2.Scar sarcoidosis: first case report in China

A 54-year-old woman presented with a five-year history of malaise with weight loss and arthralgia of both knee joints. Six months prior to the presentation, an indurated nodule developed at the site of an old scar on the temporal side of left eyebrow. Subsequently, erythema and nodules spread over the trunk and extremities, especially at the sites of old scars on the hands. Physical examination showed bilateral axillary lymphadenectasis, a plaque at the site of an old scar on the temporal side of left eyebrow, a subcutaneous mass at the extensor aspect of the right upper arm, multiple dark-erythematous nodules at the site of scars on the left hand and both soles, few small erythematous papules scattered on the left forearm and trunk. The serum angiotensin converting enzyme concentration was high (82 kU/L). Computer tomography of thorax showed multiple obscure tubercles in both lungs and swollen lymph nodes in mediastina and axillary fossa, but no hilum pulmonis lymphadenectasis was observed. Histopathology revealed non-caseating granulomas with multinucleated giant cells. Neither acid-fast bacilli nor PAS staining was positive. A diagnosis of scar sarcoidosis was established. The lesions obviously improved after one-month treatment with oral methylprednisolone. This case represents a rare subtype of sarcoidosis that arose in old sears and clinically manifests as scar, nodules,plaques, subcutaneous masses and papules. The reactivation of old scars, which manifests as erythematous swelling and nodules, may highly suggest the diagnosis of sarcoidosis.
3.Study of the Compliance of Hypertensive Medication

Objective:To investigate the influencing factors and medication compliance of hypertension.Method: 3 grade-Ⅲand 15 grade-Ⅱgeneral hospitals in Anhui Province were selected by random sampling.732 outpatients with a hypertension course of 6 months or above were interviewed during July and October of 2004 with the medication compliance schedule.All data were input into computers with the software Epi info 6.04,and analyzed with SPSS 11.5.Result:①The medication compliance schedule had high reliability and validity,which could be used in the surveys of medication compliance of hypertension.②The medication compliance rate of the 732 patients was only 46.4%.③Trust in medical pro- fessionals,more attention paid to the disease,more knowledge of non-compliance harm,satisfactory family income,and longer consulting time were the common advancing factors in improving the medication compliance,but a longer duration of the medication regimen,a larger number of preparations and drugs,and higher frequency of taking medicine were the bloc- king factors.Conclusion:The medication compliance of hypertensives was generally not good,and it was influenced by many factors.It is necessary that medical professionals,patients,families and society all take part in its improvement.
4.Food-borne disease related behavior analysis of family food providers in the floating population of Hangzhou city

Objective To study food-borne disease related behavior of the floating population in Hangzhou city. Methods A questionnaire on food-borne disease related behaviors was formulated based on Delphi method. A family-based retrospective investigation was conducted by unified trained investigators to analyze the frequency of food-borne disease related behaviors of family food providers during the past month in several floating-population centers of Hangzhou city. The investigation was conducted among 150 adults of every survey spot quarterly during a one-year period. Data were analyzed using Mann-Whitney U test. Results According to all 1 781 valid recovery questionnaires, 82.42%subjects had a family income less than 50 000 yuan per year and 87.42%of them had junior-middle-school or lower levels of education. The proportions of subjects who always cleaned kitchen tools, covered the garbage can and kept house out of flies, roaches and mice were 42.50%, 55.87% and 61.09%, respectively. The proportions of subjects who always purchased intact packaged food and food under warranty were 39.70%and 52.33%, respectively. The proportions of subjects who always used tapped water, cleaned hands before food processing, cooked eggsand other foods till well-done were 66.03%, 51.94%, 53.62%, 35.99% and 32.62%, respectively. The proportions of subjects who always stored food in containers with cover and for no more than 3 days in freezer were 20.89%and 30.88%, respectively. The frequencies of food-borne disease related behaviors mentioned above were significantly higher among migrants who originated from urban areas, received education above junior-middle-school and had a family income no less than 50 000 yuan/y than the others (P<0.05). Conclusion In order to correct the risky behaviors during food consuming, processing and storage, targeted intervention is necessary among family food providers of floating population families.
5.Detection of Th17 cells and interleukin-17 expression in peripheral blood from patients with atopic dermatitis

ObjectiveTo determine the protein and mRNA expression levels of interleukin-17 (IL-17) and proportions of Th17 cells in peripheral blood of patients with atopic dermatitis (AD),and to analyze their clinical significance.MethodsVenous blood samples were obtained from 63 patients with AD and 30 normal human controls.Enzyme linked immunosorbent assay was performed to measure the plasma level of IL-17 protein,real time fluorescence reverse transcription (RT)-PCR to detect the mRNA expression level of IL-17 in peripheral blood,flow cytometry to determine the percentage of Th17 cells in peripheral blood mononuclear cells(PBMCs).Spearman's rank correlation analysis was performed to assess the relationship of peripheral blood Th17 cell percentage and IL-17 levels with disease severity in patients with AD.Results The percentage of Th17 cells in PBMCs was 1.83% ± 0.47% in patients with acute AD,significantly higher than that in normal human controls (0.85% ± 0.45%,t =4.128,P < 0.01 ) and in patients with chronic AD ( 1.12% ± 0.69%,t =2.439,P < 0.05).There was a significant increase in the plasma level (98.37 vs.63.75 ng/L,U =168,P < 0.05) and mRNA expression level of IL-17 in peripheral blood in patients with AD compared with normal controls.Both the Th17 cell percentage and plasma IL-17 level were positively correlated with eczema area and severity index(EASI) score in patients with AD (r =0.681,P < 0.01;r =0.427,P < 0.05).ConclusionsThere is an elevated proportion of Th17 cells and an abnormal expression of IL-17 protein and mRNA in peripheral blood from patients with acute AD,which are positively correlated with disease severity,implicating the possible roles of IL-17 and Th17 cells in the pathogenesis of AD.
6.Radiosensitizing effects of cyclooxygenase-2 selective inhibitor LM-1685 on A549 human lung adenocarcinoma cells

Objective To investigate the radiosensitizing effects of cyclooxygenase-2 selective inhibitor LM-1685 on A549 cells in vitro.Methods A549 human lung adenocarcinoma cell line was used in this study.Cell growth kinetics Was determined using MTT assay.Cell survival was analyzed by clonogenic assay.The change of cell cycle Was measured by flow cytometry.Results LM-1685 inhibited the growth of A549 cells,showing a dose-dependent and time-dependent manner.LM-1685(50/μmol/L),either with or without IL-1β,showed the radiosensitizing effects on A549 cells,and the sensitizing enhancement ratio(SER)was 1.12 and 1.06,respectively.LM-1685(50 μmol/L)abrogated radiation-induced G2/M arrest of the tested A549 cells.Conclusions Cyclooxygenase-2 selective inhibitor can enhance the radiosensitivity of A549 cell line.Abrogation of radiation-induced G2/M arrest could be part of the mechanism.
7.Clinical analysis of 24 cases of Kaposi's varicelliform eruption

Objective To investigate the clinical features and treatment of Kaposi's varicelliform eruption. Methods Clinical data on 24 cases of Kaposi's varicelliform eruption were retrospectively analyzed.Results All the 24 patients were accompanied by different underlying skin diseases, including eczema in 17 cases and atopic dermatitis in 7 cases. PCR of vesicle fluid was positive for herpes simplex virus type 1 (HSV 1 ) in 16 (66.67%) patients, and culture of vesicle fluid or lesional exudates grew Staphylococcus aureus in 12 (50%) patients. Conclusion Infants with atopic dermatitis or eczema are rather susceptible to Kaposi's varicelliform eruption, which may be partly caused by herpes simplex virus infection.
9.Study of the Recognition Degree of Adverse Drug Reaction Reports among the Healthcare Professionals in our Hospital

Objective:To investigate the recognition degree and attitude to adverse drug reaction (ADR) reports among the healthcare professionals in our hospital.Method:Through random cluster sampling,the doctors,nurses and pharmacists in our hospital were interviewed on the spot.The questionnaires were input by EPI6.04 and analyzed by soft- ware SPSS11.5.Result:1077 eligible questionnaires were obtained and the recovery was 89.8%.Our bealthcare profes- sionals had a good recognition of basic ADR knowledge.The pharmacists had a better recognition degree than doctors and nurses.The better academic background the healthcare professionals had,the higher their score of recognition was,and there was a significant difference among them.There was no significant difference among the Scores of the healthcare profes- sionals with different working years,and nor was there among the scores of them with different professional titles.The atti- tude to ADR reporting was entirely right,but there were some wrong ideas.Conclusion:It is urgent to provide more train- ing courses for healthcare professionals so as to raise their consciousness of ADR reporting,ability of discrimination and lev- el of monitoring and avoid serious consequences caused by ADR.
10.Application of Kalman filter to measurement of sound power for medical ultrasonic diagnostic equipment

A data processing method for the sound power of the medical ultrasonic diagnostic equipment is introduced in this paper. The sound power for the transducer of a medical ultrasonic diagnostic equipment is measured for many times, and then the mathematical model based on the measurement data above is processed with Kalman filter.The output fluctuations for sound power of the equipment and its transducer as well as the measurement errors of sound power meter are eliminated to achieve the true value of sound power. Compared with other mean methods, the method above can improve the dicreteness of the measurements, and thus is worth being applied to processing other measured data.
