1.Research on value of multislice spiral CT to evaluate complication of acute pancreatitis

Objective:To explore the application value of multislice spiral CT scanning on abdomen for the detection of complication of acute pancreatitis.Methods: 120 patients with acute pancreatitis were divided into observation group (70 cases) and control group (50 cases) according to different detection method. The patients of control group were applied basic clinical detections included X-ray radiography and ultrasound imaging, while patients of observation group were supplemented CT detection besides these detections of control group. And situation of complication of patients in two groups were observed.Results: The positive rate of complications, which involved pancreatic pseudocyst, cellulitis, complications of gastrointestinal tract, pancreas hemorrhage and pancreatic ascites, in observation group was 97.14%, that was significant higher than that (62.0%) of control group (x2=24.74,P<0.05). Among 20 patients with pancreatic pseudocysts, there were 9 patients located at tails of pancreases, 6 patients located in bodies of pancreases, 3 patients located in small omental bursas, 2 patients located at heads of pancreases, and the CT images of them showed quasi-circular liquid form density lesions with clear boundaries appeared in lesion field. The CT images of cellulitis showed the volume of pancreas increased and the boundaries were fuzzy, and a larger range of density lesions with irregular soft tissue shape appeared in its periphery. CT detetions of gastrointestinal complications showed fuzzy point-shaped, strip-shaped and flake-shaped soft tissue-styled density shadow appeared in gastrointestinal tracts. The CT image of pancreatic ascites showed intraperitoneal fluid density shadow.Conclusion: Abdominal CT scan applied on patients with acute pancreatitis can increase the positive rate of complications and may be used as preferred detection method for clinical evaluation on the complications of acute pancreatitis.
2.Inhibition effects of Fuerkang Injection and its ingredients on SMMC-7721 and NB4 cell lines

AIM: To study the inhibitiory effects of Fuerkang Injection (sweet worm wood oil, glycyrrhetate and Vitamin A acetate). and its ingredients on SMMC-7721 and NB4 cell lines, in order to understand its antitumour spectrum and reasonability of its composition. METHODS: SMMC-7721 and NB4 cell lines were treated with Fuerkang Injection and its ingredients under the same concentration for 48 h, adfiamycin was used as standard com-parison,inhibitiory effect on growth of two cell lines was detected by MTT method. RESULTS: At 1.260 μg/mL, the inhibition rate of Fuerkang Injection and its decomposed recipes preparations 1,2,3 to SMMC-7721 cell lines were 54.78%, 55.49%, 53.18%, 53.79%, hemi-inhibitory concentration IC_(50) were 0.415,0.376,0.715 and 0.636 μg/mL,respectively. The inhibition rate of NB4 cell lines were 88.6% ,82.5% ,77.9% and 76.9% ,hemi-inhibitory concentration IC_(50) were 0.091,0.108 1,0.084 and 0.120 μg/mL. CONCLUSION: Fuerkang Injection and its ingredients have significant inhibitory action on SMMC-7721 and NB4 cell lines,each component herb in the compound preparation has synergetic effects.

4.Effects of monitored anesthesia care with deep sedation on gastrointestinal endoscopy

Objective To explore effects of monitored anesthesia care (MAC) on gastrointestinal endoscopy.Methods All patients who underwent gastrointestinal endoscopy in Peking University People's Hospital from September 2010 to September 2015 were analyzed retrospectively and divided into two groups:the MAC group and control group by usage of MAC.Success rates of removal of foreign bodies by upper gastrointestinal endoscopy were compared between two groups during this period.Detection rates of ectopia of gastric mucosa in upper esophagus were compared between two groups from September 2014 to September 2015 when esophagogastroduodenoscopy was performed.The monthly cases of gastrointestinal endoscopies were compared between the two groups,which were separately collected from one MAC operating table and one routine operating table from September 2014 to September 2015.Results The success rate of removal of foreign bodies in the MAC group was higher than that in the control group (100.0% VS 89.7%,x2 =11.737,P=0.001).The detection rate of ectopia of gastric mucosa in upper esophagus in the MAC group was higher than that in the control group(4.3% VS 1.7%,x2 =58.751,P<0.001).The monthly cases of gastrointestinal endoscopies in the MAC group was higher than that in the control group(350.2±36.2 cases VS 213.2±27.9 cases,t =17.591,P<0.001).Conclusion The use of MAC for gastrointestinal endoscopy improves operating success rate,accuracy and operating time.MAC in gastrointestinal endoscopy can be recommended.

The effect of maltol to prevent erythrocyte from auto-oxidation is presented. When erythrocytes were incubated at 37 ℃ for 24 h in vitro, oxyhemoglobin was decreased, methemoglobin, superoxide freeradical, lipofuscin and Heinz's body were increased as well as membrane proteins were changed. These reactions could be inhibited by maltol.

Objective: To investigate the mechanism that glutamine downregulates the cytokine expression in lipopolysaccharide-stimulated human peripheral blood mononuclear cells.Methods:PMBC were extracted from healthy volunteer by density gradient centrifugation,and stimulated by lipopolysaccharide.The release of TNF-α and IL-10 was analyzed via enzyme-linked immunosorbent assay and p-P38 via western blot. Results:Gln led to an increase in HSP70 expression,and decreased TNF-α、IL-10 release at 4 h after LPS stimulation.The expressions level of TNF-α、IL-10 and P-P38 were decreased after using SB203580.Conclusion:These results indicate that the effect of Gln could attenuate cytokine release in PBMC and might may be related to the inhibition of P38 MAPK.
8.Investigation on implementation of three standards by central sterile sup-ply departments

Objective To realize the status in the implementation of three industry standards of central sterile sup-ply department (CSSD),and provide a scientific basis for carrying out of the standards. Methods According to three standards,investigation forms were designed by specialists,written survey on 365 hospitals in 9 provinces and field investigation on 1 5 hospitals in 3 provinces were performed,the implementation of three standards were investi-gated.Results Of 365 hospitals,the number of provincial and ministerial level,municipal level,and county level hospitals were 90,87 and 188 respectively. More than 94% of hospitals established CSSD management system and regulations,>90% of hospitals met the requirements of CSSD layout;All hospitals were equipped with pressure steam sterilizer,all levels of hospitals basically equipped with the necessary equipments and facilities;CSSD respon-sible officers of 94.52% (345/365)of hospitals participated in training on standards;69.61% (252/362)of hospi-tals were using or developing CSSD information systems;>92% of the CSSD responsible officers considered that three standards played an important role in facilitating centralized management,and improving the quality of clean-ing,disinfection and sterilization.Conclusion Hospitals need to strengthen the management and training on stand-ards of CSSD,management of loaner instruments and development of information system need to be standardized.
9.Diagnosis and treatment of adrenal teratoma: a report of 5 cases and review of literature

Objective To discuss the feature,diagnosis and treatment,and proguosis of adrenal teratoma.Methods Clinical data of 5 patients with confirmed adrenal teratoma were retrospectively reviewed.There were 2 males and 3 females,with a median age of 57 years (35-64).The mean tumor size was 5-8.5 cm.The tumors were located on the right side in 3 cases and the left side in 2.Two cases were detected by routine health check up,and 2 were detected with the complains of fatigue and 1 with complains of back pain.All cases had normal value of endocrinology and biochemistry tests.Ultrasound examination showed cystic tumor on adrenal gland with uneven echo.CT showed irregular,inhomogeneous adrenal neoplasms in adrenal gland.The tumor presented as inhomogenous high signal intensity on MRI T1 and T2.Results Five patients underwent laparoscopic adrenalectomy,and histopathology showed adrenal mature teratoma.No recurrence occurred after operation in all patients.Conclusions These cases combined with literature review indicate that adrenal teratoma is extremely rare which has no characteristic clinical manifestations; and laparoscopic adrenalectomy is a feasible,effective technique with excellent results.
10.~1H-MRS study on radiation-induced injury of the brain in early acute reaction stage after nasopharyngeal carcinoma radiotherapy

Objective To evaluate the diagnostic value of in vivo 1H-MRS in the hyper acute reaction stage of radiation-induced brain injury after nasopharyngeal carcinoma radiotherapy. Methods Eighteen patients with nasopharyngeal carcinoma accepted radiotherapy for the first time. Bilateral temporal lobes in all cases were examined by conventional MRI and 1H-MRS before and after radiotherapy with radiation dose of 20 Gy, 40 Gy, and 60 Gy, respectively. MR image was performed with T_1-weighted gradient- and spin-echo, T_2-weighted spin-echo, fluid-attenuated inversion-recovery, and point resolution spectroscopy. The change of the spectroscopic morphology and the ratios of the metabolites were observed. Results There were no changes of signal in the temporal lobes with conventional MRI, but the peak height of NAA and Cho decreased obviously in the anterior half of the temporal lobes with 1H-MRS after radiotherapy. The post-radiotherapy changes of different metabolic peak in the anterior temporal lobes presented as two types: one type was that Cho and NAA peak were of the same height simultaneously. There were 20 locations (56%) of the 36 reaching the peak when accepting 20 Gy, 22 (61%) when accepting 40 Gy, and 13 (36%) when accepting 60 Gy, respectively. The other type was that Cho peak became the highest, whereas NAA peak was obviously lower and decreased to be the second highest. There were 4 locations(11%) of the 36 reaching the peak when accepting 20 Gy, 10 (28%) when accepting 40 Gy, and 23 (64%) when accepting 60 Gy, respectively. The ratio of Cho/Cr, NAA/Cr, and NAA/Cho decreased in the same position too. In addition, there was positive correlation between the ratios of the metabolites and the dose of radiotherapy.Conclusion 1H-MRS can be used to diagnose the radiation-induced injury of the brain in early acute reaction stage after nasopharyngeal carcinoma radiotherapy through reflecting histiocytic metabolism, and provide objective basis for pathology development and cellular metabolism study, also it can provide feasible projects for the clinic.
