1.Clinical study in acupressure time after abdominal injection of low-molecular-weight heparin calcium

Objective To investigate the relation of acupressure time and incidence rate of sub-cutaneous hemorrhage after abdominal injection of low-molecular-weight beparin calcium.At the same time to seek the most appropriate acupressure time to reduce incidence rate and area of subcutaneous hemor-rhage. Methods Patients(60 cases)undergoing abdominal injection of low-molecular-weight heparin cal-cium from October 2005 to August 2006 were selected.Each patient received 16 times of injection.They were divided into 8 groups according to acupressure time,the acupressure time for each group was 3,4,5,6,7,8,9,10 minutes. Incidence rate and area of subcutaneous hemorrhage was observed. Results Lo-cal acupressure time was related to the incidence rate and area of subcutaneous hemorrhage,P<0.01. The incidence rate and area of subcutaneous hemorrhage was statistically different between acupressure time of 6 minutes and 7 minutes,P<0.01,while other adjacent groups had no statistical difference. Conclusions Local acupressure time of 7 minutes can effectively reduce incidence rate and area of subcutaneous hemor-rhage after abdominal injection of low-molecular-weight heparin calcium.
2.Role of Proteasome Peptidase Activities in the Ethanol-Induced Cytotoxicity on PC12 Cells

Objective To clarify whether ethanol induced cytotoxicity of neurons was inhibited by proteasome peptidase activities. Methods A rat pheochromocytoma(PC12) was used in the present study. Cytotoxicity and proteasome peptidase activities were assessed in PC12 cells exposed to 50, 100, 150 mmol/L, 200 and 300 mmol/L ethanol. Results After 48 hours of treatment, ethanol inhibited proteasome peptidase activity in a dose-dependent manner at the concentration above 100 mmol/L. Moreover, the effect of ethanol on activities of proteasome was also in a time-dependent manner from 24 h. Cytotoxicity was still observed as ethanol concentrations increasing, started from 100 mmol/L, as assessed by MTT method. Conclusion Our results demonstrates ethanol may disturb the metabolism of protein by inhibiting proteasome peptidase activities, which is related to the toxic mechanism of ethanol on PC12 cells.
3.Research Progress of Intravesical Electrical Stimulation for Underactive Bladder (review)

Underactive bladder is the main part of lower urinary tract symptoms. As a conservative treatment, intravesical electrical stimulation can improve the function of bladder sensation, and promote micturition. Intravesical electrical stimulation is effective and safe on underactive bladder, both idiopathic and neurogenic. Rectilinear biphasic waveform, low frequency and short pulse width stimulation was recommended.
4.Septum strut implantation for rhinoplasty simultaneous with cleft alveolar bone grafting in cleft lip and palate patients

Objective To pursue a more efficient and effective treatment for cleft lip and palate deformities. Methods Patients with unilateral complete cleft lip and palate at their age of 9 years after were chosen for simultaneous cleft alveolar repair and nasal deformity correction. Muco-periosteal pocket and iliacgranular bone was prepared, and bone grafting was performed conventionally. At the same time of iliac cancellous bone harvesting, a cortical plate was taken and sculpted into a strut of 18 mm in length, 6 mm in width, and 1.5 mm in thickness. A flying bird incision was made at the alar ram and across columella in a V-shape. Then the alar cartilage was detached from the overlying skin, a socket was made at the site of anterior nasal spine. The strut was inserted into the socket between the two medial crura of the alar cartilage. The medial crura was lift 3 mm above the superior edge of the strut, and mattress suture technique was used to secure the bilateral medial crura to the strut graft. Results 24 patients were treated by this technique. All the patients healed uneventfully. Depressed alar base, tilted columella and lower nasal tip were corrected satisfactorily. Conclusion There is no interference in simultaneous cleft alveolar bone grafting and rhinoplasty. Septum strut can provide favorable support for tilted nasal structure and satisfactorily correct nasal deformities. Simultaneous with alveolar grafting, it is much easier in harvesting, and the time of anesthesia and operation is also decreased.[
5.Implantation of artificial urinary sphincter for treatment of trauma-induced genuine stress incontinence (report of 2 cases and review of literature)

Objective To evaluate the therapeutic effect of implantation of artificial urinary sphincter (AUS) for treatment of patients with trauma-induced genuine stress incontinence. Methods Two patients (1 male and 1 female) with genuine stress incontinence due to urethral rupture underwent implantation of AUS device (AMS 800).In the male case,the cuff of the AUS device was used to wrap the bulbar urethra.The reservoir was placed into the retropubic space.The controllable pump was implanted into the right scrotum. In the female case,the cuff was used to wrap the neck of bladder.The reservoir was placed into the retropubic space. The pump was implanted into the right subcutaneous labium majus.The literature was reviewed to show the indication,efficacy,complication and durability of implantation of AUS device. Results After operation the male patient was followed up for 17 months,and he achieved complete continence.The female was followed up for 14 months,and she had significant improvement in continence with changing 1 or 2 small pads every day.There were no infection, erosion, urethral atrophy and mechanical failure during follow-up.The review of the literature showed that in patients with implantation of AUS the mean of continence rate was 85.5%;the incidence rates of infection,erosion,urethral atrophy and mechanical failure were 6.1%,7.5%,6.1% and 16.7%,respectively. Conclusions The implantation of AUS is an effective,reliable method for patients with trauma-induced genuine stress incontinence.
6.The roles and administration methods of insulin in the early treatment of acute brain injury

Objective To explore the roles of insulin in the early treatment of acute brain injury and its administration methods.Methods 253 patients were randomly divided into the intensive insulin therapy group and the conventional therapy group.Infection rates,the short-term effect (APACHE Ⅱ assessment),and long-term efficacy (GOS prognosis) was compared between two groups.Results The results of the strengthen treatment group in the rate of infection (25.95% vs 39.34%,x2 =5.17,P <0.05),the short-term effect (11.33 ± 7.66 vs 16.49 ± 14.97,u =3.42,P < 0.05) and the long-term efficacy (40.46%,55.73%,3.82% vs 25.41%,68.85%,5.74%,x2 =7.62,P <0.05) were significantly better than the conventional therapy group with the statistically significant differences (P < 0.05).Conclusions The hypoglycemic effect,neuroprotective effect,regulatory role,and nutrition role of insulin occurred in the early treatment of acute brain injury.After acute brain injury,patients with hyperglycemia should be treated early with an enough volume,continuous,and uniform insulin.
7.Separation and Purification of Prim-O-glucosylcimifugin and 5-O-methylvisammioside from Saposhnikovia Divaricata by High-speed Counter-current Chromatography

High-speed counter-current chromatographic ( HSCCC ) method was successfully used to separate and purify prim-O-glucosylcimifugin and 5-0-methylvisammioside from Saposhnikovia divaricata(Thicz. ) Schis-chk. ,a traditional Chinese herb,with a solvent system composed of ethyl acetate-n-butanol-water(2:7:9,V/V). The lower phase of the system was used as the mobile phase at the flow rate of 2.0 mL/min,and the upper phase was used as the stationary phase. The separation produced a total 13.9 mg prim-O-glucosylcimifugin and 25.0 mg 5-0-methylvisammioside with the purity of 98. 1% and 99.2% determined by high performance liquid chromatography (HPLC) from 316 mg of crude sample from S. divaricata. The structures of isolated compounds were identified by ESI-MS,~1H NMR and ~(13)C NMR.
8.Expression of membrane CD14, Toll-like receptor 4 and nuclear factor-kappa B in peripheral blood leukocytes from patients with psoriasis vulgaris

Objective To investigate the expression of membrane CD14 (mCD14), Toll-like receptor (TLR4) and nuclear factor-kappa B (NF-kB) in peripheral blood ieukocytes from patients with psoriasis vulgaris (PV). Methods Blood samples were obtained from 29 normal controls and 32 patients with PV. Patient cohort consisted of 19 males and 13 females, with an average age of 30.28 ±13.11 years, disease duration of 8.21±10.21 years, and PASI score of 21.19±5.85. Flow cytometry was used to measure the expression of mCD14, TLR4 and activated NF-kB in peripheral blood leukocytes in these subjects. Results The expressions of mCD14 and TLR4 in peripheral blood leukocytes were significantly higher in patients than in the normal controls (5.454±2.78 vs 3.937±1.964, P < 0.05; 17.641±18.120 vs 7.527±8.574, P <0.01 ). Although the expression of activated NF-kB in total blood leukocytes in patients was higher than that in normal controls, no statistical difference was noticed (63.538±8.650 vs 62.236±6.72, P > 0.05). Conclusion There is an abnormal expression of mCD14 and TLR4 in patients with PV, suggesting that the abnormal activation of endotoxin signal transduction pathway may exert a role in the induction of psoriasis.
9.Different wavelength krypton lasers in treatment of diabetic retinopathy

Objective To investigate the therapeutic effect of different wavelength krypton lasers on diabetic retinopathy. Methods A total of 55 eyes (35 cases) with diabetic retinopathy underwent different wavelength krypton lasers photocoagulation treatment, according to the different manifestation of the affected eyes. The visual acuity, intraocular pressure,visual field,visual evoked potential were examined, and slit lamp, ophthalmoscopy, B ultrasonography, and fundus fluorescein angiography were performed preoperatively. The patients were followed up for at least 12 months after krypton laser treatment. Results The resulting effect on visual acuity after 12 months of photocoagulation in this series revealed that, 20 eyes (36.4%) improved, 34 eyes (61.8%) remained no change, and one eye (1.8%) decreased. Conclusions Different wavelength krypton lasers photocoagulation can be used in treatment of diabetic retinopathy and can improve the visual acuity at certain extent.
10.Urethral stent implantation in treatment for detrusor-sphincter dyssynergia caused by neurogenic lower urinary tract dysfunction

ObjectiveTo observe the effect of urethral stent implantation on detrusor-sphincter dyssynergia caused by neurogenic lower urinary tract dysfunction.Methods13 patients with detrusor-sphincter dyssynergia caused by neurogenic lower urinary tract dysfunction were treated with the operation of the urethral stent implantation. Voiding function, renal function, residual urine volume and hytronephrosis were examined before and after the operation to evaluate the effect of this procedure.ResultsAfter operation, 7 patients normally emptied their bladders and 6 patients had urinary incontinence. Urethral stents were removed from 2 patients in this group due to the irritation symptoms, the second implantation was performed in a patient due to the voiding difficulty. The renal function of patients after the operation had a non-significant improvement, but the residual urine volume and hytronephrosis improved significantly.Conclusion Urethral stent implantation can decrease residual urine volume and hytronephrosis in patients with detrusor-sphincter dyssynergia caused by neurogenic lower urinary tract dysfunction.
