5.Finite element model of artificial lumbar disc replacement

BACKGROUND: Artificial lumbar disc replacement is a novel approach to treat lumbar disc protrusion. But its theoreticalbasis needs further verification.OBJECTIVE:To establish finite element model of lumbar disc replacement for biomechanical studies.DESIGN: A single sample study taking the data of L4-5 three-dimensional infinite element model as object of research.SETTING: Orthopedic Department of Xiangya Hospital of SunYat-sen University.PARTICIPANTS: The trial was conducted in the Laboratory of the Orthopedic Department of Xiangya Hospital from December 2003 through August 2004. A healthy male volunteer served as simulation. His T12-S1 underwent continuous CT scanning. There were altogether 264 images with 2 mm inthickness each. Three-dimensional images were reconstructed every 15° in order to obtain the data for three-dimensional model.METHODS: The CT images and human anatomical data were processed by 3DSMAX software to establish three-dimensional L4-5 model of normal Chinese males. It was then transformed to finite element model after processed by SAP2000 software together with Charite SB Ⅲ disc prosthesis model.MAIN OUTCOME MEASURES: The three-dimensional model and finiteelement model of lumbar spine were successfully established.RESULTS: The finite element model of Charite SB Ⅲ disc replacement in L4_5 spine was established. The total nodes were 2542 and there were 1924Solid units, 592 Area units and 50 link units.CONCLUSION:The finite element model of artificial disc replacement can be established by CT scanning, digital processor and computer aideddesign,and used for further study on spinal biomechanics.

BACKGROUND: Some scholars have used finite element analysis to simulate spinal biodynamics. But there are few reports on finite element imitation of lumbar biodynamics system before and after artificial disc replacement. OBJECTIVE: This study aimed to do finite element analysis by establishing new three dimensional finite element models of SB-Chaite Ⅲ lumbar disc replacement. DESIGN, TIME AND SETTING: The observational experiments were performed at the Laboratory of Orthopaedics of Xiangya Hospital of Central South University from December 2003 to August 2004. 7.Pathogenesis,diagnosis and treatment of recurrent spontaneous abortion with immune type

Recurrent spontaneous abortion (RSA),which affects 1% to 5% of women of reproductive age,is difficult to treat in the clinical setting. In the investigations of immunopathogenesis,diagnosis and treatment of RSA since the late 1980s,it was found that RSA was associated with abnormal maternal local or systemic immune response,the pathogenesis of autoimmune RSA was mainly associated with antiphosphlipid antibody (APA),while that of alloimmune RSA was due to the disturbance of maternofetal immunological tolerance. Systemic etiological screening process and diagnosis systems of RSA with immune type were developed,and anticardiolipin (ACL)+? 2-GP1 combining multiple assay for effective diagnosis of RSA with immune type was initially established. According to dynamic monitoring clinical parameters before and during gestation,low-dose,short-course and individual immunosuppressive therapy and lymphocyte immunotherapy for RSA with immune type were initiated. The outcomes of the offsprings of patients with RSA were followed up,and the safety and validity of the therapies were confirmed. The research achievement leads to great progress in the diagnosis and treatment of RSA in China.
8.Relationship between serum leptin and thyroid hormone in children with abnormal thyroid function

Objective To study the relationship between serum leptin and thyroid function by investigating the change of serum leptin concentration in children with abnormal thyroid function (hyperthyroidism and hypothyroidism).Methods The levels of serum leptin was determined by radio-immunoassay(RIA) in 20 cases of hyperthyroidism, 17 cases of hypothyroidism, 25 cases of normal controls, respectively .Meanwhile, the serum levels of free triiodothyronine(FT 3), free thyroxine(FT 4) and thyroid-stimulating hormone(TSH) were measured by micro-particle-chemiluminescence immunoassay.Results The levels of serum leptin in hypothyroidism group before treatment was significantly lower than that of normal control groups (P0.05).Conclusions The levels of serum leptin in hypothyroidism markedly descend and no change in the hyperthyroidism. Thyroid hormone can promote the secretion of serum leptin eligibly.
9.The expression of TLR4 on peripheral blood monocytes from uremic patients with diabetic nephropathy and its relation with plasma MCP-1 concentration

Objective:To explore the expression of TLR4 on peripheral blood monocytes in uremic patients with diabetic nephropathy and the relation between TLR4 and diabetic nephropathy.Methods:RT-PCR analysis and flow cytometry were undertaken to measure the expression of TLR4 mRNA and protein.The plasma concentration of MCP-1 was measured with ELISA.Results:The gene and protein expression of TLR4 on peripheral blood monocytes in patients with diabetic nephropathy was significantly increased compared with that of the health control,and TLR4 expression was positively related with the the plasma concentration of MCP-1.Conclusion:TLR4 expression is upregulated on peripheral blood monocytes in patients with diabetic nephropathy and the upregulation is related with the pathogenesis of diabetic nephropathy.
10.Fresh and old tibial intercondylar eminence fracture by arthroscopic wire fixation

BACKGROUND:Arthroscopic treatment of tibial eminence fracture requires tiny wire,induces less epiphyseal plate damage during operations,obtains strong fixations,alows early movement postoperatively,obviously attenuate the injuries,facilitate wound healing,and reduce complications.OBJECTIVE:To investigate the biocompatibility of arthroscopic reduction and wire fixation in treatment of fresh and old tibial eminence fractures.METHODS:Twenty-five patients with fresh and old tibial eminence fractures were treated with arthroscopic reduction and wire fixation from January 2010 to April 2012 in the Second People's Hospital of Yichang City.The postoperative fracture healing time and knee function improvement in the two groups were observed.RESULTS AND CONCLUSION: Compared with the fresh fracture patients,the fracture healing time was longer,Lachman test positive rate was higher in the old fracture patients than (P<0.05),knee flexion and extension activity and Lysholm score were lower (P<0.05).Experimental findings indicate that,arthroscopic reduction and wire fixation is a feasible clinical treatment of fresh and old tibial intercondylar eminence fractures.
