1.The Effects of Preoperative Bronchial Arterial Infusion Chemotherapy for Stage III_A NSCLC

Objective To evaluate preoperative bronchial atery infusion(PBAI) chemotherapy combined with surgery for patients with stage III A non-small cell lung cancer(NSCLC). Methods Seldinger procedure was used to insert the tube to bronchial artery branch which supplied blood to the tumor. The chemotherapeutic drugs were infused through the tube. After 1~3 courses of PBAI chemotherapy, the patients received operation. The patients receiving surgical treatment without PBAI chemotherapy at the corresponding period were chosen as control group. The radical resection rate and survival rate of the two groups were analyzsed. Results The response rate (CR+PR) of PBAI chemotherapy was 66.8%, and radical resection rate (90.3%) in PBAI chemotherapy group was significantly higher than that in control group(61.1%,P

3.EFFECTS OF DIET DEFICIENT IN SELENIUM AND PHOSPHOLIPID ON THE MYOCARDIAL CYTO-CHROME OXIDASE ACTIVITY IN GUINEA PIGS

A dynamic study for the activity of myocardial cytochrome oxidase from guinea pigs fed with low selenium and low phospholipid semi-synthetic diet (basal diet) or diet supplemented with selenium, phospholipid or other agents was conducted with Pye Unicam SP-1800 UV spectrophotome-ter and Circular C100.The results showed that the activity of myocardial cytochrome oxidase from animals fed with basal diet was the lowest. The single supplement of selenium or phospholipids did not increase the enzymic activity significantly. Only when selenium and phospholipids, especially all the ingredients related to the phospholipids metabolism, such as choline and methionine were supplemented simultaneously, the enzymic activity (velocity constant K) then increased significantly, and sometime even exceeded that of the group fed with normal diet. In the sametime the ratio of phospholipids in heart and the cardiolipin content was close to that of control animals fed with normal diet. The significance of boundary lipids in heart for membranous enzyme and the pathogenesis of Keshan disease was discussed.
4.Relationship between anti-cyclic Citrullinated petide antibody and cardiovascular complication associated with rheumatoid arthritis

Objective The aim of this study was to assess the relationship between the level of anti-CCP antibody and cardiovascular(CV) risk in rheumatoid arthritis(RA) patients.Methods A total of 78 RA patients were enrolled in the study.They were divided into two groups:40 cases with CV comphtations which were secondary from RA and 38 cases with no complications.The anti-CCP antibody was diagnosed by enzyme - linked immunoabsorbent assay(ELISA) and rheumatoid factor(RF) were assessed by immunoturbidimetry method.The relationship between the levels of the two sera auto-antibodies and CV disease associated with RA was analyzed.Results The results showed that the level of anti-CCP antibody in RA-CV disease group was significantly higher than that in simple RA group(74.6% VS 87.3% ).Compared with the simple RA group,the level of anti-CCP antibody in the RA-CV disease group was higher,and with significant difference between the two groups( x2 =5.07,9.04,6.48,all P < 0.05 ).RF positive rate was 95.8% of the patients with RA-CV disease while it was 76.5% in simple RA group,and the difference between the two groups was statistically significant( x2 =33.41,26.05,34.14,all P < 0.05 ).The morbidi ty between the different level of RF had remarkable difference.In RA patients with CV complations,we compared antiCCP antibody and RF by Spearman analysis,whish showed no relationship between them ( r =0.301,P =0.174 ).Conclusion The results indicated that CV disease secondary from RA was more common in old patients.In RA patients the high level of anti-CCP antibody and RF maybe play an important role in the appearance of CV disease.
5.Bronchial-pulmonary arterial sleeve resection and reconstruction in the treatment of central non-small cell lung cancer

Objective To evaluate the indications and surgical procedure of bronchial-pulmonary arterial sleeve resection for patients with centrally located non-small cell lung cancer (NSCLC),and to prevent complications. Methods From October 1987 to December 2004, 96 cases of central NSCLC were treated with bronchial-pulmonary arterial sleeve resection and reconstruction. The results were retrospectively analyzed. Results The complication rate was 19.8%(19/96), the mortality rate in 30-day postoperation was 3.1%(3/96), the overall 1, 3, 5 year survival rates were 82.6%(76/91), 57.8%(37/64) and 39.1%(18/46) respectively. Conclusion Bronchial-pulmonary arterial sleeve resection and reconstruction in the treatment of patients with central NSCLC can not only maximize preservation of functional pulmonary parenchyma and improve the quality of life, but also provide an opportunity for those patients with poor pulmonary function to receive surgical resection of the tumor.

BACKGROUND: Simple polyvinyl alcohol (PVA) has limited ability to cell adhesion. There are not generally accepted studies on improved effects of collagen protein modified polyvinyl alcohol on cell adhesion and proliferation.OBJECTIVE: To investigate the feasibility of PVA/type Ⅰ college (COL-Ⅰ) as anterior cruciate ligament (ACL) scaffolds in tissue engineering.DESIGN, TIME AND SETTING: The controlled observation experiment was performed at the Fourth Affiliated Hospital, Medical College. Ji'nan University, Guangzhou Red Cross Hospital, Guangzhou Institute of Trauma Surgery from August 2006 to October 2007.MATERIALS: COL-Ⅰ gel was produced by Guangzhou Institute of Trauma Surgery.METHODS: PVA filature was used to weave fascicular scaffolds. NIH-3T3 cell line and human ACL cells were in vitro incubated, amplified, and then implanted on the PVA/COL scaffolds.MAIN OUTCOME MEASURES: The growth of NIH-3T3 cell line and human ACL cells on the PVA/COL scaffolds and the secretion of extracellular matrix were observed using scanning electron microscope. Cell compatibility of PVA/COL scaffolds was assessed. Mechanics characteristic of PVA/COL scaffolds was measured by using the electric. tensile force apparatus. Mechanical property of PVA/COL scaffolds was analyzed using the SPSS 11.5 software package.RESULTS: NIH-3T3 cell line and human ACL cells on the PVA/COL scaffolds adhered, proliferated, and secreted extracellular matrix. NIH-3T3 cell line highly grew compared with human ACL cells on the PVA/COL scaffolds. The adhered number of NIH-3T3 cell line and human ACL cells was significantly increased on the PVA/COL scaffolds. NIH-3T3 cell line and human ACL cells presented well morphology on the PVA/COL scaffolds. COL-Ⅰ could promote the secretion of extracellular matrix from NIH-3T3 cells, but its effects on human ACL cells were not significant. Tensile force test showed that load-extension curve of the materials was identical to ACL of human and rabbits, and the scaffolds possessed strong flexibility. The maximal load, ultimate stress and elastic modulus were respectively 52.61 N, 14.96 MPa and 202.08 MPa.CONCLUSION: COL-Ⅰ accelerates the adhesion and proliferation of NIH-3T3 cell line and human ACL cells on the surface and in the pore of the PVA/COL scaffolds, promotes the secretion of extracellular matrix from NIH-3T3, and PVA filature material has mechanical property and good cell compatibility.
7.Comparison of 3D conformal radiotherapy with conventional radiotherapy in gliomas on dosimetry and prognosis

Objective In contrast to conventional radiotherapy ,analyze the dosimetry and prognosis of 3D conformal radiothera-py in the treatment of gliomas with different pathological stages .Methods 97 patients with gliomas were selected ,and divided into two groups as conventional radiotherapy group (CR group) and 3D conformal radiotherapy group(3D group) .analyzed the influence of different radiotherapy to the gliomas in the same pathological stage .Compared the dosimetry differences between these two radio-therapies .Results There was no obvious differences in 3 year survival rate between CR group and 3D group at low grade gliomas . But 3D group had better 3 year survival rate than CR group at high grade gliomas .And 3D group had lower rate of relevant sequel after radiotherapy .3D group had higher conformal index (CI) and average dosage of radiation than CR group .Conclusion 3D radio-therapy has better dosimetry and conformal index than conventional radiotherapy ,especially has better prognosis in high grade glio-mas and lower rate of relevant sequel after radiotherapy .
8.Assessment of effect of laboratory external quality assessment program on cytogenetic testing

Objective To analyze the change of chromosome test level after participating in the inter‐laboratory proficiency test(PT) program of the College of American Pathologists (CAP) .Methods The results for participating in CAP PT during 2011-2014 were analyzed ,the accuracy of the cytogenetic chromosome quality control test in this laboratory was obtained accord‐ing to the CAP evaluation results ,and the effect of CAP external quality assessment item on increasing the chromosome detection level in this laboratory was evaluated by analyzing the chromosome band levels before and after participating in CAP .Results This laboratory participated in CAP PT test for 10 times during 2011-2014 ,a total of 59 cases were analyzed ,the accuracy rate for jud‐ging chromosome karyotype was 100% ,the karyotype description accuracy rate was 95 .1% .The chromosome test results of clinical cases in this laboratory displayed that peripheral blood chromosome abnormal detection rate was 18 .9% and bone marrow chromo‐some abnormal detection rate was 25 .9% ,the abnormal rate of newly diagnosed leukemia was 66 .8% ;the detection failure rates of peripheral blood chromosome and bone marrow chromosome were 0 .5% and 5 .0% respectively ;the detection failure rates of pe‐ripheral blood chromosome and bone marrow chromosome after participating in CAP were decreased ,the chromosome band average level was improved ,showing statistical difference compared with those before participating in CAP (P<0 .01) .Conclusion Partici‐pating in high quality external laboratory assessment item can increase the clinical analytic level of cytogenetic chromosome test .
9.Clinicopathological analysis of IgA nephropathy patients with mild proteinuria and/or hematuria

Objective To clarify the relationship between clinical manifestation and pathological features of IgA nephropathy (IgAN) patients with mild proteinuria and/or hematuria.Methods Clinicopathological data from 316 biopsy-proven IgAN cases (proteinuria<1 g/24 h and/or hematuria, and Scr<133 μmol/L) from our hospital between January 1993 and October 2009 were studied retrospectively. The renal histopathology was quantified according to Lee's grading and Katafuchi's semi-quantitative standard, and the risk factors for renal pathological lesions were evaluated using multifactor logistic regression analysis. Results Among these 316 patients, 123 were male and 193 patients were female. The mean age at the time of renal biopsy was (33.10±10.69) years old. Clinical features were found as follows: hematuria with proteinuria was found in 267 patients (84.5%), isolated hematuria in 24 patients (7.6%), and isolated proteinuria in 25 patients (7.9%). 16.5% of patients had hypertension. The percentages of CKD stage Ⅰ, Ⅱ, Ⅲ were 76.9%, 20.9% and 2.2%, respectively. 31.3% of patients presented Lee's grade Ⅲ or more severe.52.8% of patients had various degrees of glomerulosclerosis. Crescent formation was observed in 20.3% of patients. 22.5% of patients showed tubular atrophy;16.8% showed interstitial fibrosis and 24.7% also had renal vascular lesions. The extent of glomerulosclerosis was negatively correlated with eGFR levels, but positively correlated with the amount of proteinuria and mean arterial pressure (MAP) level (P<0.05). The score of tubulointerstitial lesion was positively correlated with the amount of proteinuria and negatively correlated with eGFR and hemoglobin (Hb)level (P<0.05). The degree of renal vascular lesion was also correlated to MAP level positively and eGFR level negatively (P<0.05). Multifactor logistic regression analysis revealed that proteinuria, Scr and Hb at the time of renal biopsy were independent risk factors for severe renal pathological lesions (Lee's grade Ⅲ or more severe) with odds ratio of 8.564, 1.031 and 0.975 respectively (all P<0.01). Conclusions Severe renal histological lesions and decrease of renal function may be seen in some IgAN patients with mild proteinuria and/or hematuria. The levels of proteinuria,Scr and Hb are the independent risk factors for severe renal pathological lesions. Renal biopsy is important in these patients in order to make diagnosis and individual treatment.
10.Result survey analysis of prenatal chromosome karyotyping in an external quality assessment program.

OBJECTIVETo analyze the results of prenatal karyotype of the external quality assessment program in 2013 in order to provide references and recommendations for improving the capability and performances of karyotype analysis of prenatal screening laboratories.
METHODSFive lots of quality control cell photos were sent to 500 laboratories. The participants were asked to decide whether the photos have demonstrated any abnormal karyotype and determine the abnormal type. The results should be submitted before the deadline and compared with the standard results to evaluate the performances of the laboratory.
RESULTSOne hundred forty three laboratories have returned their karyotype results for the survey. The standard answers were 7,XX,+18, 46,X,i(X)(q10), 46,XY,i(21)(q10) or 46,XY,+21,der(21;21)(q10;q10), 46,XY and 47,XY,+21 in sequential order, which were used to estimate the score of each participant. The pass rates for five lots were 97.9%, 97.2%, 95.8%, 100.0% and 97.9%, respectively. The total pass rate was 97.7%. The error rates were 2.1%, 2.8%, 4.2%, 0 and 2.1%, respectively. The total error rate was 2.3%.
CONCLUSIONSome laboratories did not correctly identify the abnormal karyotypes, while some could not determine the right type of karyotype. The external quality assessment program of prenatal diagnosis of karyotype analysis should be conducted annually in order to improve the capability and performances of karyotype analysis of prenatal screening laboratories.
