1.Effect of microsomal triglyceride transfer protein(MTP) gene polymorphism on angiopathy in type 2 diabetes

Objective To investigate the relationship of MTP gene variation with type 2 diabetic angiopathy. Methods PCR and restriction-fragment-length polymorphism(PCR-RFLP) analysis and gene sequencing were used to investigate the influence of a common MTP gene polymorphism in promoter region on diabetic macroangiopathy. Results (1) A single nucleotide mutation of G→T was detected at 493 bp of upstream from the transcriptional start point.There was a significant difference in genotype frequency(χ2=9.162,P=0.01) and T allele frequency(χ2=10.322,P=0.001) between macroangiopathy group and non-macroangiopathy group.(2) MTP-493 TT group had the significantly higher serum levels of TC, TG, LDL-C, VLDL-C and lower HDL-C than other genotype groups.(3) Multivariate stepwise regression analysis showed that T allele, age, disease duration, diastolic blood pressure and total cholesterol were independent risk factors against type 2 diabetic macroangiopathy.The HDL-C level was negatively related with diabetic macroangiopathy. Conclusions MTP-493 TT genotype may be deeply involved in production of atherogenic dyslipidemia and associated with development of macroangiopathy in type 2 diabetic subjects.
2.The application of blood pressure self-measurements in the management of hypertension in elderly patients in the community

Objective To compare the effects of different methods of blood pressure management in elderly hypertensive patients in the community,so as to explore the effective model for the management of chronic diseases.Methods 302 elderly hypertensive patients in Puhuangyu community health service center were divided into two groups by random number method:(1) receiving conventional antihypertensive drugs (control group,n=152);(2) receiving antihypertensive drugs combined with blood pressure self-measurements at least twice-weekly and real-time adjusting antihypertensive management (blood pressure self-measurements group,n =150).Medication adherence,levels of blood pressure control and blood pressure-targeting rate were compared between groups one year after intervention.Results Medication adherence rate was higher in self-measurements group than in the control group [94.7% (142 cases) vs.75.7% (115 cases),P< 0.05].After intervention,systolic pressure was decreased from (168.5±37.7) mmHg to (155.3±25.2) mmHg(1 mmHg=0.133 kPa) in the control group and from (168.0±38.1)mmHg to (141.7 ±22.9) mmHg in self-measurements group (all P<0.05).Diastolic pressure level was similar in the two groups.There was a significant difference in the blood pressure-targeting rate between groups (P <0.05).Conclusions Blood pressure self-measurements are more effective in controlling blood pressure and can improve the self-management ability in elderly patients with chronic diseases in the community.
3.Clinical observation of Yiling-Qutan Decoction used to treat sub-health

Objective To explore the clinical efficacy of the Yiling-Qutan Decoction on treating Sub-health and the mechanism of traditional Chinese medicine.Methods 219 cases of Sub-health and belonging to dampness-phlegm syndrome and were collected and treated with Yiling-Qutan Decoction for 1 month.The effective rate of syndrome of traditional Chinese medicine and state of Sub-health,syndrome score of traditional Chinese medicine and sub-health status score were observed.Results The effective rate of syndrome of traditional Chinese medicine:7 patients were cured,35 cases were significantly effective,171 cases were effective and 6 cases were unfruitful.The improvement of syndrome score of and Sub-health status were also significantly better (P<0.01).Conclusion It was effective to treat sub-health with Yiling-Qutan Decoction.

Objective To study the effects of compound rhizoma bletillae paint (CRBP) on scalded rats. Methods Rats were randomly divided into test groups (CRBP low, high dose group), CRBP-CA (CRBP without chlorhexidine acetate) group, positive control group (Jingwanhong) and negative control group (no treatment) respectively, with 10 rats in each group. Objective To study the diagnostic value of transperineum color Doppler ultrasonography (TPCDU) and transabdominal color Doppler ultrasonography (TACDU) in pelvic varicosis (PV). Methods Thirty-eight patients with PV were detected by using TACDU and TPCDU separately, and the diagnostic value between the two ways was compared. Operational results was used as standard, and the degree of lesion in patients with PV was evaluated based on the results of surgery. 6.Minimally invasive plate osteosynthesis versus ORIF in treatment of intra-articular fracture of knee joint

[Objective]To comparatively analyze the clinical results of minimally invasive plate osteosynthesis (MIPO) and ORIF in treatment of intra-articular fracture of knee joint and to investigate the utilization value of biological fixation.[Method]According to the principles of conservation of the vascularity,21 cases of intra-articular fractures of knee joint underwent surgical treatment of MIPO in recent years.In accordance with the type of fracture and other anticipated factors influenced bony healing,paired-samples treated of ORIF were selected.The two groups were compared following: operation time,intra-operative blood loss,bone union time,axial malalignment radiologically of knee joint,and the ROM of knee.HSS criteria were used to evaluate the results.[Result]In MIPO group,all 21 fractures were followed up for averagely 14.2 months (range 10~16 months).The mean duration of surgery was 60.0 min (range 50~70 min) and bone grafting underwent in 2 cases.Complete fracture healing was achieved with the average time of 10.0 weeks and assessment of vallgus/varus alignment showed a deviation more than 5 degrees in 3 cases.The average HSS score was 86.67.In ORIF group,all 21 fractures were followed up for averagely 21.2months (range 12~48 months).The mean duration of surgery was 79.52 min (range 65-95 min) and bone grafting underwent in 18 cases.Complete fracture healing was achieved with the average time of 12.24 weeks and assessment of valgus/varus alignment showed a deviation more than 5 degrees in 2 cases.The average HSS score was 82.14.All patients got bony union and no patient developed infection,implant failure in both groups.There was significant difference in operation time,union time and function of knee between the two groups.[Conclusion]To the suitable cases,MIPO appears to stabilize fractures with a low intra-operative blood loss and a low incidence of bone grafting; on the other hand it offers many advantages in the aspects of operation procedures and functional recovery postoperatively.
7.Anti-inflammatory Effect of Fukang Granules

OBJECTIVE:To study the anti-inflammatory effect of Fukang granules (FK,prepared from traditional Chinese herbs,such as Radix Astragali,Radix Codonopsis,Angelica sinensis,etc.). METHODS:The anti-inflammatory action of FK was studied by carrying out rat paw edema test,mouse auricular swelling test,rat chronic pelvic inflammation test and rabbit salpingitis test.RESULTS:FK could lessen rat paw edema,mouse auricular swelling and rat pelvic inflammation and rabbit salpingitis,reduce blood viscosity and blood reduction viscosity and the coefficients of cell aggregation and thrombosis. CONCLUSION:FK has anti-inflammatory effect and the action of activating blood circulation to dissipate blood stasis.
8.Identification of mutation in the ?-L-iduronidase gene (IDUA) in Hurler syndromes in Chinese populations

AIM: To investigate the mutation type in the IDUA gene of Liaoning district mucopolysaccharidosis I (MPS-I) patients. METHODS: The mutation type and polymorphism site in the IDUA gene of Liaoning district MPS-I patients were detected by PCR-RFLP, SSCP and DNA sequencing. RESULTS: ① There is a new mutation (1278-g-a) in the IDUA gene of Liaoning district MPS-I patients. ② There is no the common mutation (W402X and Q70X) of European patients and the common mutation (R89Q) of Japanese patients in the 10 families we studied. CONCLUSION: The mutation type in the IDUA gene of Liaoning district MPS-I patients is different from that of other countries and districts.
9.EFFECT OF INSERTING NASOGASTRIC TUBE DURING RADIOTHERAPY FOR ULCERATING OR FISTULIZED ESOPHAGEAL CARCINOMA

From 1990 to 1992, 41 patients with ulcerating or fistulized esophageal carcinoma (EC) were treated by the conventional irradiaton after a nasogastric tube was inserted into the esophageal lumen. The total doses were 60~65GY/6~6.5WKS. The results were: 1/1 CR in patients with small ulcer, 10/13 CR and 2/13 PR in patients with medium-sized ulcer, 2/7 CR and 3/7 PR in patients with large ulcer, 3/10 CR and 5/10 PR in patients with esophageal mediastinal fistula and 1/10 CR and 4/10 PR in patients with esophagel tracheal or bronchial fistula. The total remission rate was 75.6%(31/41). The 1-and 2-year survival rates were 42.8%, 26.6% for EC with ulcer and 25%, 10% for EC with fistula, respectively.
10.Clinical observation of Yubaisang-Zhikefang decoction on treating 128 patients withchronic bronchitis

Objective To observe effects of theYubaisang-Zhikefangdecoction on chronic bronchitis.MethodsA total of 128 patients were randomly divided into two groups. The control group was given the conventional therapy, and the treatment group was added theYubaisang-Zhikefang decoction based on the conventional therapy.ResultsThe total effective rate of treatment group was significantly better than the control group (92.58%vs. 79.37%;χ2=4.434,P=0.035). The treatment group significantly improved the symptoms of time of cough disappearance(3.28 ± 1.21 d vs. 5.77 ± 1.45 d;t=10.562,P<0.05), time of asthma stop (8.02 ± 3.29d vs. 10.83 ± 7.17 d;t=2.864,P<0.05), time of sputum disappearance (3.27 ± 1.18d vs. 5.18 ± 1.16 d;t=9.232,P<0.05), time of moist sounds disappearance(4.53 ± 1.27 vs. 7.96 ± 1.45 d;t=14.249, P<0.05). And the treatment group significantly improved the results of blood tests and lung imaging, like the WBC (5.9×109/L ± 2.7×109/L vs. 8.1×109/L ± 2.6×109/L;t=4.693,P<0.05), N% (63.3% ± 6.1% vs. 69.2% ± 6.5%;t=-4.68,P<0.05), CRP (14.5 ± 5.9 mg/L vs. 18.4 ± 4.8 mg/L;t=5.297,P<0.05).ConclusionThe integrated treatment ofYubaisang-Zhikefangdecoction plus convertional the rapy showed good effects on the patients with chronic bronchitis.
