1.Protective Effect of Salvianolic Acid B in Goto-kakizaki Rats Complicated with Macroangiopathy
China Pharmacist 2016;19(5):836-838
Objective:To study the protective effect and possible mechanism of salvianolic acid B in GK rats complicated with macroangiopathy. Methods:Totally 32 GK rats were randomly divided into four groups:the model group and low, middle and high dose salvianolic acid B groups(40,80 and 160 mg·kg - 1 ),and 8 Wistar rats were used as the normal group. All of GK rats were giv-en high-glucose and high-fat diet. After 16 weeks,GK rats were orally administrated with 10 mg·kg - 1 ·d - 1 N-ω-nitro-L-arginine methyl ester for 8 weeks. At the beginning and the end of the experiment, blood glucose and blood pressure were measured, abdominal aortic blood was collected after anesthesia, thoracic aorta was used to make paraffin sections after bloodletting to observe vasculopathy and the expression of VEGF was detected by Western-blot. Results:The blood glucose was increased in all GK rats with no statistical significance between the drug group and the model group(P > 0. 05). At the end of the experiment, the blood pressure was significant-ly increased in GK rats, indicating that salvianolic acid B could notably reduce the blood pressure in GK rats in a dose-dependent man-ner with statistical significance(P < 0. 05);and it could also remarkably reduce the total white blood cell count and differential blood count, and ameliorate the lesion extent and reduce VEGF expression. Conclusion:Salvianolic acid B has protective effects on aorta in GK rats, and its mechanism may be related with blood pressure lowering,anti-inflammation and reduction of VEGF expression.

4.Effects of Traditional Chinese Medicine Ruanganyin on TGF-1/Smad Signaling Pathway in Carbon Tetrachloride-induced Hepatic Fibrosis of Rats
Journal of Zhejiang Chinese Medical University 2006;0(02):-
[Aim]To study the effects of Ruanganyin on TGF-1,Smad3,Smad7 expressions of the experimental hepatic fibrosis induced by carbon tetrachloride (CCl4) in rats, and to explore its anti-fibrotic molecular mechanism.[Method] Forty healthy Wistar rats were randomly divided into normal control group, vacant control group,CCl4-induced hepatic fibrosis group, and Ruanganyin treated group. The rats in normal group were treated with normal saline by intragastrical irrigation, and the other groups were subcutaneous injected with CCl4 to establish hepatic fibrosis model. The histological changes of the liver tissues were examined by pathological examination. The protein expression of transforming growth factor beta 1 (TGF-1) and Smad3,Smad7 were measured by immunohistochemical technique.[Result] In comparison with those inmodel group, the levels of TGF-1 were significantly decreased after treatment with Ruanganyin.Meanwhile, the levels of Smad3 was also down-regulated.However,the level of Smad7 expression was significantly up-regulated.The pathological changes of liver tissues in Ruanganyin groups were also markedly alleviated.[Conclusion] Ruanganyin has significant inhibitory effects on CCl4-induced hepatic fibrosis in rats, and its mechanism may be associated with the inhibition of TGF-1 expression,down-regulation of Smad3 expression and up-regulation of Smad7 expression.
5.Anti-inflammatory effect of tranexamic acid in the perioperative period of total knee arthroplasty
Chinese Journal of Primary Medicine and Pharmacy 2021;28(3):400-404
Objective:To investigate the anti-inflammatory effect and safety of tranexamic acid in the perioperative period of total knee arthroplasty. Methods:Eighty patients who underwent primary unilateral total knee arthroplasty because of knee osteoarthritis in Anqing Hospital of Anhui Medical University from October 2018 to March 2020 were included in this study. They were divided into an observation group and a control group ( n = 40/group) according to whether they received tranexamic acid treatment. In the observation group, 1 g tranexamic acid was intravenously administered at 10 minutes before releasing tourniquet and once every 3 hours starting at 24 hours after surgery. In the control group, 100 mL 0.9% sodium chloride injection was identically administered. Before surgery, immediately after surgery, and 1, 2 and 3 days after surgery, interleukin-6 level, erythrocyte sedimentation rate and C-reactive protein level in the venous blood were compared between the observation and control groups. Coagulation function and lower extremity deep venous thrombosis in the two groups were monitored. Postoperative pain, nausea, vomiting and muscle strength were compared between the two groups. Results:There were no significant differences in age, gender, body mass index and Hospital for Special Surgery knee score between the two groups (all P > 0.05). Univariate analysis of variance revealed that there were significant differences in interleukin-6 level ( t1 = 9.364, t2 = 11.027, t3 = 11.068), erythrocyte sedimentation rate ( t1 = 20.94, t2 = 18.898, t3 = 22.797) and C-reactive protein level ( t1 = 14.079, t2 = 12.099, t3 = 23.416) between the two groups at 1, 2 and 3 days after surgery (all P < 0.05). Regression analysis revealed that the maximum differences in erythrocyte sedimentation rate ( β = 0.217, P = 0.038), interleukin-6 ( β = 0.143, P = 0.025) and C-reactive protein ( β = 0.032, P = 0.010) between before and after total knee arthroplasty in the observation group were significantly lower compared with the control group ( β > 0, P < 0.10). Moreover, the differences in erythrocyte sedimentation rate, interleukin-6 and C-reactive protein at 3 days after surgery were significantly lower compared with at the other time points studied. Before and after surgery, there was no significant difference in coagulation function between the two groups (both P > 0.05). Lower extremity deep venous thrombosis occurred in neither groups. The recovery of postoperative pain, nausea, vomiting, and muscle strength in the observation group was significantly superior to that in the control group (all P < 0.05). Conclusion:Application of tranexamic acid in primary unilateral total knee arthroplasty can effectively attenuate perioperative inflammatory reaction, reduce postoperative adverse reactions and promote functional recovery, which deserves clinical application.
6.Advances of tumor necrosis factor-α inhibitor in the treatment of junior idiopathic arthritis associated uveitis
Chinese Journal of Ocular Fundus Diseases 2021;37(5):399-403
Uveitis is the most common extra-articular manifestation of juvenile idiopathic arthritis, typically as chronic anterior uveitis with insidious onset. Delayed and inadequate treatment may result in loss of patients' vision and even blindness. For refractory or severe uveitis related to juvenile idiopathic arthritis, systemic immunosuppressive agents should be used as early as possible. With the advantage of controlling ocular inflammation, avoiding ocular complications and reducing the use of traditional immunosuppressant drugs and glucocorticoid, tumor necrosis factor-α inhibitors have been new therapeutic options for uveitis associated with juvenile idiopathic arthritis, although methotrexate is known as the first-line approach. However, there are no internationally unified guidelines for clinical issues regarding the timing of application, reduction and withdrawal of tumor necrosis factor-α inhibitors, and no agreement on the application of tumor necrosis factor-α inhibitors in the management of ocular complications either. An in-depth understanding of the application status and progress of tumor necrosis factor alpha inhibitors in the treatment of juvenile idiopathic arthritis-associated uveitis has important clinical significance.
