1.Comparison of the clinical effect of MC and LC in treatment of elderly patients with cholelithiasis

Objective To observe the clinical effect of MC and LC in treatment of elderly patients with cholelithiasis and dise~s the best treatment in elderly patients with cholelithiasis.Methods Of 798 elderly patients with cholelithiasis,412 patients were divided into MC group with minilaparotomy cholecystectomy treatment,and 386 patients were divided into LC group with laparoseopic cholecystectomy treatment,then compare clinical effect and complications after operation.Results There was no significant differences in incision length,operative time,blood loss,bed time,hospital stay(all P<0.05);There Was significant statistical significance in cost of treatment,complications after operation(all P<0.05).Conclusion Minilaparotomy cholecystectomy was suitable for elderly patients with cholelithiasis,and it Was good at cost of treatment,complications after operation.
2.Appraisal of the repair gastroschisis with autogenous umbilical cord

Objective To review the treatment of repair gastroschisis with autogenous umbilical cord and evaluate its effect. Methods Thirty newborns who underwent the repair gastroschisis with autogenous umbilical cord between August 1992 to October 2007, 26 cases survived under observed and followed-up, observing physical growth, intelligence measuring and whether the area of operated in abdomen need staged repair or not. Compared with 15 cases who underwent traditional operation method at the same time. Results Two cases died, 2 cases abandoned, and 26 ease received survive (survive rate 86.7%)and their growth was well in 26 cases. But in those 15 cases who underwent traditional operation method, 5 cases survived (survive rate 33.3%). There were significant difference in the survive rate, the mean operative time and postoperative hospital stay time between the two operation methods (P <0.05).Conclusion The material is adopted easily in the operation, autogenous umbilical cord is elastic tissue and no toxicity, it can relax the abdominal press effectively after the operation, the survive rate is high.
3.The compounds from n-butanol fraction of Alpinia oxyphylla.

Nine compounds were isolated from the n-butanol fraction of 95% ethanol extract of the fruit of Alpinia oxyphylla Miq. with a combination of various chromatographic approaches, including MDS resin, silica gel, reverse phase C18 and preparative HPLC. On the basis of spectroscopic data analysis, they were elucidated as (1R, 4R, 10R)-1β, 4α-dihydroxy-11, 12, 13-trinor-5, 6-eudesmen-7-one (1), 1β, 4β-dihydroxy-11, 12, 13-trinor-8, 9-eudesmen-7-one (2), oxyphyllenone A (3), oxyphyllenone B (4), rhamnocitrin (5), staphylionoside D (6), benzyl-1-O-β-D-glucopyranoside (7), 2-O-β-D-glucopyranosyl-(1S)-phenylethylene glycol (8), and (S)-1-phenylethyl-β-D-glucopyranoside (9). Among them, compound 1 is a new sesquiterpene, named as oxyphyllenone C; compounds 8 and 9 are new natural products; compounds 2 and 6 were isolated from the genus Alpinia for the first time, and compound 7 was isolated from A. oxyphylla for the first time.

5.Changes of brain gangliosides in rats transient global cerebral ischemia/reperfusion models after hyperbaric oxygen treatment

BACKGROUND: Ganglioside (Gls) is a kind of N-acetylneuraminatecontaining glycosphingolipid, which is abundant in neural tissues and exerts neuroprotective roles, and has been found its changes in content and composition pattern after cerebral ischemia or hypoxia diseases.OBJECTIVE: To investigate the effect of hyperbaric oxygen on brain gangliosides after cerebral ischemia/reperfusion and explore the possible mechanism of hyperbaric oxygen treatment to ameliorate cerebral ischemia/reperfusion damage.DESIGN: Randomized-control observation.SETTING: Department of Hyperbaric Oxygen, Chaoyang Hospital Affiliated to Capital University of Medical Sciences and Department of Biochemistry and Molecular Biology, Basic College, Capital University of Medical Sciences.MATERIALS: Animal models were established at Department of Hyperbaric Oxygen, Chaoyang Hospital Affiliated to Capital University of Medical Sciences (Key Laboratory of Beijing) from March to April 2002. All indexes were determined at the Department of Biochemistry and Molecular Biology, Basic College, Capital University of Medical Sciences. A total of 54 SD male rats were selected and randomly divided into 9 groups: shamoperated group; ischemia/reperfusion 6, 24, 48 and 96 hours group; and hyperbaric oxygen 6, 24, 48 and 96 hours group with 6 rats in each group.METHODS: Animal models with cerebral ischemic/reperfusion were established in the other 8 groups, except the sham-operation group. Global cerebral ischemia was induced by a four-vessel occlusion and reperfusion allowed after 20-minute ischemia. The rats in sham-operation group were operated in the same way except of arterial occlusion. The rats in the HBO group were placed in experimental animal chamber. After 5-minute wash by pure oxygen, the pressure of oxygen cabins was increased to 0.1 MPa in 5 minutes and kept stable for 45 minutes, then decreased to ambient level within 10 minutes. The rats in the HBO groups were treated once by hyperbaric oxygen at reperfusion 3, 24, 48 and 96 hours respectively; while the rats in the ischemia/reperfusion group and sham-operation group were placed in normal atmospheric environment. The rats of HBO and ischemia/reperfusion groups were killed by decapitation at the 6th, 24th, 48th and 96th hours respectively, and the brains removed quickly. Gls and its percentage content in each group were detected with high performance thin-layer chromatography plate.MAIN OUTCOME MEASURES: Total content of gangliosde in the whole brain tissue of rats and its percentage content.RESULTS: Totally 54 rats were involved in the result analysis without drop out. ①The contents of Gangliosides in HBO groups at 24 and 48 hours was significantly higher than those in sham-operation group and ischemia/reperfusion group at corresponding time phase (F=12.730,122.246,P < 0.01), but there was no significant difference between ischemia/reperfusion and sham-operation groups (P > 0.05). ② N-acetylneuraminosylgalactosyl-N-acetylgalactosaminyl- (N-acetylneuraminosylα2→8- N-acetylneuraminosyl) -galactosylglucosylceramide (GT1b) proportion in the ischemia/reperfusion 24 hours group was lower markedly than that in the S-O (F=13.575,P < 0.01); Both galactosyl-N-acetylgalactosaminyl- (N-acetylneuraminosyl- α2→8-N-acetylneuraminosyl) - galactosylglucosylceramide (GD1b) and galactosyl- N-acetylgalactosaminyl-(N-acetylneuraminosyl)galactosylglucosylceramide (GM1) in the reperfusion 48 hours group were lower than those in the sham-operation group (F= 4.015,3.979,P < 0.05); (N-acetylneuraminosyl)galactosylglucosylceramide (GM3)in ischemia/reperfusion 24 hours group was much higher than that in sham-operation and any other ischemia/reperfusion groups (F=21.450,P< 0.01 ); An unknown increase of GM3 was found again at the 96th hour;③GM1and GM3 proportion in the hyperbaric oxygen group was higher than that of sham-operation group at the 24th hour (F=3.970,21.450,P< 0.05, < 0.01), and all of GD1a, GD1b and GT1b were lower than that in the sham-operation group at the same times (F= 13.575,5.745,8.783, P< 0.05-0.01), but GT1b was remarkably higher than that in ischemia/reperfusion group (F=8.783 ,P < 0.05).CONCLUSION: The pattern of brain gangliosides changed after transient whole cerebral ischemia/reperfusion. The new mechanism of neuron damage after transient whole cerebral ischemia/reperfusion might be the decreasing of GT1b, GD1b and GM1 with increasing of GM3 proportion. The hyperbaric oxygen treatment could ameliorate cerebral ischemia/reperfusion damage by increasing total gangliosides content and GM1 proportion and accelerating GT1b restoration to normal level. It is unknown that the effect of percentage content of GD1a and GD1b decreased.
6.Efficacy of Methylphenidate Hydrochloride controlled-release tablets on Attention-Deficit Hyperactivity Disorder in Children

Objective To observe the therapeutic effect of methylphenidate hydrochloride controlled-release tablets (OROS-MPH) on attention-deficit hyperactivity disorder (ADHD). Methods Seventy-two cases of children with ADHD were randomly divided into treatment group (40) and control group (32). Cases of treatment group were given 0.8-1.0 mg??kg-1 of OROS-MPH for three months. Cases of control group were given 1.2-1.4 mg??kg-1 of atomoxetine hydrochloride for three months. After 12 weeks treatment, children were evaluated by Wechsler intelligence test, Integrated visual and auditory continuous performance test (IVA-CPT), the SNAP-Ⅳ effect assessment scale and TESS scale. Results The treatment efficiency was similar in both groups. Attention deficit and hyperactivity in both groups were improved obviously. Wechsler intelligence score was significantly elevated ( P<0. 05), SNAP-Ⅳ score was significantly decreased ( P<0. 05), and IVA-CPT score was increased significantly after treatment ( P<0.05) . The changes of scores on hyperactivity, auditory attention and visual attention were more in OROS-MPH group than those in atomoxetine group(P<0.05). There was loss of appetite in 10 children of OROS-MPH group and in 14 children of atomoxetine group. There was drowsiness in 1 child of OROS-MPH group and in 5 children of atomoxetine group, as well as difficulty to fall asleep in 6 children of OROS-MPH group and 1 child of atomoxetine group (P<0.05). One child developed a transient spasm after 4-month treatment. Conclusion Both of OROS-MPH and atomoxetine hydrochloride can improve learning ability and the symptom of attention deficit and hyperactivity, and they are similarly effective and safe in children with ADHD, but OROS-MPH can work faster.
7.Enhancement of Extracts from Celastrus orbiculatus on Maturation and Function of Dendritic Cells in vitro and in vivo

Objective To examine the immunoregulation of Celastrus orbiculatus extracts(COE),a traditional Chinese medicine,on maturation and function of dendritic cells(DCs)in vitro and in vivo.Methods In vitro,after treated with COE indifferent nontoxic concentrations(0,10,20,40,80,and 160 μg/mL)for 5 d,the surface immunological molecules andcytokine secretion of mice bone marrow-derived DCs in response to COE were analyzed by flow cytometric analysis(FACS)and enzyme linked immunosorbent assay(ELISA),respectively.In vivo,mouse hepatoma cells(Hepal-6,1 ×106)were injected sc and were treated with different dosages of COE(10,20 or 40 mg/kg/d).Effects on tumor growth were determined by tumor volume and histology analysis after 28 d administration of COE.The relative proportions ofmature DCs and CD8+ T cells were measured in mononuclear cells that had been isolated from spleen by FACS.Results COE stimulated IL-2 and IFN-γ secretion of DCs,simultaneously enhanced the maturation of DCs byenhancing immunological molecule(CD40,CDS0,CD86,H-2Kb,and I-Ab)expression in a dose-dependent manner.Furthermore,the chcmotactic responses of DCs were significantly higher in COE-treated than untreated DCs,in association with higher chcmokine receptor 7 expression.Furthermore,COE increased DCs produce IFN-γ and IL-2 ina dose-dependent manner when the concentration of COE less than 40 μg/mL,decreased DCs produce IL-10 and IL-4also in a dose-dependent manner.In in vivo studies,COE can not only suppress growth of malignant hepatocellularcarcinomas but also stimulate maturation of DCs,associated with strongly enhanced CD8+ CTL responses.ConclusionThese data provide new insight into the mechanism of action of COE and indicate that the stimulation of maturation andfunction of DCs by COE contributes to its immunoregulatory effects.
8.Association of methylenetetrahydrofolate reductase A1298C polymorphisms with non-syndromic cleft lip with or without cleft palate

Objective To investigate the association between a polymorphism of methylenetetrahydrofolate reductase with Non-syndromic cleft lip with or without cleft palate(NSCL/P)in Chinese population. Methods The polymerase chain reaction (PCR)-based restriction fragment length polymorphism(RFLP)technique was used to detect a single nucleotide polymorphism(SNP), rs1801131, at the methylenetetrahydrofolate reductase(MTHFR)gene in both 158 patients with NSCL/P and 192 healthy individuals. The Hardy-Weinberg equilibrium for genotypic distributions was estimated by the goodness-of-fit test. The UNPHASED program was applied to perform the association analysis. Results The genotypic distribution of A1298C was not deviated from the Hardy-Weinberg equilibrium in both controls and patients. No association was found between cleft lip with or without palate(CL/P)and controls. There was significant difference of cleft palate only(CPO)and the healthy individuals(χ2=4.256, P=0.039). The frequency of AC+CC genotype was higher in control group than that in CPO group(OR=0.8, 95%CI=0.381-1.683),26 among 100 healthy individuals carried AC+CC genetypes,which were carried by 22% of CPO patients. Conclusions The polymorphism of MTHFR A1298C may be involved in the occurrence of non-syndromic cleft palate only in Chinese population.
9.A prospective cohort study on hormone-dependant metastatic breast cancer treated with Shuganyishen formula

Objective Observing the effect of Shuganyishen formula on hormone-dependant metastatic breast cancer and the mechanisms of anti-resistance and endocrine therapy. Methods 226 ITT cases were divided into two cohorts by treatment. 126 cases were treated with the combination of Shuganyishen formula and standard western medicine including endocrine therapy, and 100 cases were treated with western medicine alone. The end points were time to progress (TTP) and overall survival and quality of life (QOF). The effect of Shuganyishen formula on growth of TAM-resistance cell line in vitro was observed. Results There were statistical significances of median TTP between combination and western medicine in Luminal A (ER+ /PR+, HER2-) as well as Luminal B (ER+/PR+, HER2+), but no significances of OS; The extent and incidence of hot flush, sweat and syndromes related to bone were decreased significantly in combination treated group. Shuganyishen formula could reverse resistance to TAM. Conclusion Patients with LuminalA and LuminalB metastatic breast cancer benefit from Shuganyishen formula, which maybe related with its reverse resistance to endocrine therapy.
10.Value of serum and saliva anti-α-fodrin antibodies in patients with sjfigren's syndrome

Objective To evaluate the value of IgA and IgG antibodies against α-fodrin in both serum antibodies in SS is also assessed.Methods Samples from 39 patients with SS(25 primary and 14 secondary),8 patients with systemic lupus erythematosus(SLE),and 15 patients with rheumatoid arthritis (RA)as well as 10 healthy blood donors were collected.Anti-α-fodrin antibodies were measured using ELISA.Results The titer of serum anti-α-fodrin was higher in SS than in other connective tissue diseases group and healthy group(P<0.01).IgA type anti-α-fodrin antibodies was detected in 60%.44% of serum and saliva in patients with pSS respectively.IgG antibodies were detected in 43% of sera,and 29% of saliva of patients with pSS.The sensitivity and specificity of serum anti-α-fodrin IgA in SS was 54%and 85%.The level of anti-α-fodrin was positively associated with xerostomia and parotid swelling (P<0.05),and was negatively associated with xeroma,renal tubule acidosis,lung interstitial disease and hepatic damages(P>0.05).Conclusion Saliva and serLlm anti-α-fodrin level may be diagnostic for SS.It may be a useful screening marker.
