1.Operative technique and curative effect of self-fixation intervertebral disc endoscopyon on lumbar intervertebral disc herniation

This summary is aimed to study the virtue,operative technique and curative effect for lumbar intervertebral disc herniation (LIDH) using self-fixation intervertebral disc endoscopy. A total of 520 LIDH patients accomplishing nucleotomy with self-fixation intervertebral disc endoscopy were adopted in the experiment,including 340 males and 180 females. Their courses of disease arranged from 4 months to 10 years (17.6 months average),and follow-ups for 4-12 months. The curative effect was evaluated according to uniform standard. The curative effects were excellent in 455 cases,good in 53 cases,poor in 10 case,and the better effect rate reached 98.2%. Dura mater laceration and cerebrospinal fluid leak happened in 3 cases,coccygeal nerve damage resulting in leg numbness appeared in 1 case. Those cases all received operations at the early stage,and there were no similar complication or infection presenting in intervertebral gap. Self-fixation intervertebral disc endoscopy has the characteristics of mini-invasion,handling facility and quick postoperative recovery,so as to be suitable for the treatment of single segmental LIDH. Additionally,it plays an impact on the cases with local spinal stenosis,osteophyma hyperplasia,lateral crypt constriction and nerve canal constriction.
2.Role of intracranial electrode EEG monitoring for localizing epileptogenic zone in epilepsy

Objective To explore the role of intracranial electrode EEG monitoring for localizing epileptogenic zone in epilepsy.Methods 20 patients with refractory temporal lobe epilepsy,whose seizure onsets were unable to be localized by MRI、routine EEG and clinical characteristic,accepted long-term intracranial electrode EEG monitoring.Bitemporal lobe subdural implanted trip electrode were used to perform the ictal and interictal EEG monitoring under the guiding of the stereotactic technique.The results routine of EEG,MRI and intracranial electrode EEG were combined.The accuracy for localization of epilepitogenic zone was evaluated based on the followed-up.Results The intracranial electrode EEG monitoring was implanted within 1~5 d.Each patient was recorded the ictal and interictal discharge and was detected clinical onsets at least twice.All patients could be localized by ictal EEG.The consistency between interical EEG and ictal EEG were observated in 15 cases,the inconsistency in 3 cases,and 2 cases were bilateral discharge in interictal.The clinical outcome showed that patients were seizure-free in 13 cases(65%),improved significantly in 3 cases(15%),improved in 3 cases(15%),unimproved in 1 case(5%)according to Engel's criteria after surgery.No significant complication was found in all patients.Conclusion The intracranial electrode EEG monitoring can localize epileptogenic zone and offer reliable evidence for epilepsy surgery.
3.Effect of curcumin on the induction of glutathione S-transferases and NADP(H):quinone oxidoreductase and its possible mechanism of action

This study is to investigate the effect of curcumin on the induction of glutathione Stransferases (GST) and NADP(H):quinone oxidoreductase (NQO) and explore their possible molecular mechanism. The activity of GST, NQO and cellular reduced glutathione (GSH) content were measured by spectrophotometrical methods. Cellular changes in the distribution of NF-E2 related factor 2 (Nrf2) were detected by Western blotting analysis. Nrf2-AREs (antioxidant-responsive elements) binding activity was examined by electrophoretic mobility shift assay (EMSA). Treatment of HT-29 human colon adenocarcinoma cells with curcumin dramatically induced the activity of GST and NQO at the range of 10-30 μmol·L-1. Curcumin exposure caused a significant increase in cellular GSH content rapidly as early as 3 h. Moreover, curcumin triggered the accumulation of Nrf2 in nucleus, and increased Nrf2 content in ARE complexes. These results demonstrated that induction of GST and NQO activity by curcumin may be mediated by translocation of transcription factor Nrf2 from cytoplasm to nuclear and increased binding activity of Nrf2-ARE complexes.
4.Effects of chronic inflammation and oxidative stress on skeletal muscle mass and strength in elderly patients with type 2 diabetes mellitus

Objective:To analyze the mean levels of skeletal muscle mass and strength in elderly patients with type 2 diabetes mellitus(T2DM), and to investigate the effects of chronic inflammatory factors and oxidative stress on them.Methods:A cross-sectional study was conducted on 120 patients with T2DM aged over 60 years and 126 elderly patients without diabetes(the control group). Skeletal muscle mass, strength and serum levels of chronic inflammatory factors interleukin-6(IL-6), tumor necrosis factor-α(TNF-α)and 8-hydroxy-2′-deoxyguanosine(8-OHdG)were determined, and their effects on skeletal muscle mass and strength in elderly patients with T2DM were analyzed.Results:Compared with the control group, grip strength decreased in elderly patients with T2DM(25.03±7.85)kg vs.(29.52±7.73)kg( P<0.01), and skeletal muscle mass decreased(21.36±5.46)kg vs.(22.01±5.22)kg with no significant difference( P>0.05). Serum levels of 8-OHdG were higher in elderly patients with T2DM than in the control group(3.08±0.26)ng/L vs.(2.59±0.16)ng/L( P<0.01). Correlation and regression analysis results showed that 8-OHdG was an influencing factor for muscle strength in elderly patients with T2DM( R2=0.457)and that height and weight could be influencing factors for skeletal muscle mass in elderly patients with T2DM( R2=0.822). Conclusions:Skeletal muscle mass and strength decline in elderly T2DM patients, probably as a result of increased levels of oxidative stress.These findings may serve as evidence for sarcopenia intervention in elderly T2DM patients.

Objective To explore the advantages of the bedside DR,taking the quality analysis of the chest image taken from the bedside digital radiography systems (DR) and computed radiography system (CR).Method All of the 900 pieces chest image taken by bedside DR,compared with the 900 pieces CR image randomly chosen.Results Bedside DR could further reduce the rate of the remake and the radiation dose,and provided more excellent image information.Conclusion The chest image taken by bedside DR has obviously advantage than CR.It can play a positive role in the diagnosis and treatment of the critical patient and surgical patient.
6.A Model Study: Simulation of Marker Gene Deletion and Target Gene Replacement Mediated by Cre/lox System in Cell Line

The incorporation of site-specific recombination systems can help to overcome bottlenecks in livestock transgenic technology. For evaluating the efficiency of Cre/lox mediated DNA recombination in embryos and somatic cells, a working model was established using rat mammary carcinoma cell line SHZ-88, aimed at creation of and use repeatedly of selected "friendly loci" in transgenic livestock. An integration vector pTE-lox2272-DsRed-loxP-GFP-loxP, which red fluorescence gene DsRed served as the first target gene and green fluorescence gene GFP as marker gene, was constructed for introduction of acceptor loci in genome. At the same time a replacement vector pT-lox2272-neo-loxP in which Neo coding sequence served as the second target gene was also constructed for replacing DsRed gene. Transgenic cell clones were produced by electroporating SHZ-88 cell with the integration vector. Cells from three transgenic clones selected randomly were further amplified and were then co-electroporated with the replacement vector as well as cre gene. Analysis of the expression patterns of DsRed and GFP indicated that among the 1 070 cell colonies the efficiency on marker GFP deletion was 91.1% and the efficiency on gene replacement was 29.3%. Molecular analysis by PCR and Southern blotting confirmed that the color patterns as expressed by cell colonies could represent the actual molecular events. This working model mediated by Cre/lox system should be useful for the improvement of the present animal transgenic technology.
7.Computed tomography pulmonary angiography with low dose contrast medium optimization research in dual-source CT

Objective To explore the feasibility of high-pitch CT pulmonary angiography (CTPA)with low dose contrast medium optimization in dual-source CT.Methods Eighty patients with high risk of pulmonary embolism who underwent CTPA on a dual-source CT were divided into two groups randomly:Group A,40 patients,50 mL contrast medium per patient,with conventional bolus tracing scanning;Group B,40 patients,contrast medium=body weight×0.3 mL/kg,with test bolus of small dose contrast agent. Pulmonary artery CT value,the differences of CT values in the paired PA and PV,SNR,CNR,radiation dose of two Groups were compared by two sample t-test.The positive rate of pulmonary embolism and the subjective indexes for assessing CT image quality were compared byχ2 test.Results There were no difference of the positive rate of PE between two Groups (P =0.655),and no sta-tistical difference for image quality scores between two Groups(P =0.1 74).The MPA CT value and the differences of CT values in the paired PA and PV in L-SⅢ of Group B were higher than those in Group A (P =0.001).However,no significant differences in CNR and SNR in MPA were observed between the two Groups(P =0.250,0.203,respectively).The ED of Group B was lower than that of Group A (P =0.001).Conclusion High-pitch CTPA in dual-source CT with lower dose contrast medium optimization can minimize the amount of contrast agent while meeting the clinical requirements.
8.Iso-C3D navigation to implant cervical pedicle screw with intraoperative aiding

Objective To evaluate the accuracy of Intraoperative Iso-C3D navigation guiding cervical pedicle screw fixation. Methods Twenty-two cases adopted cervical pedicle screw fixation of cervical spine, including 9 cervical spine fractures, 4 cervical tumors, 6 cervical destabilizing, 3 cervical syndrome. The cervical pedicle screw position were assessed with post-operative CT by grade. Results One hundred and twelve screws were fixed in 22 cases successfully. Grade: 107 in A grade (95.5%); 3 in B grade: 1 in C grade; 1 in D grade. There were not any complication. Conclusion Intraoperative Iso-C3D navigation can improve precision of cervical petiole screw fixation distinctively.
9.Test-retest Reliability and Measurement Error in Using A Jamar Dynamometer to Determine Grip Strength for Elderly Men

ObjectiveTo examine the test-retest reliability and measurement error in using a Jamar dynamometer to determine grip strength for elder men. Methods40 elder men with a mean age of 82 years old volunteered to participate in this study. The grip strength was administered twice using a Jamar dynamometer by specially trained testers; subjects were re-tested at the same time of day a week later by the same testers. ResultsThe infraclass correlation coefficient(ICC) was 0.99. The standard error of measurement (SEM and SEM%) were 1.18~1.69 and 4.11%~6.41%. The smallest real difference (SRD and SRD%) were 3.27~4.68 and 11.39%~17.75%. The Bland-Altman analysis revealed no systematic errors between test and retest. ConclusionThe Jamar dynamometer is reliable and with acceptable measurement errors to determine grip strength for elder men.
10.Application of infrared fluoroscopic navigation in guiding pedicle screw fixation of thoraic-lumbar spine

Objective To evaluate the application of infrared fluoroscopic navigation guiding pedicle screwfixation of thoracic-lumbarspine.Methods The pedicle screw fixation of thoraic-lumbar spine was adopted in 32 patients,including 19 cases with thoracic-lumbarspine fractures,5 cases with protrusion of lumbar intervertebral disc or vertebral canal stenosis,8 cases with lumbar spine slippage.The timefor pedicle screwfixation and amount of bleeding were recorded.The pedicle screwposition was assessed with post-operative CTby using An-drewgrade.Results Totally148 screws were fixed in 32 cases successfully,including10 screws in T11,20 in T12,18 in L1,36 in L2,16 inL3,26 in L4,28 in L5,and 14 in S1.According to andrewgrade,165 screws were inⅠgrade(98%),2 inⅡgrade,and 1 inⅢ grade.Nonerve and spinal cord damage was observed after operation.The average time of per pedicle screwfixation was(10?2.4) min.The averageamount of bleeding was(400?52.3) ml in thoracic-Lumbar Spine fractures,(200?36.8) ml in protrusion of lumbar intervertebral disc orvertebral canal stenosis,and(300?44.6) ml in Lumbar Spine slippage.Conclusion Infrared fluoroscopic navigation can improve the pre-cision of pedicle screw fixation of thoraic-lumbar spine,reduce the amount of bleeding,and shorten the operative time.
