gal staining, bcl-2 in situ hybrid and immunohistochemical staining and GAP-43 immunohistochemical staining. The postoperative functional recovery of the rear limbs of rats was dynamically observed by using sciatic nerve functional index (SFI). Results The GAP-43 expression in the motor neurons of anterior horn of spinal cord in group Ad/s-bcl-2 was higher than that in other groups (P0.05). Conclusions Gene transfer of bcl-2 can promote regeneration and functional recovery of the peripheral nerves following sciatic nerve injury.
7. Identification and culture of neural stem cells isolated from adult rat subventricular zone following fluid percussion brain injury

Objective: To analyze proliferation and differentiation of glial fibrillary acid protein (GFAP)- and nestin-positive (GFAP+/nestin+) cells isolated from the subventricular zone following fluid percussion brain injury to determine whether GFAP+/nestin+ cells exhibit characteristics of neural stem cells. Methods: Male Sprague-Dawley rats, aged 12 weeks and weighing 200-250 g, were randomly and evenly assigned to normal control group and model group. In the model group, a rat model of fluid percussion brain injury was established. Five days later, subventricular zone tissue was resected from each group and made into single cell suspension. After serum-free neural stem cell medium culture and subsequent serum-induced differentiation, cell type, proliferation and differentiation capacities were determined by immunofluorescence staining and flow cytometry. Results: At 3-7 days after fluid percussion brain injury, nestin+/GFAP+ cells in the single cell suspension from the model group significantly outnumbered those from the normal control group (P < 0.01). In the model group, an increased number of small neurospheres with smooth cell edge and bulged center formed after primary culture, and were clearly visible with the increase of culture time and medium replacement. After several passages, many clonal spheres were obtained, suggesting strong self-proliferatiing capacity. Neurospheres from the model group differentiated into astrocytes, neurons and oligodendrocytes. Conclusion: GFAP+/nestin+ cells isolated from the adult rat subventricular zone after fluid percussion brain injury are thought to be neural stem cells because of their self-renewal and multi-differentiation capacities.

9.Application of laparoscopy in closed abdominal trauma

Objective To study the clinical application of laparoscopy in the treatment of closed abdominal trauma. Methods Emergent laparoscopic exploration and treatment was conducted in 21 patients with closed abdominal trauma, who had stable hemodynamic parameters,from July 2000 to December 2003. Results Diagnosis was clarified laparoscopically in all the 21 patients. There were 4 cases of open liver repair, 2 cases of laparoscopic liver repair, 5 cases of open splenectomy, 3 cases of hand-assisted laparoscopic splenectomy, 2 cases of video-assisted small incisional intestinal repair, 4 cases of partial intestinal resection and 1 case of laparoscopic greater omentum hemostasis. Conclusions Laparoscopy in the diagnosis and treatment of closed abdominal trauma is accurate, safe and effective.
10.Clinical Observation of Recombinant Human Epidermal Growth Factor Combined with Lipoic Acid in the Treatment of Diabetic Foot

OBJECTIVE:To investigate the clinical efficacy and safety of recombinant human epidermal growth factor(rhEGF) combined with lipoic acid in the treatment of diabetic foot. METHODS:A total of 68 patients with diabetic foot were randomly divid-ed into observation group and control group with 34 cases in each group. Both groups were given conventional therapy and Lipolic acid injection 0.6 ml added into Sodium chloride injection 250 ml. ivgtt(protecting form light),qd;observation group was additionally giv-en rhEGF for external use 1 ml and vaseline on the wounds,gauze dressing,qd. Treatment course of 2 groups lasted for 7 d. Clinical efficacy,ulcer area,the expression of Bcl-2 and Bax were compared between 2 groups as well as the occurrence of ADR. RESULTS:The total effective rate of observation group(94.1%)was significantly higher than that of control group(67.6%),with statistical sig-nificance (P<0.05). Before treatment,there was no statistical significance in ulcer area,the levels of Bcl-2 and Bax between 2 groups(P>0.05). After treatment,ulcer area decreased significantly, the level of Bcl-2 increased significantly in 2 groups,while the level of Bax decreased significantly;the improvement of observation group was more significant than that of control group,with statis-tical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLU-SIONS:rhEGF combined with lipoic acid is effective for diabetic foot with good safety.
