1.Application of laparoscopy in assisted reproduction techniques

Objective To investigate the clinical significance of gamete intrafallopian transfer (GIFT) under laparoscope. Methods The gonadotropin was adopted to induce superovulation and the spermatic fluid was managed with the swim-up method. After oviums were dislodged by puncture under laparoscope, gametes were transplanted by intubating the fallopian tube. Results Among a total of 19 patients, 4 were pregnant, the pregnancy rate being 21.1% (4/19). Three women had a delivery at term and 1 had an abortion. No serious complications were seen. Of the 15 unfertilized patients, normal pregnancy was observed within 6 months after surgery in 5 patients, the circle pregnancy rate being 5.6% (5/89). Conclusions GIFT under laparoscope is a practical assisted reproductive technique (ART) that may achieve a relatively high pregnancy rate. Some other pelvic diseases resulting in infertility can be treated simultaneously, which contributes to natural fertilization in the future.
2. Effect of anemia on prognosis of non-small cell lung cancer patients

Objectives: To investigate the correlation of the hemoglobin levels with prognosis of non-small cell lung cancer (NSCLC) patients. Methods: The clinical data from 500 patients with NSCLC who were admitted in the Shanghai Chest Hospital from November 2003 to March 2005 were retrospectively analyzed. Statistical difference was analyzed by χ2 test. Survival analysis was evaluated by Kaplan-Meier and log-rank test. Multivariate analysis was performed by COX stepwise regression model. The hemoglobin levels were obtained at the initial visit to the hospital. Anemia is defined as a hemoglobin level <120 g/ L in men and <110 g/L in women. Results: The median survival time for all the 500 patients was 27.17 months, 1-year survival rate was 73.4%. Among them, 147 (29.4%) patients had anemia and 353(70.6%) patients without anemia. The incidence of anemia was higher in those patients who received chemotherapy. After the four-cycle chemotherapy, the proportion of patients experiencing anemia was 84.3%. The median survival time was 21.38 months for the patients with anemia and 30.82 months for patients without anemia. The 1-year survival rate was significantly lower in patients with anemia than those without anemia (64.3% vs 76.8%, P = 0.028). Anemia had significant influence on the survival time of female patients (P = 0.049). There was significant difference in the survival time between III stage patients with and without anemia (P = 0.035). Anemia had no significant effect on the survival of NSCLC patients at stages I, II, and IV. The COX stepwise regression analysis showed that anemia, sex, pathological classification, clinical stage, liver function, and surgical resection were independent prognostic factors for the NSCLC patients. Conclusion: Anemia determined by primary diagnosis was related with the survival time of NSCLC patients. Anemia is an independent prognostic factor for NSCLC patients.
3.Effects of baicalin on the apoptosis and cell cycle of colorectal cancer cells in orthotopic transplantation mice model with mismatch repair gene deficient

Objective To study the effect of baicalin on the apoptosis and cell cycle of colorectal cancer cells in orthotopic transplantation mice model with mismatch repair gene hMLH1 deficient.Methods Sixty orthotopic transplantation mice models of human colorectal cancer cell line HCT1 16 expressing green fluorescent protein (GFP) were established,and were randomly divided into the control group and the 50,100,200 mg/kg baicalin groups according to the random number table.Mice in the 50,100,200 mg/kg baicalin groups received intragastric infusion of baicalin at the corresponding dosages twice a day,while mice in the control group received intragastric infusion of 5% NaHCO3.Cell cycles and apoptotic rates of the HCT116-GFP cells were detected by flow cytometry and TUNEL method respectively.Differences between the 2 groups were analyzed using the analysis of variance or chi-square test,and differences within each group were analyzed using the LSD-t test.Results The orthotopic transplantation mice models of human colorecta] cancer were successfully constructed,and there was no significant difference in the body weight of the mice and tumor size among the 4 groups (F =0.343,0.107,P >0.05).The proportion of HCT116-GFP cells in the G2/M phase in the 50,100,200 mg/kg baicalin groups were 22%±6%,18%±7% and 19%±6%,which were significantly higher than 7% ±5% of the control group (t =5.421,3.483,3.575,P <0.05).There were no significant differences in the proportion of HCT116-GFP cells in the G2/M phase among the 50,100,200 mg/kg baicalin groups (F =1.291,P > 0.05).The apoptotic rates of HCT116-GFP cells in the 50,100,200 mg/kg baicalin groups were significantly higher than the control group (t =7.163,3.703,2.688,P <0.05).The apoptotic rate of the 50 mg/kg baicalin group was significantly higher than that of the 200 mg/kg baicalin group (t =2.259,P < 0.05).Conclusions Baicalin significantly inhibits tumor growth in the orthotopic transplantation mice model with mismatch repair gene hMLH1 deficient.After treated with baicalin,the cell cycle is arrested at the G2/M phase,thus the tumor growth is inhibited.
4.Expression of EGFR mutation-specific antibody and its significance in lung adenocarcinoma

Purpose To investigate the expression and significance of epiderma1 growth factor receptor( EGFR)mutation-specific anti-bodies in 1ung adenocarcinoma. Methods Immunohistochemica1( IHC)technique was used to detect the expression of EGFR muta-tion-specific antibodies(EGFR-19,EGFR-21)and EGFR tota1 protein antibody(EGFR-P)in 171 cases of 1ung adenocarcinoma with resected specimens,and EGFR gene mutation was a1so performed by amp1ification refractory mutation aystem-PCR( ARMS-PCR). The expression of EGFR-19,EGFR-21 and EGFR-P mutant proteins was compared with EGFR gene mutation and their re1ationship with histo1ogica1 c1assification and c1inica1 characteristics were ana1yzed. Results The expression of EGFR mutant protein was corre1ated with the poor differentiation group inc1uding micropapi11ary and so1id predominant types( P=0. 021 ). EGFR-21 high expression was re1ated to p1eura1 invasion(P=0. 005). The coherence of IHC(with EGFR-19/21 antibodies)and ARMS-PCR was existed(Kappa>0. 4 ). Taking ARMS-PCR as a standard,the sensitivity and specificity of EGFR-19 and EGFR-21 were 65. 0% and 89. 4%, 70. 0% and 97. 6%,respective1y. Conclusions Expression of EGFR mutation-specific antibodies is associated with poor differentia-tion and p1eura1 invasion. It suggests a worse prognosis indicator in 1ung adenocarcinoma. Because of the coherence with ARMS-PCR, using IHC with mutation-specific antibodies to detect the mutant proteins of EGFR-19/21 may act as a pre1iminary screening method of EGFR gene mutation.

6.Effect of comprehensive rehabilitation on severe cerebral injury at recovering stage

Objective To observe the effectiveness of comprehensive rehabilitation on the recovery of motor function of patients with severe cerebral injury in recovery phase.Methods 72 cases with severe cerebral injury in recovery phase were randomly divided into rehabilitation group (n=38) and control group (n=34). Patients in control group were treated with routine method, and those in rehabilitation group with comprehensive rehabilitation treatment in addition.Results Motor function, activities of daily living (ADL) and complication of patients were evaluated at the first day and two months later after be in hospital. The effect of comprehensive rehabilitation treatment were better than that of control group(P<0.05,P<0.01). Conclusion Comprehensive rehabilitation is important to the patients with severe cerebral injury of recovering, not only in recovery of motor function but also in reducing the complication.
7.Effect of Tripterygium wilfordii polyglycosidium on caspase expression of islet?-cell in NOD mice

NOD mice were treated intraperitoneally with tripterygium wilfordii ployglycosidium (TWP) for 4 weeks to observe the incidence of cyclophosphamide accelerated diabetes.The apoptosis of?-cells was detected by TUNEL,the expression of caspase-3 in islets of the NOD mice was detected by immunohistochemistry and the expression of caspase-8 mRNA in pancreas by RT-PCR.The results revealed that the incidence of diabetes in TWP group was lower than that in control group.The apoptosis index of?-cells was decreased in TWP group. The expression of caspase-3 in islets and the expression of caspase-8 mRNA in pancreas in TWP group were lower than those in control group.
8.Effects of HO-1 on Lipopolysaccharide-induced Endoplasmic Reticulum Stress of Rat Hepatocytes.

OBJECTIVE:
To investigate effects of antioxidant stress protein heme oxygenase-1 (HO-1) on lipopolysaccharide (LPS)-induced endoplasmic reticulum stress (ERS) of rat hepatocytes.
METHODS:
The BRL cells (rat hepatocyte cell line) were cultured. The hepatocytes were treated with LPS, LPS+HO-1 siRNA, HO-1 siRNA and PBS solution, respectively. The cell viability was measured by trypan blue exclusion test. The apoptosis cells were detected by the fluorescent dye Hoechst 33258. Expressions of GRP78, CHOP, caspase-12 and HO-1 were detected by Western blotting.
RESULTS:
LPS caused an increase of HO-1 protein expression of rat hepatocytes in a dose-dependent and time-dependent manner, a up-regulation of GRP78, CHOP and caspase-12, a decrease in cell viability, and an increase in apoptosis rate of hepatocytes. Pretreatment of HO-1 siRNA inhibited the up-regulation of LPS-induced HO-1, however, aggravated ERS and cellular injury.
CONCLUSION
HO-1 inhibites ERS-mediated cellular injury of rat hepatocytes induced by LPS.

9.Color Doppler ultrasonographic findings in active ankylosing sacroiliitis

Objective To evaluate the color Doppler ultrasonographic findings of active ankylosing sacroiliitis.Methods Thirty three patients (66 sacroiliac joints in total) with active ankylosing sacroiliitis identified by MRI and twenty eight healthy volunteers (56 sacroiliac joints) underwent color Doppler ultrasound examinations of sacroiliac joints.Degree of blood flow within and around the sacroiliac joints was evaluated by semi-quantitative scale (0 - 4 grade).For arteries, resistive index was also measured.Statistical analyses were performed to compare blood flow demonstration rate, blood flow signal scale distribution,degree of blow flow and RI value between the two groups.Results Higher blood flow demonstration rate was found in patients with active ankylosing sacroiliitis(60/66,90.1%) than control group (25/56,44.6%)( P = 0.000).The scale distribution and degree of blow flow between these two groups were statistically different (all P = 0.000) ,with mainly 2~3 grade for the patients and 0~1 grade for the control.Mean value of RI in the patients was 0.57 ± 0.07, which was lower than the control (0.66 ± 0.04, P = 0.000).Conclusions Patients with active ankylosing sacroiliitis tend to have higher degree of blood flow in sacroiliac joints and lower RI values of artery.Color Doppler ultrasound may help to evaluate the activity of ankylosing sacroiliitis.
