1.Comparison and enlightenment of two doctor-patient communication textbooks between China and foreign countries

Differences are existed in two textbooks of doctor-patient communication between China and foreign countries concerning perspectives of doctor-patient communication,teaching goals and applicable scope based on the consensus on importance of doctor-patient communication and education training.Contents in textbooks of china emphasize the logic of discipline while that of foreign countries pay attention to the logic of clinical treatment.The advantages of textbooks of China are having strong theoretical basis and ample contents while those of foreign text books are practicability and effectiveness.Some suggestions which to benefit the textbook development of Chinese doctor-patient communication has been offered:textbooks of doctor-patient communication in our country should be separated from those of nursing communication; advantages and experiences of foreign textbooks should be drew from; in-depth development of doctor-patient communication theory research and education reform should be conducted; diversification of teaching materials of doctor-patient communication should be gradually perfected ; teaching quality of doctor-patient communication should be improved.
2.Repair of segmental bone defects with intramedullary nailing and bone cement

Objective:To investigate the clinical efficacy of intramedullary nailing combined with bone cement in repair of segmental bone defects after tumor resection.Methods:A retrospective analysis was conducted of the 5 patients with malignant bone tumor who had been treated at Department of Orthopaedics, Qiannan People's Hospital from April 2018 to September 2019 for remaining segmental bone defects following limb salvage surgery. They were 4 males and one female, aged from 11 to 55 years (average, 35.4 years). Their defects ranged from 6 to 21 cm (average, 12.3 cm) after tumor resection. By the Karnofsky performance score (KPS) for long-term quality of life, all of them scored less than 50 points. Of them, 3 were treated by interlocking intramedullary nails and bone cement filling, and 2 by elastic intramedullary nails and bone cement filling. In the 2 cases with defects of 21 cm and 13 cm, the fixation was assisted by a plate and an external fixator. Defect length after resection, operation time and intraoperative bleeding were recorded; the efficacy was evaluated by the Enneking functional evaluation of reconstructive procedures after surgical treatment of tumors of the musculoskeletal system, visual analogue scale (VAS), and KPS.Results:All the 5 patients had uneventful surgery, with operation time ranging from 112 to 225 min (average, 154.2 min), intraoperative bleeding from 300 to 500 mL (average, 382 mL), and defect length after resection from 6 to 21 cm. The 5 patients were followed up for 6 to 28 months. Of them, 2 died of disease progression 6 and 7 months after surgery, respectively. According to the Enneking's evaluation, one patient scored 28 points, 2 patients 23 points and 2 patients 21 points, giving a high degree of satisfaction. Their VAS scores 6 months after surgery ranged from 1 to 6, averaging 3.6; their postoperative KPS scores ranged from 60 to 80, averaging 72.Conclusion:In repair of segmental bone defects after tumor resection, intramedullary nailing combined with bone cement filling can relieve pain of patients and lead to satisfactory short-term curative efficacy.

A new steroid with a long cross-conjugation structure, 15a-hydroxy-(22E, 24R)-ergosta-3, 5, 8 (14), 22-tetraen-7-one (1), was isolated from the marine-derived fungus Aspergillus aculeatus. 8.The recurrent rate of nasal endoscopic microsurgical skills for the treatment of nasal inverted papilloma: a meta-analysis.

OBJECTIVE:
To investigate the recurrence rate of nasal inverted papilloma treating by endoscopic and non-endoscopic approach.
METHOD:
A search on Pubmed, Medline, Springer and Elsevier databases was done to collect the reports (2001-2013) concerning different surgery treating nasal inverted papillomas, and meta-analysis was performed with RevMan 5.0 software.
RESULT:
Twelve papers (2001-2013) concerning the different surgery approach treating nasal inverted papillomas were retrieved. The heterogeneity test indicated that the 12 studies were consistent statistically (Q = 14.64, df = 11, P = 0.20), the data from these 12 studies could be analyzed by fixed effect model. After combination of these data, those of 1012 subjects accepted endoscopic surgical intervention and 359 subjects treating by non-endoscopic surgical intervention were collected. Test of overall effect by fixed effect model showed that the recurrence rate of inverted papilloma was significantly lower in endoscopic group than in non-endoscopic group (OR = 0.49, 95% CI was 0.35-0.69, P < 0.01). Funnel plot implied that publication bias was not obvious.
CONCLUSION
The recurrence rate of inverted papilloma was significantly lower in endoscopic group than in non-endoscopic group.

9.Pentacam anterior segment analysis system in evaluating the efficacy of phacoemulsification for angle-closure glaucoma

Objective:To observe the changes in the anatomy of anterior segment before and after phacoemulsification for treatment of angle-closure glaucoma by Pentacam system,so as to assess the efficacy of phacoemulsification combined with artificial lens implantation(ALI)in treatment of early-and middle-stage angle-closure glaucoma.Methods:Forty-two patients with early-and middle-stage angle-closure glaucoma(44 eyes)were included in the present study.The central anterior chamber depth,anterior chamber volume and peripheral anterior chamber angle were examined by Pentacam system pre-operatively and 1 day,1 week and 3 months post operatively.Meanwhile,A ultrasound was used to examine the anterior chamber depth before and 3 months after operation.and the results were compared with those by Pentacam system.Results:Pentacam system could clearly demonstrate the 3-dimensional structures of the anterior eye segment before and after operation.The central anterior chamber depth,anterior chamber volume and anterior chamber angle were increased significantly after operation(P

Objectives To summarize the characteristics,differential diagnosis and management of incomplete 17 alpha-hydroxylase/17,20-1yase deficiency(17 OHD)of Chinese patients.Methods Six cases of incomplete 17 OHD from Peking Union Medical College Hospital were studied retrospectively through analyzing their clinical data,and the molecular pathogenic mechanism was discussed after literature review.Results Four cases of 46,XX incomplete 17 OHD were reported.The clinical characteristics included female phenotype,various degrees of breast development and absent or sparse axillary/pubic hair, oligomenorrhea or secondary amenorrhea,recurrent luteinized ovarian cysts,hypogonadism with persistent hyperprogesteronemia or high serum 17 alpha-hydroxyprogesterone level,with or without hypokalemic hypertension.There were also 2 cases of 46,XY incomplete 17 OHD,in which ambiguous genitalia were present besides hypokalemic hypertension.Conclusions Incomplete 17 OHD is a very rare form of congenital enzymatic deficiencies of steroid synthesis,which should be included in the differential diagnosis when there are menstrual disorders,sexual infantilism,recurrent ovarian cysts or ambiguous genitalia.Under such circumstances,hyperprogesteronemia offers a valuable clue for further investigation.
