1.Immediate Tissue Expander Insertion for Breast Reconstruction Following Mastectomy for Breast Cancer Patients. Our Experience of Breast Surgeon - MDbP 101.

PURPOSE: Although breast reconstruction provides some advantages for women following mastectomy, few Korean breast cancer patients currently receive such reconstruction. Routine provision of breast reconstruction requires simplicity and easy availability for the procedure. This paper reports the possibility of performing breast reconstruction by insertion of a tissue expander by the breast surgeon. METHODS: We studied 22 cases of breast cancer patients who were treated in the Breast service of KangNam Cha Hospital. Nine cases were the group of immediate expander insertion and 13 were the group of MRM only. We evaluated age, histopathologic stage, starting time of chemotherapy, operation time, drainage amounts and periods, medication periods, time of discharge and depression score. RESULTS: The mean age of the expander insertion group was 41, which was younger than that of the MRM only group by 3 years. Histopathologic state was better in the expander insertion group and the time for chemotherapy start was almost the same between the two groups. Mean operation time in the expander insertion group was 2 hours and 41 minutes, and it was longer than the MRM only group by 1 hour, but it included additional wasting time to check the results of frozen biopsy. Periods for drainage were longer and amounts were larger, but this only delayed the medication period and time for discharge by two days. There were no other complications and mental suffering was alleviated. CONCLUSION: Tissue expander insertion for breast reconstruction could be offered on a routine basis by breast surgeons without problems. Breast reconstruction will become a more essential process for breast cancer patients to improve the quality of life. It is ideal if the same surgeon participates in both oncology and reconstruction surgery.

3.Midterm results of bipolar artificial femoral head arthroplasty in treatment of elder femoral neck fracture

[Objective]To discuss the midterm effects of bipolar artificial femoral head arthroplasty in treatment of elder femoral neck fracture.[Method]From September 1995 to Febuary 2005,73 elderly patients with femoral neck fracture were operated with domestic bipolar artificial femoral head arthroplasty (average 74 years old).Forty-seven cases had been followed up for average 65 months.Harris score system and X-ray had been used for evaluation.[Result]There was no vessel and nerve injury,no infection and dislocation,no perioperative death.The complications included one greater trochanteric fracture,3 cases with pneumonitis,2 urinary infection,1 cerebral infarction,1 dysfunction in central nerve system and 1 case with deep venous thrombosis.Of all the 36 X-rays there were 3 cases with prosthesis subsidence,lucent line lesser 2mm in 2 cases,narrow joint space in 21 cases,protrusion in 1 case.According to the Harris scoring system,there were excellent in 12,good in 26,fair in 7,poor in 2.[Conclusion]Bipolar artificial femoral head arthroplasty are reasonable choice for treatment of femoral neck fracture in the elderly.Proper indication and perioperative management is critical to get good results.
4.Pathological changes of CT scan on thermochemotherapy during and after human glioma operation

Objective To explore the pathological changes by CT scan on localized thermochemotherapy dur- ing and after the operation of human gliomas.Methods Retrospective analysis was given to the CT scan of 37 pa- tients receiving thermochemotherapy during and after the operation,and the relation of the tumorous cells and mi- crovessels and CT density by EM were analyzed.Changes of tumorous cells and microvessels after localized ther- mochemotherapy on C_6 gliomas in rat were analyzed.Results When the tumor was low dense on CT pattern,less cellular number with increasing the amount of fluid between the cells was demonstrated pathologically.On EM,a lower cellular electron density was observed.The amount of fluid in cytoplasm was increased,the cytoplasm was porous,swelling denaturation was chiefly seen in organelle.If the tumor had mixed density on CT,cellular number was more,the amount of fluid was less.On EM,cellular electron density increased correspondingly,the fluid in cyto- plasm decreased,organdie was aggregated.After thermochemotherapy,the tumor reduced,liquefied,and vanished by CT scan.It could be observed that the tumorous cell become smaller,concentrated and cataclased,finally formed apoprotic bodies and separated from the cell in C_6 gliomas in EM.The tumorous vessels was less,smaller and thinker. Some vessels only could see the base membrane and no endothelioid cells.Conclusion The remaining tumors is van- ished by CT scan.The mechanisms of tumors disappearance proposes to explain that thermochemotherapy can dam- age C_6 glioma cells and microvessels,decrease microvessels density and induce tumor ceils apoptosis.That inhibits tu- morous angiogenesis and proliferation.

6.Relationship between Candidas and melanin

Objective Candidas rank as the second factor of deep fungal infection .This study aimed to investigate the rela-tionship between Candidas and melanin by screening clinically isolated melanin -producing Candidas . Methods We incubated 360 strains of clinically isolated Candidas with caffeic acid , DOPA agar , and minimal medium at a temperature of 35℃, with Cryptococcus neoformans as the positive control .We observed the changes in the culture media using transmission electron microscopy (TEM) and electron spin resonance ( ESR) spectroscopy . Results Cryptococcus neoforman produced pigmentation in all the three types of medi -um.At 5 days, a standard strain of Candida albicans SC 5314 turned dark brown in the DOPA agar and minimal medium and , at 10 days, 12 and 15 of the 360 Candida strains turned dark brown in the DOPA agar and minimal medium , respectively, which were not shown to be melanin by TEM and ESR spectroscopy . Conclusion Culture media are not reliable enough for the identification of mel-anin-producing Candidas , and the observations need to be further verified by TEM and ESR spectroscopy .
7.Relation of radiotherapy dose and sternocleidomastoid muscle (SM) mass or late response in patients with nasopharyngeal carcinoma

Objective To investigate the relationship of radiation dose with the volume and late toxicity of the sternocleidomastoid muscle ( SM) in patients with nasopharyngeal carcinoma. Methods SM was divided into upper part and lower part based on the lower edge of cricoid cartilage. Patients were divided into three groups according to the prescribed dose for clinical target volume at the lower neck ( CTV2 ) ( 0, 54,60 Gy) . The dosimetric parameters included Dmean , V66 , and V60 for the upper, lower, and whole SM. SM was delineated and the volume was calculated on computed tomography images in the treatment planning system before and at 6, 12, and 18 months after treatment. The anteroposterior and transversal diameters of SM at C3?C4 , C4?C5 , C5?C6 , and C6?C7 levels were measured and recorded. Late toxicity of neck skin and SM was evaluated according to the Common Terminology Criteria for Adverse Events V4 .0 criteria. Between?group comparison was made by t?test or Kruskal?Wallis non?parametric test. Between?group comparison of the sample rate was made by one?way analysis of variance. The correlation analysis was made by Spearman correlation. Results There were significant difference in SM volume between the three time points after treatment ( P=0. 000) . At 12 or 18 months after treatment, the volume of SM wasignificantly reduced ( P=0. 000,0. 000);the reduction in SM volume was significantly correlated with V66 of the SM and the upper SM ( P=0. 015,0. 020) . At 18 months after treatment, SM fibrosis was significantly correlated with V60 of the upper SM ( P=0. 030);the fibrosis of neck skin was significantly correlated with the Dmean and V60 of the upper SM ( P=0. 029,0. 005) . Conclusions In order to prevent the incidence of the fibrosis of neck skin and SM, the dose homogeneity should be as high as possible, while the number of hot spots should be as small as possible.

9.Color Doppler Ultrasonography in the Evaluation of the Acute Scrotum.

Thirty seven patients with acute scrotal pain were evaluated by color doppler ultrasonography. The diagnosis was made by clinical finding in 26 cases and surgical finding in 11 cases. There were inflammatory diseases in 22 cases, testicular torsion in 5 cases, other diseases in 10 cases. 20 cases of inflammatory disease showed increased blood flow in color doppler ultrasonography and 4 cases of testicular torsion showed absent blood flow. Thus, color doppler ultrasonographic evaluation had diagnostic value that was 80% in sensitivity and 96% specificity in differentiation of testicular torsion from other acute scrotal diseases. We think that color doppler ultrasonography in acute scrotum is a valuable diagnostic method for differentiation of testicular torsion from other acute scrotal diseases.

10.Effect of HDAC6 down-regulation on the growth of xenografted human laryngeal carcinoma cell line Hep-2 in nude mice and underlying mechanism

Objective To study the effect of histone deacetylation 6(HDAC6) siRNA on the growth of xenografted human laryngeal squamous cell carcinoma cell line Hep-2 in nude mice and underlying mechanism.Methods Laryngeal squamous cell carcinoma cell line Hep-2 cells were subcutaneously injected to the back of nude mice and transplanted tumor model was established after one week.Nude mice was divided into three groups including blank control group,empty vector group and HDAC6 siRNA group,and the tumor growth was observed. Ki-67 proliferation index was detected by immunohistochemistry.Western blot,in situ hybridization and immunohistochemistry were used to detect the mRNA and protein expressions of HDAC6 in xenograft.The expressions of Bcl-2 and Bax proteins were examined by Western blotting.Cell apoptosis was detected by TUNEL.Results The mean volume of xenograft transfected with HDAC6 siRNA was less than that of xenograft transfected with empty vector or that of xenograft with blank control treatment (P < 0.05 ).HDAC6 siRNA effectively down-regulated the expressions of HDAC6 mRNA and the expressions of HDAC6 and Bcl-2 proteins,but up-regulated the expression of Bcl-2 protein in xenografts,with significant differences ( all P < 0.05 ).The proliferation index of Ki-67 in HDAC6 siRNA transfection group was significantly lower than that in blank control group or emptry vector group ( P < 0.05).TUNEL assay demonstrated that HDAC6 evidently evoked cell apoptosis( P < 0.05 ).Conclusion HDAC6 siRNA could effectively inhibited the growth of xenografted human laryngeal carcinoma cell line Hep-2 in nude mice,down-regulate the expressions of HDAC6 and bcl-2,and up-regulate the expression of bax.
