1.CLINICAL APPLICATION AND ANATOMICAL STUDIES OF THE PARASCAPULAR FLAP

The authors reported the results of anatomicat study of the scapular region (ninety-four sides) and the use of the parascapular composite flap in the repair of skin defects of the hand and foot.The parascapular flap is supplied by the circumflex scapular vessels. For exposure of the vascular pedicle an incision is made 7cm below the mid-point of the scapular spine, extending from the posterior axillary fold to the mid-point of the spine. Muscles are retracted and the circumflex scapular vessels are seen winding over the axillary border of the teres minor. Then the incision is extended from both ends of the above incision toward the inferior angle of the scapula to form an elliptical incision. The flap is raised by subfascial dissection from the mid-point of the spine towards the axilla, and ended by dividing the pedicle at the base of the circumflex scapular vssels. Since October 1982, the authors have applied flaps of such kind to repairing massive skin delects in the hand and the foot in 7 patients with success.
3.Content Determination of Berberine in Zhenjing San by HPLC

Objective To establish the method of content determination of Berberine in Zhenjing San. Method The analysis was carried out on C18 column (4.6 mm?250 mm, 5 ?m) by HPLC. The mobile please was acetonitrile-0.033 mol/L KH2PO4 (22∶78), the flow rate was 0.800 mL/min and detective wave length was 345 nm. Result The cabibration curves showed good linearity in the range of 12.012~600.600 ?g. The average recovery was 98.85% (RSD=1.09%). Conclusion The method is good sensitive, accurate and reliable.

The feasibility and mechanism study on intravenously injected bone marrow mesenchymal stem cells (MSCs) improving bladder function after spinal cord injury (SCI) were discussed by review the biological characteristics of MSCs, the changes and mechanism of the lower urinary tracts after SCI, and the study on the MSCs promoting recovery of bladder function.
5.Application of radionuclide bone imaging in diagnosis and localization of osteoporotic vertebral compressive fracture

Objective To investigate the value of radionuclide bone imaging in the diagnosis and localization of osteoporotic vertebral compressive fracture.Methods Percutaneous vertebroplasty was applied to 40 patients suffering from osteoporotic vertebral compressive fracture.Responsible vertebral bodys located according to the radionuclide bone imaging was involved as bone scan group,and responsible vertebral bodys located according to the physical examination,X-ray,CT and MRI as standard group.Then the Kappa test was carried out.Result In bone scan group,56 vertebrae were located by radionuclide bone imaging,and in standard group,58 vertebrae were located by X-ray,54 vertebrae by CT and 52 vertebrae by MRI.The Kappa value was 0.76,indicating essentially excellent agreement and statistical significance.Conclusion Radionuclide bone imaging was another good choice for the diagnosis of osteoporotic vertebral compressive fracture.Especially for cases with contraindication of MRI,it offers a reliable choice.
6.Percutaneous vertebroplasty combined with granulated allogeneic bone grafting for thoracolumbar compressive fractures: the completeness and stability of fractured vertebrae

BACKGROUND:Percutaneous vertebroplasty is mainly used to treat senile osteoporotic compressive fractures,while for young patients,pedicle screw internal fixation is usually applied,but heavy trauma,nail broken,secondary surgery and other problems can be found occasionally.OBJECTIVE:To investigate the clinical efficacy of percutaneous vertebroplasty combined with granulated allogeneic bone grafting for thoracolumbar compressive fractures.METHODS:Twenty-one patients suffering thoracolumbar compressive fractures were recruited,and underwent percutaneous vertebroplasty combined with granulated allogeneic bone grafting.The Visual Analogue Scale scores,height ratio of fractured vertebrae and Cobb angle were detected before and after surgery.RESULTS AND CONCLUSION:(1) Compared with baseline,the Visual Analogue Scale scores were improved significantly at 1 day and 6 months postoperatively (P < 0.05).(2) The height ratio of fractured vertebrae at 1 day and 6 months postoperatively was significantly higher than that before treatment (P < 0.05).(3) The Cobb angle of fractured vertebrae at 1 day and 6 months postoperatively was significantly less than that before treatment (P < 0.05).(4) The mean operation time was (52±12.3) minutes,and the mean blood loss was (11.5±1.5) mL.(5) These results suggest that percutaneous vertebroplasty combined with granulated allogeneic bone grafting can effectively restore the anterior fractured vertebral height,eliminate the cavity in the fractured vertebrae,renew the completeness and stabilization of the fractured vertebrae,which keep the spinal range of motion;therefore,it is a minimal invasive method in the treatment of thoracolumbar fractures.
7.The Effects of Short-term Intensive Therapy with Glargine Versus Glimepiride on ?-cell Function in Newly Diagnosed Type 2 Diabetic Patients

Objective To evaluate the effects of two different short-term intensive therapies on ?-cell function in newly diagnosed type 2 diabetic patients.Methods Twenty newly diagnosed type 2 diabetic patients were divided into two groups of 10 patients each and randomly treated with glargine or glimepiride plus metformin for 4 weeks.The improvement of ?-cell function,FPG,PPG,HbA1c were measured before and after intensive therapy in each group.The difference in the improvement of ?-cell function,blood glucose and the frequency of hypoglycemia were compared between groups.Results After the treatment,FPG(12.25 vs.6.05 and 11.83 vs.6.35mmol/L),2hPG(18.67 vs.8.11 and 16.95 vs.8.73mmol/L),HbA1c(8.98% vs.7.38% and 9.17% vs.7.71%) were significantly decreased(all P
8.Association between SUR1 Ser1369Ala polymorphism and the efficacy of sulfonylurea

Objective To investigate the association between a missense mutation (T/G, Ser 1369 Ala) in exon 33 of sulfonylurea receptor 1(SUR1) gene in type 2 diabetes patients and the glucose-lowering effect of Gliclazide. Methods One hundred and four type 2 diabetes patients were selected and orally administered with Gliclazide for 56 days. Venous fasting plasma glucose levels (FPG), plasma glucose levels half an hour after taking 75 g glucose (OGH) and two hours (OGT) after taking Gliclazide were measured. Ser1369Ala polymorphism genotypes(TT, TG, GG) of SUR1 gene were determined by Taqman method. The glucose-lowering effect of Gliclazide was compared among different groups. If FPG was reduced more than 20%, we define it as effective. People whose blood sugar level was effectively reduced and not significantly reduced were compared to find whether their genotype groups were different.Results The change of FPG,OGH and OGT on day 1 and day 57 among TT,TG and GG genotype groups had no statistically significant difference. There was no statistically significant difference of the efficacy of Gliclazide in recessive model (TT+TG vs GG). The combination of TG and GG was more effective in reducing blood sugar level than TT. Conclusion Patients carrying one or two copies of the G allele were more sensitive to Gliclazide than patients carrying T allele.
9.The analysis of surgical nurses' learning motivation of attending continuing nursing education

Objective To describe surgical nurses' learning motivation of attending continuing nursing education (CNE) and to analyze influencing factors of learning motivation.All of these would be reference for managers of hospitals and administrative department to improve the quality of CNE and make related policy.Methods 140 surgical nurses from one general hospital were recruited to fill a questionnaire of demographic data of participants and revised Education Participation Scale (EPS).Results The score of EPS of surgical nurses was (89.26±13.67),the first dimension was professional advancement,followed by serving society,learning interest,social relation,external expectations and escape/stimulation.The score of EPS of nurses who were younger than 35 years old,single,no administrative posts was higher than nurses who were older than 35 years old,married or in administrative position.Education level,working life,professional ritle,financial resources and numbers participating in CNE in recent 1 year made some differences for EPS score.Conclusions The score of EPS of surgical nurses was at middle level,factors affecting EPS were different.The managers should take interventions to improve the motivation of attending CNE according to nurses' characters in order to increase the professional level and nursing quality of nurses.
10.miRNA-199 a-5 p inhibit the invasion of MDA-MB-231 cells via regulating ERK5 through SP1

Purpose To study the effect and mechanism of miR-199a-5p on the invasion of breast cancer MDA-MB-231 cells. Meth-ods miR-199a-5p mimic was transfected into MDA-MB-231 cells. Influence of miR-199a-5p on the invasion of MDA-MB-231 cell was displayed by Transwell, the expression of epithelial-mesenchymal transition ( EMT) molecular markers ( E-cadherin, vimentin) regulated by miR-199a-5p was determined using immunofluorescence and Western blot. Western blot was employed to assess the levels of ERK5, pERK5, EGF and SP1 in MDA-MB-231 cells dealt with miR-199a-5p mimic and LNA-siRNA. Chromatin immunoprecipita-tion (CHIP) was applied for displaying the reaction of SP1 with ERK5 promoter. Results miR-199a-5p could inhibit the invasion of MDA-MB-231 cells, decrease the expression of vimentin and enhance E-cadherin. Meanwhile, miR-199a-5p decreased the expression of ERK5 and pERK5, the levels of EGF and SP1 were also downregulated. On the contrary, the levels of EGF, SP1, ERK5 and pERK5 were enhanced by employing LNA-siRNA targeting miR-199a-5p. SP1 could bind with ERK5 promoter. Conclusions miR-199a-5p could reduce the expression of ERK5 and pERK5 through regulating EGF and SP1, which functioning the inhibitory effect on invasion of MDA-MB-231 breast cancer cells.
